Prediction and evaluation criteria in perazine therapy of acute schizophrenics. Pharmacokinetic data.

Twenty-eight patients with acute schizophrenic illness received an oral daily dose of 200-800 mg perazine (Taxilan) for 4 weeks. Weekly plasma level determinations showed a constant perazine concentration from day 7 to day 28, whereas the equilibrium level of its metabolite desmethyl perazine was only achieved at day 14; on an average it amounted to twice the level of perazine. Additional measurements were carried out 2 and 4 h after administration of the morning dose on day 14. The maximal increase of the perazine concentration was usually reached after 2 h; though it varied between 7 and 240% of the morning level, a close correlation existed between minimal and maximal levels. The perazine fraction not bound to plasma proteins was found to be 3.1-5.5% on day 21. The percent improvement in target syndromes during 4 weeks of neuroleptic therapy, as documented with the AMDP system, was most marked in those patients who had perazine levels in the 100-230 ng/ml range at day 28; patients with lower or higher levels improved significantly less. Curvilinear relationships also appeared to exist between improvement and free perazine concentration as well as maximal level on day 14. With regard to total scores on the Brief Psychiatric Rating Scale or scores of higher-order factors, no significant relationship between improvement and perazine level was found. The desmethyl perazine concentration did not exhibit a significant relationship to the therapeutic result. The pharmacokinetic parameters investigated seem to have a limited influence on the clinical outcome.(ABSTRACT TRUNCATED AT 250 WORDS)

[The significance of laparoscopic examination in acute adnexitis for bacteriological diagnosis].

Cultures of abdominal bioptic material from 102 cases of acute pelvic inflammatory disease diagnosed laparoscopically revealed that anaerobes were involved in 42.2% of the cases; 18.6% of the cultures were sterile. There were no signs that anaerobes predominate in cases, in which an abscess has already formed. The proportion of predominantly polymicrobial multiple infections was 54.2%, the remaining 45.8% of the cases involving mono-microbial infection. Scrutiny of the abdominal and cervical distribution of the germs in 42 of our female patients confirmed that cervical smears are inadequate for detecting the causal germ involved in a case of acute pelvic laparoscopy it seems advisable to use abdominal cultures of tissue taken from the locality of the inflammation in order to identify exactly the causal germ and to test these cultures also for anaerobes and gonococci.

[Detection of antibody-coated bacteria in the urine of adults with chronic pyelonephritis].

Urine samples of 59 patients suffering from chronic pyelonephritis were tested repeatedly for antibody-coated bacteria (ACB). We demonstrated that the number of positive results increased with the frequency of the tests performed. It was observed, however, that antibody-coated bacteria were not found consistently in every sample during a longer period of observation.

[Typing of E. coli rough strains with phages (author's transl)].

Two phage-sets were used for the typing of 408 E. coli rough strains isolated from patients with urinary tract infections, Using the 13 phages A to M (11), we were able to type 225 (55.1 per cent) strains. 143 (35 per cent) of these strains were found to have K antigen, 84 different phage patterns could be recognized. Phage patterns GHK, M, F, and AM were the most common ones found. The application of the typical rough phages Br10, C21, 6SR, FP1, T4, and the Felix O (FO) phage led to the successful typing of 143 (35 per cent) strains, 80 of which (19.6 per cent) had a K antigen. About 70 per cent of the strains typable were found to belong to phage patterns which were represented by single phages, 24 different R phage patterns were observed. Considering both phage-sets the total typability amounted to 67 per cent. It is concluded that phage typing may be useful for the typing of E. coli rough strains in urinary tract infections in order to distinguish between relapse and reinfection.

[Microbiological aspects of human pyelonephritis].

Issuing from the adhesive ability of bacteria to human mucous membranes the phenomenon of the adhesion of E. coli to uroepithelial cells is analysed in detail. Fimbrias and colonisation factors might be of importance. In own experiments the first clues concerning the numberous appearance of these adhesive factors could be got. In further investigations the author adopts a definite attitude to the problems of the rough forms of E. coli as the pathogenic agent of a pyelonephritis of man. It is to be assumed that by the occurrence of certain factors of virulence rough forms possess importance as causal agents of pyelonephritis.

