[Diagnostic procedures in acute gastrointestinal hemorrhage].

In gastrointestinal bleeding, the diagnosis is predominantly made by endoscopy. However, if the bleeding site is in a part of the intestine that cannot be reached by endoscopy, than the diagnosis is based on radiological and scintigraphic methods. In the past 5 years 35 patients with such cases of gastrointestinal bleeding had angiography and/or scintigraphy (n = 15) in our department. Based on our retrospective study of those cases, we advocate a management protocol for such patients based on the hemodynamic presentation.

[The integration of positron emission tomography into noninvasive cardiovascular diagnosis].

Cardiac positron emission tomography is a noninvasive imaging modality that can be used to assess regional myocardial blood flow and metabolism. This combined noninvasive approach to the evaluation of acute or chronic injury to the myocardium is superior to all other imaging modalities. With increasingly widespread use of positron emission tomography it is becoming evident that patients with impaired left ventricular ejection fraction (less than or equal to 35%) resulting from congestive heart failure and patients soon (72-96 h) after acute myocardial infarction in particular may benefit from definition of metabolically active myocardium to stratify therapeutic assessment.

Relation of technetium-99m pyrophosphate accumulation to time interval after onset of acute myocardial infarction as assessed by a tomographic acquisition technique.

Technetium-99m pyrophosphate (Tc-99m PYP) myocardial scintigraphy was performed in 110 clinically stable patients with acute or healed acute myocardial infarction (AMI). Tomography was performed 12 hours to 7 days (group A), 7 to 30 days (Group B), 1 to 6 months (Group C) and after greater than 6 months (group D) after AMI. All 40 patients in group A, 9 of 31 in group B, 1 of 22 in group C, and no patient (0 of 17) in group D had a pathologic Tc-99m PYP tomogram. Relative Tc-99m PYP accumulation within the area of infarction was measured as infarct zone to blood pool ratio, which decreased significantly (p less than 0.001) from group A (1.54 +/- 0.39) to group B (0.89 +/- 0.24), group C (0.8 +/- 0.19) and group D (0.76 +/- 0.13). These data were confirmed by sequential scintigraphy in 17 patients. It is concluded that a persisting Tc-99m PYP uptake is rarely found greater than 1 month after AMI using tomographic imaging techniques in clinically stable patients with coronary artery disease. Positive results on Tc-99m PYP tomography are a reliable indicator of AMI. Thus, Tc-99m PYP tomography is not only a sensitive but also a specific imaging technique for AMI, which might be especially useful for diagnosis of reinfarction.

Brain dysfunction during motor activation and corpus callosum alterations in schizophrenia measured by cerebral blood flow and magnetic resonance imaging.

Sixteen unmedicated (14 never-medicated, 2 with washout periods of 1-2 weeks) schizophrenic patients displaying positive symptoms (e.g., formal thought disorder, hallucinations, delusions) without negative symptoms (e.g., flattening of affect, loss of energy, anhedonia--type I patients), 15 unmedicated (with washout periods from 1 week to 2 years) patients with marked negative symptomatology [type II patients; criterion score below 15/above 35 on the Munich version of the Scale of Assessment of Negative Symptoms (SANS), respectively], and 31 matched normal controls were investigated using regional cerebral blood flow [rCBF; dynamic single-photon emission computerized tomography (SPECT) with Xenon-133 as tracer] and magnetic resonance imaging (MRI; spin-echo technique, T1 weighted, midsagittal cuts). rCBF measurements were performed during both resting conditions and simple motor activation. Separately, on the same day, we performed a planimetric evaluation of the callosal-brain ratio in all subjects using MRI. In accordance with previous results on a smaller sample, we found signs of diffuse bilateral rCBF hyperactivation in type I patients, as compared with signs of nonreactivity in type II schizophrenics. Both activation patterns were different from a strictly contralateral sensorimotor rCBF activation seen in normal persons (only 8 studied with SPECT). The planimetry of relative callosal area did not reveal differences compared to normal persons, when type I/II patients were taken together. However, the threefold increased variance as compared with that found in normal persons suggested biological heterogeneity in patients. We found an increase of relative callosal size in type I as compared with type II patients. In the light of some recent findings linking lack of laterality of several brain functions to increased callosal size, we propose lack of laterality/diffuse hyperactivation and increased callosal size to be connected with positive symptomatology/good prognosis schizophrenia, and vice versa.

Evaluation of a multicentre study with Iomazenil--a benzodiazepine receptor ligand.

