[Incidence of cancers and socio-professional status: data from the Bas-Rhin tumor Registry from 1975 to 1979].

Data recorded in the Bas-Rhin cancer registry from 1975 to 1979 were used for studying the relationship between the incidence of some categories of cancer and social class. The relative risks associated with specific standardized incidence ratio (SIOR) of each cancer and social class were measured. Most of the results do not differ from other studies in developed countries. This study shows a positive gradient for colorectal cancers (SIOR = 58.4 in the lower class, 144.5 (non significant) in the upper class) and a negative gradient for cancers of the upper respiratory tract (SIOR = 51.5 in the upper class, 140.9 in the lower class). Urinary cancers (kidney, bladder) are not distributed as usual (SIOR = 201.5 in the upper class, 63.6 in the lower class). The validity of the measurement indices (SIR, SIOR, SPIR) is discussed.

[Repair surgery in the treatment of cancer of the cervical remnant].

The protocol recommended at the Centre Alexis Vautrin for the treatment of cancer of the cervical remnant is purely radiographic. The authors investigates repair surgery after the failure of radiotherapy in 8 cases, representing 9.3% of cases. He defines cancer of the cervical remnant by stressing the importance of a particularly difficult assessment of the extension and the methods of treatment. The series included 5 cases of cervicocolpectomies and 3 pelvic exenterations. The context was particularly unfavorable since the tumor had been irradiated and had developed a scarred region. Serious complications were not present in this series, but they should be recognized so that they can be prevented and some procedures adapted to local conditions, which include bladder, rectal and ureteral fistulae, occlusion and pelvic abscess. The rationale for surgery is based on the possibility of a cure and the prevention of locoregional progress which may be particularly painful and handicapping.

Long-term disability and prolonged sick leaves as outcome measurements in ankylosing spondylitis. Possible predictive factors.

Prognostic factors for the occurrence of longterm disability and prolonged sick leave were determined in 182 patients with ankylosing spondylitis. A significant relationship appeared between functional disability (determined by the Stanford Health Assessment Questionnaire) and sex, age at disease onset, and the number of peripheral joints involved. Using a life-table approach, the probability of prolonged sick leaves was associated with peripheral joint involvement and work that involved carrying heavy loads. Long-term disability was more frequent after work involving exposure to cold conditions (relative risk [RR] = 2.01) and prolonged standing postures (RR = 1.34), while sedentary work (RR = 0.35) and formal vocational rehabilitation programs (RR = 0.57) seemed to protect against long-term disability.

Pre-B lymphocytes with intracytoplasmic mu chains in the peripheral blood of rheumatoid arthritis patients.

A dysregulation of B-cell differentiation and activation has long been evidenced in rheumatoid arthritis (RA). Such analyses have, however, usually focused on the latest stages of B-cell development. Using a classical technique of immunofluorescence labeling on cytospins, we investigated the presence of peripheral pre-B lymphocytes in 92 RA patients and 23 controls. Cells with intracytoplasmic mu chains were evidenced in 58.7% of the RA patients studied, ranging between 0 and 30%, while small numbers of c-mu cells, never exceeding 6% of peripheral blood lymphocytes, were observed in 9 controls. Relationships between this feature and clinical or laboratory data were investigated, showing a negative correlation between the percentage of c-mu + lymphocytes and Ritchie's index (P = 0.05), the number of tender or swollen joints (P = 0.05), erythrocyte sedimentation rate (P = 0.005), seropositivity (P = 0.05), and disease duration (P = 0.01).

Vaccination against pseudorabies with glycoprotein gI+ or glycoprotein gI- vaccine.

Subunit pseudorabies vaccines that contained only purified glycoproteins of either of 2 strains of pseudorabies virus (PRV) were prepared and subsequently tested for safety and efficacy. The strains of virus used for vaccine production differed in at least 2 properties. One strain (Kojnok) was virulent for pigs and was believed to code for the entire complement of viral glycoproteins. The other (Kaplan) was a deletion mutant that was unable to code for structural viral glycoproteins gI and gp63. Purified glycoproteins were dispersed in an oil-in-water emulsion and were administered IM to pigs. Both vaccines were found to be safe and effective immunogens. Neither caused any local or general reactions, as verified by examination of the injection site (local safety) and by vaccination of pregnant sows in PRV-infected and noninfected herds. Sows vaccinated with the gI+ or gI- vaccine protected their pigs at levels of 93 and 92%, respectively, against a severe challenge exposure that killed 98% of pigs born from nonvaccinated sows. Vaccinated pigs were tested for active immunity by intranasal challenge exposure with the NIA 3 strain. Protection was quantitated by measuring the relative daily weight difference, expressed in percent per day, between vaccinated and control pigs during the first week after challenge exposure (delta G7); the estimated differences were 2.25 and 2.13% for gI+ and gI- vaccines, respectively. The absence of gI and gp63 did not affect the efficacy of this type of subunit glycoprotein vaccines.(ABSTRACT TRUNCATED AT 250 WORDS)

[Conservative treatment of breast cancer. Carcinologic and cosmetic results of combination quadrantectomy and irradiation at the Centre Alexis-Vautrin].

We studied the association of quadrantectomy and irradiation in 238 patients with a breast carcinoma treated from 1974 to 1984 in the Alexis-Vautrin Center. Five and ten year survival rate are respectively 89 and 77%. Local relapse rate at five and ten year are 2, and 9%. Local relapse rate is correlated with a young age, a high tumor grade and an internal site of tumor. We studied the cosmetic and functional results and compared objective results according to subjective self-evaluation by the patients.

[Cervical spondylodiscitis. A rare complication of intubation].

The authors present two cases of cervical spondylodiscitis occurring following intubation. The most probably mechanism was direct contamination of the inter-vertebral disk through a lesion of the posterior pharyngeal wall, but other possible mechanisms for the spread of infection are discussed. This very rare complication should be borne in mind and routinely sought when there is persistence of cervical pain following intubation.

Use of the caprinised strain of rinderpest virus for potency testing an attenuated cell culture rinderpest vaccine.

The caprinised strain of rinderpest virus was inoculated into goats to produce a challenge stock. These goats were kept with control animals (goats, sheep, calves). In this trial the caprinised strain was shown to have a mild pathogenicity for goats and it spread to one of two contact goats but not from goats to other species. The caprinised strain was then tested on cattle where a febrile reaction was observed. The caprinised strain also did not spread between cattle. The cattle vaccinated with a freeze-dried vaccine produced from the attenuated Kabete RBKO strain on bovine kidney cells were then challenged with the caprinised strain with good results.

[Rare localizations of digestive adenocarcinoma in children. Apropos of 2 cases].

The authors report two cases of adenocarcinoma of the digestive system in children both of which were located in unusual sites. A child of 9 years old had an adenocarcinoma of the small bowel and a boy of 16 years who was known to have a long-standing hiatus hernia developed an adenocarcinoma of the oesophagus. In children these tumors have some characteristic features. They develop on a preexisting lesion. Evolution is rapid. From the pathological point of view, anaplasic and colloide patterns are frequent.
