Prognostic indicators of ultimate long-term survival following advanced life support.

In an attempt to establish prognostic indicators for ultimate long-term survival following advanced life support (ALS), a two-year (1978-79) retrospective study was made of 92 survivors. The survivors were divided into two groups: the first survived admission to the intensive care unit, but subsequently died in the hospital; the second survived to ultimate discharge from the hospital. The groups were compared for sex, age, basic and advanced provider response time, scene an transport time, witnessed versus unwitnessed arrest, traumatic versus medical arrest, type of airway, presence of aspiration, initial cardiac rhythms, post-arrest vital signs, number of defibrillations, post-arrest ECG changes, essential and useful drugs used, and duration of basic and advanced life support. Of all prognostic indicators studied, duration of ALS correlated most closely with ultimate survival. This article advances the definition of ultimate survival of ALS to mean discharge of the patient from the hospital, and advocates this as the most objective evaluation of successful ALS. The article furthermore offers duration of ALS as an effective prognostic indicator of a patient's chance for ultimate survival.

Analysis of urine sediments after kidney transplantation.

We applied multiparameter flow cytometry for the first time to the classification of cells and particles of urine sediments after kidney transplantation. After fluorescent staining with acridine orange, cells and particles were checked for their size and their peak intensity of green and red fluorescences, known to be related to the amounts of intracellular DNA and intracellular RNA, respectively. Urine sediments were tested daily during a period of up to three weeks. Occurrence and disappearance of certain classes of cells and particles were found to be similar among different patients during normal healing. The results from patients without rejection of the graft are suggested as baseline data, differences from which should be detectable with high sensitivity due to the large number of cells being examined.

Femoral osteomyelitis secondary to diverticulitis.

Colonic diverticulitis may result in single or multiple fistulae communicating either internally or externally. We report an unusual case of fistulous communication between the colon and the left hip joint secondary to diverticulitis, with subsequent septic arthritis and osteomyelitis of the proximal femur.

Effect of administering a high dosage glucocorticoid bolus on the survival and subcapsular hydrostatic pressure of rat kidney allografts..

In order to measure the subcapsular pressure in normal and allotransplanted rat kidneys, a microcatheter was inserted into the subcapsular space and the hydrostatic pressure (SHP) continuously recorded with a transducer. The SHP was found to increase significantly up to the 5th day after transplantation. A single bolus injection of prednisolone on the day of transplantation or on the 4th post-operative day resulted in considerably lower SHPs being recorded. In contrast, graft survival was prolonged only when prednisolone was given on day 0 but not on day 4. Thus, it was concluded that the mode of action of prednisolone differs according to its time of administration.

Cyclohexyl analogues of some antiinflammatory drugs.

Some new compounds containing cyclohexyl substituents, with a structure similar or analogous to that of traditional antiinflammatory drugs, were synthesized and their antiinflammatory activity was investigated. The strongest activity was shown by alpha-(p-cyclohexylphenyl)-gamma-valerolactone W 8.

[Pharmacological treatment of chronic arterial occlusive disease (author's transl)].

Two thirds of the patients with peripheral arterial occlusive disease have to be treated conservatively, for only up to 30% can be revascularized by operative methods. Using the pharmacological differential treatment the grade of compensation and localization of the obliterative process has to be considered. Ignoring the usual basic therapy (elimination of heart failure and pathological bradycardia, systemic walking-exercise, anticoagulation etc.) intrafemoral long-term application of energetic phosphate (i.e. nucleotid-nucleosid-mixtures) leads to a positive result in nearly two thirds (n = 97 legs) with a degree of II to IV of Fontaine. Whereas the snakes' encyme Ancrod with the effect of defibrination was successful in almost 70% of the patients with arterial insufficiency (n = 45) including the degree II B (painless walking-distance under 100 meters). Energetic phosphates, applied to the arteria femoralis, are most successful in degree II with claudication intermittens. Ancrod should be used respectively for patients with pain during rest. These results are discussed with respect to compensation and localization of arterial occlusive disease, acute and chronic measurements of the hemodynamics by use of Doppler ultrasound and strain gauge plethysmography and with respect to variation of the concentration of the metabolic parameters lactate and pyruvate--the latter when defibrination was performed.

[Effect of high-dosage prednisolone administration on interstitial transplantation edema following allogeneic kidney transplantation in rats].

To measure subcapsular pressure of rat kidneys and kidney allografts, microcatheters were implanted in the subcapsular space and the pressure was continuously recorded with a transducer. Subcapsular pressure was found to increase significantly during allograft rejection. After single bolus injection of prednisolone 300 mg/kg on the day of transplantation or on the fourth postoperative day, considerably lower subcapsular pressures were recorded. This steroid effect together with other immunosuppressive measures could have an important influence on graft function and prognosis.

[The effect of high-dosage prednisolone administration on antibody formation and graft survival time following allogenic rat kidney transplantation].

The dose and time dependence of the immunosuppressive effect of prednisolone on renal allograft survival and antibody formation was studied in inbred rats. A single injection of 50 - 300 mg prednisolone/kg body wt. on the day of transplantation resulted in a dose-dependent prolongation of mean graft survival to 12.8 days as compared to 7.8 days for untreated controls. The daily postoperative administration of 10 and 30 mg prednisolone/kg prolonged the mean survival time to a lesser extent. A single bolus injection of 300 mg prednisolone/kg at different times after transplantatin resulted in a significant prolongation of graft survival and suppression of cytotoxic antibody only when given no later than 2 days postoperatively or following antigen injection.

Studies on derivatives of tetrahydro [1,3]-thiazine.

In the course of studies on compounds with expected antiinflammatory and immunosuppressive activity a series of new derivatives of tetrahydro [1,3]-thiazine-4-one-6-carboxylic acid, tetrahydro [1,3]-thiazine-4,6-dione and 2-thiobarbituric acid have been synthetized. Several new and unexpected details of the chemical behavior of these compounds were revealed. The chemical structure of the new compounds was confirmed by degradation and identification of the decomposition products and spectral analysis. Pharmacologic analysis showed that some of the compounds exerted an antiinflammatory and immunosuppressive activity.

[Advantages of Ocusert treatment (author's transl)].

The medical service of Deutsche Bundesbahn supervises through its eye-doctors all those employees who, affected to glaucoma caused by shift and stress, are rated approximately 170 per year. The drug carrier Ocusert (Federal Republic of Germany: Chemie Grünenthal) which is applied on most of the glaucomas is discussed-especially with regard to social-medical aspects.

[Causes and risks in a interruption of systemic immunosuppressive therapy following kidney transplantation].

The immunosuppressive therapy of 22 transplant recipients was temporarily suspended 25 times resulting in rejection reactions in 7 cases with the eventual loss of five transplants. One patient succumbed to fungal infection following the loss of kidney function. No connection between the risk of rejection and the previous number of rejection treatments or the histocompatibility could be established. Acute rejection reactions mostly occurred immediately after the interruption of therapy. There was no increased risk associated with a longer break in treatment. Interruption of therapy during the first period of immunosuppressive treatment, that is approximately 3 months after transplantation, was associated with the lowest risk of rejection. As a result of our experience in 25 cases it appears that the temporary suspension of immunosuppressive therapy is associated with little risk and avoids a drug intoxication as well as reduces the mortality due to infection.
