Effect of intermittent pneumatic and graduated static compression on factor VIII and the fibrinolytic system.

Intermittent pneumatic compression (IPC) or graduated static compression stockings, both effective mechanical methods in the prophylaxis of postoperative deep venous thrombosis, have been investigated in 50 patients regarding the effect of compression on the fibrinolytic system and factor VIII activators. All the patients were subjected to one of the two methods before operation upon varicose veins. In 38 patients IPC with a pressure of 40 mmHg was applied on one leg or arm for at least two hours. In 23 of these patients the compression pattern was slow, two minutes' inflation and two minutes' deflation period. In the remaining 15 patients the compression pattern was quick, three seconds' inflation, followed by 20 seconds' deflation, three cycles a minute. Twelve patients were treated with a compression stocking on one leg for about 24 hours before the operation. Blood samples for determination of fibrinolytic activity and factor VIII in plasma were obtained before and immediately after the end of compression and application of a stocking, respectively. Also the level of plasminogen activator activity (PA) in the vein wall was determined. Neither IPC nor the application of an elastic stocking had any demonstrable effect on the variables studied. In addition, the results did not vary with the rate (slow or quick) or with the duration of IPC, sex or site of application (arm or leg) of IPC. Blood samples from the compressed leg were also uninfluenced.

Intestinal trauma, Analysis of 101 cases.

Intestinal injuries sustained by 101 Swedish patients during the period 1950-1979 are reviewed. The abdominal trauma was blunt in 78 cases and penetrating in 23. Small-bowel and mesenteric, but not large-bowel, injuries showed increasing frequency. This was associated with rising numbers of motorcar accidents. Injuries to other abdominal organs were found in 56 of the 101 patients, particularly in those with mesenteric or large-bowel trauma. Clear physical signs of intra-abdominal injury led to rapid surgical exploration in most cases, but some operations were performed after relatively long observation. Most injuries, including those in the colon, were treated with primary repair or resection. Decompressive colostomy or exteriorization were rarely performed. Deaths were mainly caused by other factors than the type of operation. The mortality rate (35%) was constant.

Upper gastrointestinal trauma. Analysis of 45 cases of gastric, duodenal or pancreatic injury.

Injuries to the upper gastrointestinal tract occurring over a 30-year period in a geographically well defined region were analyzed. The incidence was low in comparison with other abdominal injuries, but duodenal and pancreatic injuries were increasingly common in the study's last decade. The stomach injuries were mainly caused by penetrating trauma. The duodenal and pancreatic injuries most commonly resulted from road-traffic accidents. Two duodenal injuries were overlooked at the initial exploration, in which the duodenum was incompletely exposed. Anastomotic insufficiency occurred in one of the three patients who underwent duodenal resection. Most of the pancreatic injuries were contusions that could be managed with drainage. The pancreas was severely damaged in five patients, all of whom had multiple intra-abdominal injuries. Three of the five patients died before or during operation.

Haemostatic platelet plug formation in the isolated rabbit mesenteric preparation--an analysis of red blood cell participation.

In a model using an isolated rabbit mesenteric preparation microvessels were transected and the time until haemostatic plugs formed was registered. Perfusion of platelet rich plasma gave no haemostasis whereas whole blood did. Addition of chlorpromazine or adenosine to the whole blood significantly prolonged the time for haemostasis, and addition of ADP to the platelet rich plasma significantly shortened it. It is concluded that red cells are necessary for a normal haemostasis in this model, probably by a combination of a haemodynamic and ADP releasing effect.

Prophylaxis of postoperative venous thrombosis in a controlled trial comparing dextran 70 and low-dose heparin.

A controlled, randomized trial comparing dextran 70 and low-dose heparin with no treatment for prophylaxis of postoperative deep vein thrombosis is reported. A total of 168 patients who underwent general and urologic surgical operations participated in the trial. The 125I-fibrinogen test was used for diagnosis. The frequency of deep venous thrombosis was 27% in controls, 29% in patients who also received dextran 70, and 13% in patients treated with low-dose heparin. Only heparin therapy reduced the incidence of thrombosis. Epidural analgesia did not reduce the frequency of thrombosis. In patients with malilgnant diseases, the thrombotic process always extended proximally, at least to the knee. The highest frequency of thrombosis was seen after urologic surgery. Complications did not differ between the groups.

