Oxytocin was synthesized via the solid-phase method using dehydroalanine as pseudo-protecting group of the carboxyl-terminal as well as the omega-amide functions of asparagine and glutamine in endo-position. Starting with Boc-Gly-Dha-resin and using Boc-L-Asp(Dha-NHEt)-OH and Boc-L-Glu(Dha-NHEt)-OH as precursors of asparagine and glutamine, respectively, oxytocin was assembled in stepwise manner under solid phase synthesis conditions. Treatment of the protected [Glu(Dha-NHEt)4, Asp(Dha-NHEt)5]-oxytocin-Dha-resin with 1 n HCl in glacial acetic acid in the presence of 3 equiv. water removed the peptide from the support with the simultaneous formation of the asparagine and glutamine residues to give the protected nonapeptide amide: Cbz-Cys(Bzl)-Tyr(Bzl)-Ile-Gln-Asn-Cys(Bzl)-Pro-Leu-Gly-NH2, which was deprotected with sodium in liquid ammonia and then oxidized with diiodoethane to give oxytocin. After purification by gel chromatography and countercurrent distribution, the product displayed the chemical and physical properties and oxytocic activity (533 +/- 301U/mg) of a standard oxytocin preparation.
Neutral selenophenol in DMF accomplished the removal of the beta-phenacyl protecting group of aspartic acid in solution, on the peptide Boc-Asp (beta-OPac)- Phe amide and on the resin peptide Boc-Trp(Nin-For)-Met-Asp (beta-OPac)-P (CCK 30-33) without alpha, beta-rearrangement.
In the solid-phase synthesis of cholecystokinin 30-33, Trp-Met-Asp-Phe- amide, the beta-phenacyl ester was used to protect the beta-carboxyl of aspartyl residue. The ester was cleaved, on the solid support, with a 1 M solution of sodium thiophenoxide in DMF, prior to ammonolysis. The product, after purification by countercurrent distribution, was identified as a mixture of isoasparaginyl and aspartyl peptides. A study of the deprotection step, with sodium thiophenoxide, on a model peptide (t-butyloxycarbonyl-beta-phenacyl-aspartyl-phenylalanineamide) showed the rapid formation of the aminosuccinyl derivative, catalyzed by this reagent.
The different ways of metastases of carcinoma of the anus can hardly be removed by radical surgery and require other procedures than the lower carcinoma of the rectum. In comparing Miles operation in T3/T4 carcinomas of the anus with the combined therapy of local excision and radiotherapy results showed no great difference in T1/T2 tumors the Miles operation is not necessary. In T3/T4 carcinomas irradiation derives its worth from the few possible procedures ro radical surgery. In T1/T2 tumors irradiation can be an alternative (T1) or an additional treatment (T2) to the local excision.

Dipeptides of threo- and erythro-beta-hydroxyaspartic acid were transformed into O-tosyl derivatives, which on base-catalyzed elimination gave aminofumaric acid derivatives. The possible mechanism of this process and the spectroscopic properties of the elimination products are discussed.

Very often in advanced stages of malignant melanoma several organs are involved. In a small number of patients only intraabdominal metastases seem to occur. The analysis of 106 cases found in the literature and 6 cases from our department revealed the small bowel and the stomach as the main site of metastases, giving rise to intestinal bleeding and obstruction. Apart from emergency cases, even in the advanced stage of disease palliative procedures are indicated since the prognosis in individual cases is quite unpredictable, as seen in some cases with a survival time of up to seven years.
In order to establish possible routes for the synthesis of AM-toxins, cyclotetradepsipeptides containing Dha, Pyr-L-Ala-L-Hmb-L-Tyr-NH2 were prepared and treated with TFA or anhydrous HF. The product was identified to be cyclo (-alpha-Hyala-L-Ala-L-Hmb-L-Tyr-), resulting from intramolecular condensation of the alpha-keto and amide group with the formation of the alpha-Hyala residue. Cyclo (-alpha-Hyala-L-Ala-L-Hmb-L-Phe-) and cyclo (-alpha-Hyala-L-Ala-L-Val-L-Phe-) were obtained by acid-treatment of the corresponding pyruvyl-tripeptide amides. The cyclic tetrapeptides did not show the activity of AM-toxin.

To test sphincter function after rectal resection with colon-anal anastomosis performed with Park's peranal technique, 17 patients were examined using the Kelly code and manometric studies. Compliance and anal resting pressure were reduced. Sphincter internal relaxation after rectal distension was also less compared to the norm. Continence was normal for 10 patients and 4 had satisfactory function.
1. Iron status has been assessed in 171 blood donors and 58 controls. 2. In men, ferritinemia was lower in blood donors, as compared to controls, but hemoglobin and erythrocytes values are found normal. 3. In women, both M.C.V. and ferritinemia were lower in donors. Ferritinemia could reach values below the threshold of iron deficiency. Microcytosis occurred 7 times. It appeared to be related either to the association of hypermenorrhea with blood donation, or to a number of 4 blood donations a year. 4. Iron supplementation was randomized. The comparison of the two groups of donors with and without iron supplementation showed that supplementation had no effect on male blood donors, but improved both ferritinemia and M.C.V. values in females. 5. However, systematic iron supplementation does not seem to be appropriate in France, as long as one excludes hypermenorrheic women, and if the periodicity of blood donation does not exceed three times a year for females.
1. N tau-Methylhistidine, nitrogen and creatinine were measured in the urine of 10 volunteers on normal and meat-free diets and in 10 vegetarians, and compared with the results from the urine of eight patients with intestine fistulae on intravenous or enteral nutrition containing no meat. The values obtained were used to calculate fractional breakdown rate of myofibrillar protein. 2. There was a significant fall in the excretion of N tau-methylhistidine and creatinine and in apparent fractional breakdown rates after 2 days on a meat-free diet. 3. One of the patients had lower, and two of the patients had higher, fractional breakdown rates compared with the vegetarians. 4. N tau-Methylhistidine and creatinine excretion may be a useful and non-invasive measurement of myofibrillar protein degradation in patients on meat-free diets. Firm conclusions cannot, however, be drawn without confirmatory, direct measurement of the breakdown rates of muscle protein in vivo.

In this study in seven primary care facilities the proportion of children recognized as having behavioral, educational, or social problems was much higher than generally assumed. Although there was great variability among the facilities, at least 5% and as many as 15% of children seen in one year were diagnosed as having these problems in all but the hospital teaching facilities. The prevalence was even higher among children from poor families. The variability among facilities was much less for psychosomatic problems, which were diagnosed in 8% to 10% of the children. For both psychosocial and psychosomatic types of problems, but especially for psychosocial ones, the proportion of visits with the diagnoses was much lower than the proportion of children with them, so that these problems engendered fewer visits for their management than might have been expected from their frequency in the population. However, available evidence suggests that individuals with unresolved psychosocial problems make more than their share of visits for other diagnoses. The findings of this study have implications for the content of educational programs for primary care practitioners, for the organization of primary care practice, and for the current debate over policy concerning reimbursement and benefit packages.
