[A preliminary study for cellular, albumin and immunoglobulin components of bronchoalveolar lavage fluid in normal control, pulmonary T.B. and malignant lung diseases].

Bronchoalveolar lavage (BAL) is a new diagnostic tool which could be applied repeatedly to investigate the nature of lesion in pulmonary diseases. To evaluate the clinical significance of the difference in immunoglobulin content for the differential diagnosis between benign and malignant pulmonary diseases, we performed bronchoalveolar lavage for 64 patients. They included 12 patients with bronchogenic carcinoma, 12 patients with pulmonary tuberculosis, 20 patients with benign non-specific bronchopulmonary diseases (these including 6 patients with pneumoconiosis, 6 patients with bronchietasis, 2 patients with paraquate intoxication, 6 patients with pneumonia) and 20 healthy persons as of the control group. We analyzed the following items: (1) cell count (2) differential count (3) protein content (4) immunoglobulin content. The results showed that there were increase in PMN cells in paraquate intoxication, pnuenmonia and bronchiectasis. While there was lymphocytosis in pulmonary tuberculosis. In comparison with the control group, there was elevation of statistical significance in the IgA/Albumin ratio in bronchogenic carcinoma and pulmonary tuberculosis. Although the IgG/Albumin ratio would be elevated with statistical significance among a variety of pulmonary diseases, it was particularly high in pulmonary tuberculosis. In contrast, the IgA/IgG ratio was significantly increased only in bronchogenic carcinoma. In conclusion, there were differences in the cell number, differential cell count, protein content and immunoglobulin content among various pulmonary diseases. It may be useful in making differential diagnosis between benign and malignant pulmonary disease and the prediction of prognosis.(ABSTRACT TRUNCATED AT 250 WORDS)

[T-cell subpopulation of bronchoalveolar lavage fluid in patients of sarcoidosis and hypersensitivity pneumonitis--2 cases report].

Diffuse interstitial lung diseases include more than 100 varieties. Even for a chest specialist differential diagnosis is difficult; accurate diagnosis without transbronchial lung biopsy or open lung biopsy was particularly hard. Bronchoalveolar Lavage (BAL) is a new diagnostic procedure which is relatively safe and repeatable. It takes about 30 minutes more, during the usual bronchoscopic examination, to obtain fluid and cells from the lower respiratory tract. These specimens aid differential diagnosis and an understanding of pathogenesis in the early stage of diffuse interstitial lung disease. Two cases are reported. One was sarcoidosis, as proved by mediastinoscopic lymph node biopsy; the other one was hypersensitivity pneumonitis by clinical impression. Both patients received bronchoscopic examination with bronchoalveolar lavage. The bronchoalveolar lavage fluid showed an increase in total cell count, lymphocyte count and lymphocyte percentage. In sarcoidosis, the main T-lymphocyte subpopulation was helper T-lymphocytes while, in hypersensitivity pneumonitis, the main T-lymphocyte subpopulation was suppressor T-lymphocytes. We concluded that, if the patients of diffuse interstitial lung diseases had increased lymphocyte count and percentage in bronchoalveolar Lavage fluid, examination of the lymphocyte subpopulation would help differential diagnosis.

Familial paracentric inversions inv(2)(q31q35) and inv(8)(q22.3q24.13) ascertained through reproductive abnormalities.

Two cases of familial paracentric inversion, one in the long arm of chromosome 2 and the other in the long arm of chromosome 8, are described. The first was ascertained in a woman who was studied because of recurrent abortions. The second was ascertained in the father of a girl with the trichorhinophalangeal syndrome and an interstitial deletion in 8q. The latter is the first case in which unequal crossing over in an inversion loop can be inferred in a male carrier of a paracentric inversion. The reasons for the relatively low frequency of paracentric inversions observed and factors which affect the pregnancy outcome are discussed.

Cytogenetic and biological characterization of two new human plasma cell lines.

