[Occupational medicine otorhinolaryngologic analysis of employees of a district socialist agricultural facility within the scope of expanded public mass roentgen screening].

For the purpose of registration of subjective complaints and pathological changes of the respiratory tract 1,023 employees of socialist agriculture were additionally examined in connection with an expanded x-ray-screening from the points of view of otorhinolaryngology and industrial medicine. The employees in industrial livestock breeding, especially in industrial poultry breeding, turned out to be the group with the most frequent complaints and pathological findings in the respiratory tract. The findings suggest, that chronic, non-specific changes develop on the basis of the summation of several noxious factors in industrial live-stock breeding. In conclusion of the examination recommendations for the optimization of the working conditions for employees in industrial livestock breeding are given from the points of view of industrial hygiene and occupational medicine.

[Psychosocial aspects and rehabilitation problems of patients with tumors of the ORL region with special reference to laryngectomees].

The operational therapy of malign tumours in the ORL is often connected with visible larger tissue organ lesions respectively. By this reason intensive efforts regarding comprehensive rehabilitation are necessary with the aim of overcoming the multiple impairments of familiar, social and occupational life. It is demonstrated with making inquiries concerning tight cooperation between patients, physicians, relatives, speech therapists and rehabilitation centres by example of laryngectomees as a premise for an successful reincorporation into a normal life.

[Aspects of drug supplies for veterinarians].

Problems of drug supply by veterinary doctors are discussed and explained. It has to be pointed out that the cancellation of drug substances needed for drugs and veterinary drugs, respectively, can lead to a sensible gap in the veterinary drug assortment, which is often difficult to compensate. Proposals to solve this problem are offered. In addition a growing demand on the treatment of small animals and pets, which are kept in increasing numbers, has to be considered. An adequate training and further education of pharmaceutical staff would be recommendable to meet the demands of an effective drug supply as well as a sufficient ability to give proper advice to proprietors of animals at the pharmacist's. This is already realised in Czechoslovakia in an exemplary way.

[Bacterial colonization of the gastric mucosa].

In 380 histologically prepared biopsy specimens taken from the mucous membrane of the stomach, the employment of Giemsa staining and Warthin-Starry silver staining has revealed an often surprisingly abundant (i. e., to the extent of 66.1%) bacterial colonization of the surface of the gastric mucous membrane and of the gastric foveolae. The bacteria encountered here involve gram-negative and morphologically uniform flora of thin, S-shaped or slightly curved bacteria designated as campylobacter-like organisms (CLO). Evidence for Campylobacter pyloridis was also obtained in 55.1% of cultures from 49 native, microbiologically prepared gastric biopsy specimens. Bacterial colonization of the gastric mucous membrane by CLO is definitely correlated with inflammatory alterations in gastric membranes. Active forms of gastritis, however, are not more abundantly colonized by bacteria than are dormant forms. The positive correlation between inflammatory gastric alterations of various degrees and bacterial colonization of the gastric mucous membrane, furthermore, is not proof of an etiological relationship. These bacteria, obviously adapted to the mucus in the gastric foveolae and on the surface of stomach lining, are not evidence of an aggressive behavior or of penetration tendencies. Facultatively pathogenetic significance cannot, however, be excluded for bacteria occurring in such abundance in intimate epithelial contact. Pathogenetic influence of abundant colonization by CLO could well develop if additional noxae become effective.

Judgement of measured values of intestinal iron absorption.

The value of a medicinal iron preparation is influenced by several factors e.g. therapeutical efficacy, frequency of side effects, and prescriptions for intake. Since the accurate quantitation of therapeutical efficacy is difficult to establish, the value of fractional intestinal absorption of iron, which is a prerequisite for the effect on haemoglobin regeneration, represents a useful criterion for that particular factor. When intestinal absorption is investigated the selection of subjects is of importance. Studies should be performed on subjects with iron deficiency since the adaptation of intestinal absorption to the body's need is the basis of oral iron supplementation. Different methods have been used to assess intestinal absorption, some of which give almost identical results or show a close correlation. However, mixing data from different investigations in order to convert semi-quantitative values obtained from plasma iron tolerance curves into amounts of absorbed iron by means of regression equations estimated from studies on different subjects and under different conditions may lead to considerable uncertainties of the calculated results. Even more difficult to evaluate is the relation between fractional absorption and costs of therapy, since there are additional factors influencing the duration of supplementation and therefore the total amount of iron necessary.

Bioavailability and therapeutic efficacy of bivalent and trivalent iron preparations.

