Mass survey for coronary lesions in school children with history of Kawasaki disease in Shimane Prefecture.

In 1981, a mass survey for Kawasaki disease was performed in Shimane Prefecture. The incidence of the disease was 0.3% in elementary and 0.1% in junior high schools, respectively. The number of children with a past history of Kawasaki disease was 40. Selective coronary arteriography (S-CAG) was performed on 37 children by December 1983 and 3 cases having coronary lesions were detected. In all these 3 children, more than 8 years had elapsed since the onset of the disease and in one of them there were severe stenotic changes in the coronary artery, with coexisting mitral incompetence. Two-dimensional echocardiography (2D echo) was able to detect dilatatory coronary lesions, but it failed to delineate stenotic changes adequately in these 3 cases. The present data indicate that 2D echo cannot demonstrate the stenotic segments of the coronary artery and therefore S-CAG is necessary for that purpose. We suggest coronary arteriography (CAG) for school children with a past history of Kawasaki disease.

Transmural conduction velosity index in healthy schoolchildren.

Transmural conduction velosity index (TCVI) was obtained in 174 healthy children. TCVI was defined as the echocardiographically determined interventricular septal thickness (IVST) divided by the ventricular activation time (VAT) measured by body surface potential mapping. TCVI ranged from 14 to 49 cm/sec and was highly correlated with IVST (r = 0.75, TCVI = 3.25 X IVST + 0.56). It was concluded that the left ventricular hypertrophy with muscular thickness does cause the greater conduction velosity in healthy children.

Changes in hemodynamics with advancing age in conscious spontaneously hypertensive rats.

The changes of hemodynamics were measured in spontaneously hypertensive rats (SHR) of increasing ages. Male SHR and Wistar rats of the Kyoto strain (WKY) at 4, 12, 24 and 48 weeks of age were used. The right jugular vein and the left femoral artery were cannulated and a thermistor was placed in the ascending aorta. After 24-hour rest, heart rate (HR), mean arterial pressure (MAP) and cardiac output (CO) were measured. The ratio of left ventricular weight (LVMI) of 4 week-old SHR had already increased significantly when compared to WKY. The HR in 4-week-old SHR was significantly higher than WKY. The increased HR in young SHR indicates the hypersensitivity of the sympathetic nervous system. Increased CO in 4 week-old SHR was due to high HR. The ratio of heart work to left ventricular mass (HW/LVM) of SHR at all age groups was not different from that of WKY, although the ratio of heart work to body weight (HWI) had a tendency to rise in SHR as compared to that in WKY. Our conclusion is that the development of LVM adapts to HW.

[Successful treatment of male urethritis with Chlamydia trachomatis by doxycycline].

Twenty male patients with urethritis caused by C. trachomatis were subjected to the clinical study of treatment with doxycycline (DOXY) 100 mg twice daily for 14 days. C. trachomatis was identified from urethral swab, both by direct fluorescein-conjugated anti-C. trachomatis species-specific monoclonal antibody technique (CTFA) and by culture using HeLa 229, in all cases before treatment. Almost complete agreement between CTFA and culture was obtained in the urethral swabs taken before treatment and after the 7th day of treatment. Disagreement between CTFA and culture especially CTFA-positive, culture-negative cases were observed at the third day of treatment. During the treatment course there might be the period where C. trachomatis antibody can be detected, but C. trachomatis can not be isolated by culture. After the 7th day of treatment C. trachomatis could not be identified neither by CTFA, nor by culture in all 20 cases, even after 2 to 5 weeks after the completion of the treatment. No notable side effect was experienced. Above results lead to the conclusion that treatment with DOXY 100 mg twice daily for 14 days is effective against male urethritis by C. trachomatis.

The relationship between blood pressure and left ventricular muscle volume in normal children and adolescents--the Shimane Heart study.

Blood pressure (BP) and left ventricular muscle volume (LVMV) were measured in 408 children and adolescents aged 9-15 years. LVMV was determined by M-mode echocardiography. Lean body weight was approximately calculated using body weight, upper arm circumference and triceps skin fold thickness. Lean-BSA was obtained by substituting lean body weight for body weight in the formula for BSA. Subjects were divided into the high and the low BP percentile groups according to their BP percentile values. LVMVI (= LVMV/BSA) and lean-LVMVI (= LVMV/L-BSA) were compared between the two groups. LVMVI and L-LVMVI were larger in the high BP percentile group than in the low one. Regression lines of LVMV to BSA were also compared between the two groups. Regression lines of the high BP percentile group were located in a higher position than those of the low percentile group. These results suggest that LVMV are larger in the high BP percentile group than in the low percentile group even if their BSAs are equal. We concluded that left ventricular hypertrophy has been already initiated in the pre-hypertensive stage of human beings as observed in spontaneously hypertensive rats.

Computer analysis of frank vectorcardiograms in normal children--maximal spatial QRS vector.

