[Results and indications of cochlear implant in 19 cases of total pre-speech deafness].

From June, 1987, to January, 1900, 19 patients aged 3 to 56 years have been rehabilitated for total deafness occurring prior to the acquisition of speech, ie. before age 2, whatever its cause. Some degree of hearing was achieved in all cases. However, the rate and steadiness of progress in auditory recognition, the daily period of use of the device and the changes in the behavior were inversely proportional to the patient's age. This leads to proposing cochlear implants for all patients, especially children, and all the more largely as they are young.

Seromonitoring in small laboratory animal colonies. A five year survey: 1984-1988.

From 1984 to 1988 one thousand serologic investigations of laboratory animal colonies originating from 10 different European countries were performed. The most prevalent infections in mouse stocks were caused by Mouse hepatitis virus (MHV), Minute virus of mice (MVM), Theiler's encephalomyelitis virus (TEMV), Reovirus type 3 (Reo3), Sendaivirus, and Pneumonia virus of mice (PVM). In mice no infections with Lymphocytic choriomeningitis virus (LCM), Polyomavirus, Mouse adenovirus, and K-virus were recorded. Only two colonies were infected by Ectromelia virus. The first six virus infections of mice were also found in rat colonies as well as the rat-specific Coronaviruses (Sialodacryoadenitisvirus--SDA, Rat corona virus--RCV) and Parvovirus (Kilham rat virus--KRV, Toolan H-1 virus) being endemic. Antibodies to Bacillus piliformis were detectable in about 50% of rat stocks screened. This is in contrast to the mouse, where only about 10% of the colonies were found to be positive. A similar picture was seen for M. pulmonis which is primarily an infection of the rat. In mice no case was detected during the last two years. The number of investigations performed from guineapig, hamster and rabbit colonies was relatively low. Nevertheless, antibodies against the following antigens were detectable: In guineapig stocks: Reo3, PVM, Sendai, Simian virus 5 (SV5) and B. piliformis; in rabbits: Reo3, Sendavirus, SV5, and B. piliformis; in hamsters: PVM, LCM and B. piliformis. The overall contamination rate showed a continuous decrease until 1988. Nevertheless, about 50% of mouse and rat stocks still exhibited antibodies to one or more viral infections.

[Clinical results of the surgical treatment of 1222 cases of chronic snoring].

Uvulopalatoplasty was performed (whether or not associated with tonsillectomy and/or septoplasty) in 1,222 cases of chronic rhonchopathy. Among them, 65 cases consisted of pre- and postoperatively registered sleep apnea syndrome. Clinical results were assessed on improvement of preoperative snoring, apneas, morning asthenia, daily sleepiness. Surgery was totally successful in 50% of cases, partially in 35%. Failure was encountered in 15% of cases. Failure or partial improvement are related to the preoperative presence of one or several of five particular risk factors: neck shortness, tongue hypertrophia, retrognathia, obesity and nasal pathology. Surgical complications were rare. Severe rhinolalia occurred in five cases: only three of them were sufficiently to accept plastic reconstruction which was successful.

[Physiopathologic value of cephalometric teleradiography study and magnetic resonance imaging in sleep apnea syndromes. Therapeutic deductions].

A cephalometric study was carried out on 98 patients assigned to 3 groups including 48 patients with sleep apnea, 25 patients with chronic rhonchopathy without apnea, and 25 controls, respectively. 44 patients benefited from lateral teleroentgenographic investigations and 54 had sagittal-median NMR sections visualized. Various distance, angle and area measurements were obtained. The authors found that anomalies were the more severe that snoring was associated with sleep apnea. These arguments speak for an evolutional character of the disease. The various cephalometric anomalies were primarily related to the hyoglossal apparatus, more rarely to the maxillary-mandibular basal bones. After this analysis, the authors propose a therapeutic strategy for the management of chronic snoring associated with sleep apnea.

[New analysis of the results of uvulo-palato-pharyngoplasty in sleep apnea syndromes. Apropos of 65 cases].

65 cases of sleep apnea syndrome due to undergo uvulo-palato-pharyngoplasty were subjected to pre-operative and then post-operatoire, after 3 months, polysomnographic recording. The authors analysed the results as a function of several criteria of success described in the literature: postoperative apnea index less than 10%, less than 20% and less than 50% of the pre-operative apnea index. They found a success rate approximately 50%. The importance of the apnea index and pre-operative weight were the only factors predicting the results of surgery. However, the spread in individual values did not allow completely reliable criteria to be established.

[Value of extracorporeal piezoelectric lithotripsy in treatment of gallstone disease].

