Serological study of amoebiasis and toxoplasmosis in the Malili area, South Sulawesi, Indonesia.

Results of a serology survey in September 1972 for Entamoeba histolytica and Toxoplasma gondii antibodies among 915 volunteers from the Malili area of South Sulawesi (Celebes), Indonesia are presented. Indirect hemagglutination antibody titers for amoebiasis were found in 22.7% of the sampled population while 9.5% demonstrated antibody titers greater than or equal to 1:128. The frequency distribution of antibody titers were similar by sex and age. There was no significant correlation between stool positivity and seropositivity. Indirect hemagglutination antibody titers for T. gondii were detected in 62% of the population sampled, and titers greater than or equal to 1:32 were demonstrated in 29% of the samples tested. The distribution of antibody titers was similar in males and females. In both sexes the prevalence of IHA antibody titer against T. gondii increased with age. There was no consistent association between altitude and prevalence of seropositive samples.

Intracranial aneurysms: a clinical, angiographic and computerized tomographic study.

Fifty-six patients with 63 aneurysms were selected from a larger group of patients because they had at least one angiographic and one CT scan study. A total of 102 CT scans and 136 angiograms were analyzed. In five patients the aneurysm was visualized on the CT scan. There was a tendency for the location of the subarachnoid hemorrhage and intracerebral hematomas visualized on the CT scans to correlate with the site of the aneurysm as seen on the angiogram. The CT scan was useful in demonstrating the evolution of hydrocephalus and in differentiating hematoma from edema. The patients with normal CT scans were more likely to have good neurological grades. No single angiographic or CT scan feature correlated significantly with survival at lower than the 5 percent level.

Effect of isoprenaline and nitroglycerine on pressure time indices and coronary graft blood flow in man.

In five patients studied 1 to 3 d after coronary artery surgery isoprenaline and nitroglycerine have been used to alter the systolic (TTI) and diastolic (DPTI) pressure time indices. A close correlation with the predicted relationship between diastolic coronary graft flow and the DPTI/TTI ratio has been demonstrated during isoprenaline-induced tachycardia. Nitroglycerine reduced diastolic coronary graft resistance and increased the DPTI/TTI ratio, effects which are desirable for the relief of angina.

Biphasic response on oral glucose tolerance testing in myotonic dystrophy.

Oral glucose tolerance tests have been carried out on 40 patients with myotonic dystrophy, and 50 hospitalized control patients. A biphasic response was found in 19 myotonic dystrophy patients (49 per cent), compared with 12 of the control population (24 per cent). In 9 patients with myotonic dystrophy (23 per cent), the height of the second peak was greater than or equal to 20 mg/100 ml, the corresponding figure for the control group being 2 patients (4 per cent) (P less than 0.005).

Development of the psychopathology instrument for Mentally Retarded Adults-Sexuality Scale (PIMRA-S).

Little research or attention has been focused on identifying sexual problems or difficulties that people with mental retardation commonly experience. Scale development represents an important area for study to help identify these problems and to evaluate treatment outcome. To address this need, the Psychopathology Instrument for Mentally Retarded Adults-Sexuality Scale (PIMRA-S) was designed to assess psychosexual disorders in mild and moderate mentally retarded persons. Eighty-six mild and moderately mentally retarded adults, ages 20 to 60, were studied using the PIMRA-S. Scale development of this type was considered important because little has been done to assess sexual problems among mentally retarded persons. Fifty-eight items were developed based on evaluations of the research literature and interviews of experienced professionals. A preliminary assessment of reliability was conducted. The psychometric characteristics of these preliminary analyses were favorable. In addition, information was reported on the rate of sexually aberrant behavior exhibited by people identified with mild and moderate mental retardation. Differences in the rate of sexually aberrant behavior were addressed as a function of living and work placement, positive history of sexual abuse, need for treatment of sexual problems, and psychiatric diagnosis. Implications of these results for further scale development are discussed.

Expression and characterization of bryodin 1 and a bryodin 1-based single-chain immunotoxin from tobacco cell culture.

Bryodin 1 (BD1) is a potent ribosome-inactivating protein (RIP) isolated from the plant Bryonia dioica. It is relatively nontoxic in rodents (LD50 > 40 mg/kg) and represents a potential improvement over other RIPs and bacterial toxins that have been used in immunotoxins. Recombinant BD1, expressed in Escherichia coli, localizes to insoluble inclusion bodies necessitating denaturation and refolding steps to generate active protein. In this report, BD1 was expressed as a soluble recombinant protein in tobacco cell culture (ntBD1) and purified to near homogeneity with yields of up to 30 mg/(L of culture). The protein synthesis inhibition activity of ntBD1 was identical to that of both native BD1 isolated from the roots of B. dioica and recombinant BD1 expressed in E. coli. Toxicology analysis showed that ntBD1 was well tolerated in rats at doses that cannot be achieved with most other toxin components of immunotoxins. Additionally, a single-chain immunotoxin composed of BD1 fused to the single-chain Fv region of the anti-CD40 antibody G28-5 (ntBD1-G28-5 sFv) was expressed in tobacco tissue culture as a soluble protein and was specifically cytotoxic toward CD40 expressing non-Hodgkin's lymphoma cells in vitro. These data indicate that tobacco tissue culture is a viable system for soluble expression of BD1 and BD1-containing immunotoxins.

Technical note: preventive maintenance quality assurance.

Performance improvements in healthcare and, specifically, in clinical engineering are required by the Joint Commission on the Accreditation of Healthcare Organizations' Plant Technology and Safety Management (PTSM) documents. The goals of healthcare service quality assurance are to establish performance requirements and improve patient outcomes. It is important that these goals be kept in the forefront of the design of clinical engineering quality assurance planning. Indicator/threshold pairs must be carefully chosen on the basis of some logical relation to the desired goals of the overall program.
