Comparative morphological and biochemical investigation of healthy human myocardium.

In order to study healthy myocardium ultrastructural and biochemical investigations were performed on human heart in biopsies taken from different locations from brain dead organ donors. To study healthy myocardium and to recognize early signs of myocardial impairment, morphological and biochemical investigations were performed on human myocardial tissue in various conditions. The lactate dehydrogenase was investigated cytochemically in different part of the myocardium. The cytochemical reaction did not show difference. Lactate dehydrogenase and its isoenzyme activities were determined in different parts of the normal human myocardium. In conclusion LD isoenzyme dominates in all parts of the normal human myocardium. The activities of total LD, H subunit and LD1 increase from atria and auricular appendages via right ventricle to left ventricle. The activities of both H and M subunit are different in the different parts of the myocardium. Combining the morphologic and enzymatic evaluation of endomyocardial biopsy samples, it is possible to get closer to the understanding of different cardiac diseases and to increase the value of diagnostics.

[Restoration of unilateral borderline edentulousness (Fábián-Fejérdy prosthetic class 2A-2B)].

A frequent case of unilateral row and edentulousness is when, on one side, all three molars are missing and, thus, the second promolar is the last tooth. To solve such edentulousness a removable denture or a combined fix plus removable plate denture is suggested. By employing the principles of the prosthetical classification (Fábián-Fejérdy) the justification of the distally free end bridge is proved.

[The damaged environment. Heart and lung changes].

Author evaluates cardiac and pulmonary changes developed by environmental pollution, from the point of view of forensic expert. The myocardium and parenchyma are the target organs of multicasual damages. Attention is called to the role of forensic expert in prevention of environmental damages and in the evaluation of the effects.

The asymmetry of 3H-imipramine binding may predict psychiatric illness.

The Bmax and Kd values for 3H-imipramine binding were measured in post-mortem human brains from drug-free selected psychiatric subject homicide victims (n = 15) and normal controls (n = 15). The two groups were comparable in age and gender. The number of imipramine binding sites (Bmax) in the frontal cortices of psychiatric subjects had significantly higher Bmax values in the left hemisphere than in the right hemisphere. Inversely, the number of imipramine binding sites (Bmax) in the frontal cortices of normal controls were significantly higher in the right brain than in the left brain. It was postulated that the inhibiting effect of central serotonin (5-HT) has weakened in psychiatric cases, therefore the change of presynaptic serotonergic activity might be associated with psychiatric illness in the left hemisphere of human brain.

Lactate dehydrogenase and its isoenzyme activities in different parts of the normal human heart.

Activity and distribution of lactate dehydrogenase (LD) and its isoenzymes (LD1-5) were determined and both the heart (H) and muscle (M) subunit activities were calculated in myocardial samples from six brain dead human organ donors with normal hearts. Ten parts of each heart were analysed. LD1-3 were found to be the main LD isoenzymes. The LD1 activity predominated in all parts analysed. The activities of total LD, H subunit and LD1 increased from atria and auricular appendages via the right ventricle to the left ventricle. The H subunit activity varied more than twofold and the M subunit activity by only 20% between different locations of the heart. The left ventricular papillary muscle was found to have higher activities than other locations of both H and M subunits. It is concluded that the isoenzyme profile could be a metabolic adaptation to divergent work demand on the different heart chambers.

Immunofluorescence detection of digoxin with monoclonal digoxin specific antibody.

The present study primarily focuses on the analysis of digoxin binding of the heart muscle cells. The primary aim of the investigation was to demonstrate the cardiac glycoside morphologically. The direct immunofluorescence staining technique with digoxin specific monoclonal antibody or Fab fragments and FITC or Texas-Red conjugated antisera are useful for morphological demonstration of digoxin binding and localization in cardiac cells. With the immunofluorescence method, linkage can be observed on the sarcolemma membrane and on the wall of capillaries and arterioles in myocardial cells treated by cardiac glycoside. The specificity of reaction is provided by the negative reaction of cells, not treated by digoxin. Intensity of reaction depends on concentration. The photometric measuring of fluorescence enables the quantitative analysis of cardiac glycoside. It shows the sensitivity of the method in that cardiac glycoside linked to the cell membrane can be detected in the upper sphere of a therapeutic dose. Application of the immunofluorescence method is manifold and relatively simple, and this quick method can be used in diagnoses and in the study of cardiac glycoside receptors of cell membrane. On the basis of our own experiments it is possible to study the kinetics of digoxin linkage. The use of this method is demonstrated for investigation of single cell suspension and cryostat sections.

