Ureteral motility after renal autotransplantation.

Twelve kidneys were autotransplanted to the internal iliac vessels in 11 patients for the treatment of renovascular hypertension. The ureter had been left unresected in all but one case, where a transuretero-ureterostomy was performed. A radiographic study of the ureteral motility was obtained during the postoperative period recording images every 1.5 s with a spot-camera system. The ureters appeared to follow different courses, due to the different surgical procedures. In all but two cases the transport of urine was obtained through separate boluses, frequency and direction of peristalsis were within normal ranges, and significant dilation was never observed.

Parathyroid hormone bioassay using human kidney cortical cells in primary culture.

The biological activity of parathyroid hormone (PTH) has been investigated by measuring intracellular accumulation of cyclic AMP (cAMP) in human kidney cortical cultures. Enzyme dispersed cortical cells from non-invaded kidney poles of patients undergoing nephrectomy for cancer were used after 5 days of primary culture. Bovine PTH (1-84) produced a significant increase of cAMP accumulation in cultured cells at a dose (53.7 ng/ml) 10-fold lower than that found for the minimal stimulatory effect when using preparations of human purified plasma membranes. The action of bovine PTH (1-84) was very rapid, a response was detected after 5 min and a ceiling effect after 30 min. Cortical cells showed a slightly lower sensitivity to synthetic bovine PTH (1-34) (half maximal increase dose: 0.66 microgram/ml), compared to bovine PTH (1-84) (half maximal increase dose: 0.32 microgram/ml), but revealed a higher sensitivity to human PTH purified from the medium of parathyroid cell cultures (half maximal increase dose: 11.2 ng/ml). Arginine-vasopressin (AVP) also increased the cAMP accumulation of kidney cortical cultured cells, with a potency and efficacy lower than that of human 'culture' PTH, while in kidney medullary cells in primary culture AVP exerted a strong response and the effect of PTH was poor or absent. Calcitonin and glucagon were weak stimulators of kidney cortical cell cAMP accumulation.

Surgical treatment of retroperitoneal fibrosis with omentoplasty.

The main goal of surgery for retroperitoneal fibrosis is to free the ureters from sclerotic tissue and pressure them from further extension of the disease; the omentum may be used as an efficient barrier against recurrence of fibrosis. We present 11 patients with idiopathic retroperitoneal fibrosis treated with ureterolysis and omentoplasty - in four patients the procedure was performed bilaterally, in seven the omentum was used to protect only one ureter, but in six there was a surgical or functional solitary kidney. Out of 15 ureters, residual dilatation was observed only in two, with marked clinical improvement in all patients. Mean follow-up was 51 months.

Angiographic aspects of renal cell carcinoma metastatic to the female genitalia.

Selective angiography of the hypogastric artery was performed in two patients presenting vaginal metastases from renal cell carcinoma, in order to better define the extent of the metastases. In other case left renal angiography revealed an unsuspected metastasis to the left ovary, fed by a middle perforating capsular artery.

Conservative surgery for renal cell carcinoma.

The authors present their clinical experience in conservative surgery for renal cell carcinoma, which was performed in two cases of simultaneous bilateral tumours, in a case of neoplasm in solitary kidney, in two cases of cancers with damaged contralateral kidney and in two cases of association between cyst and small tumour. All patients are alive and without evidence of residual or recurrent malignancy with a followup ranging between 6 months and 7 years. The pros and cons of the various surgical solutions are discussed.

Polyorchidism and seminoma in a child.

A case is reported of a neoplasm involving a supernumerary testicle in a 9-year-old boy. The histologic diagnosis was anaplastic seminoma. We have not been able to find a similar case in the literature.

The role of prostaglandins in the maintenance of the tone of the rabbit urinary bladder.

The effects of bladder distension on the production of prostaglandins (PGs) were studied in an isolated rabbit bladder preparation. Increasing bladder distensions induced a proportional release of PGs together with a parallel increase in endovesical pressure; pretreatment with indomethacin prevented both. Infusion of exogenous PGs into the arteries of bladders previously filled and treated with indomethacin produced a rise in endovesical pressure similar to that observed subsequent to distension of the untreated bladder. Our results suggest that PGs and particularly PGE2 play a role in the maintenance of vesical tone during distension.

Ex situ surgery of the kidney: experience with 20 autotransplantations.

The authors propose a classification of ex situ surgery of the kidney and present their clinical experience of 20 renal autotransplantations over a period of 6 years: 16 were performed for renovascular hypertension with 2 failures and 4 for recurrent staghorn calculi, with 1 stone recurrence. Based on their experience and on the literature they discuss the present status and the future implications of ex situ surgery of the kidney.
