[Effect of immunization with Escherichia coli J5 on the course of experimental pyelonephritis in rats].

Among the germs of urinary tract infections is a great heterogenicity. Therefore, in search of vaccine candidates of pyelonephritis such germs must be checked the antigens of which to be occurred as much in other pyelonephritis germs as possible. E. coli J5 as a re-mutante of E. coli O111 has a core-antigen which is found in the most enterobacteria. We have examined whether a pyelonephritis in rats was prevented by prophylactic use of a E. coli J5 vaccine and if the course of the disease was influenced by vaccine treatment of animals with pyelonephritis, respectively. Neither a pyelonephritis was prevented nor an existing infection was influenced favourable by the vaccine of E. coli J5.

[Local autovaccine treatment of experimental urinary tract infections in the rat].

The effect of a local autovaccination therapy on the course of an experimental pyelonephritis should be investigated in the rat. A total of 114 animals were infected by E. coli O2:K1:H4 by the hematogenic route. In the first group a suspension of formalin-inactivated germs of the infection strain was applicated, in the second group a suspension of formalin-inactivated germs of a E. coli rough strain and in the third group isotonic saline solution was given on the 6th, 7th, and 8th week after infection, respectively. The fourth group was untreated after infection. The autovaccination therapy led to a significantly decrease of bacterially contaminated kidneys and a significantly reduction of pyelonephritis foci in the primary infected kidneys. An ascending reinfection resulted in a higher bacterial colonization of the kidneys in the vaccinated animals than in the control animals. Nevertheless, in the vaccinated animals fewer kidneys were affected by inflammatory changes.

[Assessment of occupational factors of influence on pregnancy].

This study has a clinical design. The aim is, to gather more information about professional influences on the negative outcome of pregnancy in form of abortion or malformation. We analysed 90 histological confirmed abortions of patients visiting a sterility consulting outpatients clinic and 343 malformations detected during postdelivery stay in the hospital. Each woman with abortion or malformated child was pair-matched with a referent pregnant woman in the same age and normal outcome of pregnancy, whose pregnancy or delivery was at the same time. Their professional exposure data arranged in different expositions have been compared. Women with abortions or malformated newborns were more often in a profession with a risk to health than the pairmatched women without abortions or malformated newborns. A proposal for attention to professional influences usable in a computer program of obstetric clinics is offered.

Effect of immunization with Escherichia coli K1 antigen on the course of experimental infection of the urinary tract of the rat.

The influence of vaccination with an immunogenic K1 antigen of Escherichia coli on the course of pyelonephritis induced experimentally in rats by infection with Escherichia coli O2:K1:H4 was investigated. The immunogenic properties of K1 antigen are poor. A conjugate of K1 antigen with bovine serum albumin (BSA) was therefore tested. Preliminary experiments showed that the K1-BSA conjugate is immunogenic. Renal inflammatory activity in rats after retrograde infection was significantly reduced after vaccination of the animals with K1-BSA conjugate within six weeks of infection.

[Modification by cyclophosphamide of the effect of autovaccination in chronic pyelonephritis in an animal experiment].

In an existing chronic pyelonephritis the influence of cyclophosphamide on the efficacy of an immunisation with autopathogenic antigens and the surface antigen of an E. coli rough strain was tested in an animal experiment. After an existence of the infection of about five weeks a decisive decrease of the antibody production by cyclophosphamide was not to be observed. The animals treated with cyclophosphamide did not show any improved elimination of germs. The histological findings of the kidneys documented the cytotoxical effect of the cyclophosphamide on the renal tissues and the epithelium of the urinary tract. The immunisation of animals with chronic pyelonephritis with various autopathogenic antigens altogether led to a statistically significant decrease of inflammatorily changed kidneys.

[Autovaccination in chronic pyelonephritis in the animal experiment--effect of cyclophosphamide on splenic and thymus lymphocytes].

In rats with experimental pyelonephritis, cyclophosphamide leads to changes in the spleen and thymus lymphocyte populations. When cyclophosphamide was given five times a week from the fifth week of infection on, the population of electrophoretically slow moving spleen lymphocytes, which are believed to be B-lymphocytes, decreased in the seventh week. This effect is independent of autovaccination. During the same period the number of immature slow moving thymus lymphocytes decreased. Further treatment with cyclophosphamide until the twelfth week of infection led to the differences almost disappearing in the histogramm. Cyclophosphamide showed a slight effect on antibody production and did not influence the elimination of infective agents from the kidneys of animals with pyelonephritis.

[Effect of immunization with K1-antigen of Escherichia coli on the course of experimental urinary tract infection in the rat].

