A new force-field program for the calculation of glycopeptides and its application to a heptacosapeptide-decasaccharide of immunoglobulin G1. Importance of 1-6-glycosidic linkages in carbohydrate.peptide interactions.

Energetically favored conformations of glycopeptide 1 were calculated using the newly developed force-field program, GEGOP (geometry of glycopeptides). The three-dimensional structure of glycopeptide 1, which is part of the Fc fragment of IgG1, has been calculated. 1 contains 27 amino acid residues from Pro291 to Lys317 and a biantennary decasaccharide N-linked to Asn297. The conformations of the peptide and the carbohydrate parts are shown to be mutually dependent. Single glycosyl residues of 1 exhibit interaction energies of up to -31.8 kJ/mol with the peptide portion. Generally, only a few of the glycosyl residues of the oligosaccharide moiety express significant interaction energies with the peptide part. No easy prediction is possible of glycosyl residues which exhibit favorable interaction energies. However, in all of the calculated structures, the glycosyl residues of the 1-6-linked branches show strong attractive forces for the peptide part. 1-6-glycosidically linked branches can adopt a larger number of conformations than other linkages due to their high flexibility which allows more favorable interactions with proteins. We developed the GEGOP program in order to be able to study the preferred conformations of large glycopeptides. The program is based on the GESA (geometry of saccharides) program and utilizes the HSEA (hard sphere exo anomeric) force field for the carbohydrate part and the ECEPP/2 (empirical conformation energy program for peptides) force field [Némethy, G., Pottle, M. S. & Scheraga, H. A. (1983) J. Phys. Chem. 87, 1883-1887] for the peptide part. The GEGOP program allows the simultaneous relaxation of all rotational degrees of freedom of these glycoconjugates during the energy optimization process. Thus, mutual interactions between glycosyl and amino acid residues can be studied in detail.

[Significance of family anamnesis in colorectal carcinoma].

To assess the familial risk of colorectal cancer among first-degree relatives of colorectal cancer patients, a tumour-related family history study was conducted in 1982-1988 on 184 patients (median age 60 [28-92] years). These family data were compared with the data recorded in the cancer register of the population of Basel. An enhanced risk among first-degree relatives was noted (relative risk: 1.8%; 95% confidence interval: 1.4-2.3). The enhanced risk was attributed exclusively to seven families involving 2-3 relatives each (the relative risk in these families being 34). In four of these seven families an autosomal dominant site-specific cancer of the colon (Lynch's syndrome I) was diagnosed. Hence, family case history of colorectal cancer patients enables the identification of relatives with a high risk for colorectal carcinoma.

Effect of canine surfactant protein (SP-A) on the respiratory burst of phagocytic cells.

Cells obtained from bronchoalveolar lavage, or neutrophils of peripheral blood of dog, were incubated with the canine surfactant-associated protein A (SP-A). A significant decrease of the production of superoxide anion was observed after subsequent stimulation with phorbol-12-myristate-13-acetate (PMA) as measured by the lucigenin-dependent chemiluminescence (CL). Several other proteins used for control experiments did not decrease lucigenin-dependent CL, indicating a specific effect of SP-A on phagocytes. Treatment of SP-A with collagenase prior to incubation with neutrophils destroyed the depleting effect on oxygen radical production of PMA-stimulated cells. We propose that SP-A acts as a regulatory factor of the respiratory burst of alveolar macrophages and neutrophils in the lungs. The inhibitory effect of SP-A is down-regulated by collagenase released from stimulated alveolar macrophages.

[A year experience with extracorporeal shockwave lithotripsy of gallstones].

43 patients with symptomatic gallbladder stones were treated by extracorporeal shockwave lithotripsy and oral bile acids. In all patients the stones were successfully fragmented during the first lithotripsy session. 33 patients underwent 2-4 treatment sessions. In 16 out of 43 patients the stones disappeared within 7.9 months. The rate of stone dissolution was dependent on the number and size of stones. 3 patients required surgery because of frequent colic in one case, cholecystitis in one case and lack of cooperation in one case. No important side effects were noted except mild pancreatitis 3 weeks after lithotripsy in one patient. Results at this center of extracorporeal shockwave lithotripsy combined with oral bile acids indicate that this treatment may become an alternative to cholecystectomy in patients with a small number (less than 3) of stones not exceeding 30 mm in diameter.

Two-dimensional electrophoresis of dog bronchoalveolar lavage fluid proteins.

Proteins of dog bronchoalveolar lavage fluid, obtained by washing the epithelial lining layer of lungs with phosphate-buffered saline, were separated by two-dimensional electrophoresis. Due to the low protein and high salt content of the bronchoalveolar lavage fluid, samples had to be concentrated and desalted. Following electrophoresis the protein spots were visualized by silver staining. Comparing the two-dimensional protein patterns of bronchoalveolar lavage fluid with that from serum, several lung-specific proteins were detected. The most prominent protein, most probably a surfactant-associated protein, showed isoforms with isoelectric points in the range of pH 4.2-4.8, and a molecular mass of 32 kDa on sodium dodecyl sulfate-polyacrylamide gel electrophoresis after reduction with dithiothreitol.