Two new human plasma cell lines designated as ACB-885 and ACB-1085 have been established from a 39-year-old patient with multiple myeloma. These cell lines have definitive plasma cell features by morphologic examination, and essentially all of the cells are positive for cytoplasmic IgG kappa immunoglobulin. These cells are negative for standard T-cell surface markers and mature B-cell markers, such as B1, B2, and HLA-DR, but are strongly positive for the antigen defined by OKT-10. The cells are negative for Epstein-Barr virus. The cell lines have a doubling time of 30-35 hours and a growth fraction approaching 100%. Cytogenetic analysis showed a 2n chromosome number of 45-46 with very similar karyotypic abnormalities in both the plasma cell lines and the original tumor material. One of the chromosomes in each of the pairs of chromosomes number #1, #2, #6, #7, #8, #10, #12, #13, and #22 were consistently missing. These were replaced by eight marker chromosomes that resulted from chromosomal rearrangements involving mainly these missing chromosomes. Almost all of the breakpoints occurring in the marker chromosomes were identified, and eight of these breakpoints have been reported in other studies of myeloma plasma cells. Homogeneously staining regions were observed in two marker chromosomes suggesting gene amplification in these chromosomal regions.

Comparative studies on aetiology and epidemiology of viral conjunctivitis in three countries of East Asia--Japan, Taiwan and South Korea.

A total number of 1105 cases with viral conjunctivitis, mainly epidemic kerato-conjunctivitis (EKC) consisting of 354 cases from Sapporo, Japan, of 628 from Kaohsiung, Taiwan and of 123 from Busan, Korea, encountered during the periods, 1980-81 and/or 1983, were studied aetiologically and epidemiologically. Patients were aged from 27 days to 88 years with a peak in the 20-29 year age group. Aetiological diagnoses were established in 610 cases (55%), consisting of 536 cases (49%) caused by adenoviruses and of 74 (7%) by EV 70. The most frequently detected agent was Ad 8 (57%), followed by EV 70 (12%), Ad 3 (9%) and Ad 19 (7%). The aetiological profiles of viral conjunctivitis were generally similar in three cities of East Asia. EKC (70%) was mainly caused by Ad 8, Ad 19 and Ad 37, but AHC (13%) by EV 70 and PCF (5%) by Ad 3 and Ad 11. Ad 19 and Ad 37 isolates in three cities were compared with the cleavage patterns with restriction endonucleases, BamHl, Smal, Sall and Hindlll. All of the Ad 19 isolates tested were identical to Ad 19a reported by Wadell et al, and all but three isolates of Ad 37 were identical to the Ad 37 prototype prevalent in Europe and the US. The three isolates of Ad 37, different from the prototype in cleavage pattern with Hindlll, designated as Ad 37A, were detected in Sapporo and Kaohsiung in 1980. From the cleavage patterns with four restriction endonucleases, Hindlll, BamHl, Sa1l and Sstl, the Ad 8 isolates in 1983 were divided into three subtypes, which were associated with the cities isolated.(ABSTRACT TRUNCATED AT 250 WORDS)

Structural identity of the subunits of pigeon liver malic enzyme.

Pigeon liver malic enzyme was found to have arginine, alanine, and tyrosine as the only N-terminal, N-1, and N-2 amino acids, respectively. Hydrolysis of the reduced and carboxymethylated malic enzyme by carboxypeptidase A yielded quantitative evidence for the following C-terminal sequence: -Leu-(Phe-Ala)-Ile-Leu-COOH. Fifty-five trypsin-digested peptides were separated by HPLC, in accordance with the arginine and lysine contents of each subunit. This more direct structural evidence strongly supports the conclusion that pigeon liver malic enzyme is composed of four chemically identical subunits.

DNA clonal heterogeneity of hepatocellular carcinoma demonstrated by Feulgen-DNA analysis.

To demonstrate DNA clonal heterogeneity of hepatocellular carcinomas (HCC), the DNA histographic pattern of both primary HCCs and their recurrent or metastatic lesions were studied among 36 patients (33 men and 3 women). Thirty-six paired aspirations or imprints taken from primary, recurrent or metastatic lesions were stained, using the modified Feulgen method, and the DNA content was measured with a scanning microdensitometer at a wavelength of 550 nm. Paired aspirations or imprints taken from different parts of the same HCC were examined in 17 cases; the DNA distribution patterns were similar in 15 (88%) and differed in only two (12%). A similar DNA histogram was also shown among different tumors in 10 (71%) of 14 patients with multiple HCCs, with a DNA ploidy discrepancy in only four (29%). Two of two subcutaneous metastases and two of three recurrent tumors showed DNA distribution patterns similar to those in their primary HCCs. In summary, a DNA clonal heterogeneity of HCC was found in 19% (7/36). In contrast, the similar DNA histographic patterns found in most instances among different parts of the HCC and between the primary and recurrent or metastatic lesions suggest that HCC may derive from a single cell clone in the majority of cases.

