The continuous occurrence of particularly severe epileptic attacks represent a great stress for the patients. For this reason, we have conducted anticonvulsant blood-level studies particularly in these patients, this paper being based on 146 first determinations. Among the patients, most of whom are adults and are treated with combinations of drugs, only 24 per cent reached the assumed normal phenytoin range while 69 per cent remained below it. With the administration of phenobarbital, however, 50 per cent reached the therapeutic range and only 14 per cent reached the lower limiting value. It is only i the last-mentioned group that we assumed primarily an insufficient intake of the drugs while with DPH the problems of the determination of the plasma water range had to be discussed. In practice, blood level determinations have their special value for the detection of the intake of a wrong dosage of the drugs as well as bland anticonvulsant intoxications.
In the renal dispensary of the medical polyclinic of the County Hospital Cottbus 6 physicians care for 3,640 patients. Here a group of 74 prospective dialysis patients plays a particular role, who are systematically prepared to the dialysis by psychosocial care measures by a team, consisting of the physician, the psychologist, the social worker and the assistant dietician. The care system became possible by the concentration of this group of patients in the county dispensary and it gives the therapy team the necessary time for the solution of the specific tasks. By the findings and experiences established in the result of this team work a decisive influence may be done on the later conduction of the patients in the dialysis programme.

The diagnosis of peripheral nerve lesions remains difficult despite the sophisticated technology which is currently available. Experience with the Doppler, photoelectrical plethysmography and flow studies is analysed. Recording of antidromic sensory action potentials in dependence on acral thermometry as the only objective diagnostic procedure proved correct when compared with the indirect procedures via arterial vascular system, namely Doppler, photoelectrical plethysmography and flow studies. The application of the above techniques in a case report with sphygmographic assessment is described.

Allyl isopropyl acetamide (AIA) is believed to destroy P-450 and to enhance the d-aminolevulinic acid synthetase (ALAS) activity by lowering the heme pool of the liver parenchymal cell, in this way weakening the negative feed back mechanism, which regulates ALAS synthesis at the translational level. Dose and time dependence of the AIA influence on cytochrome P-450 concentration and P-450 activity argues against this theory, supporting the hypothesis that AIA destroys P-450 only after administration of higher doses (> 300 mg/kg i. p.), but does not influence or even enhances P-450 concentration when given in lower doses which exert an increase in ALAS activity. 25--100 mg/kg AIA i. p. have no significant effect on ALAS activity, 200 mg/kg and higher doses up to 400 mg/kg cause dose-dependent increases in ALAS activity.
A report is given on the possible therapeutic influence of diphenylhydantoin on dystrophic myotonia. After studying the correlations of the clinical-neurological findings, EEG changes and electromyo-electroneurographic findings with respect to the DPH serum concentration, it was found that a long-term therapy is only indicated in individual cases and when the patient is taken thorough care of.

From 1968 until 1977 in 148 patients 202 arterio-venous fistulas for chronic hemodialysis were performed 41 out of them in the angle of the elbow. By the fistula in the angle of the elbow the V. mediana basilica is sutured end to side of the A. brachialis. This fistula is recommended, when the performance of a fistula in the forearm is failed.

The zinc content of the liver and the thigh muscle of the rat were measured by means of atomic absorption spectrophotometry under different conditions: standard diet, carbohydrate diet, an experimental dystrophy model and its recovery period. The influence of a parental mineral substitution on the whole body and the organ weight was studied. Carbohydrate diet caused a lower rise of the whole body weight, a decrease of the zinc content of the liver and an increase of the zinc content of the thigh muscle. In dystrophy model, the liver weight decreased to one fifth of the controls. The zinc concentration of the liver increased markedly, the zinc content of the whole liver is equal to that of control animals. The thigh muscle showed an increase of the zinc content, too. The parenteral mineral application had no effect upon the whole body weight and the organ weights. Little influences on the zinc content of the skeletal muscle were seen. The zinc content of parenchymatous organs varies markedly, and it is influenced by diets and disease. In the dystrophy model, zinc is concentrated in atrophic organs in order to protect the organism against zinc deficiency.
Eleven women with menopausal symptoms were treated with 150 micrograms Clonidine (dixarit) daily. Before and during therapy, plasma estradiol-17 beta, LH, FSH and prolactin levels were measured by specific radio-immunoassays. In addition, each patient recorded the number of hot flushes a day and symptoms were monitored by the Kupperman-index. A highly significant fall in the number of hot flushes by day and night occurred during therapy (p less than 0.001) and the Kupperman-index similarly improved. Significant pulse and blood pressure changes were not noted and plasma hormone concentrations remained unaltered. Medication was well tolerated. Clonidine therapy would appear to be the treatment of choice for menopausal symptoms if estrogens are contraindicated.