The influence of the vaccination with an immunogenic K1-antigen of Escherichia coli on the course of an experimental pyelonephritis in rats due to Escherichia coli O2:K1:H3 was investigated. K1-antigen is a poor immunogen. Therefore a conjugate of K1-antigen with bovine serum albumin (BSA) was used. This K1-BSA-conjugate was immunogenic in preliminary experiments. The influence of a vaccination on the course of pyelonephritis may be dependent on the route of infection. Rats which are infected by a retrograde route shows significant less inflammated kidneys when they received a vaccination by K1-BSA-conjugate within the first six weeks of infection.

[Phagocytosis in patients with chronic pyelonephritis].

In 30 patients with a non-obstructive chronic pyelonephritis and two to three clinically manifest urinary tract infections per year as well as in 50 healthy test persons we determined the rate of phagocytosis and the index of phagocytosis of neutrophil granulocytes in absence of serum with Candida albicans and latex particles in double disposition. The mean values of the phagocytosis rate und phagocytosis index of the patients were significantly decreased both for Candida albicans with 40.80% and 1.29 and for latex particles with 67.80% and 2.33 compared with healthy test persons with 54.24% and 1.69 and 86.54% and 3.51, respectively, disturbances of the cell metabolism of the neutrophil granulocytes with an impairment of the function of the cell membrane were discussed as cause of the reduced effect of phagocytosis.

Effect of autovaccination on the course of urinary tract infection in animal experiment.

Autovaccination of rats with chronic pyelonephritis carried out approximately two months after the onset of infection does not result in an improved histological picture in the infected but can prevent destructive processes in the controlateral kidney. Cyclophosphamide administered in three doses of 30 mg/kg simultaneously with autovaccination slightly modulates the immune response to the infectious strain. The temporal relationship between immunization and cyclophosphamide administration determines the mode of action of cyclophosphamide. In the present experiment, the concept of Miller has not proved to be applicable. Cyclophosphamide administration causes a distinct increase in inflammatory processes in the kidney. Should an enhancement phenomenon be involved in the bacterial infection of the kidney, of which we have found no proof, cyclophosphamide therapy as it was used in the present study would not result in its removal and thus in improved elimination of the infectious organism. Additional experiments are required to determine whether animals subjected to autovaccination are protected against a new episode of urinary tract infection.

[Preliminary results of the vaccine treatment of chronic pyelonephritis in animal experiments].

The immunization of rats with a formaldehyde-inactivated vaccine of the homologous infection strain does not lead to an improved tendency for an existing infection of the urinary tract to heal in the primarily infected kidney, but it can prevent it spreading to the other one. The dose is probably significant. In previously published investigations this effect did not occur after six vaccinations (11). In existing infection cyclophosphamide treatment does not prevent any possible enhancement and thus does not improve the elimination of germs.

Prediction and evaluation criteria in perazine therapy of acute schizophrenics. Methodological considerations, sample population, and research variables.

In an open study of schizophrenic patients treated with perazine (Taxilan), a multidimensional approach comprising the following three areas was chosen: 1. psychopathology (AMDP, BPRS); 2. psychophysiology (habituation of skin resistance reaction, pulse rate); 3. pharmacokinetics (perazine-desmethylperazine plasma levels). Looking beyond the known global effect of the neuroleptic drug, interactions between these three areas were studied. Furthermore, methodological questions concerning possible predictor variables for treatment outcome were examined. Finally, an improved method for analyzing psychopathological data was employed. The methodological framework of the study, the examination techniques, and patient population are presented.

Prediction and evaluation criteria in perazine therapy of acute schizophrenics: psychopathological results.

In a pharmacopsychiatric pilot study 28 patients with an acute schizophrenic illness were treated for 4 weeks with perazine (Taxilan). Research goals in the psychopathological investigation were: (1) to assess the range of efficacy, target symptoms and syndromes of perazine and to compare these results with previous studies; (2) to compare the quality, interrater reliability, and practicability of two instruments of psychopathological assessment (AMDP and BPRS) for measuring change during treatment; (3) to apply new mathematical methods for finding statistically significant changes in symptomatology on the item level as well as on the levels of syndrome and total score; (4) to test whether pre-treatment prediction based on psychopathological evaluation or psychophysiological data is possible. The target symptoms and syndromes of perazine were the "positive" schizophrenic syndromes, above all thought disturbances and delusional phenomena. This result was consistent with a former perazine investigation. The BPRS proved to be a practicable and reliable instrument for a more global evaluation of change in psychopathology. The AMDP seemed to be especially useful for evaluating the homogeneity of a sample and showing differentiated psychopathological profiles. On this scale, changes in symptomatology became statistically apparent more clearly and more quickly on the syndrome level rather than on the level of single items. As to interrater reliability, both scales perform well. Dichotomizing the original scaling may mean a loss of information from both scales (AMDP and BPRS) in their evaluation of change. Hence it seems necessary to introduce new mathematical procedures (e.g., Friedman and Dunn-Rankin tests) into pharmacopsychiatric research.(ABSTRACT TRUNCATED AT 250 WORDS)
