[Andrologic appraisal in a court of law. Current problems in daily practice].

In a court of law the andrological expert is a servant of the court, not a judge. In the case of oligozoospermia manifested at the present time, where anamnestic data regarding any past sickness are lacking, it must be assumed that during the legal time of conception a condition similar to that now detected also existed. This means that the present finding was already in a state of effect in the past. Furthermore the presence of the expert at the time of masturbation by the patient controlling the semen production is discussed. The defense attorney asked the court for the presence of the expert. This presence does not cause any additional difficulty for the patient.

Diagnosis, Differential↗

[Mycological findings in andrological patients. Statistical study in the years 1971-1981].

In the years 1971 to 1981, clinical signs of mycotic skin infection were found in 589 andrological patients (2.7%). Microscopic proof and absence of positive microscopic results, respectively, are not sufficient for diagnosis; therefore, the culture is a conditio sine qua non for exact diagnosis. A positive mycotic culture was found in 13% of the cases with a negative microscopic investigation.

[The patient's face, a follow-up study of a leper].

We report on a 20-year follow-up study dealing with a male patient suffering from leprosy. Observation of the feature allows to determine a deterioration in the general condition; it becomes possible to fix the moment of the manifestation of the bronchial cancer the patient finally died from.

Screening for fra(X)(q) in a population of mentally retarded males.

Among 242 institutionalized mentally retarded males in Northern Germany screened for fra(X)(q), 15 (6.2%) with severe mental retardation expressing fra(X)(q) were detected. One patient displayed Klinefelter's syndrome in addition. All fra(X)(q) males showed the typical facial signs, but three of the adults did not express macroorchidism. A preliminary estimation of an overall frequency of 1:2000 males for the fra(X)(q) condition is suggested.

Vas deferens aplasia: clinical and anatomical features of 90 cases.

Out of 531 patients with excretory azoospermia, scrotal exploration for attempted epididymo-vasostomy showed vas aplasia in 90 of them (= 17%). In the latter group complete bilateral agenesis of the vas was seen in 64 cases while the remaining showed partial vas aplasia or different findings on both sides. The anatomical findings were classified into various groups of frequency. The typical peroperative picture included extensive paraepididymal bodies of fat, venous conglomerations at the place of missing epididymal structures and frequent Morgagni's hydatides and spermatoceles. Preoperative palpation and surgical findings did not correlate. Laboratory examination of the ejaculate showed volume and fructose values below the norm in the majority of cases. Electron- and light-microscopy showed dissolution of spermatozoa and sperm-phagocytosis within the dilated epididymal canal as well as within the bordering tissue. Since laboratory- and physical examinations are misleading in some cases with vas aplasia surgical exploration is necessary. A cup-shaped Silicone-Dacron reservoir was implanted upon the remaining epididymal structures in 16 men with vas aplasia, but insemination of the aspirated material in their wives did not lead to any lasting pregnancy. In other centers 3 conceptions and normal births were obtained by this method.

[Treatment of fertility disturbances in the male with a combination of mesterolone and kallikrein].

73 males with different fertility disturbances (53 X oligozoospermia, 4 X asthenozoospermia and 16 X teratozoospermia) were treated by a combination of mesterolone (50 mg daily) and kallikrein (6 X 100 KU daily; every two hours one tablet); duration of treatment: three months. Before starting the treatment two semen analyses were performed; furthermore semen control at the end of the treatment as well as three and six months after ending the therapy. The sperm density don't demonstrate any changes. The normal morphology of the spermatozoa increased about 10%. In selected cases with a very bad sperm morphology a testicular biopsy were performed; in these cases the percentage of normal spermatozoa increased for about 21%. The motility rate showed no significant changes. It can be demonstrated that this combination don't better effects than other treatment. The best way is a treatment at a well selected material with a special drug for a special case.

[Surgical results of epididymovasostomy and vaso-vasostomy].

In the Urology Clinic of the University of Hamburg 326 epididymo-vasostomies have been done. A follow-up of 241 men was possible. Bilateral anastomosis resulted in a 47% permeability; after unilateral recanalisation a patency-rate of 32% was obtained. Vaso-vasotomy after vasoresection was performed in 56 men. Following a splinted one-layer anastomosis, which we abandoned in the meantime, patency was obtained in 64% of patients. Current techniques in one or two-layers without a splint resulted in a 88% permeability. The double layer anastomosis does not offer an advantage over the one-layer method.

[Experiences with vasectomy. Catamnestic study over a 10-year period].

Vasectomy can be a valuable contribution of the male to family planning. Vasectomy is a final method of contraception; it is not correct to make any hope for a later refertilisation because it is not sure that this operation will be successful. If a contraception is final, then it is not advisable to refer to methods of reconstruction or cryoconservation of sperm, because these methods are not always successful. Without intensive discussion with the couple and without sufficient time for the couple's decision, vasectomy can be very dangerous. Some specific examples are presented.

["What is normal?" - Ideals Exemplified on andrological aspects (author's transl)].

In andrology ther term "normal" stands for mean value but indicates also male fertility. Some data (ejaculate parameters, testis volume, coitus frequency, hormone levels) are discussed with special reference to the relation of age and fertility. The general situation of the patients has to be evaluated not single data only. It is postulated that the sexual behaviour of a couple is normal if both partners tolerate this. We promise a reconsideration of the present norm values of the ejaculate.

[Findings after vasectomy (author's transl)].

Investigations in a collective of 73 men sterilized by vasectomy showed, that the preoperative explanations about possible complaints were insufficient. This concerns too in pointing out the necessity of contraception methods after the operation until andrological examination has proved the success of the sterilisation.
