Urea metabolism in an azotaemic woman with normal renal function.

The blood-urea of a 43-year-old woman with normal renal function was 150 mg. per 100 ml. because she regularly consumed about 400 g. of protein daily. She had no symptoms of uraemia and her haemoglobin concentration was normal. Although she had a very large body-urea pool she degraded no more urea than healthy individuals on a normal diet. The failure of endogenous urea hydrolysis to increase in proportion to the blood-urea in renal failure has tentatively been ascribed to toxic effects of compounds retained with urea in renal failure; the findings in this azotaemic patient, who had neither signs nor symptoms of uraemia, cast doubt upon that explanation.

Report of a family with inherited medullary carcinoma of the thyroid and phaeochromocytoma.

A family in which several members have medullary carcinoma and phaeochromocytoma is described. Four of the 5 patients in this family who had a phaeochromocytoma have died; in one of these the medullary carcinoma may have contributed to death but in general the thyroid cancers were slow growing. In our experience successful diagnosis and treatment of the phaeochromocytomas have presented the greatest challenge. The welfare of future generations with this syndrome would seem to depend upon regular clinical and biochemical surveillance of the individuals and expeditious diagnosis and treatment of their tumours, particularly the phaeochromocytomas.

The incorporation of ammonia nitrogen into albumin in man: the effects of diet, uremia and growth hormone.

15 N-ammonium chloride was given orally to 17 individuals in 20 studies. Retention of 15N by those with normal renal function was inversely proportional to the protein intake; enrichment of albumin with 15N increased during protein restriction. Protein restriction appeared to be a more potent stimulus to the synthetic use of ammonia than a large pool of urea nitrogen. Both retention of isotope and enrichment of albumin decreased when a high protein diet was given. Enrichment of albumin with 15N in response to 10 or 20 mg of growth hormone daily was similar to that in healthy individuals on a 20 g protein diet for 3 weeks. The relevance of these findings to the nutritional potential of recycled urea nitrogen is discussed.

Ovarian cytology. An application of cytology in an attempt at the early detection of ovarian carcinoma.

The possibility of detecting pre-clinical ovarian carcinoma by ovarian cytology taken at the time of laparotomy has been studied in 472 patients. Malignant cells have been recovered from ovarian carcinomas but never from macroscopically normal ovaries. It is suggested that this simple, inexpensive technique of sampling cells from the ovarian surface should be continued to be practised on all occasions at which ovaries present such as at laparotomy or at laparoscopy, as with further experience this technique may prove to be of help in the early diagnosis of ovarian carcinoma.