The bioavailability of iron on Fe(III)-hydroxide-polymaltose complex was compared intraindividually with that of Fe (II)-ascorbate (iron absorption) and a Fe (II)-sulphate quick release preparation (haemoglobin regeneration test). The study was carried out in a population of 16 healthy male volunteers, phlebotomized in weekly intervals until development of an iron deficiency anaemia in order to establish a test population with only small variations of their individual body iron status. Intestinal iron absorption in fasting state as measured by 59Fe whole body retention and simultaneous estimation from plasma iron tolerance curves was low for the 59Fe (III)-complex (1.2 +/- 0.1% (means +/- SD] as compared to the 59Fe (II)-ascorbate (43.7 +/- 7.1% (means +/- SD]. Iron administration together with a test meal did not affect the absorption from the 59Fe (II)-ascorbate, whereas the 59Fe (III)-complex showed a significant increase in absorption (8.8 +/- 4.7% (means +/- SD]. Haemoglobin (Hb) regeneration after 100 mg of iron as Fe (III)-complex and Fe (II)-sulphate administered during 28 days with meals amounted to a mean of 0.68 +/- 0.2 g/l and 1.1 +/- 0.3 g/l (means +/- SD) of total daily Hb-increase and a net-Hb-increase of 0.31 +/- 0.37 g/l and 0.79 +/- 0.36 g/l (means +/- SD), respectively. There was also a small but significant rise in serum ferritin during the oral treatment in both treatment groups. The results of Hb-regeneration after treatment with the Fe (III)-complex were in the range which could be expected from the absorption measurements.(ABSTRACT TRUNCATED AT 250 WORDS)

RNA metabolism in nuclei: selective transport of kappa exons from myeloma nuclei and adenoviral transcripts from infected HeLa nuclei.

Two independent systems and several analytical procedures have been used to establish that isolated mammalian nuclei selectively transport mature RNA polymerase I and II products. Murine myeloma nuclei retain physiologic restriction in our transport assay as assessed by the transport of the immunoglobulin kappa light chain mRNA and 18S and 28S rRNAs. Nearly 50% of the total kappa exons are transported as structurally intact mature mRNA molecules while less than 8% of either pulse-labeled or steady state kappa intron sequences are detected in the transported fraction. Ribosomal external transcribed spacer sequences also are absent in transported RNA. Release of cytoplasmic RNA from the outer nuclear membrane during the transport assay accounts for less than 10% of transported RNA. Nuclei isolated from adenovirus-infected HeLa cells at 20 hours post infection retain cellular actin mRNA and transport viral poly A+RNA. Ribosomal RNA is transported from infected nuclei although at a reduced rate compared to transport from mock-infected nuclei. Inhibition of transport of host mRNA is paralleled by the absence of pulse-labeled actin mRNA in the cytoplasm of infected cells. The implications of our transport data in relationship to intranuclear RNA trafficking are discussed.

[Smoke cloud pneumonia, a new atypical pneumonia in influenza diseases].

Since 1980 we have to reckon with a novel atypical pneumonia in influenzae. The clinical course of these fire-damp pneumonias is very discrete. In X-ray the changes of the lungs are uniform in density and considerably multiform in extent, in which cases the topographic-anatomic borderlines are not taken into consideration. They disappear within 6 to 8 days. There are no participations of pleura and hilus.

[Development of body weight in males of the Wurzen district 1961-1983].

In 1961, 1971 and 1983 additional medical parameters, among them body length and body weight, were established during the mass radiographies in the district of Wurzen. From the data of 7,670 males from the year 1983 for two 10-year age groups regression equations between body weight and body length were determined. The calculation of relative weights was performed on the basis of the optimum weights after Ott. The longitudinal comparison from 1961 to 1971 resulted in an increase of the number of persons with overweight by 17.7% during 10 years. The results of the examinations of the year 1983 showed that the trend to the increase of body weight continues in the male inhabitants of the district of Wurzen.

[Cryptococcus neoformans meningoencephalitis in AIDS].

The risk for AIDS patients from Cryptococcus neoformans is outlined on the basis of a case report on a 28-year old male patient whose disease was complicated by cryptococcosis. Beside the description of the diagnosis of cryptococcosis (demonstration of the agent and its antigen), epidemiological associations (habitat of Cr. neoformans in fecal matter of birds) and the clinically, mostly not recognized, route of infection via the lungs is stressed. The effective therapy with the combination of amphotericin B and 5-flucytosine, which also in this case has been successful is described from the clinical and microbiological angles. Finally, proposals for the prevention of Cr. neoformans infections in AIDS patients and for a special mycological surveillance directed at this fungus are made.

[Significance of the "21-day cough" and the patient with "variable bronchitis"].

The word formations "21-day-cough" and "contrast-articular bronchitics" have become a fixed term in the practice of pneumology. They signal possible pulmonary diseases, especially tuberculosis or bronchial carcinoma. In many countries they have become the method of choice for recognition of several pulmonary diseases, particulary pulmonary tuberculosis.
