[Cloverleaf skull deformity with thanatophoric dysplasia in the 24th week of pregnancy].

A case of cloverleaf skull anomaly with thanatophoric dysplasia could be examined pathologico-anatomically in the age of 24 weeks of gestation. There were typical radiological and morphological signs of a mild form of thanatophoric dysplasia. Additionally a cloverleaf skull anomaly with premature fusion of sutures and a disturbed ossification without any signs of haemangiomatosis of the desmocranium was found. A hydrocephalus was not demonstrable.

[The assessment of disability in epileptic patients].

Report is given concerning 16 epileptics in whom annuity could be suspended. Types of seizures did not differ from those analysed in a formerly study of further invalid epileptics. On the contrary, the beginning of treatment was very soon after manifestation of disease, and the treatment was in the hand of neuropsychiatrists with mostly good or very good attention. Improvement could be obtained mainly by decrease of seizure frequency of freedom from seizures, and after few years invalidity could be annulled.

[Significance of the EEG for the prognosis of therapy in adult epileptic patients].

The importance of EEG findings regarding prognosis of therapy and relapses of epilepsies is controversally discussed in the literature. On the basis of 495 patients being in attendance for many years there were some important indications for prognosis, for instance 59 percent of the patients with normal EEG at all times during the cours attained freedom from seizures for at least five years. The best possibilities for differentiation followed from EEG features during medication, especially from degree od disturbances of background activity. Only 13 percent of patients with more than slight disturbances in resting EEG remained seizure free for a long time. Also as a distinctly unfavourable sign proved the combination of generalized and focal epileptiform pattern.

[Morphometric studies on numerical area density of glomerular cells (nuclear section) in the pathogenetic evaluation of minimal glomerular abnormalities].

Minor glomerular abnormalities (MGA) with hypercellularity (according to the WHO-nomenclature) are one of the most frequent glomerular lesions. Minimal proliferative changes in the glomerular structure can be proven only with the aid of morphometric methods. Morphometric glomerular studies were carried out in MGA with hypercellularity and with rare or minor deposits of immunoglobulins and/or complement (C3) or without deposits. Clinical symptomatology was variable (minimal to mild proteinuria, massive proteinuria, haematuria or haematuria with proteinuria). The groups examined were divided as follows: MGA without deposits and with minimal (mild) proteinuria. MGA with minor deposits and with minimal (mild) proteinuria. These two groups were statistically compared with: normal glomerular structure (after Wehner 1974); MGA with massive proteinuria or nephrotic syndrome (includes the minimal changes with nephrotic syndrome), which showed minor deposits of complement; diffuse mesangial proliferative glomerulonephritis (IgA-nephritis). The glomerular cell density, i.e. the total number of glomerular cells, averaged 3.48 cells/1,000 micron 2 in the first group and 4.26 cells/1,000 micron 2 in the second group. In normal renal corpuscles the average was 2.30 cells/1,000 micron 2. The first group has an average of 51% and the second group has an average of 84% higher cell density than normal renal corpuscles. The cell density in diffuse mesangial proliferative glomerulonephritis (IgA-nephritis) is increased by 160% compared with the norm. The first group was not statistically different in cell density from MGA with high proteinuria. We conclude, that MGA with hypercellularity and minimal to mild proteinuria represents a minimal mesangial proliferative glomerulonephritis.

[Results of a multicenter study of the early detection of glomerulonephritis. 1].

In the framework of a multicentric retrospective study between selected clinics of the Republic Austria and the GDR anamnestic, clinical and paraclinical data were investigated in their valency for the early recognition of glomerulonephritis. Data of 583 patients were evaluated. Hereby it was shown that independent of the size of excretion and the reproducibility the findings "proteinuria" are of particular significance for the early recognition. The serological investigations usually performed within the diagnostic of glomerulonephritis proved as insignificant for the early recognition. Since the establishing of an exact diagnosis is up to now possible only with the help of invasive methods, a call on research is made to develop reliable, non-invasive diagnostic methods.

[Hepatitis prevention and its results--report of experiences in a dialysis department].

The results show that, taking into consideration the specificity of a dialysis ward, when the factors of hepatitis prophylaxis are performed consistently with the help of so-called usual measures the hepatitis contamination may decisively be influenced. They indicate that in the period from 1982 to 30 November 1985 neither in the staff nor in the patients the HbsAg was proved or the clinical picture of an infectious hepatitis appeared. Apart from the consistent observance of existing dialysis-specific sanitary rules the characterized infection dialysis unit and the dietary regimen described in a particular degree might have contributed to this result. The fundamental attitude described should be maintained also after active immunization, since unsuccessful vaccinations are known and the Non-A-non-B-hepatitis is by no means without importance for dialysis wards.

