Comparison of PGE2, 6-keto PGF1 alpha and renin release from dog kidneys.

Several renal cell types synthesize prostaglandin E2 (PGE2) and prostacyclin (PGI2). To examine whether the release of these prostaglandins varies in proportion, prostaglandin synthesis was stimulated in anaesthetized dogs by renal arterial constriction, ureteral occlusion, intrarenal angiotensin II infusion and infusion of arachidonic acid, the precursor of PG synthesis. PGI2 was measured as its stable hydrolysed product, 6-keto PGF1 alpha. The two former procedures raised PGE2 release to 13 +/- 2 pmol min-1, 6-keto PGF1 alpha release to 5 +/- 2 pmol min-1 and renin release to 23 +/- 5 micrograms AI min-1. Angiotensin II infusion, reducing the renal blood flow by 30%, increased PGE2 and 6-keto PGF1 alpha release only half as much as ureteral and renal arterial constriction, and exerted no significant effect on renin release. By increasing the infusion rate of angiotensin II up to 10 times, the renal blood flow remained unaltered in four dogs and fell to 50% of control in two dogs, but PGE2 and 6-keto PGF1 alpha release did not increase further in any of the experiments. Arachidonic acid, infused at 40 and 160 micrograms kg-1 min-1, increased prostaglandin release in proportion to the infusion rate. At the highest infusion rate, PGE2 release averaged 166 +/- 37 pmol min-1 and 6-keto PGF1 alpha release 98 +/- 28 pmol min-1. All procedures increased PGE2 and 6-keto PGF1 alpha release in a fixed proportion of about 2.5:1, whereas renin release increased only during autoregulatory vasodilation.

Rapid effect of prednisolone on serum 1,25-dihydroxycholecalciferol levels in hypercalcemic sarcoidosis.

We have studied a hypercalcemic patient with sarcoidosis and advanced renal failure. Bone biopsy and urinary cAMP excretion indicated suppression of parathyroid function. 1,25(OH)2D levels were moderately elevated and dropped to low normal levels during prednisolone treatment. Discontinuation of prednisolone treatment caused deterioration of renal function and hypercalcemia, 1,25(OH)2D serum levels being within the normal range. Our data demonstrate the rapid speed at which prednisolone causes a drop in serum 1,25(OH)2D level. Since hypercalcemia was observed both during periods of hypercalciuria and normal serum 1,25(OH)2D levels, increased sensitivity to active vitamin D seems likely. There was no significant correlation between 25(OH)D, 24,25(OH)2D or 25,26(OH)2D. Furthermore there was no correlation between any of these three metabolites and either 1,25(OH)2D or serum calcium.

Dissociation between renal prostaglandin E2 and renin release. Effects of glucagon, dopamine and cyclic AMP in dogs.

To examine the relationship between prostaglandin E2 (PGE2) and renin release, glucagon, dopamine and dibutyryl cyclic AMP (DB-cAMP) were infused into dog kidneys during autoregulatory dilation of preglomerular vessels. Autoregulatory vasodilation, which enhances PGE2 and renin release, was induced by renal arterial constriction or ureteral occlusion. Glucagon infusion increased both PGE2 and renin release during autoregulatory vasodilation, and renin release was almost abolished after inhibiting PGE2 release by indomethacin. In contrast, dopamine and DB-cAMP infused during autoregulatory vasodilation increased renin release without significantly changing PGE2 release. Stimulation of renin release was not dependent on vasodilatory effects, which for all drugs were greatly diminished during autoregulatory vasodilation. Hence, glucagon stimulates both PGE2 and renin release. Most of the increase in renin release during glucagon infusion is prostaglandin-dependent since indomethacin greatly reduced the stimulatory effect. In contrast, dopamine and DB-cAMP stimulate renin release without increasing PGE2 release as previously found for beta-adrenergic stimulation.

Stereological analysis of human parietal cells before and 6 months after vagotomy.