Frank vectorcardiograms recorded on magnetic tape were analyzed by a computer. The study population was 835 normal children aged from 6 to 16. Magnitudes, angles (azimuth, elevation) of the maximal spatial QRS vector and QRS duration were calculated. The magnitude of maximal spatial QRS vector increased significantly in males compared to those in females at ages 9-10, 11-12 (p less than 0.01) and 15-16 (p less than 0.001). Though there was no significant difference of elevation in either sex, azimuth tended to move more anteriorly in females than in males. QRS duration in males became longer with increasing age, while in females at age 15-16, it decreased (p less than 0.001).

A network data base system for children with cardiac disease--echocardiographic data of MCLS.

We developed a computer-assisted network data base system for children with cardiac diseases. It aims at storage and exchange of medical information from many hospitals to prevent school children from sudden death caused by heart diseases. The computer system used was PANAFACOM U-1500 introduced at Shimane Medical University Hospital in 1980. The format of the questionnaire is designed to standardize the computer-assisted data-entry method among different hospitals. The entries of questionnaires are composed of basic information about the patient and laboratory information. The former is identification data of the patient, and the latter principally the values of M-mode echocardiographic measurement. Two types of output-tables were made from the data base. One is a table of each disease, and the other each hospital.

Measurement of cardiac output by the thermodilution method in conscious spontaneously hypertensive rats.

The usefulness of the thermodilution method for measuring cardiac output (CO) was evaluated in awake rats by comparison with electromagnetic flowmetry. CO was measured in 3- and 6-month-old conscious spontaneously hypertensive rats (SHR) and normotensive Wistar-Kyoto rats (WKY). The correlation co-efficient between CO obtained by the two methods was 0.66 (p less than 0.01). Although CO values obtained by the thermodilution technique tended to be overestimated in comparison with those determined by electromagnetic flowmetry, this method was shown to be useful for measuring CO in unanesthetized rats because of its technical simplicity. Left ventricular mass (LVM) and the ratio of CO to LVM were significantly greater in SHR than in WKY at both ages and CO/LVM increased with increasing age in SHR. The ratio of heart work (HW) to body weight (HW/BW) was increased only in 3-month-old SHR compared with WKY and there was no difference in HW/LVM in 3- and 6-month-old SHR compared with age-matched WKY. The present results suggest that the development of cardiac hypertrophy in SHR is an adaptation to the increased HW due to high afterload.

[The effects of parotin components on testosterone biosynthesis in rats].

During our studies on the active moiety of parotin, we succeeded in purifying MP-parotin (MW = 130000) and parotin-subunit (MW = 45000) from crude parotin. Furthermore, AA-1 (MW = 9100) was isolated after the tryptic cleavage of the parotin-subunit. AA-1 was the smallest unit ever obtained which had serum Ca-decreasing activity and circulating leucocyte-increasing activity together with the nature of specific localization in bone. Therefore, AA-1 was considered to contain the essential residues for parotin activity in its structure. Since crude parotin was also known to show andromimetic action, these parotin components reduced in size were assayed in this regard. A daily injection of MP-parotin (500 micrograms/kg), parotin-subunit (20 micrograms/kg) or AA-1 (20 micrograms/kg) was administered subcutaneously for two weeks to male rats weighing 200 approximately 220 g. The dynamics of the testicular biosynthesis of testosterone from 3H-pregnenolone and 14C-progesterone via delta 4, delta 5-pathway and the transition of delta 5 to delta 4-steroids were measured. MP-parotin and the parotin-subunit stimulated the pathway of pregnenolone----17-hydroxypregnenolone----dehydroepiandrosterone---- androstenedione----testosterone and resulted in the elevation of serum testosterone levels. Cyclic AMP levels in the testicular homogenate and the motility of epididymal sperm were also increased by the treatment. On the other hand, AA-1 showed no effect on these parameters. It was concluded that andromimetic activity, which is involved in MP-parotin or parotin subunit was independent of parotin activity on the bone tissue.

Relationship between cardiac parameters and cube root of BSA corrected by subcutaneous fat weight.

Left ventricular internal dimension (LVIDd), aortic root dimension (AOD) and left atrial dimension (LAD) were measured echocardiographically in 688 normal children, ranging in age from 5 to 15. Lean body surface area (BSA) was calculated from height and lean body weight which was estimated from triceps skin fold thickness, upper arm circumference and body weight. Cardiac parameters (LVIDd, AOD, LAD) were well correlated with 3 square root BSA or 3 square root lean BSA (r = 0.72 approximately 0.89). Regression lines of LVIDd and AOD against 3 square root BSA or 3 square root lean BSA were compared between boys and girls. Statistically significant sex difference was observed in regression lines of LVIDd and AOD plotted against BSA (p less than 0.005), but this difference disappeared when plotted against lean BSA. It is concluded that the sex difference of LVIDd and AOD corresponding to a similar BSA depended on the difference of lean body weight between boys and girls.
