Serum prolactin response to acute and chronic cimetidine administration in man.

UNLABELLED: The effects of cimetidine administration on the serum prolactin (PRL) response has been studied in twenty healthy volunteers and 46 duodenal ulcer patients. Cimetidine acute administration in two different doses (200 or 400 mg i.v.) induced a similar conspicuous rise of PRL while CB154, a dopaminergic drug, inhibited said increase. No high PRL level was observed when the drug was administered per os in a single acute dose (800 mg), nor in the chronic short-term treatment (1 g/die/28 days) for duodenal ulcer. CONCLUSIONS: 1. Cimetidine has no antidopaminergic activity or this hypothetic action is very slight; 2. serum PRL increase after the drug is probably ascribable to the H2-receptor blocking, 3. this effect is observable only with the bolus i.v. of cimetidine, but not during or after acute and chronic oral administration.

Clinical and immunological aspects of sporotrichosis cutanea.

A series of 14 cases of sporotrichosis cutanea is reported. Epidemiological and clinical aspects of the disease are discussed in the light of recent data. In addition to cultures, the diagnostic methods to be considered include an indirect immunofluorescence test, which is positive at variable titre and for which use may be made of normal human antiserum or anti-immunoglobulin, mainly IgG and IgM, antisera. The mainstay of the treatment is preparations of iodine, with trimethoprim- sulpha-methoxazole as a possible alternative.

The role of caerulein in tests of exocrine pancreatic function.

Secretin (1 CU/Kg) plus Caerulein (100 ng/Kg) or Cholecystokinin (1 or 2 IvyU/Kg) were given by rapid intravenous injection (Schedule 1) or by continuous infusion (Schedule 2) to 63 control subjects (C) and 69 patients affected by chronic pancreatitis (CP). Duodenal juice was collected for two and four 30-minute periods in schedule 1 and schedule 2, respectively. Volume, bicarbonate, and enzyme content were measured. Secretin-Daerulein, by rapid intravenous injection, showed a strong overlapping between C and CP values and led to some side-effects. Secretin-Caerulein by continuous intravenous infusion gave almost identical results as the Secretin-Cholecystokinin.

Effect of histamine-H2-receptor antagonists on the exocrine pancreatic secretion.

Pancreatic exocrine secretion has been studied in 10 patients with documented duodenal ulcer before and after 4 weeks of cimetidine treatment (1 g/die). This chronic 'short term' cimetidine administration did not result in any substantial modification of exo-pancreatic response stimulated by Secretin and Caerulein. From this point of view the drug could be employed in the treatment of pancreatic insufficiency in order to obtain a better utilization of pancreatic enzyme supplement. It is, however, questionable if such a 'long-term' therapy may be responsible for the appearance or the relapse of a peptic ulcer.

Angiography in chronic pancreatitis and pancreatic cancer. A critical evaluation.

A critical "blind" evaluation of 129 randomly selected angiographic examinations was carried out including 37 control patients, 58 patients affected by proven chronic relapsing pancreatitis and 34 patients with cancer of the pancreas. In 48.5% of the control patients a completely normal angiographic picture was found. The false positives were found in 10.8% of chronic pancreatitis and in pancreatic carcinoma in 5.5% of the cases. Equivocal signs were found in 35.2%. The percentage of the false negative results in chronic pancreatitis was 34.4% (of which 8.6% were suggestive of pancreatic cancer). In pancreatic cancer positive results were seen in 70.6% of the cases. The percentage of the false negatives was 26.5% (suggestive of chronic pancreatitis); equivocal signs were found in 2.9% of these patients. Notwithstanding the not-negligible percentage of errors, angiography can be usefully employed in diagnosis of pancreatic disorders.

Behaviour of contact allergy and new sensitivities on subsequent patch tests.

Patch tests repeated one or more times over periods of up to 3 years in 208 subjects with allergic contact dermatitis produced evidence of persistent specific sensitivities in almost 50% of cases. In about 25% the reactivity disappeared and over 25% developed new sensitivities. No new sensitivities appeared in 253 patients with non-allergic contact dermatitis or with other types of dermatitis in whom the same patch tests were carried out. The history and follow-up of patients with new sensitivities brought into focus the presumed aetiopathogenetic role of further contacts, especially occupational or after topical applications. Patch tests themselves, provided the correct technique is employed especially as regards the concentration of allergens, do not seem to be responsible for new sensitizations to any significant extent.

Contact and bacterial allergy in children with atopic dermatitis.

