Are PPS payments adequate? Issues for updating and assessing rates.

Declining operating margins under Medicare's prospective payment system (PPS) have focused attention on the adequacy of payment rates. The question of whether annual updates to the rates have been too low or cost increases too high has become important. In this article we discuss issues relevant to updating PPS rates and judging their adequacy. We describe a modification to the current framework for recommending annual update factors. This framework is then used to retrospectively assess PPS payment and cost growth since 1985. The preliminary results suggest that current rates are more than adequate to support the cost of efficient care. Also discussed are why using financial margins to evaluate rates is problematic and alternative methods that might be employed.

Medicare prospective payment without separate urban and rural rates.

The elimination of urban-rural differences in the Medicare prospective payment system (PPS) standard rates implies a need to re-examine all the PPS payment adjustments. Refinements for case mix, outliers, and the wage index can make a significant contribution to avoiding payment disparities in a single-rate system. However, changes in the adjustments for teaching and disproportionate-share (DSH) hospitals are also needed. The typically urban location of these hospitals makes it difficult to balance PPS payments and costs among major groups of urban and rural hospitals without some form of higher payment for all hospitals located in large urban areas.

Computed tomography of bronchogenic carcinoma: findings and reevaluation after radiotherapy.

493 computed tomograms were performed in 150 patients with bronchogenic carcinomas after radiotherapy. In early controls after termination of the irradiation, tumors with atelectasis could be delineated slightly better. In tumors without atelectasis, however, delineation deteriorated. In 50 late controls the radiation-induced pulmonary injury resulted in further masking of the tumor area, independent of the initial degrees of airway obstruction. Post-radiation changes were always earlier and more extensively visible by CT than by conventional radiography. No radiation effect on delineation of the mediastinal lymph nodes was provable. In 87% of all cases the remission rate of the tumors could be adequately evaluated by CT. In 43% of the late controls a delayed complete remission was seen. Fifteen out of 20 tumor recurrences or progressions were recognizable only by CT. The most important diagnostic criteria after radiotherapy of bronchogenic carcinomas are: 1. Consideration of the irradiation modalities and timing of the CT examination. 2. knowledge about the course of the pulmonary reaction to irradiation and 3. subtle comparisons of serial CT examinations in tumor follow-up.

[Drug clearance as a decision aid for further invasive liver diagnosis--studies with hexobarbital as a model substrate].

The clearance of a drug predominantly metabolized in the liver may serve as an estimate of quantitative liver function. In 260 consecutive patients presenting with a history of liver disease and abnormal laboratory findings but without a current definite diagnosis we have measured the clearance of hexobarbital and investigated if low values in patients are able to support the decision for an invasive diagnostic procedure such as needle biopsy or laparoscopy. 250 mg of hexobarbital was given orally to the patients between 8 and 10 hrs p.m. 12 hrs later blood samples were taken. Hexobarbital was determined by gas chromatography with N-selective detection, and a single point clearance was calculated. We recommended liver biopsy or laparoscopy to all patients with a hexobarbital clearance below 2.7 ml/min/kg body weight (normal 2.66-5.34 ml/min/kg). 73 out of 260 patients showed a reduced hexobarbital clearance. In 44 patients blind liver biopsy (n = 14) or laparoscopy (n = 30) was performed, 29 patients refused an invasive diagnostic procedure. 17 out of 26 patients with the tentative diagnosis chronic hepatitis had already an incomplete or complete liver cirrhosis. In 11 out of 18 patients with the tentative diagnosis alcohol toxic liver injury we found a progressive portal fibrosis or complete liver cirrhosis. Reduced drug clearance reflecting quantitative liver function can be an indicator of advanced liver disease, thus adding substantially to the decision for further invasive diagnostic procedures.

[Digital luminescence radiography of premature and term infants. Possibilities and limitations].

Postmortem images of seven premature and term newborns, acquired with various conventional screen-film combinations and by a digital technique with appropriate radiation exposures, were compared. Images without and with simulated pathology were used in this study. With electronic manipulation of the computed radiographs we obtained well-balanced images with good contrast, and in many cases the artifacts were better visualized than with the conventional screen film. However, in our study no significant dose reduction for this young age group was possible with the digital technique compared with conventional screen film.

