Measurement of hand bone mineral content using single-photon absorptiometry.

A single photon absorption imaging technique has been developed to assess the bone mass of the hand, especially in patients with rheumatoid arthritis or bronchial asthma. A modified rectilinear scanner images the hand by transmission scanning in a water bath with a 7.4 GBq 125I source. A microcomputer is used to calculate the bone mineral distribution, and the total bone mineral content (BMC) of the hand is determined from that distribution. The precision (coefficient of variation) of the measurement is 1.9%. A control population of 20 men and 58 women has been studied to determine normal variations in hand bone mineral content with age, sex, body size, hand volume and years since menopause. The normal men are found to have an average hand BMC of 25.1 g with a coefficient of variation (CV) of 22%, which is reduced to 12% by normalising for body size using span. The normal women had an average hand BMC of 18.0 g +/- 15%. The CV is reduced to 13% by normalising for span and years post-menopause.

Anisoylated plasminogen streptokinase activator complex versus placebo. A preliminary multicentre study of safety and early mortality in acute myocardial infarction.

90 patients were enrolled into this preliminary multicentre study of the efficacy and safety of 30 units intravenous anisoylated plasminogen streptokinase activator complex (APSAC) compared with placebo in patients with acute myocardial infarction. 45 patients received APSAC and 45 placebo; the groups were similar for age, weight and site of infarction. There were significantly more women treated with APSAC (p less than 0.02). The mean time to treatment was 3.3 hours after symptoms of myocardial infarction for APSAC and 3 hours for placebo. The 30-day mortality was 7 patients in the placebo group and 1 in the APSAC group (p = 0.058). Adverse events were generally minor and were of similar overall frequency in both groups. There were more haemorrhagic events with APSAC, from which all patients recovered, and more cardiovascular events with placebo including 2 deaths from cardiogenic shock. APSAC showed a trend towards a reduction in 30-day mortality. Experience from this study has led to the initiation of the APSAC in myocardial infarction multicentre mortality study (AIMS).

Discriminative stimulus properties of beta-phenylethylamine, deuterated beta-phenylethylamine, phenylethanolamine and some metabolites of phenylethylamine in rodents.

The discriminative stimulus (cue) properties of phenylethylamine (PEA) were analysed in rodents in a conventional two lever FR10 operant drug discrimination task. Rats trained to discriminate phenylethylamine at 30 mg/kg showed complete dose-related generalization to PEA and to two potential PEA metabolites: phenylethanolamine (PEOH) and N-Methyl PEA (NMPEA). Only partial (50%) generalization was seen with N-Methylphenylethanolamine (NMPEOH), another potential PEA metabolite. The specificity of PEA's action as a discriminative stimulus was demonstrated by the finding that fenfluramine, a substituted phenylethylamine, failed to generalize to PEA even at high doses with marked behavioural effects which are known to have discriminative stimulus properties themselves. These data suggest that NMPEA and PEOH may be functionally important active metabolites of PEA, particularly if the major pathway of PEA metabolism to phenylacetic acid under the influence of MAO Type B is for any reason impaired. A long acting deuterium substituted form of PEA (alpha, alpha, d2 PEA), which is resistant to metabolism by MAO, produced complete dose-related generalization to the PEA cue but was more potent than PEA, due presumably to its resistance to metabolism by MAO. Deuterated PEA may therefore be a useful agent to use in future studies of the PEA cue, because the discriminability of PEA itself appears to be low due to its very rapid metabolism in vivo.

Rubella: immunity and vaccination in schoolgirls.

Of 191 schoolgirls, 128 volunteered to take part in a feasibility study of serotesting before and after rubella vaccination, and all responded to RA 27/3 vaccine. Had the serum samples been taken by a fingerprick method the number of volunteers would probably have increased considerably. A change in policy for rubella vaccination to testing both before and after vaccination would cost no more than the existing policy, would ensure primary response, and would differentiate those women who were protected by the vaccine from those with antibody to wild virus.

Illness associated with a package holiday in Romania.

