[Basal cell epithelioma with lymphogenic and hematogenic formation of metastases (a.o. into the myocardium) (author's transl)].

This report deals with a basal cell epithelioma, partially adenoid and partially morphea-like in structure, which despite intensive X-ray treatment relapsed constantly and which finally developed into an ulcus terebrans. Approximately 13 years after the primary tumor had developed (located on the left wing of the nose) both a lymphogenic and a hematogenic formation of metastases occurred with a subsequent exitus letalis 4 months later. Besides the metastases of the skin, there were multiple metastases in the lymph nodes, vertebral column, ribs, spleen, liver stomach, pleura, and peritoneum as well as in the mycardium of both ventricles and in the perimysium of the skeletal muscles. Their histological structure was similar to a partly adenoid, partly morphea-like basal cell epithelioma. The possible influence of X-ray treatment on the tumor tissue in way of benignity or malignancy is discussed in view of relevant literature of this topic. The alterations of basal cell epitheliomas into the so called transitional epitheliomas in also analyzed.

[Vasectomy. Pro and contra (author's transl)].

Surgical contraception on the part of the male (vasectomy) is a contraceptive method which has a character of finality. This statement must be firmly adhered to because refertilization is not 100% successful. This intervention on the male needs very thorough preparation by intensive consultative discussions in which the female partner must also be included. Both the resons for and against vasectomy are presented with descriptions of some pertinent examples. In doing so it is demonstrated in detail that vasectomy is more than a purely technical procedure; in the assessment of the pros and contras, the whole relation to the partner relations must be integrated.

The interdisciplinary cooperation between andrology and gynaecology as consideration to the effectivity of treatment.

The author advocates a close cooperation between gynaecologist and andrologist in an effort to solve the infertility problems of married couples. It is emphasized that in the presence of oligozoospermias the fertilizing capacity of the ejaculate should be substantially improved by transferring the semen to a cervical cup previously fitted to the vaginal cervix uteri.

New development in surgical andrology. Alloplastic spermatocele.

We developed a cup-shaped silicone-dacron prosthesis that may be used as a sperm reservoir. Spermatozoa aspirated from this reservoir and stored as pellets in liquid nitrogen for 6 weeks induced pregnancies and normal births in 50 per cent of inseminated cows. Because of aplasia or long stenosis of the vas deferens 26 prostheses were implanted upon the epididymis in 14 patients. Percutaneous aspirations up to 7 months postsurgery showed 0.3 to 105 million spermatozoa the quality and quantity of which decreased with time. Insemination of patients' wives has not resulted in pregnancy.

Neutral sugar composition of proteins of human seminal plasma from different andrological diagnoses.

The neutral sugar content of proteins of human seminal plasma from different clinical diagnoses has been determined by gas-liquid-chromatography. Galactose, mannose, glucose and fucose were the predominant carbohydrates. Sometimes small quantities of arabinose, rhamnose, xylose and ribose were detected. Our analysis indicate that the concentration of galactose and fucose was distinctly higher in the group with azoospermia.

Changing trends in skin diseases at the University Clinic Hamburg.

The author reports about statistical analyses of the outpatients from the dermatological department of the University hospital Hamburg 1935-1966. It can be demonstrated in general a change of the frequency of some skin diseases e.g. a regression of bacterial diseases and scabies, a increase of mycoses and of neurodermatitis and of eczema.

[Pathogenesis and therapy of varicocele (author's transl)].

During the last 13 years 519 patients were operated upon for varicocele in the refertilisation center Hamburg. Out of 209 cases examined thereafter, 174 showed an improved spermiogram, and only in this group 40 children were fathered. Pathogenesis, diagnosis and surgical treatment are presented. A persisting varicocele (in our clientele in 1 per cent) was corrected in 18 patients. The reasons for recurrent surgery were overlooked branches of the V. spermatica interna and not yet described collaterals to neighbouring venous systems. For optimal surgical planning the latter are best demonstrated by preoperative phlebography.