Inhibition of intestinal glucose absorption in rats treated with 2,3,7,8-tetrachlorodibenzo-p-dioxin.

Male Sprague-Dawley rats were injected with either 125 micrograms 2,3,7,8-tetrachlorodibenzo-p-dioxin (TCDD)/kg or vehicle (pair-fed and ad libitum-fed controls). Transfer of water, electrolytes and D-glucose as well as fats of a tracer dose of the non-metabolizable radioactive marker 3-O-methyl-D-[U-14C]glucose was studied in isolated perfused jejunal segments 1, 2, 7, and 21 d after treatment (TCDD-treated and pair-fed control rats) and after 26 d in ad libitum-fed controls. TCDD-treated rats demonstrated reduced feed consumption and loss of body weight. Active intestinal absorption of glucose was significantly inhibited 30 and 22% compared to pair-fed controls, respectively 2 and 7 d after TCDD treatment. After 21 d the inhibition (14%) was less significant. There were no differences in glucose transfer between severely starved pair-fed controls (body weights 370 +/- 26 g) and ad libitum-fed rats (body weights 512 +/- 15 g). Water absorption and transfer of sodium and calcium was not influenced by TCDD treatment. However, a significant increase of potassium transfer was observed in parallel with impaired glucose absorption. The uptake of 3-O-methylglucose into mucosal tissue was not impaired, whereas the transfer to the serosal side was significantly inhibited by 30-60% compared to pair-fed as well as ad libitum-fed animals from day 2 until the end of the experiment. These results suggest that TCDD is involved in an inhibition of glucose transport at the basolateral membrane.

Increased copper concentrations in rat tissues after acute intoxication with 2,3,7,8-tetrachlorodibenzo-p-dioxin.

Recently, acutely toxic doses of the environmental pollutant 2,3,7,8-tetrachlorodibenzo-p-dioxin (TCDD) have been reported to affect the hepatic distribution of essential metals in the rat. However, the reduced food intake by TCDD was not taken into account. Therefore, metal concentrations were determined in different rat tissues at the end of a toxicity study with TCDD in which a pair-fed control group was introduced. Male Sprague-Dawley rats received a single i.p. injection of corn oil/acetone with or without TCDD at 125 micrograms/kg. Controls and TCDD-treated rats were fed ad libitum; additionally, pair-fed controls received the amount of food consumed by their TCDD-treated partners 1 day before. Twenty-one days after dosing rats were killed and samples of liver, kidney and jejunum were taken for the analysis of Ca, Cu, Fe, Mg, Mn, and Zn. After acid digestion of the tissues metals were determined by atomic emission spectrometry (AES). The most outstanding effect of TCDD treatment was an increase of the copper levels in the kidney (4-fold, versus pair-fed controls) and in the liver (greater than 2-fold, versus pair-fed controls). Other metals were mainly affected by the reduced food intake only. Since Cu represents a trace metal the homeostasis of which depends on its biliary excretion and since TCDD is known to impair biliary flow and excretion, an impaired biliary excretion of Cu by TCDD is suggested as the causal mechanism.

[Neonatal alloimmune thrombocytopenia--can cryopreserved thrombocytes help? (Case report)].

We present a case report of the transfusion of cryopreserved HPA 1-negative platelets to a newborn suffering from neonatal alloimmune thrombocytopenia (NAT). A boy weighing 3450 g, born of a HPA1-negative mother with a platelet count of 9 Gpt/l after delivery, received two transfusions of cryopreserved HPA1-negative platelet concentrates. The first transfusion resulted in a platelet increment to 32 Gpt/l (CCI 11). A second transfusion was needed after a further drop of the platelet count on day 4 to 18 Gpt/l, resulting in an increment to 64 Gpt/l (CCI 36). Normal development during the first six months was obtained. In our opinion, cryopreserved HPA1-negative platelets could be helpful in the therapeutical strategy of NAT.

[Efficiency of mechanical plasmapheresis].

