Vitamin A and beta-carotene content of seminal fluid and sperm of normospermic and oligozoospermic men.

Beta-carotene and vitamin A were estimated in 125 specimens of human seminal fluid and spermatozoa. In 72 of the specimens triiodothyronine, tetraiodothyronine, and T4-binding globulin were also determined. Vitamin A in seminal fluid increased from 10.4+/-l.8 microgram (SE) in severe oligozoospermia up to 19.1+/-2.5 microgram/100ml in specimens with high sperm densities. In spermatozoa, however, there was a trend for a decrease in vitamin content from 2.1+/-0.8 to 0.2 +/-0.07 microgram/108. In the majority of cases beta-carotene was undetectable and, when present, the levels were much lower than those of vitamin A. The amounts of triiodothyronine and tetraiodothyronine in seminal fluid were 0.19+/-0.03 ng/ml and 1.1+/-0.18 microgram/100ml, respectively. No correlation was found between levels of both hormones and quality of seminal fluid. T4-binding globulin was either very low or undetectable.

Inhibition and enhancement of macrophage migration in the presence of lymphocytes and seminal fluid of men with fertility disorders.

Macrophage migration inhibition factor (MIF) towards autologous seminal fluid was studied in 29 infertile men, 16 of whom were also examined towards normal seminal fluid. In 10 patients, inhibition of macrophage migration was observed in the presence of 50 microgram of protein of autologous semen sonicates. Nine patients had enhanced macrophage migration in the presence of 5 microgram protein. Macrophage enhancement was more antigen dependent than the migration inhibition. In all patients seminal abnormalities were observed, being milder in the patients whose lymphocytes enhanced macrophage migration.

Estimation of hyaluronidase activity of human semen and its relationship with sperm density by means of a simplified method.

Hyaluronidase activity was examined in 109 specimens of human semen of various sperm densities, seminal plasma, and spermatozoa sedimented by centrifugation. We used a modification of a method originally devised for estimation of hyaluronidase activity in Clostridia and which was bases on measurements of the area of hyaluronate digestion in agar plates. Enzyme activities in both semen and seminal plasma increased with increase in sperm density. The activity in seminal plasma, which appeared promptly after liquefaction and represented enzyme released from spermatozoa, ranged from 31% to 61% of the activity in semen. Activities of spermatozoa in sediments exhibited lower values than those calculated for sperm of whole semen, possibly due to leakage. Both activities, calculated per million sperm cells, gradually increased with decrease in sperm density. The possibility that with severe oligozoospermia the acrosome may become less prone to enzyme release or that the initial activity per cell may increase is discussed. These phenomena would represent additional characteristics of oligozoospermia.

Radionuclide cystography: the significance of retention time of the refluxed radioisotope.

We studied 36 patients with vesicoureteral reflux into 45 renal units by means of isotope cystography and a dynamic renal function scan performed 24 to 48 hours after the cystography. Functional impairment was found in 21 renal units. To evaluate the significance of the time between voiding and complete disappearance of the radioisotope that refluxed from the kidney (retention time) the presence of the labeled particles in the kidney was followed continuously and intermittently during a prolonged interval. There was a significant difference between the retention time found in the impaired kidneys and that in the normally functioning kidneys. In 91.7 per cent of the normally functioning kidneys retention time was less than 5 minutes, whereas in 86 per cent of the impaired kidneys it was more than 10 minutes and in 56 per cent it was more than 20 minutes. No significant correlation was found between the retention time and intrarenal reflux or dilatation of the collecting system (patients classified as having radiological grades III and IV reflux were not included in this study). A retention time greater than 10 minutes may be correlated with decreased renal function. The retention of technetium sulfur colloid particles may be analogous to the retention of similar size bacteria and suggests a possible pathophysiologic explanation for the development of renal dysfunction owing to the prolonged presence of bacteria in the kidney after reflux.

The treatment of osteitis pubis with heparin.

We treated 7 patients with osteitis pubis by heparinization. Of the 7 patients 2 had a dramatic improvement. Small doses of heparin given routinely preoperatively and postoperatively were not found to prevent the development of osteitis pubis. In view of these results as well as the fact that there presently is no effective method of treatment of this disorder, it is considered justifiable to subject all patients with postoperative osteitis pubis to a therapeutic trial with heparinization.

Hydrocortisone and inhibitors of prostaglandin synthesis. Potentiation of allograft survival in mice.

We studied the effects on survival of allogeneic skin grafts after treatment with hydrocortisone and/or inhibitors of prostaglandin synthesis: indomethacin and flufenamate. We found a marked synergistic effect of combined treatment with hydrocortisone and indomethacin or flufenamate. Neither hydrocortisone nor indomethacin or flufenamate, when given alone in relatively small doses, caused delayed graft rejection. However, when small doses of hydrocortisone were used in combination with flufenamate or indomethacin, the median survival time (MST) of allogeneic grafts was prolonged from 11.4 days to 20.9 and 23.8 days, respectively. Moreover, the increase in graft survival was comparable to that obtained by treatment with relatively high doses of azathioprine alone or combined with hydrocortisone. The finding of synergism between low doses of prostaglandin synthesis inhibitors and glucocorticoids in delaying graft rejection suggests that such treatment might provide a relatively safer means of achieving clinical immunosuppression than the high doses of steroids and azathioprine currently in use.

'Malignant' macroscopic appearance of an inflammatory testicular lesion--a reminder for surgeons.

Inflammatory changes in the tunica vaginalis testis may assume a misleading appearance which is highly suggestive of malignancy. Obviously, histological examination is crucial for establishing the corrent diagnosis. This phenomenon has been described in texts dealing specifically with testicular pathology, but is not emphasized in standard text books of urology, surgery or pathology. An illustrative case serves as the basis for discussing this entity, for the purpose of familiarizing it to surgeons, urologists and pathologists who are yet unaware of it.