After showing in an earlier publication that Iomazenil is a potent benzodiazepine receptor antagonist, the substance has been distributed to 11 clinical centres in Europe for further tests. The protocol asked for volunteers, epileptic cases and patients with Alzheimer's disease. Prior to the Iomazenil examination, flow images by perfusamine or HMPAO were required, and as comparative methods EEG, computed tomography (CT) and magnetic resonance imaging (MRI) were performed. The results allowed first the determination of the normal distribution of the benzodiazepine receptors in the human brain. The highest uptake was found in medial occipital cortex. Second, the evaluation of the epileptic cases shows a 100% positive prediction value for Iomazenil compared to 92% for flow images. Negative prediction values were calculated as 81% for Iomazenil and 54% for flow images. Furthermore, one group reported the successful diagnosis of Alzheimer's disease at an early stage. The visual image examination was tentatively compared to a more objective semiquantitative one based on quotients of corresponding left/right regions of interest. This semiquantitative method has not proved successful yet, but the problems have been identified. A more precise protocol for further studies is therefore proposed.

[Captopril-renal function scintigraphy in the clarification of arterial hypertension].

Renal artery stenosis (RAS) is a rare cause of hypertension. Radiological tests can disclose the morphological changes, but not their functional effect on renal function and perfusion. Normalization of the blood pressure can be achieved by intervention (operation, percutaneous transluminal renal angiography; PTRA), in cases of prolonged RAS-induced hypertension long-term preservation of the organ function is most important. The purpose of this study was the validation of captopril renography as a screening test for hypertension secondary to RAS prior to PTRA. Captopril renography with 99mTc-MAG 3 has a high sensitivity (94%) and acceptable specificity (88%) for the screening of hypertensive patients. The positive predictive value is 74% and the negative predictive value 98%, compared with the "gold standard" of angiography.

[Immunoscintigraphy using 111Indium-labeled antimyosin in suspected myocarditis].

111Indium-monoclonal antimyosin scans were carried out in 21 patients with suspected myocarditis, confirmed by reduced ejection volume, pericardial effusion and clinical follow up in 12 patients. Coronary heart disease was excluded angiographically in all cases. Quantitative evaluation of myocardial 111In-antimyosin accumulation 48 hours after injection showed a pathological uptake in 10/12 patients with increased heart/lung ratios (Q48 greater than 1.58). Ratios were also elevated in 2 patients with cardiomyopathy, 2 suffering from vasculitis and 1 with dermatomyositis. Four patients without proven cardiac disease had normal ratios (Q48 less than or equal to 1.58). Examination after 24 hours was of limited value, depending on the residual blood pool activity. Visual analysis of the scans showed a high interobserver variation despite a positive correlation with quantitative analysis (48 h p.i.: r = 0.72; p less than 0.001), and is not recommended. The results show the value of the 111In-antimyosin scan as a screening method prior to myocardial biopsy. However, scintigraphy cannot definitely elucidate the cause of myocardial damage. Therefore, myocardial biopsy is still recommended after positive antimyosin scans.

Viability testing of transplantation donor liver by 1H NMR relaxometry.

We present preliminary results of water proton relaxation time measurements on perfused, cold-stored rat livers as a basis of a simple method for testing the viability of human donor livers in liver transplantation surgery. Proton T1 and T2 data correlated strongly (p less than 0.01) with bile flow measurements, indicating that relaxation times reflect organ viability and therefore might be used for rapid testing of donor livers shortly before transplantation.

Value of 67gallium scintigraphy in primary diagnosis and follow-up of opportunistic pneumonia in patients with AIDS.

Opportunistic pneumonias are a life-threatening complication in patients with AIDS. Early diagnosis and therapy is necessary to improve the prognosis. This study was designed to assess the value of 67gallium scintigraphy in the primary detection and follow-up of these special pneumonias. 67Gallium scintigraphy was performed in 40 patients: 10 normal controls and 30 HIV-positive patients with AIDS or AIDS-related complex (ARC). 67Gallium scan results were compared with current chest x-rays and the results of pathogen detection. The evaluation of positive scans was based on a quantification of the pulmonary uptake, expressed as a pulmonary/soft tissue uptake ratio. Only 8 of 30 patients had a normal scan, while 22 of 30 showed diffuse (13/22) or focal (9/22) increases of pulmonary uptake. In seven of eight patients with normal scans the chest radiograph was negative as well. The one patient with negative scan but positive chest radiograph had pulmonary Kaposi's sarcoma. In 11 of 22 patients, the 67gallium scan and chest x-ray were positive simultaneously. In the other 11 of 22 patients with positive scans, chest radiographs were initially negative but showed pathology in five cases within 1-2 weeks. The reason for positive scans in most cases was an opportunistic lung infection; other forms of pneumonia were observed only in two cases. The defined uptake ratio was demonstrated to be a highly sensitive parameter for monitoring pneumonia and the effects of therapy in follow-up studies. In conclusion, quantitative 67gallium scintigraphy proved to be a reliable and highly sensitive method for primary detection and follow-up of opportunistic pneumonias in patients with AIDS.

