Decreased fetal movements associated with umbilical cord complications.

In three of 1,094 cases of cord complications the mothers experienced reduction of fetal movments until cessation. In these three instances the fetal heart beat was audible but changes appeared on the fetal heart rate monitor. The course of loss of fetal movements resembled that seen in cases of placental insufficiency. It is suggested that the reduced fetal movements and the changes in fetal heart rate were due to a diminished blood flow in the cord vessels as a result of gradual cord compression.

Effect of indomethacin and cyproheptadine on onset of labour in rats.

An attempt was made to postpone term in 59 pregnant rats by s.c. injections of indomethacin or cyproheptadine, or a combination of both. The cyproheptadine group gave birth to their litters on days 20-22, yet indomethacin postponed labour to the 23rd day, both when given alone or in combination with cyproheptadine. As the fourth saline group went into labour on the 20--23rd day the indomethacin postponement cannot be considered significant. Indomethacin being a prostaglandin antagonist and cyproheptadine a serotinin antagonist, it may be concluded that neither prostaglandin nor serotonin are decisive for the intricate process which triggers parturition. Other factors, such as decrease of progesterone, increase of oestrogen and perhaps foetal oxytocin, as well as placental ACTH, seem to concur in inducing labour, their effect being fortified by serotonin and prostaglandins during parturition.

Serum prolactin in normal and pathologic pregnancy.

Serum prolactin values in normal pregnant women showed a progressive increase from a mean value of 50 ng/ml in the 12th week to 270 ng/ml at term, with the range at term being 100-600 ng/ml. There was a fairly good correlation (r = 0.7) between the values of 24-hour urine estriol in 138 determinations and in the serum prolactin in 133 pregnant women. The regression lines of serum hPRL values with time of gestation in cases of intrauterine growth retardation (IUGR) and diabetes mellitus were less steep than those seen in normal pregnancy. The serum hPRL value of patients with preeclamptic toxemia, latent diabetes, premature rupture of membranes, or multiple pregnancies were found not to differ significantly from the values observed in normal pregnancy. The results indicate that prolactin determinations in pathologic pregnancies are not useful as an aid in their evaluation.

Foam test, amniotic fluid cytology, creatinine and fetography in evaluation of fetal maturity.

Amniocenteses were performed in 73 pregnant women for maturity assessment in cases of high risk pregnancies, pregnancies of unknown gestational age and before an elective cesarean section. The evaluation of fetal maturity was based on: 1) foam test; 2)amniotic fluid cytology; 3)creatinine concentration; 4) fetography and 5) clinical criteria. The results which were assessed as to the proven infant maturity were as follows: Foam test 84.3%, lipid laden cells 75%; Creatinine 62.5% and fetography 56.3%.

Serum prolactin and the suppression of lactation.

Bromocriptine (2 bromo-alpha-ergocryptine), stilboestrol, clomiphene citrate, testosterone propionate and a placebo were given to 75 postpartum women for the suppression of puerperal lactation. An additional 15 women who breast-fed their babies served as a control group. Blood samples were taken for the determination of serum prolactin levels by a specific homologous double antibody radioimmunoassay. Concurrently, the clinical effectiveness of the various treatments was assessed. High levels of prolactin were found at the time of delivery. Bromocriptine effectively reduced serum prolactin and prevented lactation; stilboestrol increased serum prolactin and partially suppressed lactation; clomiphene citrate and testosterone propionate both lowered serum prolactin levels and partially suppressed lactation. The placebo showed almost no effect on serum prolactin. It appeared that bromocriptine was the drug of choice in the suppression of puerperal lactation.

The Soichet intrauterine device in the treatment of intrauterine adhesions.

IUD in the shape of a "Y" was inserted following curettage to 32 patients suffering from missed abortion or post-partum hemorrhage for the prevention of intrauterine adhesion formation, and to 48 patients suffering from sterility as a result of intrauterine adhesions. The rate of adhesion formation or recurrence of adhesions after their removal by curettage was very low and dropped significantly compared to other IUD's.

Preeclampsia and fetal well-being.

