[Pseudolupus induced by a vasculotropic drug (author's transl)].

The authors report eight cases of women suffering from pseudolupus erythematosus, with positive antimitochondrial antibodies. In six in the past history, a vasculotropic drug, Venocuran or Venopyridum, had been taken. All the patients had had episodes of fever, a pericarditis and/or myocarditis. A pleural effusion was present in six and pulmonary infiltrates in four. The course of the disease was for a limited period (1 to 3 months) in four cases and was chronic in the other four. One patient developed severe mitral incompetence and a cardiomyopathy.

[Diagnostic value of anti-mitochondria antibodies].

The histological lesions found in patients with hepatic disease and positive anti-mitochondrial antibodies (AMA) by immunofluorescence were compared with those in 10 subjects of the same age and sex with negative AMA. The majority of the patients with positive AMA were suffering from primary biliary cirrhosis, the diagnosis being based upon the histological appearance (Stage 1 of Scheuer's classification: 40%) or the presence of clinical signs of cholestasis with a compatible histological appearance (40%). None of the remaining patients (20%) with positive AMA nor the 10 individuals with negative AMA showed histological signs suggestive of primary biliary cirrhosis. The absence of certain clinico-biological factors, with the exception of Stage 1 histology, gives a high degree of value to a high level of AMA in the diagnostic of PBC.

[Early diagnosis of respiratory distress in premature infants. Application of the Clements test to the gastric fluid].

The stomach of the newborn infant contains swallowed amniotic fluid. The gastric fluid of 66 newborn infants was placed in contact with alcohol, following the technique reported by Clements in relation to amniotic fluid. The stability of the bubbles formed after mixing correspond to the presence of surfactant. In the case of respiratory distress in a premature infant, a positive test is in favour of transient respiratory distress. Intermediate or negative results are seen essentially in the presence of idiopathic respiratory distress. Application of the Clements test to gastric fluid collected immediately after birth is an excellent mean of diagnostic orientation in the presence of early respiratory problems in a premature infant. When negative, the child should be transferred to a specialised centre in order that intermittent positive pressure therapy may be started, the effectiveness of the latter being related to how soon it is begun.

Sucrose density differences of Chlamydia psittaci 6BC in relation to its host.

Previous studies on Chlamydia psittaci 6BC propagated in different hosts have shown differences in cytotoxicity but no differences in the ultrastructure of the individual particles. It is shown here that the 6BC strain derived from yolk sac of infected chick embryo sedimented in sucrose gradients at lower densities than the 6BC strain derived from L-cells. Host-related modifications of lipid concentrations of the 6BC strains have been previously documented by others. It is thought that the phenomenon of host-induced modifications of physical properties of the agent might provide a new approach to the study of chlamydial pathogenicity.

Systolic time intervals in the syndrome of midsystolic click and late systolic murmur.

Left ventricular function, as measured by systolic time intervals, was studied in 30 subjects with the syndrome of midsystolic click and late systolic murmur (ages, 20 to 53 years; mean, 34 +/- 9 years). Twenty-one were asymptomatic, six had chest pain, two had palpitations, one had shortness of breath, and one had hypotensive episodes. Each patient had the typical auscultatory and echocardiographic findings of the syndrome of midsystolic click and late systolic murmur. In the study the values for the interval between the onset of the QRS complex (Q) and the aortic component of the second heart sound (A2) (377 +/- 42 msec), the Q-A2 index (532 +/- 39 msec), the preejection period (PEP) (97 +/- 17 MSEC), THE PEP index (127 +/- 16 msec), the left ventricular ejection time index (403 +/- 26 msec) and the ratio of PEP to left ventricular ejection time (0.35 +/- 0.01) were not significantly different from the values in 17 controls matched for age. Out study, then, supports the contention that left ventricular function remains well preserved in most patients with the syndrome of midsystolic click and late systolic murmur.

[Islet cell adenoma with neonatal onset. Clinical, hormonological and ultrastructural study of a case].

A case of hyperinsulinism occuring in a newborn, with a birthweight of 4,050 g, is reported. The hypoglycaemia was refractory to the usual therapy (increase of glucose administration per os, and I.V., corticosteroids, glucagon, diazoxide). At surgery, undertaken at 9 days of age, an adenomatous nodule was removed along with the left part of the pancreas. Death occurred at 18 days, after the child had developed a transitory acidoketosic diabetes and an encephalopathy. Measurement of insulin by radio-immunoassay revealed a strong increase in the ratio insulin/glycaemia, characteristic of nesidioblastoma, as well as a high concentration of insulin in the tumor as compared to normal tissue. On the ultrastructural level, the observed features differed from those seen in children and adults and showed an abnormal overload of dense deposits in the cytoplasm of some histiocytes.

[Fluctuating deafness (author's transl)].

The problem of fluctuating deafness merits a separate place in otological pathology. On the basis of 52 cases of recurrent fluctuations in hearing in patients observed over a sufficiently long period, the authors provide evidence to the effect that, in addition to pressure disturbances accepted up to now, other mechanisms may be responsible. An aetiological study emphasises the role of local factors, but examination for general causes is also necessary. Clinical study of fluctuating deafness also reveals interesting data which, taken in combination with pathogenic hypotheses, are of therapeutic significance.

Possible high dosage gentamycin therapy in infected neutropenic patients.

Serum gentamycin levels have been studied in 23 neutropenic infected patients after high doses (62 mg/m2 and 72 mg/m2) of the drug, we observed: a higher gentamicin level than with an usual dosage, levels under the ototoxic 10/ml level, no renal failure, and, no patient ototoxicity.