[Scintigraphic control and x-ray findings following the cement-free implantation of the PM shaft in connection with hip joint allo-arthroplasty].

147 bone scans with 99mTc-MDP following 60 cementless hip arthroplasties were analysed at 3 to 160 weeks after operation. Results are compared to clinical evaluation according to Merle d'Aubigné. X-ray changes in a number of patients are included. In uncomplicated cases, bone scan activity decreased quickly within 26 weeks postoperatively. Decrease of scintigraphic activity was markedly delayed in patients with low Merle d'Aubigné values or persisting pain. In 4 cases, bone scan activity increased again. In these cases, reoperation revealed rotational instability of the shaft endoprostheses. Unusual x-ray phenomena such as development of a bony cup at the tip of the prosthesis or seams were found mainly in patients with less satisfying results. We consider nuclear imaging an important additional examination to evaluate bony integration of PM shaft endoprostheses after hip replacement, especially as interpretation of x-ray features alone is still doubtful.

Detection of traumatic myocardial injury by means of simultaneous Tl-201/Tc-99m pyrophosphate tomography--report of three cases.

Trauma to the chest can result in cardiac damage, which may be missed by clinical examination because of associated injuries. Routinely performed non-invasive tests may also be non-diagnostic. Tc-99m pyrophosphate (PPi) tomography, in this study combined with Tl-201, is a promising addition to non-invasive evaluation. In three patients with cardiac injury, this technique successfully detected and localized myocardial necrosis.

Acute myocardial infarction delineated by noninvasive thallium-201/technetium-99m pyrophosphate tomography.

The purpose of this study was to validate different scintigraphic approaches for assessing acute myocardial infarctions. 201Tl, 99Tcm pyrophosphate (PPi) and combined 201Tl/99Tcm PPi tomograms were evaluated in 115 consecutive patients, in 85 of whom clinical examination revealed acute myocardial infarction. The overall sensitivity and specificity for infarct detection was 80% versus 56% for 201Tl alone, 61% versus 97% for 99Tcm PPi alone, and 100% each for the combined 201Tl/99Tcm PPi imaging. The sensitivity for nontransmural infarcts was 57% for 201Tl, 37% for 99Tcm PPi and 100% for combined imaging. The overlay of 201Tl and 99Tcm PPi images increases the observer's confidence in the diagnosis and provides better localization of the infarction. 201Tl alone could localize the infarction in 80%, 99Tcm PPi alone in 49% and 201Tl/99Tcm PPi in 100% of the cases. ECG and 201Tl/99Tcm PPi tomography concurred upon infarct localization in 98% of the patients. Based on the 201Tl/99Tcm accumulation pattern, 94% of the Q-wave infarctions were judged to be transmural and 83% of the non-Q-wave infarctions were judged to be nontransmural. It is concluded that combined 201Tl/99Tcm PPi tomography is highly accurate for determining the presence and location of acute myocardial infarction. This technique appears to be superior to studies where only one radioisotope is used.

[Abdominal angina: percutaneous transluminal angioplasty of the superior mesenteric artery].

Percutaneous transluminal angioplasty (PTA) of the superior mesenteric artery (SMA) was performed in a 65-year-old man with clinical and radiographic evidence of abdominal angina. The patient was relieved of abdominal pain immediately after the dilatation. At the angiographic follow-up (7 months later) the SMA was of normal caliber. At the last follow-up visit (14 months after intervention), the patient was still free of pain. PTA of SMA stenoses is an alternative to surgical revascularization, and can be repeated if symptoms recur.

Dietary principles in disease management.

Nutritional management of medical and surgical animal patients to provide better total health care is not a new concept, but one that has gained credibility in veterinary medicine in the 1980s. Therapeutic dietary management is an alteration of diets intended to provide optimal health in pets by restricting, moderating, or enhancing nutrients and/or manipulating dietary ingredients. In many cases, dietary therapy is an integral component of animal patient management that may complement or even replace drug administration or surgery.
