[Did Napoleon suffer from sleep apnea syndrome?].

Napoleon would sleep very little. He frequently woke up during night and worked. Brief sleeping time in day repaired his fatigue. He had also a short and thick neck. In the last fourth of his life he progressively suffered from obesity, daily involuntary sleepiness and his intellectual capabilities undoubtedly decreased. Our experience of 48 cases of sleep apnea syndrome diagnosed by mean of polysomnography allow no to think that Napoleon suffered from this disease. Historical consequences of this pathology is discussed.

[Acute abdomen in gynecology].

The aim of the present study is, to describe the morbidity and mortality of 196 patients with an acute abdominal condition who underwent surgery at the Department of Gynecology and Obstetrics of the TU Munich between 1982 and 1986. This is a percentage of 2.7 of all 7,167 operations carried out during this period. 118 of these patients had an extrauterine pregnancy and were therefore excluded from the study. The second group of 79 patients, mostly with inflammatory diseases, were analyzed. In most of these cases the acute abdominal condition was caused by a tuboovarian abscess (48.1%), followed by peritonitis because of a bowel-disease (11.4%). 6 patients suffered from an abscessing endometritis due to a caesarean section with sepsis in 5 cases. A generalized peritonitis occurred in 5 cases and was treated with a planned relaparatomy with lavage. 63% of the patients had no complications within 28 days after operation, 13% developed a subileus; in 7% a relaparatomy was necessary. 6% of the patients had problems of wound-healing. One patient with stomach-cancer died 3 weeks after the operation because of a fulminant lung-embolism. Thus the mortality rate was 1.5%. A further 27% were treated at the intensive care-unit and 18% needed artificial respiration. The average postoperative period of hospitalisation was 15 days. In comparison, patients with elective operations remained 13 days. The morbidity and mortality of patients due to surgery of an acute abdominal condition was relatively small; postoperative complications could be well treated in all cases and is probably the result of a positive and early indication for surgical intervention.

[Oropharyngeal form of the Lasthenie de Ferjol syndrome. Apropos of a case].

The Lasthenie de Ferjol Syndrome associates an iron-deficient anemia by blood auto-spoliation with mental disorders. In this paper, the authors present an oropharyngeal form of this disease. Based on this case review (and those reported in the literature), they remind us this syndrome's mains characteristics, underlying how exceptional and particular this clinical report is. At least they point out how to diagnose, manage, and treat patients with this strange illness.

[Current role of total esopharyngo-laryngectomy in cervico-facial cancerology. Apropos of 11 cases].

Eleven patients underwent total esophagopharyngolaryngectomy for cancer, with colic or gastric digestive reconstructive surgery. Postoperative course is analyzed, contraindications and indications outlined and other methods of pharyngeal reconstruction discussed. An extension to the technique is proposed to include tumors requiring a circular pharyngolaryngectomy. Improvement in the very poor prognosis of these tumors can only be envisaged by the use of enlarged surgery combined with chemotherapy and radiotherapy.

[A benign tumor of the cavum: Thornwald's cyst].

The non neoplastic diseases of the nasopharynx are uncommon especially in adults. Cyst of pharyngeal bursa or Thornwald cyst are located in the supero-posterior nasopharynx, in the location of the normal pharyngeal bursa which open into the pharyngeal mucosa. When it persists, a cyst can develop if the orifice becomes occluded. The have a characteristic and distinctive C.T. and N.M.R. aspects, as presented in this paper, well circumscribed round midline mass, sparing muscles lateral as well as posterior, with an high signal in T1 and T2 N.M.R. sequences. No discussion with other pathologic conditions is useful because carcinoma, adenoid and pharyngeal polyp, or Killian's polyps haven't this configuration.

[Surgical orbital decompression in Basedowian optic neuropathy. Apropos of 5 cases].

Surgical orbital decompression in 5 patients with basedowian optic neuropathy restored the visual acuity and visual field in all cases. The place of surgery among the different therapeutic options proposed for this serious complication of Basedow's disease is discussed, as well as the advantages over other decompression techniques of the procedure used : the cutaneous approach to a trans-ethmoidonasal decompression.

[Cysts of Tornwald's bursa. Apropos of 3 cases].

Thornwald's cyst is a benign bursa formation of the posterior superior wall of the rhinopharynx due to occlusion of the pharyngeal bursa, this entity has been known for more than a century but it is only exceptionally observed. A series of 3 cases have been diagnosed at Hôpital Saint-Antoine over the last 6 months and the perfect visualization of the cysts by CT scan and MR imaging wall no doubt allow their fortuitous detection more frequently.

[The monochannel Monomac and multichannel Minimac cochlear implants].

Monomac monochannel and Minimac multichannel cochlear implants bring to the remaining hearing fibers sound information that is previously converted into adequate electric signals. Both function at constant currents. The Monomac monocanal system possesses only one electrode implanted either inside or outside the cochlea and sends off sound of basic laryngeal rythm to all the nervous fibers. The Minimac is entirely numerical and sends off the whole of the sound information by splitting it into 15 frequency tracks, to 15 electrodes inserted within the scala tympani by means of an electrode-holder. Programming allows for selecting specific values from each frequency track and thus regulating the needed compression individually. Both these implants (Monomac and Minimac) work no conflictlessly but complementarily to each other because they answer different, well-defined clinical needs.