[Computer tomography identification of carriers of Duchenne progressive muscular dystrophy].

Comparison of findings obtained by computed tomography of muscles of possible carriers and normal subjects demonstrated that structural changes existed in 31 out of 74 women; the certainty of the information obtained became greater with increasing age. Six of the seven genealogically definite carriers were detected by computed tomography. The present results show that the use of computed tomography is promising for future carrier diagnosis.

[Iatrogenic peripheral nerve damage and its forensic evaluation].

Iatrogenic peripheral nervous lesions represent a regrettable complication of medical therapy. Legal regulations of the GDR make it possible for the practicing physician to remain an advocate of the patient's interests. The results of an analysis of 11 years of activities in the Magdeburg County Commission of Medical Experts are presented.

[Incidence and early detection of glomerulonephritis].

Within of a for the first time performed aimed study of the early recognition and frequency of the glomerulonephritis in 1,425 males the findings of an "asymptomatic proteinuria" were proved in 5.26% of the affected persons. As a result of a comprehensive investigation, the close of which was in every case the biopsy of the kidney with the histological examination of the renipuncture cylinder, a frequency of glomerulonephritis of 2.8% was found. Within this investigation the findings of an "asymptomatic proteinuria" proved to be the decisive parameter independent of the size of excretion and the reproducibility. The serological investigations did not show any specifity and proved to be insignificant for the early recognition. The results confirm that the diagnosis of "physiological proteinuria" may be made only then when there is an inconspicuous histological finding. In lacking renal histology every form of the proteinuria should at first be attributed to glomerular process.

[Computer tomography studies in myotonic dystrophies].

Besides clinical symptoms, progress and electromyography the computer tomography is a possibility for improvement the diagnostic of myotone dystrophy. Even small changes in muscle structure are detectable and especially m. soleus exhibits early and pronounced alterations. By means of density distribution pattern an improved characterization of the disease is possible. Additional informations are obtained by cerebralcomputertomography. Hints for atrophy of brain tissue are expectable at all patients with myotone dystrophy. According to the presented findings the computer tomography enlarges the diagnostic possibilities at myotone dystrophy.

[Neuromyotonic syndrome].

A syndrome with constant rest activity was observed in the EMG of a 28-years-old female patient. An H-reflex was detected for normal nerve conduction velocity. Hypomagnesemia was striking among the laboratory values. Bioptic investigation yielded centralisation of nuclei, variations in the diameter of muscle fibres and a predominance of type I fibres. The findings and the response to Finlepsin point to neuromyotonic syndrome.

The pathway of E. coli RNA polymerase-promoter complex formation as visualized by footprinting.

The pathway of E. coli RNA polymerase-promoter complex formation was probed by characterization of low temperature intermediates at the major coat protein promoter of phage fd DMA. Three different complexes could be distinguished. One of them represents the active 'open' complex, the other two have to be regarded as 'closed'. The promoter contacts of the lower temperature complexes were totally comprised within the contacts of the higher temperature complexes. Increase in temperature led to extension of contacts into the downstream direction, while the upstream border of the complexes remained virtually unchanged. Only in the 'open' complex contacts were extended beyond the start site of transcription.

[Gastroesophageal reflux--cause of hemorrhaging esophageal varices?].

The results of the examinations do not depend on the peptic theory of the haemorrhage of the oesophageal varices in patients with liver cirrhosis, since the relative frequency of reflux troubles and of gastrooesophageal reflux in patients with liver cirrhosis and haemorrhage of the oesophageal varices was not found greater than in patients with liver cirrhosis and oesophageal varices without haemorrhage as well as the combination of reflux oesophagitis and oesophageal varices was rarely to be observed in the endoscopic material.

[Course of heart beat variability following acute alcohol intake].

Blood pressure, heart rate and mean momentary arrhythmia were measured in 16 healthy test persons before and after acute alcohol ingestion. While the blood pressure remained unchanged, heart rate and mean momentary arrhythmia showed significant differences in comparison to the initial value, in which case the clearer change of the mean momentary arrhythmia in relation to the heart rate emphasizes the sensitivity of the parameter. The results are discussed as a sequel of the increase of sympathetic activity of alcohol/acetaldehyde.
