Aggressive meningioma with jugular vein extension. Case report with ultrastructural observations.

A meningioma that was seen initially as a mass in the upper portion of the neck in a 44-year-old woman was studied by light and electron microscopy. Light microscopy demonstrated that the tumor was within the lumen of the internal jugular vein. Ultrastructural features led to the diagnosis of meningioma. The clinical findings indicate that the tumor arose within the cranial cavity, invaded a venous sinus, and extended down the internal jugular vein into the upper portion of the neck.

Superior laryngeal neuralgia.

A 46-year-old patient had a right superior laryngeal neuralgia. After extensive investigations for laryngeal, dental and submandibular gland disease, the condition was treated successfully with carbamazepine.

Enzyme polymorphism in the classification of human malignant lymphoma.

Recent studies on the polymorphism of lysosomal hydrolases have shown that all individual blood cell types in the human being possess their own isoenzyme pattern. In the present study acid phosphatase activity of normal human B-lymphocytes and of four different types of low-grade malignant non-Hodgkin's lymphomas according to the Kiel classification was estimated. In addition, the isoenzyme pattern of AcP was investigated by isoelectric focusing. Chronic lymphocytic leukemia of B-cell type (N = 9) and centroblastic centrocytic follicular lymphoma (N = 10) demonstrated significantly lower values than lymphoplasmacytic/lymphoplasmacytoid lymphomas (N = 28) and plasmacytomas (N = 8). The isoenzyme pattern of normal human B-lymphocytes comprised 12 bands between pH 6.3 and 3.85. This basical pattern was shared by all four lymphoma entities. Only lymphoplasmatic lymphoplasmacytoid lymphoma and plasmacytoma revealed additional bands, which probably account for the higher net enzyme activity in these cases.

Septic shoulders in children.

A review was carried out on nine children who were diagnosed and treated for septic arthritis of the glenohumeral joint at the University of California, San Diego affiliated hospitals. Eight of the nine patients were under 18 months of age at the time of diagnosis. Three patients had a history of antecedent trauma, and four had a previous or concurrent source of infection. The longest delay between the onset of symptoms and diagnosis was 30 days with the majority being diagnosed within 4 days of onset. Laboratory findings revealed a mean white blood cell count of 17,400 and a mean temperature of 39.3 degrees C. The sedimentation rate was always greater than 30 mm and averaged nearly 60 mm in these nine patients. Joint cultures were positive for a variety of organisms. Early radiographic findings often demonstrated widening of the joint space. Associated osteomyelitis of the proximal humerus was present in five of nine patients. Once the diagnosis has been established by aspiration, we recommend arthrotomy. Important aspects of the surgery include drainage of the joint and bicipital recess, and drilling of the proximal humeral metaphysis to rule out osteomyelitis or to more adequately decompress the bone.

[Diaplacental influence on the disseminated intravascular coagulation (DIC) in an asphyxia model. I. Influence on the vasal tonus and on the vasal wall (author's transl)].

With the aid of an asphyxia model in rabbit fetuses the possibility of diaplacental influence on the hypoxic effects of the fetal organism has been investigated. The trial to change the vasotonia during the asphyctic shock by sympathicomimetic or -lytic drugs (Dilatol, Obsidan, Ergocomb, Regitin) remained to be without any effect on the DIC in the fetal organism. Trying to seal the vascular wall by the high-molecular-weight basic polypeptide Aprotinin there seemed rather to be an increase of the intravascular fibrin sedimentation.

[Diaplacental influence on the disseminated intravascular coagulation (DIC) in an asphyxia model. II. Influence on the vessel's content (author's transl)].

There is reported on the possibilities of diaplacental influencing on the DIC arising from the hypoxia we provoked with the help of an asphyxia model in rabbit fetuses. We could demonstrate the impossibility of influencing on the fetal vasal content by means of low molecular dextran, heparin or streptokinase. On the contrary, the possibility exists to influence on the fetal hemostasis by low molecular synthetic substances. The antifibrinolytic drug PAMBA causes in case of advanced acidosis diaplacental a definite growth of the intravascular coagulation, as well. But an inhibition of the fibrin sedimentation in the terminal vessels at the asphyctic shock can be achieved by applying the ethyl ester of the 4-amidinophenylpyruvic acid (APPA).