Alternative techniques were introduced in the last 20 years for the treatment of gallstones. Among these the extracorporeal shock wave lithotripsy followed by a systemic litholytic therapy represents undoubtedly the most attractive one. A group of two surgeons and two gastroenterologists has started to evaluate this treatment in April 1988, using a piezoceramic lithotryptic system (Piezolith 2300). From April 1988 to May 1989 we have treated 32 patients who fulfilled the selection criteria-symptomatic gallstone disease, 1-3 radiolucent concrements of less than 30 mm of diameter, functioning gallbladder. We noted only one pancreatitis as a complication of this treatment. The overall stonefree rate is 16% after two months, 32% after four months and 56% after six months, depending on the size and number of stones. A definitive evaluation and final conclusion will only be possible when the rate of late recurrences after this treatment will be known.

Trial of low-dose aspirin plus dipyridamole versus anticoagulants for prevention of aortocoronary vein graft occlusion.

In a prospective randomised trial, 249 patients who had aortocoronary vein bypass surgery were assigned either to a platelet inhibitory drug regimen or to standard anticoagulant therapy. Treatment was replaced by placebo in half of the patients in each group after 3 months. The platelet inhibitory drug regimen--very low-dose aspirin combined with dipyridamole--was as effective as standard anticoagulant therapy to prevent early and late graft occlusion. Death, myocardial infarction, and severe bleeding occurred significantly more often in patients receiving anticoagulants, whereas mild drug-related gastrointestinal and cerebral side-effects were more common in patients taking platelet inhibitory drugs. Antithrombotic treatment should be continued for at least 1 year after coronary artery bypass graft surgery.

[Pain-free piezoelectric extracorporeal shock wave lithotripsy in gallbladder stones. Initial experiences].

Efficacy and side effects of lithotripsy of gallbladder stones with a piezoelectric lithotriptor are assessed. 16 treatments were performed in 8 patients (1-3 per patient). Patients required no premedication, analgesia, infusion or monitoring. Gallstone fragmentation was achieved with all treatments. Laboratory findings remained unchanged after treatment, with the exception of one patient with mild pancreatitis. With adjuvant oral bile acid treatment, 6 of the 8 patients were stone-free within 3 days to 3 months. Extracorporeal shockwave lithotripsy with piezoelectric shock waves provides painless and efficient gallstone fragmentation. Repeated treatments may speed complete fragment dissolution.

[Percutaneous endoscopic gastrostomy. Personal experiences with Russell's method].

Feeding with nasally placed enteral feeding tubes is a method frequently used for long-term nutrition of patients with neurologically or mechanically induced swallowing disorders. Feeding tubes, however, may be unsatisfactory due to either esthetic considerations or complications such as esophagitis, bleeding or pulmonary aspiration. Recently, endoscopic techniques for placing of feeding gastrostomies without laparotomy have been introduced. We report on our experience with 46 endoscopically controlled percutaneous gastrostomies in 38 patients according to the method described by Russell. Cerebrovascular diseases and malignancies of the oropharynx were the 2 most common indications for creation of the gastrostomy. Endoscopically assisted gastrostomy was successful in 38 of 40 patients referred. One serious and two minor complications were encountered. Gastrostomy was in use for a median time span of 3 months (range 1-83 weeks). We conclude that endoscopically assisted percutaneous gastrostomy is a simple, safe, and effective means of providing enteral nutrition in patients requiring tube feeding.

Physical characteristics of indigestible solids affect emptying from the fasting human stomach.

Gastric emptying of indigestible solids depends on their size. It is not clear whether physical characteristics other than particle size affect emptying of indigestible solids from the fasting human stomach. We studied gastric emptying of three differently shaped particles, (cubes, spheres, rods) of either hard or soft consistency during the fasting state in human volunteers. The shape of indigestible particles did not affect their emptying. The area under the gastric emptying curve (AUC: particles x hour) was for hard cubes 24.7 (2.2), for hard spheres 27.9 (1.6), for hard rods 26.9 (2.7). All soft particles emptied faster than their identically shaped hard counterparts, but there was no difference among the three shapes (AUC for soft cubes: 29.2 (3.0), for soft spheres 32.0 (1.8), for soft rods 34.1 (1.2). If gastric emptying of hard and soft particles was compared independently of their shape, soft particles emptied significantly faster than hard ones: AUC 31.8 (1.2) v 26.5 (1.3) (p less than 0.01). In conclusion, the consistency but not the shape significantly affects gastric emptying. Specific physical characteristics other than size and shape may affect gastric emptying of indigestible particles which may be of importance in the design of drugs.

[Effects after 3 months of uvulopalatopharyngoplasty in the treatment of obstructive sleep apnea syndromes in adults].

