Relationship between left ventricular muscle volume and body surface area corrected by subcutaneous fat weight in normal children.

Left ventricular muscle volume (LVMV) was measured echocardiographically in 688 normal children ranging in age from 5 to 15. Lean body surface area (BSA) was obtained from height and lean body weight, which was estimated using triceps skinfold thickness, upper arm circumference and body weight. Regression lines of LVMV against BSA or lean-BSA were compared between boys and girls. There was statistically significant sex difference in regression lines when LVMV were plotted against BSA (p less than 0.005), but this difference disappeared when plotted against lean-BSA. We conclude that the sex difference of LVMV with a similar BSA originates from the difference of lean body weight between boys and girls.

Left ventricle volume characteristics in children with tricuspid atresia before and after surgery.

Left ventricular volume variables (left ventricular end-diastolic volume, ejection fraction, mass and systolic output) were quantified in (1) 13 patients with tricuspid atresia preoperatively (type Ia, 3 patients; type Ib, 9 patients; type IIb, 1 patient), (2) 4 patients after a shunt procedure (Blalock-Taussig, 3; Glenn, 1), and (3) 1 patient after corrective surgery. Cardiac catheterization and angiography were performed at age 1 month to 5.8 years (mean 1.4 years). All patients had decreased pulmonary blood flow. The results were compared with data in 15 patients with no significantly greater than normal in 12 of the 13 patients. Left ventricular mass was significantly greater than normal, but ejection fraction was significantly less than normal. There was a positive correlation between ejection fraction and systemic oxygen saturation. Left ventricular systolic index was normal in 9 of the 13 patients preoperatively. After a shunt procedure, left ventricular end-diastolic volume and systolic index increased significantly in all four patients, and ejection fraction improved greatly in three of the four. After corrective surgery, left ventricular end-diastolic volume and left ventricular systolic index returned to the normal range and left ventricular ejection fraction improved. These findings suggest that patients with tricuspid atresia have impaired left ventricular function.

Clinical estimation of left ventricular size: precordial electrocardiographic voltage corrected by the measurement of thorax.

The left ventricular muscle volume (LVMV) was determined echocardiographically and the radius of the circle with equal circumference to the thorax (R1) was calculated from thorax-circumference in 688 children as a part of the Shimane Heart Study. correlation of the summed precordial voltage (SV1 + RV5) with LVMV was not close (r = 0.377 in boys and -0.095 in girls). The precordial voltage (SV1 + RV5) was corrected by the square of R1 to compensate for the diminution of electrical potential by distance. Correlation between (R1) 2 x (SV1 + RV5) and LVMV was good in boys (r = 0.681) and relatively good in girls (r= 0.554). This fact indicates that LVMV can be predicted from ECG and anthropometric measurements of thorax without using echocardiography. Therefore, (R1) 2 x (SV1 + RV5) seems to have clinical applicability as the index of LV size, if the differences in (R1) 2 x (SV1 + RV5) and in the correlation coefficient between both sexes are clarified further.

[Studies on antigenicity, phototoxicity and other specific toxicities of hydrocortisone 17-butyrate 21-propionate (HBP) (author's transl)].

The present experiments were undertaken to determine the antigenicity and other toxicities of HBP; such as phototoxicity, photosensitivity, ulcerogenicity, adrogenic-myotropic, estrogenic and progestational activities, and mutagenicity. No antibody formation and delayed type skin reaction of HBP were seen in rabbits. Active systemic anaphylaxis was not observed in guinea pigs challenged by HBP. In the phototoxicity and photosensitivity test, 0.1% HBP ointment, 0.1% HBP cream and 10% HBP acetone solution did not show any skin reaction with or without irradiation of ultraviolet light. Repeated subcutaneous administration of HBP irritated the gastric and intestinal mucosa dose dependently in rats as hydrocortisone 17-butyrate and betamethasone 17-valerate (BV). HBP had neither androgenic-myotropic nor estrogenic activity, but antiestrogenic activity was observed. The progestational activity of HBP in immature rabbit pretreated with estrone was less potent than BV. In the mutagenicity test os HBP investigated by the reverse mutation according to the method by Ames, no significant increase in the number of revertants was observed in the presence or absence of S9 mixture.

