Optimization of flow-cytometric discrimination between reticulocytes and erythrocytes.

An automatized technique to count reticulocytes by means of flow cytometry is described. Blood samples were stained by the fluorescent dye acridine orange without the use of fixative. Scatter and red fluorescence of the blood cells were measured in a flow cytometer. A discrimination between reticulocytes and erythrocytes was only achieved by using logarithmic amplification. The discrimination was better in peak mode than in area mode. The optimum dye concentration was 0.5 mg/liter acridine orange. At lower dye concentrations, not all reticulocytes were measured, whereas at higher dye concentrations the degree of discrimination between reticulocytes and erythrocytes decreased. There was a suitable discrimination between reticulocytes and erythrocytes. The reticulocyte numbers were scored by flow cytometry as well as by microscope for blood samples with 0.1-14% reticulocytes. The correlation between both methods was close.

[Urine drainage following vaginal gynecologic operations].

In a retrospective study covering 161 patients, postoperative discharge of urine by means of a suprapubic catheter (SPC) and a transurethral indwelling catheter (TIC) following vaginal hysterectomy with reconstructive surgery were compared. The incidence of urinary tract infections differed clearly in the two groups, though not significantly (SPC 20.9%, TIC 37.2%). The SPC has clear advantages, causing fewer micturition difficulties and being more readily accepted. Isolated complications such as macrohematuria, bladder tamponade, and blocking of the catheter represent disadvantages of the SPC as compared to the TIC.

The complement of native alpha-keratin polypeptides of hair-forming cells: a subset of eight polypeptides that differ from epithelial cytokeratins.

Living hair-forming cells (trichocytes) were obtained from basal portions of human, bovine and ovine hair-follicles, free from contaminations of root-sheath epithelia. Their intermediate filament (IF) cytoskeleton was studied by gel electrophoresis of the native, i.e. non-S-carboxymethylated polypeptides, by peptide-map analysis of the individual components, by reconstitution experiments and by immunological methods. The IF protein complement of trichocytes from all three species is characterized by a very similar set of eight highly conserved alpha-keratin polypeptides, comprising four members of the basic (type II; Mr 56,500-60,000) and four members of the acidic (type I; Mr 41,000-44,000) cytokeratin subfamily. None of these eight trichocyte alpha-keratin polypeptides, which form heterotypic complexes and IF in vivo and in vitro, is identical to any of the epithelial cytokeratins of the same species. All the trichocyte-specific cytokeratins are native polypeptides encoded by different mRNAs, as demonstrated by in vitro translation of hair follicle mRNA. The same polypeptides are also found in mature hairs, although with different patterns of modification. Our study provides the first analysis of the native unmodified alpha-keratin polypeptides of trichocytes and hairs and therefore allows a direct comparison of these with the epithelial cytokeratins and other IF proteins from the same species. These findings indicate that, during fetal hair-follicle formation, the differentiation of trichocytes from epithelial cells involves a complete cessation of the synthesis of epithelial cytokeratins and a marked induction of the synthesis of a complex set of trichocyte-specific cytokeratins.

[Diagnosis and course of tuberculosis especially from the viewpoint of clinically unknown deaths].

Every time the tuberculosis is present and it is to be included in the differentialdiagnosis if the occasion arrises. In the anamnesis it is necessary to pay attention to specific diseases and the risk groups like patients with "21-day-cough", silicotics, "Contrast-articularis bronchitics", diabetics, so-called "persons with fibrotic lesions" and patients with frequent influenzal infections. The symptoms unclear gastric distress, want of appetite, indifferent loss in weight, uneasiness, slight vertigo and fast tiredness already give further references. Breath-pain, haemoptysis and subfebrile temperatures are already severe symptoms. A thorax X ray-photograph, tuberculin test, heamogram, sedimentation test and intensive search for mycobacteria, belong to the diagnosis. In extrapulmonary foci the search for mycobacteria is to try by swab, puncture, control of urine and menstrual blood. It is possible, that a histologic corroboration will be necessary. Unclear fever, headache and vomiting with or without dyspnoea, cyanosis and diaphragmatic lowness indicate a ocular reflection, liver biopsy and, in special case, a lumbar puncture without delay. Sooner or later the course of an unrecognized phthisis can result in death. It is necessary to fill up the gap between welltime diagnosis and death by unknown tuberculosis. That means: Thorough knowledge of matter, insight into the disease-course and inducement of all necessary diagnostic possibilities.

[Therapy of tuberculosis and other mycobacterial infections].

At present every case of tuberculosis recognized in time is curable. The antituberculotic remedies have a causal effect, since they achieve a destruction of the germs. Decisive for the success are the observing of the treatment instructions and the cooperation with the patient in a period of 6-9 months. The action mode of the basic pharmaceutic agents, the dynamics of bacterial populations and the newest therapy recommendations are listed in tables. In the case of leprosy we look forward to similar results. Here the chemotherapy must, however, be performed for 2 years and longer. In the Buruli-ulcer (mycobacterial skin ulcer) after additional surgical interventions are still to be recommended. Mycobacterioses caused by other human-pathogenic atypical mycobacteria are also cured by a specific treatment of 1-2 years. In particular cases resection are still necessary.

[Prognosis of new cases of lung tuberculosis in 1962 and 1972 10 years later].