The results at three months of uvulopalatopharyngoplasty (UPPP) have been evaluated in thirty adult patients with an obstructive sleep apnoea syndrome (SAOS). For the group overall the mean apnoea index (IA) decreased from 57 apnoeas per hour and the mean maximal desaturation decreased to 60% post operatively. However in the overall results different individual facts overlap. 20 of the 30 patients, or 67%, have an IA post operatively of less than 50% of the pre-operative value (responders). 14 of these or 47% have a post operative IA of less than 10 apnoeas per hour, a value considered as non pathological. Finally 33% of the patients have no improvement in their post operative IA (non responders). A stricking diminution of nocturnal desaturation and of the disorganisation of sleep was seen in responders to UPPP. No predictive factor for the results of UPPP could be determined. This study shows that UPPP is an effective treatment in a large number of patients having SAOS at the price of minor and transitory complications.

Mechanisms of terbutaline-induced inhibition of exocrine pancreatic secretion in humans.

Terbutaline, a selective beta 2-adrenoreceptor agonist, has recently been advocated as a potential agent for treating patients with pancreatic fistulae. In the present study, we attempted to quantify the presumed effects of terbutaline on exocrine pancreatic secretion in humans and to characterize possible mechanisms of action. In six healthy volunteers, the pancreas was stimulated by infusion of graded doses of secretin (15.5-250 ng/kg/h) or by infusion of secretin (15.5 ng/kg/h) plus graded doses of caerulein (3.3-30 ng/kg/h). The experiments were repeated in each subject without and with administration of terbutaline. Pancreatic secretion was assessed by a marker perfusion technique and plasma somatostatin and pancreatic polypeptide (PP) levels by radioimmunoassay. Terbutaline had no significant effect on secretin-stimulated pancreatic secretion, but significantly inhibited caerulein-stimulated pancreatic fluid secretion and trypsin output. Plasma somatostatin and PP levels were not affected by terbutaline. The inhibitory effect required the administration of pharmacologically large doses of terbutaline. We conclude that the weak inhibitory effect of terbutaline on exocrine pancreatic secretion is not mediated via somatostatin nor PP and that our data do not support a major role for beta-adrenergic mechanism as regulator of pancreatic secretion.

Angiotensin II/III and substance P in human follicular fluid obtained during IVF: relation of the peptide content with follicular size.

Ovarian follicular fluid (FF) of a number of species contain regulatory peptides secreted by granulosa cells or by autonomic nerve terminals. In this report we demonstrate the presence of authentic (HPLC-verification) angiotensin II and III as well as of substance P (SP) in human FF obtained from hMG stimulated infertile patients undergoing in vitro fertilization. Angiotensin II/III (AII/III), estradiol (E2) and progesterone concentrations increase with the size of the follicles. SP concentrations did not vary significantly in FF of various sizes. These peptide concentrations in FF are about 10-fold higher than those measured in the serum of the same patients. Attempts to correlate SP, AII/III, E2 and progesterone concentrations in the individual FF with the ability of an oocyte to be fertilized, failed. Neither AII/III, SP, E2 nor progesterone concentrations were different in these subclasses of FF. Follicles of patients punctured under general anesthesia contained significantly more SP than follicles of patients which had lumbar analgesia. AII/III concentrations were the same in FF of both treatment groups. The presence of angiotensin II and III in FF in increasing concentrations depending on the maturity of the follicle and the inability of general anesthesia to affect the AII/III concentrations suggests that this peptide is produced within the ovary.

An enzyme-linked immunosorbent assay (ELISA) for the detection of antibodies to Pasteurella pneumotropica in murine colonies.

An ELISA for the detection of class specific IgG antibodies to Pasteurella pneumotropica was developed for the serological diagnosis of infections in mouse colonies. Heat inactivated whole cell preparations of an isolate of P. pneumotropica biotype Heyl (strain P 166) served as antigen for the ELISA procedure and for immune serum production in germ-free Han:NMRI mice. Cross reactions with the autochthonous flora of Han:NMRI SPF-mice were not observed, but were evident when a P. pneumotropica antiserum was tested against other antigens of the Pasteurella-Actinobacillus group. According to the reclassification of this bacterial group proposed by Mutters et al. (1), strains of the following species were tested: P. anatis, P. canis, P. dagmatis, P. langaa, Pl multocida sub. multocida, P. pneumotropica biotype Jawetz, P. stomatis, Actinobacillus equuli and A. lignieresii. Clear cross reactions could be shown with P. pneumotropica biotype Jawetz and A. equuli and to a lesser extent with P. anatis. Antibody formation profiles after nasal infection of Han:NMRI mice exhibited a primary rise of IgG-type antibody titer between 17 to 21 days post infection. Investigations of different mouse colonies free and infected with P. pneumotropica revealed good correlations between serological and bacteriological findings.
