[Prostaglandin E2 in prostatitis and prostatic adenoma].

The measurement of prostaglandin E2 (PGE2) concentrations in the serum and prostatic fluid of healthy men, patients with prostatic hyperplasia and of patients with prostatitis was attempted and correlated to the state of disease, respectively. PGE2-concentrations with prostatic fluid of healthy men were found to be significantly lower than in patients with prostatitis. Corresponding to the course of treatment concentrations normalized, being favorably influenced by sitosterin as an adjuvant medication. Compared to healthy men, PGF2 concentrations in the prostatic fluid of patients with hyperplasia of the prostate incline to lower levels.

Interaction of myelin basic protein with gangliosides and ganglioside-phospholipid mixtures.

The interaction of myelin basic protein with monosialoganglioside GM1 was investigated. It was found that the emission maximum of the tryptophan of the protein is blue-shifted due to the interaction. In mixtures of the monosialoganglioside with phosphatidylcholine, the myelin basic protein induces phase separation of the lipids as inferred from differential scanning calorimetry experiments.

Thermotropic properties of bipolar lipids of Sulfolobus solfataricus and of their mixtures with dipalmitoylphosphatidylcholine.

The thermotropic properties of the bipolar lipids, glycerol dialkylglycerol tetraether (GDGT) and glycerol dialkylnonitol tetraether (GDNT), were determined at different degrees of hydration and in mixtures with dipalmitoylphosphatidylcholine (DPPC). The number of water molecules rendered unfreezable by the GDNT molecule is 10+/-1.5 and that by the GDGT molecule 2.8+/-0.7 or about 1.1-1.5 H2O molecules per OH group. Binding of water molecules causes randomization of the two polar heads from the oriented form prevailing in the dry state. The hydration seems to be a cooperative process extending over a whole lipid domain. DPPC added in small amounts to GDNT interacts preferentially with the nonitol halves of the molecules separating them from the glycerol half molecules. In the cooperative interaction domain each DPPC molecule is surrounded by up to six GDNT molecules. Cooperative domains formed during the interaction of DPPC with GDGT are less pronounced. In both cases they affect the thermotropic properties of the system.

Dynamic cavernosography: venous outflow studies of cavernous bodies.

Based on dynamic cavernosography studies in 15 patients, including 8 with simultaneous passive erection, we present more precise details of the venous drainage of the penis. The venous drainage is comprised of 3 different systems. The superficial dorsal vein drains mainly the penile skin and prepuce, and empties via the external pudendal veins into the femoral vein. The deep dorsal vein, located between the tunica albuginea and Buck's fascia, drains the glans and all 3 corpora. The venae profundae penis emerge from each crus of the corpora cavernosa and drain only the corpora themselves. Considerable individual differences were found regarding further drainage via the pelvic venous system, including the prostatovesical plexus and internal pudendal veins. Passive erection was tried in 11 patients and was successful immediately after cavernosography in 8. The flow rates to induce an erection averaged 111 ml. per minute (range 55 to 160 ml. per minute), while the rate to maintain the erection was 48 ml. per minute (range 12 to 90 ml. per minute).

Congenital and acquired penile curvatures: diagnosis and outcome with the Nesbit procedure.

A surgical correction according to Nesbit [1] was performed in 9 patients with congenital penile curvature as well as in 5 patients with acquired penile curvature in the context of Peyronie's disease and in 1 case after an old penile fracture. Dynamic cavernosography with simultaneous passive erection and photographic documentation of the curvature in two planes served as basic preoperative diagnostic methods. Besides the recording of a local report, the postoperative results were evaluated by means of a detailed questionnaire in all patients. 8 patients in the group of congenital curvatures as well as 3 patients of the group with acquired penile curvatures were shown to be completely satisfied with the functional result of the surgical correction. With the knowledge of the result of the operation, 13 out of 14 patients would indeed agree to the operation if asked again. This experience indicates that treatment according to Nesbit is justified in the therapy of both congenital and acquired penile curvatures.

Balloon therapy for obesity--when the balloon bursts.

Intragastric balloon placement is a non-invasive treatment for morbid obesity. We report a patient who illustrates incomplete bowel obstruction and pancreatitis following dislodgement of such a balloon. Percutaneous transabdominal puncture of the balloon is an effective method of decompression.

[Effect of diet on lithogenic and inhibitory substances in the urine].