Killer toxin of Hanseniaspora uvarum.

The yeast Hanseniaspora uvarum liberates a killer toxin lethal to sensitive strains of the species Saccharomyces cerevisiae. Secretion of this killer toxin was inhibited by tunicamycin, an inhibitor of N-glycosylation, although the mature killer protein did not show any detectable carbohydrate structures. Culture supernatants of the killer strain were concentrated by ultrafiltration and the extracellular killer toxin was precipitated with ethanol and purified by ion exchange chromatography. SDS-PAGE of the electrophoretically homogenous killer protein indicated an apparent molecular mass of 18,000. Additional investigations of the primary toxin binding sites within the cell wall of sensitive yeast strains showed that the killer toxin of Hanseniaspora uvarum is bound by beta-1, 6-D-glucans.

Regulation of systolic force and control of free energy of ATP-hydrolysis in hypoxic hearts.

In isolated isometrically working rat hearts during graded, constant pressure hypoxic saline perfusion (PO2 140-700 mmHg) heart rate was changed experimentally between 80 and 400/min. The following parameters were recorded or estimated: peak systolic pressure, dP/dtmax, dP/tmin, oxygen consumption (VO2), venous PO2, ATP, ADP, phosphocreatine, creatine and inorganic P. Free energy of ATP hydrolysis (dG/d xi) was calculated from cytosolic concentration using CK-equilibrium equation. Two predominant responses of the hearts to hypoxia were observed, pronounced negative force frequency relationship; maintained energetic state (Free energy, dG/d xi) at the respective level of hypoxia if contractile force and beating rate were varied by a factor of 4-5. The enhanced force frequency relationship with a maintained free energy level is interpreted in terms of downregulation of EC-coupling (including duration of action potential) which comprises a possible cardioprotective effect.

Prevention of aortocoronary vein bypass graft occlusion: which antithrombotic treatment and for how long?

The effects of 50 mg aspirin combined with 400 mg dipyridamole were compared with those of standard anticoagulant therapy, in the prevention of aortocoronary vein bypass graft occlusion. Early graft occlusion in 249 patients, with 749 distal vein graft anastomoses, were angiographically assessed 11.5 +/- 2 days after surgery and were almost equal in both treatment groups. In half of the patients in each group, active treatment was replaced by placebo after 3 months. Repeat angiography after 1 year (360 +/- 24 days) showed that more new late graft occlusions occurred in patients with only 3 months active medication (either regimen). The incidence of major complications was significantly higher in patients treated with anticoagulants, with minor side-effects more common in the antiplatelet group. Thus, this antiplatelet drug regimen was as effective as standard anticoagulant therapy in the prevention of early and late bypass graft occlusion, but carried a significantly lower risk of severe complications. In addition, as replacement of active treatment by placebo after 3 months resulted in significantly more graft occlusions, antithrombotic treatment should be continued for at least one year after coronary artery bypass graft surgery.

A model to detect interactions between roxithromycin and oral contraceptives.

Twenty-two healthy women participated in a study to determine whether roxithromycin (a new macrolide antibiotic agent) obtunds the activity of a triphasic oral contraceptive. The duration of the study was four menstrual cycles. Medication was given as follows: (1) cycle 1, no medication to demonstrate ovulation; (2) cycle 2, triphasic oral contraceptive daily to suppress ovulation; (3) cycle 3, triphasic oral contraceptive daily plus roxithromycin, 150 mg b.i.d.; and (4) cycle 4, triphasic oral contraceptive daily plus rifampin, 300 mg daily. Sonography of the ovaries was performed on day 13, and serum progesterone was measured on day 21 of each cycle. Elevated progesterone indicated ovulation. The presence of a maturing follicle supported this finding. All volunteers ovulated in the first cycle and no volunteers ovulated in the second and third cycles. However, 11 women ovulated when rifampin and the triphasic oral contraceptive were given concomitantly. The findings suggest there is no reason to believe that roxithromycin interferes with the efficacy of oral contraceptives.

Influence of beta-lactam antibiotics on serum resistance of K1-positive blood culture isolates of Escherichia coli.

The K1-positive strains of Escherichia coli are a group with considerable clinical importance, serum resistance being a common virulence factor of these strains. In the present paper, the influences of cephaloridine, imipenem, and ceftazidime on the serum resistance of eight serum-resistant K1-positive E. coli blood culture isolates with smooth-type lipopolysaccharide were studied. All strains were rendered more serum sensitive by treatment with subinhibitory concentrations of antibiotics. The amount of the reduction of serum resistance was dependent on the concentration of the antibiotic. Amounts of K1 produced under the influence of the antibiotics were measured and were found to be reduced for almost all strains tested. To further test the hypothesis that antibiotic-induced reduction of serum resistance is mediated by inhibition of K1 expression, isogenic mutants of one strain were produced by selection for resistance against infection with K1-specific bacteriophages. These mutants were found to be highly serum sensitive. We conclude from this study that beta-lactam antibiotics can render K1-positive serum-resistant strains of E. coli highly serum sensitive and that this effect is mediated by inhibition of K1 expression.