[Animal experiments on direct drug effects on disseminated intravascular coagulation (DIC) in asphyxic shock].

In a total of 41 rabbit fetus it could be demonstrated in an asphyxia model to what extent disseminated intravascular coagulation may be influenced by directly applying drugs in the foetal circulation. Administration of heparin or streptokinase in the umbilical vessels led to a decrease of fibrin sediments in the areas of terminal coagulation, whereas an increase was caused by injecting noradrenalin or PAMBA.

[Prepartal cardiotocographic monitoring of high-risk pregnancy (author's transl)].

Non-stress and oxytocin exposure tests were applied to 321 patients with hypertensive late gestoses for cardiotocographic monitoring of their high-risk pregnancies, over two years. No prenatal and intranatal deaths of newborns were recordable in cases in which non-stress tests were repeated in two-day intervals together with oxytocin exposure tests, where indicated. Thirty-minute recordings of cardiotocography in two-day intervals, with particular attention to foetal movements, are recommended for routine prepartal monitoring of high-risk pregnancies. Oxytocin exposure tests should not be used unless indicated.

[Results of eclampsia treatment--analysis of 44 cases over 20 years (author's transl)].

Reported in this paper are 44 cases of eclampsia recorded from among 61,809 deliveries at the Gynaecological and Obstetric Hospital of the Medical Academy of Erfurt, between 1960 and 1979. Five mothers died and two more suffered permanent damage. Two infants died perinatally. However, an estimate of eclampsia risk to patients with gestosis cannot be derived from the intensity of the gestosis symptoms and thus from the gestosis index (Goecke).

Activity and isoenzymes of acid phosphatase in human B-cell lymphomas of low-grade malignancy: a novel aid in the classification of malignant lymphoma.

Activity and isoelectric focusing (IEF) pattern of lysosomal acid phosphatase (E.C.3.1.3.2.) were investigated in 55 cases of low-grade malignant B-cell lymphoma, classified as chronic B-lymphocytic leukemia (B-CLL), centroblastic/centrocytic follicular lymphoma (CB/CC), lymphoplasmacytic/lymphoplasmacytoid lymphoma (Immunocytoma, IC), and plasmacytoma (PC), applying the criteria of the Kiel classification. The results show (1) that the four lymphoma types present a characteristic range of enzyme activity in an increasing order: B-CLL, BC/CC, IC, and PC. B lymphocytes, germinal center cells, and plasmacytes are the main constituents of these lymphomas. This sequence might reflect one possible mode of B-cell transformation into plasmacytes traversing an amplification stage in germinal centers under normal conditions. (2) All cases showed the basic IEF pattern of normal B lymphocytes with 12 bands localized in three regions between pH 6.1 and 3.9. This finding supports the B-cell origin and the close phenotypical relationship among the investigated lymphomas. (3) The IEF patterns of B-CLL and CB/CC did not differ from that of normal B lymphocytes, whereas two additional isoenzymes were encountered in cases of IC and seven in PC; this suggests that the higher enzyme activity of IC and PC is at least partly due to the appearance of "new" isoenzymes. The results support the validity of the underlying classification and indicate the individually, B-cell origin, and close relationship among the four lymphoma entities investigated.

Test of a pion range monitor using the radiative capture of pions on protons pi-p leads to n gamma.

The present paper demonstrates that neutron-photon pairs from radiative capture of stopped pions on chemically bound protons can be used to measure the range of negative pions within phantoms or a patient. Experimental results are given for a polyethylene and a water target of realistic size as well as for a Rando phantom. Monte-Carlo calculations were carried out in order to study the influence of various sizes of treatment volumes, detector geometries and neutron scattering within the targets upon the accuracy of the pion range determination. The results reveal clearly that a pion range monitor for the control of therapy plans and for actual patient irradiations can be designed according to the proposed principle. The absorbed dose required for a measurement is of the order of 0.1 Gy for a single pion beam if one aims at an accuracy of range determination of a few millimeters.
