Roller traction: mobilizing patients with acetabular fractures.

Patients with acetabular fractures unamenable to open reduction and internal fixation are subject to 6-12 weeks of traction and, traditionally, bed rest. Using roller traction allows the patient to be mobilized to a bedside chair and to stand without disrupting the traction. This is accomplished by slightly modifying a standard lower extremity skeletal traction setup. Roller pulleys and stopper clamps are substituted for the usual pulleys. These are attached to extra-long horizontal bars that expand the lateral space that the patient can now use around the bed. Roller traction helps prevent the common complications of prolonged bed rest, particularly deep vein thrombosis, skin breakdown, pulmonary stasis and boredom.

Somatization disorder in sphincter of Oddi dysfunction.

OBJECTIVE: Sphincter of Oddi dysfunction (SOD) is a biliary disorder with a recognized pathophysiology and demonstrated surgical treatment. We sought to examine whether women with SOD were overrepresented on measures of somatization, sexual and physical abuse, socially compliant attitudes, and familial psychiatric illness. METHOD: We matched 33 women with SOD to 33 normal controls by age, sex, and race, and, with a case-controlled cross-sectional questionnaire, compared the groups on the study variables. Statistics included a chi 2 for categorical variables, t tests for scores of somatization and attitudes of social desirability, and Pearson correlation coefficients for post hoc associations of variables. Bonferroni corrections were used with chi 2 values to reduce capitalization by chance. RESULTS: SOD patients exhibited excessive nongastroenterological somatic complaints compared with controls (p < .0001). There was a statistical increase in reports of childhood sexual, but not physical, abuse in the SOD women (p < .02) compared with controls. The severity of the abuse correlated strongly with the severity of somatic complaints. There were no differences in social desirability attitudes or family psychiatric histories of the two groups. CONCLUSIONS: We conclude that SOD is associated with a high degree of somatization in adulthood, and a mean rate more than four times that of controls in self-reports of sexual abuse in childhood. The severity of childhood sexual abuse is correlated with the severity of somatization in later life. A psychological model for this disorder is suggested by the data. Increased psychiatric attention is indicated in the treatment of women with this disorder.

The influence of selected variables on the experience of menstrual distress in alcoholic and nonalcoholic women.

Studies indicate that some alcoholic women relate their drinking to their menstrual cycle. Studies also indicate that the menses can serve as a situational stressor for certain alcoholic women. A study of 100 alcoholic and 100 nonalcoholic women was designed to investigate factors that influence menstrual distress. The factors of age, education, income, occupational status, and menstrual distress were investigated. None of the predictor variables accounted for significant amounts of variance in the criterion variable (menstrual distress). Alcoholic women were found to have a higher degree of menstrual distress than nonalcoholic women. The relationship of race to menstrual distress deserves further investigation. Nursing implications are presented.

Glial fibrillary acidic protein expression in pleomorphic adenoma of salivary gland: an immunoelectron microscopic study.

Previous immunocytochemical studies of pleomorphic adenomas have demonstrated consistent labeling with glial fibrillary acidic protein (GFAP). Cross-reactivity with other intermediate filaments of similar structure and chemical composition has been suggested to account for this seemingly inappropriate pattern of immunoreactivity. To investigate further this phenomenon, we examined five pleomorphic adenomas by immunoelectron microscopy. Ultrastructural features were similar to those described by other investigators, with ductal epithelium being surrounded by myoepithelial cells and modified cells becoming detached to form the isolated stellate and spindle cells of the stroma. As part of this process, many neoplastic myoepithelial cells appeared to lose their specialized ultrastructural features, assuming a rather undifferentiated appearance. Single and double immunoelectron microscopic labeling showed vimentin filaments in all these neoplastic myoepithelial cells. In contrast, GFAP filaments were identified only in the most undifferentiated cells. Such restriction of GFAP filaments to an ultrastructurally definable subset of neoplastic cells provides strong evidence against nonspecific staining due to cross-reactivity. Given the previously described coexpression of vimentin and GFAP by neoplastic cartilage, it appears likely that this immunophenotype in neoplastic myoepithelial cells reflects early chondroid differentiation.

Blood pressure lowering for the prevention of cognitive decline in patients with cerebrovascular disease. PROGRESS Management Committee. Perindopril Protection Against Recurrent Stroke Study.

Cerebrovascular disease and high blood pressure both appear to increase the risk of vascular dementia. PROGRESS aims to investigate whether blood pressure lowering with an angiotensin coverting enzyme inhibitor-based regimen will reduce the risk of cognitive impairment in patients with a history of stroke or transient ischaemic attack. A total of at least 6000 patients will be randomised to receive perindopril (+/- indapamide) or matching placebo(s), with treatment and follow-up scheduled to continue for at least 4 years. Substudies will investigate the effects of treatment on cognitive decline in subgroups defined by apo-E genotype and on white matter lesions assessed by magnetic resonance imaging. Final results from the study should be available in 2001.

Altered oncogene, tumor suppressor and cell-cycle gene expression in PANC-1 cells cultured with the pleiotrophic 5-lipoxygenase inhibitor, MK886, assessed with a gene chip.