At present a good prognosis of pulmonary tuberculosis is possible in the case of well-timed diagnosis and optimum treatment. Controlled use of all necessary specific drugs is a prerequisite to a success. The analogous courses of bacillary and non-bacillary pulmonary tuberculosis confirm our experiences that culture-negative respiratory tuberculosis is existing in an important number. Therefore, this factor will have to be put into our epidemiologic and clinical considerations.

[Epidemiology of tuberculosis].

Up to now the tuberculosis passed in great waves enclosing centuries. After aimed control measures the curves of mortality varies in a typical manner. The better retrogression in the developed countries is to betraced back to the successful use of tuberculostatics. Under the conditions of an endemia the morbidity again varies in a characteristic manner. The situation in the world must also be included in our analysis and this situation is in no case such a one that we can lull ourselves in security.

Methylation pattern of fish lymphocystis disease virus DNA.

The content and distribution of 5-methylcytosine in DNA from fish lymphocystis disease virus was analyzed by high-pressure liquid chromatography, nearest-neighbor analysis, and with restriction endonucleases. We found that 22% of all C residues were methylated, including methylation of the following dinucleotide sequences: CpG to 75%, CpC to ca. 1%, and CpA to 2 to 5%. Comparison of relative digestion of viral DNA with MspI and HpaII indicated that CCGG sequences were almost completely methylated at the inner C. The degree of methylation of GCGC was much lower. The methylation pattern of fish lymphocystis disease virus DNA differed from that of the host cell DNA.

Desferrioxamine treatment for aluminium and iron overload in uraemic patients by haemodialysis or haemofiltration.

In an attempt to quantify iron and aluminium removal in patients following desferrioxamine therapy, a comparative study of haemodialysis and haemofiltration was performed in five patients. Haemofiltration was found to be the more efficient treatment especially if iron and aluminium clearances were related to urea clearances. The distribution volumes of chelated iron and aluminium at the low clearance rates obtained, behave as a single compartment which amounts to about 40 per cent of body weight.

[The pharmacokinetics of rifampicin in the intermittent treatment of patients with pulmonary tuberculosis. 1. Excretion of rifampicin in the urine].

With a rifampicin (RMP) range of 600 to 1800 mg, 12.5% of the administered dose are on an average secreted in urine. In spite of this minimal portion the course of pharmacokinetic processes can distinctly be learned from its alterations. Analyses in 84 patients revealed that the age will influence the induction and the sex-specific differences of the pharmacokinetics. Compared to male subjects, an increased RMP secretion in urine can be recorded in young female subjects, whereas in older male ones an induction-reduced RMP elimination in urine becomes more obviously. Considering these multiform processes, the determination of the bioavailability of RMP preparations by means of the RMP secretion in urine is taken to be manipulatable and uncertainly. More evident results can be gained by the determination of the RMP concentration in serum or by the calculation of the AUC.

Interrelations of radiocalcium absorption tests and their clinical relevance.

Intestinal absorption of radiocalcium from an oral test dose was measured in 84 patients with various disorders of bone and calcium metabolism. In each subject, five different methods were used to assess intestinal absorption and the estimates obtained were compared inter- and intra-individually. The whole-body counting technique was chosen as reference method and as a basis for comparison of the other four estimates. Plasma levels of radiocalcium after oral administration seem to provide a simple and rapid test which may serve its purpose in various clinical situations. Fractional intestinal calcium absorption can be assessed equally well by the whole-body counting technique, the double-isotope method or the double-isotope deconvolution technique, since these methods yield comparable results. The deconvolution technique additionally provides a quantitative description of the absorptive process of calcium with respect to time.

[New developments in mass roentgen studies].

Since 1984 in the GDR the persons older than 40 years are summoned to participate in the mass X-ray examinations in a two-year sequence. The risk groups of tuberculosis are called up at shorter intervals, this also concerns younger people. To this belong patients with silicosis, so-called "healthy carriers of lesions", persons with contacts to patients suffering from tuberculosis, diabetics and the staff of tuberculosis hospitals and pathological institutes. Furthermore, persons working in national education, kindergartens and cattle-breeding are particularly controlled. Patients with chronic diseases of the respiratory tract need our increased attention.

Processing of the external transcribed spacer of murine rRNA and site of action of actinomycin D.

The primary rRNA transcript contains a large external transcribed spacer (ETS) approximately 4,000 nucleotides in length. We have used subcloned DNA probes derived from the 5' end of the ETS in conjunction with Northern blot analysis of murine nuclear RNA to examine processing of this region. In agreement with the results of previous investigators, we find that the large rRNA precursor lacks part of the ETS region. These ETS sequences are also missing from subsequent rRNA processing intermediates. Experiments using actinomycin D confirm that the excision of portion of the ETS is an early event in rRNA processing. In addition, in the presence of actinomycin D small RNA species accumulate which hybridize to a probe specific for the 5' end of the ETS. The length of these abbreviated transcripts defines a region of rDNA which is probably a target for this drug.

[A sino-pulmonary syndrome in pneumonologic practice--alpha 1-antitrypsin deficiency in emphysema patients].

An illness in the arial of the paranasal sinuses was found in 43% of 120 persons with obstructive emphysema. In 40 cases the alpha 1-antitrypsin-concentration in serum was determined. The average of 1.73 +/- 0.19 g/1 was clearly lower than the 2.25 g/1 of normal-series. The clinical-therapeutic consequences and also the possible importance in work-medical examinations or expert-opinions of adequate occupational diseases will be pointed out.
