[Prolonged survival in 16 cases of acute myeloid leukemia [author's transl)].

16 our of 370 acute myeloid leukemias treated between 1964 and 1973 have been in long survival for more than three years. In the first period 1964-1971 the overall remission rate was 20% and 6 patients has a long survival. In the last two years of study, 10 out of 96 patients have more than three years of survival, the remission rate was over 50%. All the cases were slightly proliferative the first group did not recieve intensive chemotherapy and appear as spontaneous long remission, but in thelast patients intensive chemotherapy seems responsible for the good results.

Developmental relationships between vitelline and intra-embryonic haemopoiesis studied in avian 'yolk sac chimaeras'.

Using the quail-chick marker technique, cells have been traced in the haemopoietic organs of 149 developing chimaeras composed of a quail embryo with a chick yolk sac. The existence of intraembryonic stem cell relaying yolk sac stem cells, previously demonstrated with this system, is confirmed. Thymus, bursa of Fabricius and bone marrow are preferentially populated by intraembryonic stem cells. The spleen shows a transitory phase of colonization by chick yolk sac stem cells at 11--12 days of incubation. From 7 days onwards the yolk sac receives quail stem cell emigrating from the embryo.

BCG therapy in acute non lymphoid leukaemias.

The survival of patients with acute non-lymphoid leukaemias (ANLL) receiving chemotherapy (32 patients) or chemotherapy plus BCG (31 patients) has been compared in a randomized trial which started in Nov 1974. Fresh BCG (Institut Pasteur, Paris) was administered by Heaf gun in the interval of chemotherapy cycles. The overall survival was better in the chemo-immunotherapy group (median duration of survival of 25 months compared to 16 months in the chemotherapy group). However, the first remission duration did not differ (median duration of the 15 and 12 months, respectively). Plateau survival curves were not obtained in either group. A second complete remission was obtained in 7 out of 12 patients with bone marrow relapses in the chemo-immunotherapy group, and only in 2 out of 15 in the chemotherapy group (P less than 0.05). BCG seemed more efficient in patients older than 40 years; no difference was observed according to sex.

[Effect of dobutamine on intra-pulmonary shunt].

A study of the development of intra-pulmonary shunts was carried out in ten subjects following an intra-venous administration of a 7.5 microgram/kg/min dose of dobutamine by electric syringe. All the subjects were on a respirator (FiO2=0,6 most often) and frequently with P.E.E.P. The shunts were determined at the FiO2 by which the subject was being treated. The cardiac flow was measured by thermodilution. In 9 cases out of 10 the intra-pulmonary shunt is sharply increased already in the first half hour by dobutamine. The shunt values are in the order of: -- 17.94p. 100 +/- 7.19 before the drug -- 26.50 p. 100 +/- 12.85 half an hour after the infusion beginning. The shunt increase is thus 47p. 100 of the average original value after half an hour, which is significant (p less than 0.01). The shunt is then stable at the attained value (hourly average : 26.05 +/- 12.25). These observations are discussed in relation to the obtained effects on the cardiac outout, to the PaO2, and to the quantity of transported arterial oxygen. It appears in this series that, in spite of the shunt increase, there is no risk of cellular oxygen deficiency.