Thirty-three women with preeclamptic toxemia were retrospectively divided into three groups, according to clinical data, urinary estriol excretion, fetal growth, fetal movements and fetal heart recordings. In group 1 (six patients) there was no fetal growth retardation, and fetal motor function and heart rate were normal. In group 2 (17 patients) there was fetal growth retardation, estriol values were usually low, and fetal movements and heart rate were normal. In group 3 (10 patients) fetal movements decreased markedly almost until complete cessation for 12 to 24 h, and pathological changes were present in fetal heart recordings. A classification of preeclamptic toxemia according to these criteria is suggested. Patients who manifest placental metabolic failure, such as the women in group 2, should be hospitalized for observation only, as no immediate danger to the fetus is apparent. If there are signs of severe fetal distress and cardiomotor failure, such as in group 3, prompt delivery is essential to prevent fetal death in utero.

Prevention of postpartum venous thrombosis with low doses of heparin.

Postpartum superficial and deep venous thrombosis were found in 22.3% and 5.3% of women with varicose veins who delivered vaginally, or by cesarean section. The preventive use of low doses of heparin (5,000 U every 12 h injected subcutaneously) in 116 parturients with varicose veins reduced significantly the incidence of postpartum venous thrombosis. None of the 26 women with previous thrombosis who received preventive treatment, developed such an event.

The effect of methylxanthines on chromosomes of human lyphocytes in culture.

The effect of caffeine (I,3,7-trimethylxanthine), theophylline (I,3-dimethylxanthine), theobromine (3,7-dimethylxanthine), paraxanthine (I,7-dimethylxanthine) I-methylxanthine, 3-methylxanthine, and 7-methylxanthine added at the 48th h on the chromosomes of human lymphocytes in 72-h cultures has been investigated. Caffeine and the dimethylxanthines cause breakage at 750 mug/ml, with caffeine the most, and paraxanthine the least clastogenic. I-Methylxanthine and dimethylxanthines with a methyl group in the I-position are the most effective in depressing mitotic indices. No chromosome damage was exhibited by the monomethylxanthines.

Reduction of fear-related dental management problems with use of filmed modeling.

In this study of the modification of anxiety-related disruptive behavior in dental treatment, matched groups of inner-city children attending a pedodontic clinic were shown a videotaped demonstration of a 4-year-old black child undergoing a dental restorative procedure or were given an unrelated drawing task before dental treatment. Children who viewed the videotape demonstration of a peer model coping with dental procedures showed significantly fewer fear-related disruptive behaviors during restoration of lesions. Observations of children's anxiety levels made by dentists and independent observers validated the effectiveness of viewing the videotaped demonstration. No significant correlation was found between the children's reports of their anxiety and their behavior during dental treatment.

Puerperal endometritis and intrauterine adhesions.

The role of puerperal endometritis in intrauterine adhesion formation was studied by hysterography in 171 women who had cesarean sections. Of 28 patients who developed significant endometritis, only one developed intracervical adhesions. In the control group of 143 cases, there was also only one such case. Endometritis alone apparently does not play a significant role in intrauterine and endocervical adhesion formation. The possible role of placental fibroblasts in preventing endometrial regeneration is discussed.

The role of wedge resection of the ovary as a cause for mechanical sterility.

The role of wedge resection of the ovary in inducing secondary sterility of a mechanical nature has been evaluated. Two groups of patients were studied. Fifty-seven patients underwent wedge resection of ovary because of symptoms of the Stein-Leventhal syndrome. Of these, eight were found to have extensive peritubal and periovarian adhesions after operation. In the second group, a retrospective study was made. One hundred and ninety-eight women were known to have mechanical sterility. Of these, 28 gave a history of bilateral wedge resection of ovaries because of the Stein-Leventhal syndrome. All pelvic operations for correction of sterility carry a considerable risk of adhesion formation.

Teratogenicity of cyproheptadine in pregnant rats.

The teratogenicity of cyproheptadine HCL (Periactin) was studied in 3 groups of pregnant rats which received respectively 0.5 mg/kg/day, 2.5 mg/kg/day and 5 mg/kg/day cyproheptadine subcutaneously and in a control group of pregnant rats which received 0.2 ml/day of the solvent normal saline. Higher doses were toxic to pregnant rats. There was no significant difference in the birth rate, foetal resorption rate and malformation rate between the 4 groups. Moreover, there was no significant difference between average weights, average foetal placental weights and male/female ratio in the experimental and control foetuses. It is concluded that cyproheptadine is devoid of any teratogenic effect in rats.
