Studies on the control of bovine ostertagiasis using a morantel sustained release bolus.

The efficacy of a morantel sustained release bolus to control ostertagiasis in grazing calves was assessed. Administration of the bolus into the rumen of calves immediately before turnout on to spring pasture in May substantially reduced the level of pasture contamination with infective larvae later in the season. Compared with controls there was a 71 per cent reduction in worm burdens acquired over the entire grazing season with a mean improvement in weight gain of 24 kg. When administration was delayed until midsummer (July) a good anthelmintic effect was obtained but there was little reduction in pasture contamination levels. Worm burdens acquired over the grazing season were reduced by only 48 per cent with a mean weight gain advantage of only 9 kg compared with controls. The control obtained against Dictyocaulus viviparus by both treatments was not absolute, failing to prevent the development of mild parasitic bronchitis.

An onlay vein patch technique for the repair of small vessels.

A method for the repair of defects in small blood vessels is described, using an oversized onlay autogenous vein patch tacked to the adventitia. The technique has been evaluated experimentally and it has been shown that vessels repaired with a patch remain patient with normal blood flow. By contrast, vessels repaired by direct suturing have a high incidence of mural thrombus formation and sluggish flow. The clinical usefulness of the method is illustrated by a case report describing the salvage of a free flap with a damaged vascular pedicle.

Haemophilus vaginalis (Corynebacterium vaginale, Gardnerella vaginalis) in a family planning clinic population.

Vaginal specimens were obtained at 902 attendances from 522 women requiring vaginal examination at a family planning clinic. Haemophilus vaginalis was found in 8% of specimens either by culture or by at least two out of three microscopical tests; lactobacilli were seldom found in the presence of the organism. Increased signs and symptoms, especially an offensive odour, were found only when H vaginalis and staphylococcus were isolated together. The organism was found less often in patients using contraceptives methods which protected the vagina, thus suggesting sexual transmission. Treatment is advisable when H vaginalis is in contact with a vascular bed.

Ferritin, finger clubbing, and lung disease.

The serum ferritin concentration has been determined by an immunoradiometric assay in 90 subjects with a variety of pulmonary diseases. No association between ferritin concentrations and finger clubbing has been found in any of the diseases studied. Ferritin levels were significantly raised in the subjects with bronchial carcinoma, but were not useful in monitoring recurrence of the tumour. Pulmonary artery and pulmonary vein ferritin concentrations were similar to systemic venous concentrations. It is therefore unlikely that the tumour releases ferritin into the pulmonary circulation. Ferritin levels were raised in patients with acute pneumonias but did not correlate with the total white cell count or erythrocyte sedimentation rate. Serum ferritin concentrations were also increased in a variety of chronic lung diseases but were normal in subjects with asbestosis.

Synthetic biliary bypass.

A method for the bypass of high biliary strictures using a Dacron velour prosthesis is discussed. This has been employed in 24 patients with obstructive jaundice caused by malignant disease which could not be relieved by conventional means.

Clinical evaluation of B cell and T-regulator cell function using a protein A haemolytic plaque assay.

Using the protein A haemolytic plaque assay (PrA-HPA) and standardized, optimal conditions, pokeweed mitogen (PWM) stimulated peripheral blood lymphocytes (PBL) from normal donors produced 5,500-48,000 (mean 16,955) IgG plaque-forming cells (PFC) per 10(6) lymphocytes, 3,375-38,000 (mean 14,179) IgA-PFC/10(6) and 5,500-69,750 (mean 18,684) IgM-PFC/10(6). Addition of 4 micrograms/ml concanavalin A (Con A) at the start of PWM stimulation of PBL from normal donors gave 59-98% (mean 84%) suppression of IgG-PFC, 46-99% (mean 79%) suppression of IgA-PFC and 31-100% (mean 82%) suppression of IgM-PFC. When PFC/10(6) values in PWM-stimulated co-cultures of unfractionated PBL from pairs of unrelated normal donors were compared with PFC/10(6) obtained in the individual PWM-stimulated cultures, observed/expected ratios were between 0.81 and 1.24 (mean 0.99) for IgG-PFC, 0.85 and 1.23 (mean 0.98) for IgA-PFC and 0.86 and 1.25 (mean 1.03) for IgM-PFC. Cord blood lymphocytes (CBL) produced few PWM-stimulated PFC and co-culture of CBL with normal PBL gave markedly reduced observed/expected ratios, indicating hyperactivity of T-suppressor cells in cord blood. PBL from a patient with partial DiGeorge syndrome also responded poorly in the PWM-driven PrA-HPA, but co-culture of this patient's PBL with normal PBL and PWM gave rise to higher than expected values, indicating reduced T-helper cell function. These simple methods could be employed in clinical evaluation of the cellular defect in humoral immunodeficiency and autoimmune disease states.