A standard diet is the pre-condition for an accurate investigation of the causes of stone formation reasons in recurrent stone-formers, since the uncontrolled influences of individual diets on the composition of the urine can thus be eliminated. In patients kept on a standardized diet intake a steady state (balance of excretion of lithogenic substances) is obtained after about 5 to 7 days. Because it must be assumed that brief excretion and concentration peaks are very much more frequently the cause of nucleation, the determination of the circadian rhythm of lithogenic substances is most important. On the basis of our investigation results, an "investigation programme in stages" for research into the causes of stone formation is recommended.

Calorimetric studies on the interaction of gangliosides with phospholipids and myelin basic protein.

Differential scanning calorimetry was employed to investigate the interaction of GM1 gangliosides with phospholipids (phosphatidylethanolamine, phosphatidylserine or phosphatidylcholine). It was found that GM1 is completely miscible with phosphatidylethanolamine; however, the interaction with phosphatidylserine is minimal. Addition of excess Ca2+ to the interaction products of GM1 with phosphatidylcholine or phosphatidylethanolamine did not induce phase separation. The influence of myelin basic protein on the thermotropic behaviour of GM1 was also studied. It was found that basic protein has a very strong perturbing effect on GM1 micelles.

Plain film manifestations of retrocrural disease.

The following plain film findings of retrocrural disease were demonstrated by two patients: a) silhouetting, by a soft-tissue density, of a previously seen inner margin of the crus, b) lateral displacement of the crus, c) lateral deviation of the inferior portion of the paraspinal line, and d) abrupt obliteration of the upper portion of a well defined crus.

[Penile revascularization surgery in vascular impotence].

The application of objective diagnostic methods to patients complaining of erectile dysfunctions has proved that 30 to 40% are based on pathologic changes of the penile vascular system. The most important diagnostic procedures (NPT, Doppler, BCR-latency time, dynamic cavernosography with simultaneous passive erection, penile angiography) are emphasized and possible pitfalls with reference to misinterpretation of the diagnostic findings are discussed. Technique and results of current revascularization procedures (epigastricopenile anastomosis, revascularization of cavernous and dorsal arteries using vein grafts, deep dorsal vein arterialization) are discussed and first own experiences with aforementioned techniques presented.

[Erection and ejaculation disorders following retroperitoneal lymphadenectomy in non-seminomatous testicular tumors].

From 38 patients, who had undergone retroperitoneal lymphadenectomy in the period between April 1980 and October 1983, reliable statements were obtained referring to pre- and postoperative erectile and ejaculatory abilities. The thoracolumbar outflow (Th12-L3) of the centers for emission and psychogenic erection is usually damaged in this procedure. About 12% of the radically lymph node dissected patients complained of permanent erectile disturbances; 85% of the radically lymph node dissected patients revealed ejaculatory disorders, 58% of them with a total loss of ejaculation. About 50% of them showed a considerable psychic involvement. In 12 patients a modified lymph node dissection procedure was performed with the intention to preserve ejaculatory capability. Nevertheless 6 (50%) of them revealed postoperative ejaculatory failure, 3 of them with a total loss of ejaculation. Thus our own experiences and a review of the literature indicate that this modified lymph node dissection often fails in its purpose and can not always be considered a valuable procedure for preservation of ejaculation. The administration of sympathomimetic drugs and/or imipramine is a promising approach in the treatment of ejaculatory failure.

[Late results following sclerotherapy of varicocele].

Between May 1981 and December 1982 244 patients with varicocele were scheduled for percutaneous sclerotherapy which is performed as an out-patient procedure. In around 80% of the patients sclerotherapy was possible. Failures were due to anatomical and technical factors. Three months after treatment a persisting varicocele was observed in 3.3% as evidenced by control phlebography; 3.9% of our patients developed a painful induration of the plexus pampiniformis which necessitated a 3-day hospitalization. Six months after sclerotherapy spermiography was repeated and compared to the pretreatment results: in 50% of the patients the sperm density was significantly improved, however, sperm morphology and mobility remained virtually unchanged. The sclerotherapy of varicoceles can be recommended as an alternative procedure to surgery. The effect on sperm quality of both therapies is equal, but the sclerotherapy is performed as an outpatient procedure (sick-leave 2 days), is cheaper, better tolerated and has a lower rate of persisting varicoceles than surgical repair.

[Optimizing conservative urinary calculus expulsion].

A multicentric randomised and prospective study on conservative treatment of ureteric calculi had been performed. The frequency and transit time of spontaneously passing of the stones, side effects and cost of treatment have been compared under classic spasmoanalgetic therapy and a phytotherapeutic medicament (Urol). In the group with spasmoanalgetic therapy 85.5% of the stones passed spontaneously, in the group with Urol medication 89.3% of the stones. There was no significant difference concerning the transit time of stones between the two groups, but side effects and costs were less in the Urol-group.
