[Pemphigus vulgaris in childhood].

Histological examination and immunofluorescence technique revealed the clinical diagnosis 'Erythema exudativum multiforme' as a real Pemphigus vulgaris of a 14-year-old boy. Early diagnosis and a large dose therapy of immunosuppressives agents decide on the survival of the patients.

[The incidence of subungual erythema].

In this study, 506 patients from several hospitals, especially from departments of internal medicine, were examined in order to find out the frequency of subungual erythemas (half and half nails, brown arcs). 44 patients suffered from small erythemas (less than 30%). The nails of 69 patients were inspected after kidney transplantation, only 5 of them revealed subungual erythema, 2 our of 61 healthy persons showed the subungual erythema. There were signs of renal failure in most patients with subungual erythema.

[Psoriasis therapy with retinoid].

Retinoids are effective in all forms of psoriasis and can be used alone or in combination with other therapeutic measures (anthralin, corticoids, PUVA, SUP). The effect of retinoids on psoriasis, in the final analysis still open to question, is discussed with regard to three parameters of psoriatic epidermis (enhanced proliferation, disturbance in differentiation, leucotaxis).

[Cerebrovascular disorders in young patients with livedo racemosa generalisata (Ehrmann)].

The results of examinations on 15 patients suffering from livedo racemosa generalisata (idiopathic livedo reticularis) are described. 12 of the patients were found to have cerebrovascular disturbances (in most cases severe) and 2 of them had migraine accompagnée. The remaining patient had no neurological changes. The majority of the patients were women (12 female, 3 male). In most cases the illness began between the age of 20 and 30 years. Cerebral angiographic examinations showed the existence of peripheral angiostenosis, whereas in 9 patients pronounced cases of diffused cerebral atrophy could be found with the help of computerised tomography. Questions concerning the nosological position, pathogenesis and therapy of livedo racemosa generalisata are discussed.

Monilethrix. Comparative scanning electron microscopic study of the hair in one family.

A comparative examination of the hair of the members of a family with monilethrix was carried out. Scanning electron microscopy (SEM) was used in the examination. It could be shown that the hair of persons with monilethrix showed a clear irregularity in its surface structure. A corresponding pattern was found in the clinically healthy sister and the likewise healthy father. The surface of the mother's hair showed a normal condition. The cuticle of the monilethrix appears to be very soft. The SEM method permits more exact examinations concerning the heredity of monilethrix.

[Green hair].

A young blond woman developed green tinted hair from exposure to tap water containing excessive amounts of copper. The adsorbed copper was located in the peripheral region of the hair shaft. The copper concentration of her hair was elevated to about 3,000 ppm-5,000 ppm.

[Cockayne syndrome and epidermolysis bullosa dystrophica (Hallopeau-Siemens). Simultaneous occurrence in a family].

A Turkish family with four children is described, two boys suffering from Cockayne's syndrome and a girl from recessive dystrophic epidermolysis bullosa (Hallopeau-Siemens). The outstanding features in the brothers are a pronounced delay in growth which began in both children at the age of about 1.5 years, progeria-like facial features, and a high degree of light sensitivity noticed very soon after birth. The daughter died of septicemia at the age of 4 years. Both diseases are rare and probably the consequence of extremely high consanguinity.

[Pachydermoperiostosis (Touraine-Solente-Golé syndrome). Clinical and radiological differential diagnosis (author's transl)].

This report describes a patient with pachydermoperiostosis (Touraine-Solente-Golé-syndrome). This disorder is characterized by clubbing of the terminal phalanges with watchglass-like nails, enlargement of hands and feet, pachydermia and periostotic skeletal changes. The signs of pachydermia are cutis verticis et frontis gyrata, ptosis and increased skin-thickness of the palms of the hand and soles of the feet. Frequently, a hyperhidrosis of the hands and feet can be found. The differential diagnosis of clinical symptoms and changes in the bones is discussed in detail.

Immunological features of psoriasis. Effects of Ro-109359, concanavalin A, pokeweed mitogen, and methotrexate on cultivated lymphocytes.

Isolated peripheral mononuclear cells of psoriasis patients with different disease characteristics, e. g. head-localised, quiescent guttata, confluent active widespread and erythrodermic, were cultured in a modified Mishell-Dutton system. Using the plaque-forming cell (PFC) assay, single cell antibody formation was studied, and class distribution monitored, adding pokeweed mitogen (PWM), concanavalin A (ConA), methotrexate (MTX) or Ro-109359/31, as well as autologous sera to the culture. PFC-estimation vs. sheep red blood cells (SRBC) and burro red blood cells (BRBC) revealed a distinct suppression of primarily IgG-PFC in some of the PWM-treated patients' cultures; ConA maximally reduced PFC by only 50%, compared to 100% for normal immune cells. Ro-109359/31 reduced mainly IgG-PFC in the co-cultures with PWM or autologous sera. MTX resulted in a reduction of IgG-PFC and IgM-PFC equally. The results were compared with cultured immune cells from normal individuals. The two antigenically different indicators, the partial abolition of ConA induced suppression, broad-based immunoglobulin elevation in the sera, and the mainly IgG-formation hint at the role of polyclonal B-cell activation in the perpetuation of psoriasis, which can be specifically reduced by Ro-109359/31. Suppressor cell dysfunctions remain to be discussed.

[Undesirable side effects on the skin caused by lithium therapy--case report and references (author's transl)].

This is a report about a 54 year old woman, who was treated with lithium carbonate because of a manic-depressive illness. As side-effect of this medicament she developed a relapsing hidradenitits suppurativa. After interruption of this therapy the illness of the skin improved, but relapsed, when the therapy with lithium was started again. In this case report we give a survey about side-effects of lithium concerning the skin.

[Neurological changes in livedo racemosa generalisata (Ehrmann). Case reports and review of the literature].

Five women with idiopathic livedo reticularis (Ehrmann) are demonstrated who showed remarkable or serious cerebrovascular lesions. A review of the literature showed that cerebro-vascular disturbances are the most important complications in idiopathic livedo reticularis. Twenty-four of 33 patients (73 per cent) showed neurologic complications. The ratio men: women was 13:11, the mean of the age of the patients 37 years.

[EMO-syndrome. Clinical and roentgenologic signs].

The EMO-syndrome (thyroid acropachy) consists of the triad of exophthalmos, circumscribed pretibial myxedema and clubbing of fingers and toes. This report describes a patients with EMO-syndrome. The skeletal changes are discussed.

[Disorders of immune regulation in psoriasis].

Immune abnormalities in psoriasis vulgaris are described. Anti-stratum corneum antibodies deposited with C3 have been found in patients with psoriasis. A lot of other immune abnormalities identified in psoriasis: increased serum IgG and IgA levels, lowered DNCB-sensitization, alteration in polymorphnuclear leukocytes (PMN) function, decreased number of T cells, lack of response to concanavalin A (ConA) and an increase in the polyclonal B cell activation. the authors believe that the immunoregulatory defect is due to the disorder of the skin.