We describe a genomic response of mRNAs associated with a subset of oncogenes, tumor suppressor and cell cycle-related genes in proliferating human Panc-1 pancreatic cancer cells after 24 hours of culture with MK886, a pleotrophic 5-lipoxygenase inhibitor. Ninety-eight of these cDNAs are represented in one of the sub-arrays included in the Clontech Human cDNA Expression Array. In this initial analysis, control cells exhibited apparent widespread low levels of disparate mRNA synthesis. In cells cultured with 40 microM MK886 for 24 hr, while most expressed genes, including a number of specific proliferation-enhancing genes such as c-myc were inhibited, 19 other ones including some countervailing genes including tyrosine SRC protein kinase, cyclins B1 and D1, CDC25B phosphatase and 40s ribosomal S19, amounting to 19 percent of the cDNAs resident on the chip were up-regulated at > 1.10 experimental/control values. Therapy-induced activation of compensatory proliferative genomic responses provides an additional explanation why malignant cells can fail therapy. Among their many future uses, gene chips clearly will be an extremely powerful tool for identifying relationships between the hierarchical linear and non-linear control and implementation-related cellular events and for identifying potential molecular targets tor cancer therapy.

Surveillance for West Nile virus cases in Louisiana 2001-2004.

West Nile virus (WNV) was first detected in Louisiana during August of 2001. An outbreak of 204 human cases of neuro-invasive disease (NID) and 25 deaths occurred in 2002. In the 2 years following, lower numbers of human cases were identified (101 NID cases in 2003 and 84 in 2004) but intense localized foci were observed. The incidence of NID has been particularly high in the elderly (65 years and older). The distribution of West Nile cases has consisted of sporadic cases with a few very intense foci. Annually, human cases have occurred from June through December, with a peak number of new cases in August. As compared with other WNV serosurveys conducted in the United States, it appears that the WNV seroprevalence in Louisiana is not elevated.

Radial keratotomy in a patient with keratoconus.

A 33-year-old patient had radial keratotomy performed on both eyes for relief of longstanding myopia. Minimal reduction of myopia was obtained, and the patient underwent a second radial keratotomy in both eyes. Best corrected visual acuity decreased, and the patient had a marked, irregular astigmatism postoperatively. Careful ophthalmologic evaluation revealed that the patient had keratoconus bilaterally. Following a failed trial of contact lens wear, the patient underwent a penetrating keratoplasty of the right eye. Pathologic evaluation revealed findings of keratoconus, as well as radial keratotomy scars. This case report illustrates the importance of a complete ophthalmologic examination possibly including videokeratography, prior to keratorefractive surgery.

Plasma levels of immunoreactive parathyroid hormone in dogs chronically exposed to low levels of cadmium chloride.

A previous report had suggested that chronic cadmium intoxication interfered with calcium accumulation in bone and suggested that the mechanism may be a parathyroid-induced bone resorption. We measured C-terminal parathyroid hormone levels in the blood of four male standardized research beagles chronically exposed during 6 months to 25 ppm cadmium chloride in their drinking water while at the same time we performed measurements of the parameters of haversian bone remodelling activity. We failed to demonstrate any statistically significant difference in levels of this hormone in the experimental dogs before or during the exposure, or between samples obtained from the experimental dogs at the end of their period of exposure and normal untreated control dogs from the same colony. We conclude that in the early stages of chronic cadmium intoxication before there is evidence suggestive of intestinal or renal malfunction that may secondarily involve the parathyroid glands, the alteration observed in haversian bone remodelling rates is not mediated through abnormalities of function of the parathyroid glands.

Heat shock protects kidneys against warm ischemic injury.

The present work confirms previous observations made by others that heat shock is followed by a period of protection against tissue injury. Furthermore, this protection against associated with HSP 72 production. We have identified some detrimental effects of heat shock as a technique for induction of the HSR. Clearly there exists in living tissues a highly conserved, latent cellular mechanism that can be rapidly activated by stress, which can provide useful protection against additional stressful conditions. The ability to control the expression of this response should enable one to protect cells, tissues, and organisms against situations in which tissue injury is anticipated.

Fine needle aspiration cytology of pulmonary carcinoid tumors.

Twenty-four cases coded as pulmonary carcinoid tumors initially sampled by fine needle aspiration (FNA) biopsy were reviewed in order to determine the cytologic features most useful in making the FNA diagnosis. The diagnosis of carcinoid tumor had been confirmed in 23 cases; the remaining case, though closely resembling a carcinoid tumor on the FNA specimen, proved to be a sclerosing hemangioma of the lung. Comparison of the original and review interpretations of the FNA specimens revealed that all typical spindle cell carcinoids and all atypical carcinoids were correctly diagnosed and classified. Of the 15 typical round cell carcinoids, the original cytologic diagnosis was lymphoma in 2 cases and benign bronchial lining cells in 2 cases. Thus, it appears that diagnostic errors are most likely in "typical" carcinoids. Review of the FNA findings suggests that the frequently stripped cytoplasm (with resulting non-cohesive bare nuclei), coupled with the almost universal plexiform vascularity (seen in 21 of 23 cases), should allow an accurate cytologic diagnosis in virtually all cases.
