Rupture of the diaphragm in patients wearing seatbelts.

Six diaphragmatic ruptures seen in a consecutive series of 435 cases of blunt abdominal trauma (1946-1976) are analyzed. All of the ruptures were sustained in car accidents, three of them with fatal outcome after severe trauma with multiple injuries. Four of the patients wore safety belts: in three of them a rather specific injury combination occurred--diaphragmatic rupture, multiple costal fractures, and pelvic or vertebral fracture. The role of the seatbelt in the pathogenetic mechanism of the rupture is discussed, and the possibility that the cause is the placement too high on the trunk, especially in short individuals, is raised.

Platelet aggregability in microvascular haemostasis and the effect of local inflammation.

The effects on haemostatic plug formation and individual vessel platelet aggregability of granulocytopenia, prostaglandin E1 (PGE1) and indomethacin were studied in microvessels of the rabbit mesentery. Granulocytopenia was found to have no effect. Indomethacin abolished the differences between arterioles and venules in haemostatic plug formation time and stability as well as individual vessel platelet aggregability. PGE1 significantly inhibited haemostasis and also counteracted indomethacine. It is concluded that the haemostatic differences normally seen between arterioles and venules can be explained by prostaglandin-induced inhibition of venular platelets.

Solitary calcaneal metastasis as the first sign of gastric cancer. A case report.

A case of gastric adenocarcinoma is reported where the first symptom was a solitary calcaneal metastasis. The metastastic symptoms occurred in previously fractured bone, one year before the onset of gastric symptoms. This history is rare and the eight cases found in the literature where gastric cancer has started with solitary bone metastasis are reviewed.

The effect of O-(beta-hydroxyethyl)-rutoside (HR) on macromolecular leakage, thrombosis and haemostasis in experimental animals.

O-(beta-hydroxyethyl)-rutoside (HR) (Venoruton, Zyma AS, Nyon, Switzerland) has been investigated experimentally to evaluate the effect on microvascular permeability and thromboembolism. Permeability to macromolecules is diminished in a hamster cheek-pouch model. Haemostatic plug formation is impaired whereas laser-induced intravascular platelet aggregation is uninfluenced. There is a small but insignificant protection against sodium morrhuate (Eli Lilly and Co., Indianapolis, Indiana) induced femoral vein thrombosis.

A comparison between subcutaneous low-dose sodium and calcium heparin. Prophylaxis of postoperative deep vein thrombosis and side effects of treatment.

A controlled, randomized, prospective double-blind trial between subcutaneous low-dose sodium- and calcium-heparin was made. Heparin Vitrum was used. The only difference between the two heparins was the sodium or calcium ions. Postoperative thrombosis frequency, bleeding complications, and local reaction were analysed. For evaluation of local haematoma formation, 125I-fibrinogen uptake was measured over the injection sites. The only statistically significant difference between the two heparins was a larger haematoma formation at heparin injection sites when calcium heparin was used. Otherwise sodium- and calcium-heparin are equal in effect and side effects provided they come from the same heparin source.

The frequency and significance of postoperative plantar vein thrombosis judged by the 125I-fibrinogen test.

In 318 patients screening for the development of postoperative deep vein thrombosis was made with the 125I-fibrinogen test. In addition to the conventional measurements over the leg the planta pedis was also included. Of 221 legs with thrombosis 75 had an increased uptake in the planta pedis, but only in sixteen patients an isolated plantar thrombosis was found. In only eight of 70 thrombi there was evidence of a proximal extension of the plantar vein thrombosis. The time for the thrombosis debut did not differ between isolated plantar vein thrombi and more extensive thrombi; the majority beginning during the first four postoperative days.

Thermography. A noninvasive method for diagnosis of deep venous thrombosis.

Thermography is a simple, rapid, and noninvasive method for the diagnosis of deep venous thrombosis (DVT). A comparative study between thermography and conventional phlebography has been performed in 121 legs. The thermographic criteria for diagnosis of DVT are described; posterior views were found to be advantageous in interpretation. The diagnostic agreement between the two methods was 90.1%. The localization diagnosis was somewhat less correct. The thermographic method is a reliable alternative in the diagnosis of acute DVT.

Effect of dextran and hyaluronic acid on the development of postoperative peritoneal adhesions in experimental animals.

The development of postoperative peritoneal adhesions was studied in rats and rabbits, the frequency of adhesions in the experimental model used being very high. In the development of an adhesion, fibrin seems to be an important contributor to the bridge between different tissues. Dextran, which modifies the fibrin network and makes it more susceptible to lysis, was used as a possible prophylactic agent, but we found no difference between treated and control groups. It is concluded that the stimulus for fibrin formation in the peritoneal adhesions using this atraumatic model was too strong to be overcome by the normal fibrinolytic system. The anti-inflammatory reaction of hyaluronic acid did not diminish the frequency or degree of adhesions.

Advanced hyperbilirubinemia simulating obstructive jaundice after choledocholithotomy. Report of a case.

A case of intrahepatic cholestatic jaundice of unclear etiology and unusual course is described. A male patient underwent choledocholithotomy after PTC, showing a normal immediate postoperative recovery, but after a few days developed a severe cholestatic jaundice despite normal postoperative cholangiograms. The liver function tests showed a biphasic picture. Bilirubin was maximally 770 mumol/l and a lower level was obtained after blood exchange and subphrenic abscess drainage. It was, however, not until prednisolone treatment was induced that a slow continuous decrease of bilirubin was seen.

Microvascular haemostasis and the effect of local stimulation and inhibition of platelet function. An experimental study in rabbits.

The local stimulation and inhibition of haemostatic plug formation was studied by local infusion and superfusion of different agents - ADP, thrombin, adenosine, heparin, prostaglandin E1 (PGE1) and laster injury. ADP and laser injury shortened venular haemostatic plug formation time whereas adenosine in the superfusate and PGE1 prolonged it. Thrombin infusion had no effect but thrombin in the superfusate significantly stabilised the haemostatic plugs. It is suggested that ADP released from red cells is the most potent stimulator for haemostatic plug formation in the rabbit mesenteric microcirculation.
