HLA-A, B and C antigens in South Indian families with leprosy.

Seventy-two families, selected for having at least two children affected with leprosy, were HLA typed for 57 A, B and C locus antigens recognized by the WHO Nomenclature Committee. In addition, 20 possible new "splits" were investigated. The distribution of A, B and C locus antigens in affected and unaffected family members was similar, irrespective of the type of leprosy in the family. Gene frequencies (derived by direct gene counting from 253 haplotypes), haplotype frequencies and delta values were calculated. There is evidence for heterogeneity of B5, B15, B17, Bw16 and Bw35 and for the existence of at least one A locus and one B locus antigen not previously detected. The value of the HLA system for detecting expaternal children in a highly inbred population and the effect of inbreeding on the HLA system is discussed.

[Simplified method of methacrylate embedding for undecalcified bone sections (author's transl)].

A simplified technique of methacrylate embedded bone biopsies for the diagnosis of bone and haemoblastic disorders is described. Hard tissues are fixated for routine purposes in the Schaffner mixture. Fixations are carried out for fluorescent marked bone using Carnoy mixture and for histochemical methods using 4% purified formaldehyde buffered in 0.1 M solidum cacodylate at pH 7.2. Then, the bone cylinders are dehydrated, defetted and stored at 4 degrees C in methacrylate solution (1 vol. of methyl-methacrylate, 3 vol. of butylmethacrylate, 2.5% benzodylderoxide for 24 hours in vacuum). After the immersion the bones are transferred to plastic capsules filled with the methacrylate mixture described above. The polymerisation takes place in a water bath at 25--26 degrees C. Sections of 4--6 micron thickness can be cut on a special microtome, and after removal of the plastic material by means of toluene they are stained. Results of the method are demonstrated by micrographs.

Retinal functions in snowflake degeneration.

Retinal functions were studied in 7 patients with snowflake degeneration and correlated with fundus appearance. Despite the presence of vitreous turbidity, scotomatic glare sensitivity was slightly elevated in only one case. Retinal functions were affected more extensively in the late stages of the disease; however, abnormal retinal functions were also detected in some patients who showed only slight fundus changes on ophthalmoscopic examination. Kinetic perimetry showed peripheral defects which were more pronounced in the upper field. Flicker perimetry revealed abnormalities that kinetic perimetry could not detect. Dark-adaptation tests showed elevated rod thresholds except during the early stage of the disease. In all patients, the scotopic b-wave of the electroretinogram elicited by dim white light was low in amplitude. The photopic b-wave and the photopic flicker responses showed decreased amplitudes in some patients. The scotopic b-wave, normally recorded as a second positive peak when deep red light is used, almost disappeared, leaving only a small, single positive peak, photopic in nature, in the late stages. However, no patient had a nonrecordable electroretinogram. The electro-oculographic light peak/dark trough ratio was abnormal in only a few patients. These results indicate that in snowflake degeneration, functional abnormalities of the retina are milder than those in other progressive primary retinal degenerations typically represented by retinitis pigmentosa.

A new look at HLA genetics with particular reference to type-1 diabetes.

HLA genotypes were ascertained in 150 families with a diabetic child from the same geographical area. There was preferential zygotic assortment of the paternal HLA A1-B8 haplotype (63--65% compared with the expected 50%) in 69 diabetic families and 33 control families (pooled from elsewhere) who were informative for this haplotype. In diabetic families, the offspring also had an increased incidence of the maternal HLA A2-B15-Cw3 haplotype. Irrespective of which parent contributed the HLA A1-B8 haplotype, there was a significantly increased incidence of male children (63%) who inherited this particular haplotype. This probably explains the known excess of male diabetic children.

HLA-linked genes and leprosy: a family study in Karigiri, South India.

The evidence for a genetic determination of susceptibility to leprosy is reviewed. To test the hypothesis that an HLA (histocompatibility leukocyte antigen)-linked gene is associated with such susceptibility, the association between the distribution of leprosy within a family and the segregation of HLA haplotypes was investigated among 72 families who lived in Karigiri, Tamil Nadu State, South India. A statistically significant association was found for families in which siblings had tuberculoid leprosy and in which neither parent had leprosy. The findings from the data of this study agree with those of two previous studies carried out among smaller populations is Surinam and Wardha, Maharashtra State, India. Such an agreement suggests that a genetic determinant which is linked to the major HLA locus on chromosome 6 and which is probably recessive affects susceptibility to tuberculoid leprosy in humans.