Cell-mediated immunity to bacterial antigens was studied by intradermal testing, and to contact antigens (cosmetics, environmental chemicals and topical medicaments) by patch testing in 270 children with atopic dermatitis. The incidence of delayed-type immune cutaneous reactions in these patients was lower than in the controls. Contact allergy is a rare finding in the first 4 years of life but its incidence increases subsequently. In subjects with atopic dermatitis the incidence of sensitization by contact with allergens contained in topical medicaments proved to be lower than in subjects with eczema of other types. The data collected points to a reduced cell-mediated immune reactivity in a proportion of subjects with atopic dermatitis.

Contact dermatitis in patients with leg ulcers.

The incidence of contact allergy to 63 substances, including active principles and excipients used in a variety of topical preparations, has been studied by patch testing in 306 patients being treated conservatively for stasis dermatitis with or without ulceration. Positive reactions to one or more test compounds were recorded in 177 patients. The main offenders among drugs were neomycin, sulphonamide and promethazine; among the excipients, they were parabens, lanolins and benzoyl peroxide. Frequent reactions were observed to certain substances of the 'para' group, benzocaine, diaminodiphenylmethane, aniline and paraphenylendiamine, with the first three acting mainly as indicators of cross-sensitization. The extension and spread of allergic dermatitis can also be produced by drugs administered systematically. Recognition of the allergens responsible for allergic contact dermatitis arising at the lower leg site is a necessary condition if chronicity and spread of the dermatitis are to be prevented. For the same reason it is important to restrict topical applications to non-allergic substances.

Contact dermatitis from pyrrolnitrin (an antimycotic agent).

Two cases are described of acute contact dermatitis from an antifungal cream, containing pyrrolnitrin, employed for the treatment of tinea cruris. Positive reactions to pyrrolnitrin and to a chemically related substance, 1-chlor-2,4dinitrobenzene, were put into evidence.

Immunity in tumours of the skin. Studies on cell-mediated immune reactions.

Cell-mediated immunity (CMI) was investigated in a series of patients with epithelial tumours of the skin and with Kaposi's sarcoma. In all 53 patients the peripheral blood lymphocyte count showed no substantial difference from the control values. The blasts count after phytohaemagglutinin stimulation in 30 patients ranged from 40-50% with no noticeable difference from the control values. Intradermal tests with the staphylococcal antigen, PPD-tuberculin and streptokinase-streptodornase carried out in 59 patients proved negative in a significant number of cases, mainly those of Kaposi's sarcoma. The non immune inflammatory response to chemical irritant (benzalkonium chloride) was more often negative (17 of 59 cases) than in controls (5 of 30 subjects). The DNCB sensitization test carried out in 28 patients was negative in 5 cases (2 cases of late basal-cell epithelioma, 2 cases of advanced squamous-cell carcinoma and 1 case of Kaposi's sarcoma). The findings do not thus indicate the presence of a gross CMI defect in the material studied, which would be borne out by all the tests employed. The sometimes discordant results of the various tests suggest the existence of partial CMI defects in certain tumours, but mainly in Kaposi's sarcoma. Such partial defects, encountered even in early neoplasms, coexist, in some cases, with failure of inflammatory response to chemical irritants.

[Contribution of clinical, biohumoral, morphological and instrumental parameters to the diagnosis of some chronic liver diseases].

A comparative study concerning the diagnostic importance of clinical, humoral, morphological and instrumental parameters had been carried out on 105 patients affected by chronic liver disease (51 cases with liver cirrhosis, 15 with metastatic tumors, 10 with liver steatosis and 29 with chronic hepatitis). The final diagnosis of the patients used for the comparison was formulated considering the greatest numbers of data. The results of this study can be drawn as follows: --clinical and laboratory data show satisfactory diagnostic value principally in patients suffering from liver cirrhosis (90 per ceny) and metastatic tumors (93 per cent); --peritoneoscopy appears to be generally the most useful procedure with the most significance value above all in cirrhotic patients; --morphological examination in biopsy specimen results of the greatest importance in patients affected by chronic hepatitis and hepatosteatosis while rather high has been the percentage of false negatives in secondary tumors of the liver (40 per cent) and cirrhosis (68 per cent); --scintiphotoscanning is generally less useful as a diagnostic tool than peritoneoscopy a part of the cases of metastatic tumors. In such a condition the diagnostic power of both the procedures results to be identical; --it may be concluded that an integrating diagnostic information generally results to be the most precise and maximally capable of reducing the number of false negatives. In individual cases, of course, the less important procedure shows to have the maximum of diagnostic capacity.