A study of 370 holidaymakers returning from Romania revealed that 279 (75%) reported illness. Alimentary symptoms predominated and were recorded either alone or along with other symptoms by 71 per cent of the tourists. The highest illness rate (82%) occurred in those under 39 years of age and those over 60 years had least illness (38%). Most of the tourists attributed their illnesses to the supply, handling or preparation of food and drink. Twenty-six (21%) tourists had serological evidence of typhoid immunisation out of 121 from whom blood samples were obtained. Most of the tourists studied (85%) were immune to poliomyelitis.

Biologic properties of factor-independent nonadherent hematopoietic and adherent preadipocyte cell lines derived from continuous bone marrow culture.

Cell lines dependent for growth upon an inducer T-cell synthesized glycoprotein factor interleukin-3 have been derived from continuous mouse bone marrow cultures. These factor-dependent (FD) lines have been shown to be multipotential (erythroid/basophil/neutrophil) or (eosinophil/basophil/neutrophil); or are unipotent basophil or neutrophil granulocyte cell lines. Both classes of cloned FD lines have maintained self-renewal in vitro for several years with absolute growth dependence on freshly added IL-3. In four instances, factor-independent (FI) variant cell lines were derived, one by subculture in medium containing hydrocortisone and 25% horse serum and three by evolution of variants from cloned FD lines. One class of (FI) lines demonstrated adherent fibroblast-like morphology with differentiation to differentiated adipocytes in medium containing 10(-5) hydrocortisone. A second class of cell lines evolved from cloned FD lines and each grew in suspension culture to a saturation density over 10-fold greater than that for the parent FD line (greater than 10(7)/ml) and each contained no detectable hematopoietic cellular differentiation markers by histochemistry or cell surface receptors. In contrast to IL-3 dependent cell lines, (FI) cell lines failed to differentiate to mature granulocyte morphology in diffusion chambers in vivo. The FI cell lines formed no detectable CFUs in vivo, did not reconstitute hematopoiesis in irradiated mice and did not form tumors in vivo. The failure of the (FI) lines to form tumors and lack of detectable hematopoietic differentiation capacity indicates that these lines may represent an intermediate state between normally regulated hematopoietic cellular self-renewal and malignant transformation.

Surveillance of hepatitis B virus infection in Scotland, 1973-1982.

Since 1973 epidemiological surveillance of laboratory-confirmed hepatitis B virus infection has been undertaken in Scotland. During the ten-year period, 1973-82, 2893 persons with laboratory evidence of infection were reported and the number increased by almost threefold between the beginning and the end of this time. Males accounted for 66% of the patients and intravenous drug abuse was the most commonly encountered risk factor. The low risk to laboratory staff is confirmed, but among National Health Service hospital staff nurses accounted for 54% of those reported.

Interleukin 3-dependent hematopoietic progenitor cell lines.

Several biological phenotypes of growth factor-dependent cell lines have been described in recent years, including those with T lymphocyte, neutrophil granulocyte, basophil/mast cell, B lymphocyte, and multipotential stem cell properties. The growth factors for each cell lineage are a subject of intense study. Continuous mouse bone marrow cultures infected with RNA type C viruses (retroviruses) produce nonadherent hematopoietic cells over a longer duration than control cultures. Marrow cultures derived from strains with spontaneously induced ecotropic endogenous retrovirus demonstrate a greater longevity than those from strains with no replicating virus. Cultures infected with murine leukemia virus also generate a greater number, compared with controls, of cloned permanent suspension cell lines dependent for growth on a 41,000-dalton glycoprotein (interleukin 3 [IL 3]). Some are multipotential with capacity for differentiation to erythroid, neutrophil, eosinophil, and basophil/mast cell types. Other cloned IL 3-dependent cell lines are committed to a single pathway. Studies with Friend spleen focus-forming virus indicate that the first effect in the marrow culture is mediated through a subset of adherent hematopoietic stem cells. Bone marrow culture-derived IL 3-dependent cell lines provide a model with which to study the role of viral genes in the control of differentiation and self-renewal capacity of hematopoietic stem cells.