Pharmacokinetics and metabolism of an intravenously administered penem (Sch 34343) in humans.

The pharmacokinetics of Sch 34343, a new broad-spectrum penem antibiotic, was studied in subjects receiving 1 g of 14C-labeled drug by intravenous administration. At the end of a 30-min intravenous infusion, the mean maximum concentration of drug in serum was 39 micrograms/ml for unchanged Sch 34343 and 49 mu eq/ml for total radioactivity. The mean serum half-lives of Sch 34343 were 0.16 h for the distribution phase and 0.80 h for the elimination phase. The total body clearance of Sch 34343 was 7.52 ml/min per kg, and the mean apparent volume of distribution was 525 ml/kg. Over a 4-day period, mean urinary excretion of radioactivity accounted for 87.9% of the dose, and mean urinary excretion of unchanged Sch 34343 accounted for 23.6% of the dose. The total radioactivity in feces on days 0 to 6 accounted for only 0.8% of the dose. In serum from 0.5 and 1 h, unchanged Sch 34343 represented the major radioactive peak, with negligible amounts of several metabolites. In urine, there were at least six metabolites in addition to Sch 34343. The amount of unchanged Sch 34343 accounted for 33% of radioactivity in samples of urine from 0 to 2 h, 22% in urine from 2 to 4 h, 15% in urine from 4 to 8 h, and 0% in urine from 8 to 12 h.

Incidence and inheritance of the 1/29 and 14/20 Robertsonian translocations in Canadian beef cattle.

Metaphase chromosomes were prepared from peripheral leukocyte cultures of 253 beef animals representing 21 breeds. A total of 10 Robertsonian translocations were identified. Nine were 1/29 fusions and one was a stable 14/20 fusion identified in several Simmentals. To our knowledge, the only other case of this translocation reported was also in the Simmental breed, suggesting that this fusion may be unique to the breed. A further 54 relatives of known 1/29 and 14/20 carriers were examined, revealing that both fusions were inherited by 50% of offspring of the heterozygous carrier. Heterozygous male 1/29 carriers left fewer calves than karyotypically normal bulls when used in natural service.

The higher order structure of the centromere.

The architecture of the centromere region of mouse chromosomes has been studied in cells grown in the presence of 5-azacytidine. This drug interferes with normal condensation producing elongated centromere regions. It has been found that this effect is reversible in the presence of the drug, allowing the observation of the repackaging of the extended centromere into a structure exhibiting native centromere morphology. Light microscopy as well as transmission and scanning electron microscopy of this condensation process suggests that the native centromere is formed by the helical folding of a subfiber with an approximate diameter of 100 nm. This fiber is in turn composed of loops of the 30-nm fiber class. The boundary between successive gyres of the subfiber are obscured at the completion of condensation resulting in the formation of a homogenous 250- to 300-nm fiber that is the native centromere. These observations provide evidence for an additional level of chromatin organization within the metaphase chromosome.

Natural killer cell activity in lung cancer patients.

A total of 50 lung cancer (CA) patients and 20 normal persons as controls were included in our study. We obtained lymphocytes from heparinized blood by Ficoll-Hypaque gradient method, and after the macrophages were removed, they were incubated with 51Cr-labelled K562 cells at concentrations of 100:1, 50:1, 12.5:1 for four hours. We calculated the cytotoxicity activity and lytic unit (LU) of natural killer (NK) cells. Significant impairment was noted in the comparison of NK cytotoxicity and LU between stage III-M1 lung CA patients (n = 28) and the control group. However, significant impairment was also noted in stage III-M1 and III-M0 patients. Four cases in stage I underwent surgery, and the NK cytotoxicity before the operation was noted to be low. After the operation, the NK cytotoxicity increased. These results indicate that NK cytotoxicity assay was impaired in stage III-M1 lung CA and also suggest that it may act as a therapeutic guide in determining the results of the operation.