Serum ferritin in patients with cancer: determination with antibodies to HeLa cell and spleen ferritin.

Some malignant tissues and cell lines contain acidic isoferritins and it has been suggested that the assay of such isoferritins in serum may be of value in the diagnosis of malignancy. This paper describes a radioimmunoassay for acidic ferrtin purified from HeLa cells. Examiniation of purified heart, kidney, liver and spleen ferritin showed that the assay was highly specific for acidic isoferritins. Ferritin concentrations have been measured with antibodies to HeLa cell and spleen ferritin in extracts of normal and tumour tissue. Although the tumours contained more HeLa type ferritin than the corresponding normal tissue the HeLa/spleen type ferritin ratio was low. HeLa-type ferritin concentrations have been compared with values obtained with anti-spleen ferritin in over 1000 sera from normal subjects and patients with cancer and leukaemia. HeLa-type ferritin as not detected ( less than 2 micrograms/l) in most normal sera. Concentrations of up to 53 micrograms/l were found in sera from patients with malignant disease but the HeLa/spleen type ferritin ratio was always very low. There appears to be little application for antibodies to HeLa cell or heart ferritin in the diagnosis or monitoring of cancer.

The presentation and progress of macroscopic breast cysts.

A total of 322 cases of macroscopic breast cysts presenting at a district general hospital has been studied with a minimum follow-up period of 5 years. In almost half the patients (44.18 per cent) a further cyst occurred following aspiration, and in two-thirds of these, more than one subsequent cyst developed. There was a 50-year age span from the youngest to the oldest patient at presentation: however, the majority of cysts (61.80 per cent) occurred in the perimenopausal age group (40-50 years). The subsequent incidence of carcinoma was more than twice that which would be expected in the population at large, but no case of carcinoma that was not suspected clinically was revealed by cytology.

Haemophilus vaginalis infection. Diagnosis and treatment.

Some 4,263 women from special, family planning and gynecologic clinics were screened at random for the presence of H. vaginalis (C. vaginale) infection. Twenty-four percent of the women from the special clinics, 6% from the family planning and 4% from the gynecology clinics were culture positive although microscopic findings were somewhat higher than this. Of 582 women attending special clinics who were infected solely with H. vaginalis, only 261 complained of having an offensive discharge. Two hundred thirty-eight women reported "no symptoms," but 116 of them did have an offensive discharge on examination. These 582 women were treated with either ampicillin, ampicillin with probenecid or sulphonamide vaginal tablets. All were largely effective. Direct microscopy was regarded as being a reliable guide to H. vaginalis infection.

Effects of divalent cations on calcium levels, aggregation and morphology of human blood platelets in vitro.

Some effects of cadmium, nickel, strontium and manganese ions upon human platelets in vitro are reported. Both Mn and Cd induced aggregation, Mn being the most effective. Ni and Sr did not have this effect. Scanning electron microscopic observation showed that Mn and Cd, but not Ni and Sr induced shape changes in the platelets. X-ray microanalysis of pyroantimonate fixed platelets revealed that calcium was displaced from both membrane and cytoplasmic regions by all four cations. The magnitude of displacement occurred in the sequence Mn > Cd > Ni approximately or equal to Sr. Manganese was detected both in the cytoplasm and membrane region of those platelets treated with this ion. It is suggested that the cations are influencing directly certain macromolecular ligands whose condition controls platelet surface properties.