[Configuration frequency analysis. XI. Strategies of symptom configuration comparison before and after therapy].

Four strategies are proposed for comparing patterns of yes-no-symptoms before and after therapy (or time elapsed) in a sample of N patients: 1) a test for change of (mean) symptom score, 2) a test for (proportional) change of a specified symptom, 3) a test for (proportional) change of a specified symptom pattern and 4) a test for specified change or persistence of a selected symptom pattern. All strategies are exemplified by N = 75 schizophrenics observed as to 3 main symptoms (emotional withdrawal, thinking disturbances, and hallucinations) before and after a neuroleptic treatment.

[Escherichia coli o serotyping in different patient groups with urinary tract infections (author's transl)].

Using 44 anti-O sera 789 Escherichia coli strains isolated from the urine of patients with urinary tract infections were typed. Of the 119 E. coli strains from the urine of children, 49.6% were O-typable; only 3.4% were rough strains. Of 357 strains from pregnant women, 47% were O-typable; 12.1% were rough strains. The largest proportion of O-typable E. coli strains was found among 314 strains isolated from adults with recurrent urinary tract infections (55.4% of the 314 strains); 8% were rough strains. The higher proportion of E. coli rough strains in adults, including pregnant women, indicate that once a pyelonephritis process has been established by virulent smooth strains in childhood, it can be sustained by rough strains at a later stage. Certain O serotypes were found to occur with a variable frequency in the individual patient groups. Enteropathogenic E. coli strains were seldom found in any of the three groups.

[K antigen, colonization factor antigen, dulcitol fermentation, and hemolysin production in E. coli rough strains isolated from urine specimens (author's transl)].

E. coli rough strains (as determined by spontaneous agglutination of in boiled saline suspension) isolated in significant amounts (>10(5)/ml) from from patients with urinary tract infections were investigated for some factors possibly related to virulence. The frequencies of capsular antigen, colonization factor antigen (CFA I), dulcitol fermentation, and hemolysin production are summarized in table 1. 270 (66.2%) out of 408 strains were found to be encapsulated as determined by the inagglutinability of the living strains in saline. 180 (44.1%) strains were able to ferment dulcitol, and evidence for hemolysin production could be demonstrated in 146 (35.8%) strains. The CFA I (demonstrated by a mannose resistant hemagglutination of human red blood cells) was detected in 62 (15.2%) of the E. coli rough strains tested. The frequency of single properties in the presence or absence of capsular antigen is shown in table 2 demonstrating no significant differences in both groups. The most frequent single factor was found to be the capsular antigen with 22.1%, followed by the double combination capsular antigen/dulcitol fermentation (16.7%), dulcitol fermentation alone (10%), and capsular antigen/hemolysin production (9.8%). There were only 5 strains possessing all four factors tested. On the other hand we found 35 (8.6%) rough strains with none of the properties. Some aspects of virulence of E. coli rough strains are discussed.

[Douglas' pouch swab for detection of anaerobic asporous micro-organisms (author's transl)].

Bacteriological diagnosis was applied to Douglas' pouch swabs taken off 119 patients, most of them with non-inflammatory genital diseases. Purely anaerobic cultures were recorded from 11.8 per cent, mixed anaerobic-aerobic cultures from 8.4 per cent, and purely aerobic cultures from 56.3 per cent. Cultures remained sterile in 23.5 per cent of all cases. Forty-six of those patients were afflicted with uterine myomatosis, a group from which the following results were obtained: purely anaerobic cultures in 21.7 per cent of all cases, mixed anaerobic-aerobic cultures in 6,5 per cent, purely aerobic cultures in 54.4 per cent, and sterile cultures in 17.4 per cent. The importance of such sited flora to both genesis and kinetics of pelvic inflammations is discussed.

Border disease: persistant infection with the virus.

Two genetically related sheep that produced border disease-affected lambs from successive pregnancies were identified. These sheep, and some of their progeny, were found to be persistently infected with a virus antigenically related to bovine virus diarrhoea/mucosal disease virus. Only one of the sheep developed detectable serum antibody to the virus, and this animal only produced it 12 months after being detected as infected. The epidemiological significance of sheep persistently infected with border disease virus is discussed.