Gastric biopsies from 10 patients were studied before proximal gastric vagotomy (PGV) and 6 months after the operation. The parietal cells were studied light microscopically and electron microscopically, using point-counting stereology. The volume fraction of the mucosa made up of parietal cells was significantly reduced (p less than 0.01) from 13.9% before PGV to 10.6% 6 months after PGV (median values). This reduction corresponded with a decrease of the median cell volume from 4698 microns3 before PGV to 3737 microns3 after the operation (p = 0.01). The ultrastructural composition of the parietal cells was virtually unchanged after the vagotomy, with the exception of a slight but significant reduction of the cell fraction occupied by mitochondria. The fractions of the parietal cell volume occupied by microvilli and tubulovesicular structures were significantly altered during stimulation with pentagastrin. These relative alterations were of the same magnitude before and after PGV.

Acetylsalicylic acid compared with acetylsalicylic acid plus codeine as postoperative analgesics after removal of impacted mandibular third molars.

In a multicenter, double blind clinical trial a combination of acetylsalicylic acid 500 mg + codeine phosphate 30 mg has been compared with acetylsalicylic acid 500 mg as postoperative analgesics in patients with pain after surgical removal of impacted mandibular third molars. Evaluation of the results from 129 patients showed that the combination of acetylsalicylic acid and codeine provided better pain relief and also the number of tablets used was smaller and the time intervals between repeated doses were longer than with acetylsalicylic acid only. Adverse effects were few and similar for both drugs. It may be concluded that the combination of 500 mg acetylsalicylic acid and 30 mg codeine phosphate provides a useful analgesic for more severe pain conditions in oral surgery.

Analysis of word comprehension in a case of pure word deafness.

A case of pure word deafness due to a left temporal infarct is reported. The results of dichotic tests suggest that auditory verbal material may be processed in the right hemisphere. The inability to repeat nonsense words, the frequent semantic paraphasias in real-word repetition tasks, and the capacity to give a partial account of the meaning of a word that the patient cannot repeat show that despite the impairment of the phonological analysis, lexical semantic processing is possible. An attempt is made to demonstrate that the patient resorts to this semantic processing and that this reflects the linguistic competence of the right hemisphere.

Transdermal scopolamine, oral meclizine, and placebo in motion sickness.

The efficacy of transdermal scopolamine, oral meclizine, and placebo in protection against motion sickness was compared in a double-blind crossover study. Thirty-six healthy subjects were exposed to motion three times for 90 min in a ship-motion simulator. Transdermal applications were made and tablets were taken at least 12 and 2 hr before exposure to motion. Transdermal scopolamine provided better protection than placebo or meclizine. Dryness of mouth was the only side effect reported more frequently for one regimen, transdermal scopolamine.

Morphometric studies of human parietal cells before and 7 days after proximal gastric vagotomy.

In nine patients, acid titration tests and gastric biopsy specimens were studied before proximal gastric vagotomy (PGV) and 1 week after the operation. When postoperative biopsy specimens were compared with the preoperative ones, no change was found in the thickness of the mucosa. The volume fraction of the mucosa made up of parietal cells was significantly reduced (p less than 0.01) from 13.8% before PGV to 11.2% after PGV (median values). Both before and after the operation, stimulation with pentagastrin was followed by a significant increase in the relative volume of microvilli in parietal cells and a significant reduction of the relative volume of tubulovesicular structures. The magnitude of this ultrastructural response to pentagastrin was similar before and after PGV. The relative volume of lysosome-like structures was significantly increased in parietal cells postoperatively. With this exception, no significant alteration was found when comparing the ultrastructure before and 7 days after PGV.

Longitudinal study of body height, radius length and skeletal maturity in Danish boys with cleft lip and palate.

Body height, radius length and skeletal maturity were registered longitudinally in 48 boys with combined cleft lip and palate (CLP) and compared with measurements in a non-cleft control group. Age range was from 6 to 20 yr. Skeletal maturity was evaluated according to the TW2 method. Distance and velocity curves for radius length and body height were determined by means of a computer program which for each month calculated the median and percentiles of the distribution. Compared to the control group the CLP boys were slightly shorter from 6 to 18 yr of age. The velocity curves revealed that pubertal growth maximum occurred on an average 6 months later in the CLP boys and that the pubertal growth spurt was less marked. However, the total growth period was prolonged thus enabling the CLP group to catch up with the control group. Skeletal maturity was retarded during the whole period when compared to normal children. It is suggested that the early feeding problems and recurrent infections of the upper airways combined with the surgical procedures account for an essential part of the delay in the present CLP group.