The relevance of urinary sampling methods in patients with indwelling Foley catheters.

The relevance of different urinary sampling methods in male patients with indwelling catheters was studied. There was good agreement between the bacterial strains cultured from urine sampled at the catheter end compared to samples obtained by catheter puncture. However, one-quarter of the samples obtained by the catheter did not show complete agreement with the bladder aspiration samples. It is concluded that, in patients with an indwelling catheter, urinary sampling by the catheter does not always provide relevant information about the bacterial strains present and bladder aspiration is advised.

Blunt renal trauma. Changes in aetiology, Diagnostic procedure, treatment and complications over thirty years.

In a rural area with a relatively stable population, 216 persons were hospitalized for blunt renal trauma over a 30-year period (1946-75). In the final 10 years the frequency of such trauma increased, as also did the proportions of females and younger patients (11-30 years old). In the final 5 years the overall incidence was 6.2 cases per 100 000 inhabitants and year. Motor traffic accidents were increasingly the cause of blunt renal trauma. Injuries from compression showed a stable frequency. Renal trauma attributed to blows varied with the incidence of accidents involving horses and sports. Treatment was mainly conservative, except in major injuries. Emergency excretory urography was rarely used during the first 10 years of the study, but thereafter with increasing frequency. Early complications were seen only during the first 20 years of the study.

Effects of gliclazide--a new antidiabetic agent--on blood platelet function in vitro and in vivo.

UNLABELLED: The effect of gliclazide on platelet activity was investigated and compared to that of tolbutamide and glipizide. IN VITRO STUDIES: adenosine diphosphate induced human platelet aggregation was inhibited by gliclazide in high concentrations (0.5--2.0 mg/ml). In the same concentration range tolbutamide exhibited a slightly less pronounced inhibitory effect. Collagininduced human platelet aggregation was inhibited by gliclazide, glipizide and tolbutamide in a concentration range of 0.5--2.0 mg/ml. Gliclazide had a similar effect on rabbit platelets. In vivo studies: Both acute and seven days administration of gliclazide to rabbits resulted in prolonged primary and total hemostatic plug formation time both in arterioles and venules. Similar results were obtained with tolbutamide and glipizide. Experiments with laser-induced platelet plug formation in the rabbit ear chamber demonstrated that 100 mg gliclazide/kg body weight and 25 mg gliclazide/kg body weight during 7 days significantly reduced the number of platelet emboli formed during 10 minutes after laser injury.

Local inhibition of the fibrinolytic system in patients with massive upper gastrointestinal hemorrhage.

The effect or oral tranexamic acid on massive upper gastrointestinal hemorrhage was evaluated in a randomized double-blind study. Totally 50 patients entered the trial and seven were excluded, leaving 22 placebo treated and 21 tranexamic acid treated for analysis. The groups were comparable regarding sex, age, diagnosis, and initial laboratory data. Transfusions requirements and operation frequency did not differ. Mortality was slightly reduced and death delayed in tranexamic acid treated patients.

Traumatic splenic rupture during 30 years. An analysis of 88 cases with special attention to delayed rupture.

Eighty-eight cases of splenic rupture in non-penetrating abdominal injury are analysed. An increasing frequency was seen during the 30-year period studied (1946--1975). Teenagers and young adults were the most affected age groups. A rising incidence of traffic accidents amd multiple iinjuries was noted. Mortality was 21%. All patients with an isolated splenic rupture survived. The complication rate was low. The high incidence of delayed splenic rupture in earlier reports seems to be more a refleciton of the diagnostic diffulties than an actual delayed rupture. We have put the splenic ruptures into minor immediate, major immediate, and delayed. Minor splenic rupture was seen in 20% of patients undergoing operation, major in 75%, and delayed in only 5%. Three of our patients were operated on with splenorrhaphy with an uneventful postoperative course. In minor splenic ruptures, especially in young patients, splenorrhaphy could be a possible way of treatment but more controlled studies are needed before firm recommendations can be made.