Studies of D-penicillamine (3): immunomodulating effects of D-penicillamine.

D-penicillamine (D-PA) has beneficial therapeutic effects for patients with rheumatoid arthritis but no convincing explanation has been offered for the mode of action. Experiments reported here were designed to gain an insight into the related mechanisms. Wistar rats were inoculated with various doses of Mycobacterium tuberculosis to induce adjuvant arthritis, and on the 21st day, the lesions of paws and ears were graded according to the extent of the erythema and swelling. Rats given D-PA simultaneously with the inoculation of M. tuberculosis developed a more severe arthritis than that seen in the control group, when they were inoculated with low doses of M. tuberculosis. To investigate the effect of D-PA on hemolytic plaque forming cells (PFC) in the spleen, BDF1 mice were immunized with various doses of sheep red blood cells (SRBC) and D-PA was injected in various doses and at various times. D-PA produced either enhancement or depression of the number of PFC, depending on the dose of antigenic stimulus of SRBC. Furthermore, D-PA slightly enhanced the concanavalin A-induced blastogenesis of the spleen cells in vitro, at a concentration of 1-50 microM, but at concentrations of 500 microM, inhibition was evident. These results indicate that D-PA may act as an immunomodulating agent.

Basic study for computer analysis of the pediatric electrocardiogram: relation of the distance-corrected precordial electrocardiographic voltage with the echocardiographically determined left ventricular muscle volume.

The left ventricular muscle volume (LVMV) and the distance from the anterior chest wall to the middle of the left ventricle (D) were determined echocardiographically in 476 children as a part of the Shimane Heart Study. Correlation of the summed precordial voltage (SV1 + RV5) with LVMV was not good (r = 0.377 in males and -0.095 in females). The precordial voltage (SV1 + RV5), therefore, was corrected by the square of D to compensate the diminution of electrical potential by distance. Correlation between D2 (SV1 + RV5) and LVMV was good in males (r = 0.712) and relatively good in females (r = 0.469). Moreover, the values of D2 (SV1 + RV5) were different between both sexes at the age of 12--16 years. The thickness of subcutaneous fat may be one of the most important factors for these differences in D2 (SV1 + RV5) and correlation coefficient.

Cardiac hypertrophy in early hypertension.

Studies of cardiac hypertrophy in spontaneously hypertensive rats have indicated that left ventricular hypertrophy occurred even in the prehypertensive stage. These findings suggested that other factors besides blood pressure levels, and including possibly a genetic predisposition to myocardial hypertrophy, could play a role in structural cardiovascular alterations in spontaneously hypertensive rats. More recent studies have confirmed these anatomic results; left ventricular hypertrophy was vectorcardiographically detected even in the prehypertensive stage in voth young stroke-prone rats and stroke-resistant spontaneously hypertensive rats. Further, a close relation was found between degree of left ventricular hypertrophy and vascular hypertrophy or hyperplasia; this suggests that early detection of left ventricular hypertrophy may be a useful indicator of the incipient stage of structural vascular changes in genetic hypertension.

Echocardiographic and angiocardiographic features of supravalvular aortic stenosis in children.

Fourteen patients with SVAS served as subjects of echocardiographical and angiocardiographical studies and the data obtained were compared. There were significant decreases in aortic diameter at the level of ascending aorta compared to those of the aortic leaflets and significant differences of the percentage change of the aorta between the patients with SVAS and the normals. Echocardiography is, therefore, a useful noninvasive method for evaluating SVAS. If the values of percentage change of the aortic diameter obtained by echocardiogram are below -30%, the values of peak systolic pressure gradients across the supravalvular stenotic region should be higher than 20 mmHg. The value of LV M/V under 1.0 suggests that the pressure gradient is lower than 20 mmHg.