In a clinical study the automated donor plasmapheresis devices Autopheresis-C A 100, Plasmapur-Monitor, and Ultralite-PCS were tested. Including also manual plasmapheresis, we analyzed the efficiency of donor plasmapheresis, the reliability and breakdown susceptibility of the methods, the quality of the collected fresh plasma, and the suitability of the different machine plasmapheresis devices for routine application, using a total of 2,372 donor plasmaphereses. We also studied the behavior of some individual laboratory parameters due to plasmapheresis with Plasmapur-Monitor. The data allow us to recommend machine plasmapheresis for routine operation (Ultralite-PCS greater than Autopheresis-C A 100 greater than Plasmapur-Monitor).

[Multiple malignomas--an analysis of 352 patients].

8234 patients were irradiated between 1977 and 1988 in the Department of Radiotherapy at Würzburg, 211 patients (2.56%) developed multiple primary malignant tumors. The records of 1599 patients from the year 1958 to 1976 were reviewed and another 141 multiple tumors were found. On the whole, 352 patients with 326 cases of two tumors, 21 cases with three tumors, 4 cases with four tumors and one case with five tumors were observed. Both sexes were affected equally, but the peak age of these patients was found to be the sixth and seventh decade. The mean interval for a second primary was 5.7 years, from the second to the third and from the third to fourth tumor it was 1.6 and 0.5 years, respectively. 18 patients with a second malignoma in a formerly irradiated area were diagnosed, especially men with tumors in the aerodigestive tract and women with tumors in the pelvic region. The data of our clinic do not indicate an elevated risk for radiation-induced malignancies.

[The new Würzburg data bank for radiotherapy].

Conception, structure and realisation of a new data base for radiation therapy are presented. The data base utilizes the commercial data base system ORACLE and the data base language SQL. A program package for statistical analyses including Kaplan-Meier-calculations, logrank test and Gehan/Breslow test was elaborated. The input of the data recorded on form sheets is carried out on a data base of the Tumor Centre in the first instance. From there the data are transferred to the ORACLE data base. Up to now the courses of disease of about 13,000 patients are stored. Therefore, extensive and detailed statistical analyses are practicable.

[Experiences with simultaneous cytostatic and radiotherapy treatment of advanced cancer of the head and neck].

Fifty-seven patients with stage III and IV (stage-grouping UICC 1987) carcinoma of the head and neck were primarily treated with both chemotherapy and radiotherapy at the same time. Radiation therapy was administered in two series in daily fractions of 2 Gy up to a total dose of 70 Gy. On the first five days of each series a daily dose of 20 mg/m2 body surface Cis-diamino-dichloroplatin (II) (Cis-DDP) was given intravenously. In the group of T3-tumors, there were 78% complete remissions and 89% complete or partial remissions, while in the group of T4-tumors, 46% complete remissions and 92% complete or partial remissions were found. In cases of N2-metastases there were 63% complete remissions and 89% complete or partial remissions, 43% complete remissions and 86% complete or partial remissions occurred in the group of N3-metastases. Complete remissions were obtained in 46%, and complete or partial remissions both of the primary tumor and of eventual metastases were obtained in 79%. However, the disease-free intervals for the majority of the patients were short. Only half of them stayed recurrence-free for longer than six months, while only one-quarter were recurrence-free for longer than 12 months.

Intestinal first-pass metabolism of nitrosamines. 4. Metabolism of N-nitrosodibutylamine in vascularly autoperfused jejunal and ileal loops of rats.

N-Nitrosodi-[1-14C]butylamine (NDBA) has been shown to undergo a high first-pass metabolism in isolated perfused rat small intestinal segments. Metabolites resulting from omega-hydroxylation of NDBA, the bladder carcinogens N-nitrosobutyl-(4-hydroxybutyl)amine (NB4HBA) and N-nitrosobutyl-(3-carboxypropyl)-amine (NB3CPA), accounted for greater than 90% of the total radioactivity absorbed. In the present study using vascularly perfused rat small intestinal segments, the high first-pass metabolism of NDBA could be confirmed under near in vivo conditions despite the much higher absorption rate. At the end of the 36 min experimental period 70-80% of the dose have been absorbed via the portal blood as opposed to 1-10% of the dose after 2 h in vitro perfusion. omega-Hydroxylation was again the most important metabolic pathway. However, the relationship of NB3CPA to NB4HBA was shifted in favor of NB4HBA, indicating a concentration and absorption rate dependency in the further metabolism of NB4HBA to NB3CPA.