[Motor-evoked potentials following electric and magneto-electric stimulation: the value and a comparison of both methods].

Motor evoked potentials (MEP) were recorded in a total of 145 patients with supratentorial (N = 29), infratentorial (N = 25) and spinal (N = 91) lesions affecting the descending pathways. In all cases potentials were evoked by electrical, in addition in 55 of them by electromagnetic stimulation of the motor cortex. The peripheral conduction time was determined in all patients by electrical stimulation of the cervical and lumbar nerve roots, respectively. This study was designed to compare both stimulation techniques (electrical vs. electromagnetic) regarding their significance in recording of potentials as well as in correlation of potentials with the motor status as established by clinical examination. Our results show that potentials were obtained in 87.6% following central (cortex) and in 100% following central electromagnetic stimulation. 87.9% of the recordings in infratentorial and spinal lesions show a correct and 12.2% a "false positive" correlation with the clinical motor status. On the other hand, the correlation was false negative in 13.8%, correct in 79.3% and "false positive" in 6.9% of the supratentorial cases. There was no difference between electrical and electromagnetic stimulation regarding diagnostic significance of potentials. Our results allow the following conclusions: both electrically and electromagnetically evoked potentials are sensitive for electrophysiological assessment of infratentorial and spinal lesions, but unreliable in evaluation of supratentorial lesions, especially in the acute stage. Due to its painlessness, electromagnetic stimulation is the method of choice for transcranial eliciting of MEP in the awake patient, who is capable of facilitation by voluntary background contraction of the target muscle.(ABSTRACT TRUNCATED AT 250 WORDS)

[Nocardia mastitis in cattle. 1. Clinical observations and diagnosis in 7 particular cases].

Lately reports of nocardial mastitis have been increasing. Therefore the clinical aspects of 7 individual cases were investigated. Pathologic-anatomical changes of the udder were registered and bacteriological examination of tissue and milk probes were performed. All of the cows had a mastitis during previous lactation periods which had to be treated with antibiotics. The infected quarters recovered in no cases. Shortly after parturition 6 cows had a severe acute mastitis. The cows were treated intramammarily and parenterally with various antibiotics during several days. The infected quarter became indurated and strongly enlarged. These changes were not influenced by therapy, and the cows lost weight. 14 days after the outbreak of the illness, all 6 cows were culled. One cow had a chronic Nocardia mastitis with involution of the affected quarter. Though treated several times, this cow did not recover either. The histopathological examination of the affected quarters revealed an acute to chronic, necrotic to granulomatous mastitis. Nocardia asteroides was histologically and bacteriologically proven to be the causative agent. The reason for the frequent infections with Nocardia and the way of infection is not clear yet. Therapy with antibiotics brought no recovery in any cases. The infected cows should be removed from the herd or culled.

[Provoked oto-emissions. Parameters obtained in normal subjects using Kemp's equipment].

Response parameters relating to the stimulus (click or filtrated bursts), the patient's age and audition curve (physiological presbyacusis) were derived from 3158 provoked otoemission (POE) recordings obtained in normal subjects. Current limits imposed by the device used constitute a factor restricting indications for such devices to the screening of loss hearing in the young child, obviously, but also to the investigation of certain paucisymptomatic hearing disorders presenting with normal or subnormal pure tome audiograms.

[Presentation of the prototype of a fully implanted cochlear prosthesis].

Description of the prototype of a fully implanted cochlear implant demonstrating the feasibility of a prosthesis with no external component within the next few years. The battery is reloaded by means of electromagnetic induction, and the sound message is received by a subcutaneous microphone, which already has a satisfactory bandwitch and signal-to-noise ratio. This system can be generally applied to most types of permanent intracorporeal stimulation.

["Monosonic" one canal digital implant].

In order to decrease the gap between the single and multichannel cochlear implant efficacy we tried to improve the speech coding strategy of the Monomac, the constant current single channel system which we designed in 1987. Owing to a research computerized system, different strategies have been successively studied in the laboratory on 12 new implanted patients during the first weeks of the post-operative period to avoid habituation differences. Results led us to design a miniaturized digital emitter, the Monosonic. This emitter allows the speech therapist to program the frequency band wideness (80-1000 Hz) of the transmitted information, and the threshold level and dynamic range of the stimulating square wave as a function of its frequency. Other strategies are discussed, which have not been yet studied, but may be also programmed.

[Presentation of a French language protocol for the evaluation of communication improvement in the deaf following implantation].

We have been led to drawing up a procedure of assessment of communication as restored by implantation. This procedure is simple, and can be used by all teams, however complex the material implanted is. It can be implemented for children as well as adults, and for both pre- and post-speech deafness. Our staging allows integrating the implanted patient into the classical description of deafness, while retaining the specific features of electrical stimulation.