Field independence, lateralization and defensive style.

This study investigated the relationship between field independence and defense clustering as measured by the Defense Mechanisms Inventory and lateral eye movements. Subjects had previously been classified either as hysterical or obsessive style by the Rorschach and WAIS Comprehension subtest. Previous findings indicate that these subjects have a preferred direction of lateral eye movement in a questioning format (hysterical style = left; obsessive style = right). This study found no relationship between field independence and defense clustering and lateral eye movements. To the extent that eye gaze indexes hemispheric activation, we conclude that neither field independence nor defense clustering was related to hemispheric lateralization.

The artificial beta cell (Biostator) in the adjustment of instable diabetics--results after 20 months.

In 55 poorly controlled insulin-dependent diabetics, we tried to discover criteria for an improvement of metabolism by means of the "artificial beta-cell" (Biostator). To this end, during the first 24 h of hospitalization, blood glucose was monitored continuously under conventional insulin therapy (monitoring period). Insulin requirement was determined during the next 24 h by the artificial beta-cell (feedback period). Corrections of diabetes regimen were made with reference to the insulin consumption during the feedback period and to the extent of the postprandial blood sugar increases and decreases during the monitoring period. The resulting new diabetes regimen led to a significant improvement of the daily blood sugar profiles.

[Characterization of radioactivity distribution in the organism during constant intravenous infusion of tracer amino acids and calculation of the rate of tissue protein synthesis in rats].

Male wistar rats (100 g live mass) were given infusions into the tail vein of 14C-leucine and 14C-lysine simultaneously for 0.5; 1.0; 2.0; 3.0; 4.5; 6.0 and 7.0 hours. At the end of the infusion the specific radioactivity of the free leucine and lysine in the blood plasma, liver, M. gastrocnemius, small intestines and colon were ascertained as well as after the 6-and-7-hour infusion that of the protein-bound leucine and lysine. In all tested tissues the specific radioactivity of the free amino acids attained a plateau during the 6-and-7-hour infusion. The rate constants for the increase were calculated for each organ tested. The two amino acids used are suitable for the calculation of the fractional rate of protein synthesis in tissues. The values of the fractional rate of protein synthesis calculated on the basis of the 6-and-7-hour infusion were: 54 +/- 7.7%/day for the liver, 9.4 +/- 1.2%/day for muscles, 89 +/- 12.2%/day for the small intestines and 42 +/- 5.9%/day for the colon. The simultaneous application of two tracer amino acids is recommendable for the estimation of the precursor pool for protein synthesis and the more accurate calculation of the rate of protein synthesis.

Ambulatory ECG monitoring and bicycle ergometry: correlation with findings on coronary arteriography.

Ambulatory ECG monitoring during everyday activities (AEM) and a multistage submaximal bicycle ergometric test (BET) was performed on a series of 48 patients suffering from precordial symptoms or pain. The results of these tests were compared with the results of coronary arteriography. In 20 subjects with a negative AEM and BET the coronary arteriography was normal in 18, moderately pathological (30-60% narrowing) in 1, and severely pathological (greater than 60% narrowing) in another. In 23 subjects in whom both AEM and BET disclosed ST-T alterations, 20 had severely pathological and 1 moderately pathological coronary arteriograms; normal coronary arteries were found in only 2 subjects. Only 5 of the total of 48 patients of the series showed a discrepancy between the results of AEM and BET. The findings on coronary arteriography confirmed that patients with a normal AEM and BET have a very high probability of having normal coronary arteries, while if both tests are positive the patient will most likely have advanced coronary pathology. When the two tests were concordant, the number of false positive and false negative results were slightly less than if ergometry was performed alone. Although AEM does not provide the quantitative aspect of effort testing, it does have the advantage of avoiding the risk of unusual physical effort for the patient.