Analysis of chromosomes in paternity cases.

Authors on the base of the material investigated in the Department of Forensic Medicine of Semmelweis Medical University discuss importance of anthropological investigations as a valuable, additional to serology test. In cases when neither blood-group tests nor anthropology provided evidence sufficient to formulate a conclusive opinion chromosomal studies are recommended. The surveying of 300 cases with the C-band/showed the thickening of the heterochromatic part of chromosomes 1, 9, 16, Y/and the occurrence of total and partial inversion. Examinations with Q-band have revealed the incidence of chromosomal polymorphism.

[Evaluation of distal free-end dentures and partial plate dentures based on their effect on the remaining dentition].

The data of 3778 examined persons were processed. The effect of distally free end bridges made for replacing terminal endentulousness on the remaining teeth was compared with the condition of the remaining teeth of patients untreated, provided with partial dentures as well as with combined fix + partial denture. It was ascertained that in cases of replacements by distally free end bridges deterioration of the remaining teeth is essentially less.

Coenzyme Q10 content in different parts of the normal human heart.

Coenzyme Q10 (CoQ10) and citrate synthase (CS) activities were analysed in the myocardium of brain-dead organ donors (14-40 years). Different parts of the heart were studied: right and left auricular appendage, right and left atrium, right ventricle (septum and free wall) and left ventricle (septum, free wall, and papillary muscle). Freeze-dried, dissected myocardial samples were analysed for CoQ10 content by HPLC and CS activity by fluorometric technique. CoQ10 content in the normal human myocardium was lowest in auricular appendages and atria (0.25 +/- 0.06 mg X g-1 dry muscle), intermediate in right ventricle (0.37 +/- 0.05 mg X g-1 dm) and highest in left ventricle (0.42 +/- 0.07 mg X g-1 dm). CS activity showed the same relationship between these locations as CoQ10. The results suggest that there exist differences in CoQ10 content between different parts of the normal human myocardium. These differences were closely related to the differences in CS activity between corresponding parts. The differences between different parts of the heart may be related to divergent work demand, and the constant relationship between CS and CoQ10 may be related to their coupling to the mitochondrial oxidative metabolism.

Retrospective psychiatric assessment of 200 suicides in Budapest.

Based on an interview with the closest family member, using the Schedule of Affective Disorders and Schizophrenia--SADS, a retrospective psychiatric assessment and diagnostic classification was carried out on 200 completed suicides. Eighty-one per cent of the victims had a recent psychiatric disorder, in 63% depression. The prevalence of psychiatric illnesses was similar to that of other studies from countries with lower suicide rates.

Does 3H-imipramine binding asymmetry indicate psychiatric illness?

We have accepted that serotonin is essentially an inhibitory neurotransmitter in the human brain, so we propose that it is precisely this inhibiting effect that has weakened in psychiatric cases. We have investigated the asymmetry of tritiated imipramine binding sites (Bmax) in the frontal cortices of homicide victims (n = 6) and controls (n = 6) who died of natural causes. Of these homicide victims examined in our experiment, five proved to have been psychiatric cases and one case had no psychiatric record. The two groups were comparable in age, gender and postmortem delay. The number of imipramine binding sites (Bmax) in the frontal cortices of controls was significantly higher in the right hemisphere than in the left hemisphere. But the homicide victims who were psychiatric cases had significantly higher (Bmax) values in the left hemisphere. While we only found higher Bmax values in the left hemisphere of homicide victims with mental diseases, our data may serve to prove the direct role of the serotonergic mechanism in the development of psychiatric cases.