[Results and therapeutic indications of the electrical stimulation of the round window in 581 cases of bilateral total deafness].

Round Window Electric Stimulation necessitates to remove the ear drum to directly place the electrode tip in the round window niche. This test supplies the patient with a sound sensation in 93% of cases presenting a total deafness. It may be realized either under local or general anesthesia owing to brain stem evoked auditory responses. This test supplies the surgeon with precious informations regarding the cochlear implant indications.

[Operative indications of total deafness].

Rehabilitation of patients with total hearing loss through surgical implantation of intracochlear devices is warranted only in cases for which deafness is absolutely not relieved by conventional hearing prostheses. Furthermore, it is required that the patient respond positively to the electrostimulation test of the round window, be strongly motivated, as well as present a high index of socialization. These three parameters will also determine the type of implant to be used, i.e. mono- or multielectrode, as well as particular indications relating to children.

[Changes in the speaking fundamental frequency after cochlear implantation].

Speaking fundamental frequency (Fo) changes have been studied in 12 patients suffering from total post-lingual deafness and having been supplied with the multichannel cochlear implant CHORIMAC-12. Fo is significantly lower after implantation. But Fo variance changes depend on the laryngeal associated pathology.

[Clinical results of the multielectrode cochlear implant in the rehabilitation of 27 cases of acquired total deafness].

Multi-electrode cochlear implants allow verbal discrimination without lip reading in a manner dependent directly on the liminal threshold value during the preoperative electrical stimulation of round window test, and the quality of socialization and motivation of the patient. Overall auditory performances of 27 patients rehabilitated in this way are reviewed and compared with those obtained by other teams and other apparatuses.

[Surgical treatment of chronic snoring. Description and indications].

Chronic rhonchopathy groups all clinical forms of a disease having a common functional sign: snoring. Surgical treatments form part of a therapeutic strategy which includes medical treatment, essentially reduction in body weight, and nocturnal respiratory prosthetic treatments, essentially continuous positive pressure respiration. First intention surgery is an uvulopalatopharyngoplasty (UPP) with or without tonsillectomy. In the severe forms of chronic rhonchopathy, after failure of UPP, major surgery:protrusion mandibular osteotomy or reduction glossectomy can be associated with the other treatments.

[Results of uvulopalatopharyngoplasty in sleep apnea syndromes. Apropos of 33 cases].

Thirty-three patients with the sleep apnea syndrome were operated upon by uvulopalatopharyngoplasty and clinical and multiple sleep recording findings compared at follow up after 3 months. No exact correlation was noted between the clinical condition and recordings during sleep. The only predictive criterion for the result of this operation is the preoperative weight, and this procedure is proposed as a routine treatment as long as no better method exists to determine the obstructive site. Therapy of the sleep apnea syndrome is described.

[Long-term results and complications of uvulopalatopharyngoplasty in snoring without sleep apnea syndrome].

Results and complications were analyzed in 201 patients operated upon by uvulopalatopharyngoplasty for benign or moderately severe rhonchopathy, after elimination of the sleep apnea syndrome as confirmed after follow up for between 15 and 30 months, and showed total recovery from snoring in 60% of cases, an improvement in 26.5% and lack of efficacy in 13.5%. Social and familial relations were improved concomitantly as well as the functional and general disorders usually attributed to snoring (asthenia, somnolence, sleep hypopnea). Various usually minor functional disorders remain, including the sensation of a pharyngeal paresthesia, the most frequent sequela (19.5% of cases).

[Conformation analysis of modified tetrasaccharide sequences of the N-glycoprotein type--problem of the alpha-(1 to 6)-glycosidic bond].

The conformational analysis of the recently synthesized tetrasaccharides alpha-D-Manp (1----3)-[alpha-D-Manp-(1----6)]-4-deoxy-beta-D-lyx-hexp+ ++-(1----4)-D-GlcNAc (2) and alpha-D-Manp-(1----3)-[alpha-D-Manp-(1----6)]-beta-D-Talp -(1----4)-D-GlcNAc (3) will be described. The preferred solution conformation of 2 and 3 is a gt-conformation, which is nearly identical with the preferred conformation of the naturally occurring tetrasaccharide alpha-D-Manp-(1----3)-[alpha-D-Manp-(1----6)]-beta-D-Manp -(1----4)-D-GlcNAc (1). The main structural feature is the backfolding of the alpha-(1----6)-linked D-Man to the reducing D-GlcNAc unit. Conformational analysis of the tetrasaccharides alpha-D-Manp-(1----3)-[alpha-D-Manp-(1----6)]-beta-D-Manp -(1----4)-1,6- anhydro-beta-D-GlcNAc (4), alpha-D-Manp-(1----3)-alpha-D-Manp-(1----6)]-4-deoxy-beta-D- lyx-hexp-(1----4)- 1,6-anhydro-beta-D-GlcNAc (5), and alpha-D-Manp-(1----3)-[alpha-D-Manp-(1----6)]-beta-D-Talp -(1----4)- 1,6-anhydro-beta-D-GlcNAc (6) gave additional proof for this backfolding. The substitution of the reducing unit leads to a smaller amount of gt- and a greater amount of gg-conformers. The method used for conformational analysis of 2-6 is a combination of n.m.r.-experiments and HSEA-calculations with the program GESA. Concerning the application of new 2D-techniques, the COLOC-experiment turned out to be extremely useful in sequencing oligosaccharides.
