The use of the ultrasound Doppler flowmeter in reconstructive microvascular surgery.

A method is described for predicting the outcome of experimental free flaps at the time of surgery using a directional ultrasound Doppler flowmeter. Tests of both arterial and venous anastomoses have been developed. Provided that both anastomoses are individually assessed, the technique is 100% accurate and is recordable. Spectral analysis of truly directional Doppler signals has provided knowledge of the peripheral resistance within a flap.

Isoferritins in normal leucocytes.

Monocytes, lymphocytes and polymorphs were separated from the peripheral blood of normal human subjects. Ferritin concentrations were determined with antibodies to both human spleen and heart ferritins. The heart type ferritin concentration in monocytes was 38.4 +/- 21.6 fg/cell (mean +/- SD), in lymphocytes 8.6 +/- 6.6 fg/cell and in polymorphs 3.2 +/- 2.4 fg/cell. Spleen type ferritin concentrations (fg/cell) were 15.6 +/- 7.0 in monocytes, 6.6 +/- 5.7 in lymphocytes and 7.0 +/- 4.6 in polymorphs. The mean heart/spleen ferritin ratios were 2.8/1 for monocytes, 2.0/1 for lymphocytes and 0.6/1 for polymorphs. The cell extracts were also subjected to anion exchange chromatography. Heart type ferritin eluted at a higher chloride ion concentration than spleen type ferritin. Both H and L subunits were synthesized by mononuclear cells when incubated with 3H-leucine. Human leucocytes contain a wide range of isoferritins and ferritin concentrations may be considerably underestimated in conventional assays for serum ferritin which employ antibodies to liver or spleen ferritin.

Enrichment of erythroblasts from human bone marrow using complement-mediated lysis: measurement of ferritin.

Sequential lysis of human bone marrow cells with a monoclonal antibody directed against myeloid cells (TG1) and a rabbit antiserum raised against peripheral blood mononuclear cells gave preparations in which 78-97% of the nucleated cells were erythroid, with a 24-77% recovery. Viability was high, morphology was good and the cells were able to divide and differentiate in culture. No metabolic experiments were carried out but the ferritin content of the erythroblasts was measured in four experiments and found to be about 200-2000 times higher than that found in normal erythrocytes. The H/S ratio was high in both erythroblasts and erythrocytes. Fractionation on the basis of density of two erythroblast preparations, one from a patient with sideroblastic anaemia and one from a patient with megaloblastic anaemia, showed that the most immature erythroblasts contained the highest content of ferritin and that this fell with maturation. The H/S ratio stayed the same or fell with maturation. It was concluded that this method would be valuable for the study of the role of erythroblast ferritin in normal and pathological situations.

Kinetic analysis of subset growth and B-cell activation in pokeweed mitogen-stimulated cultures of peripheral blood mononuclear cells depleted of or enriched for OKT8+ cells.

Peripheral blood mononuclear cells (PBMN) that were depleted of OKT8+ cells and stimulated with pokeweed mitogen (PWM) produced higher cell yields and higher numbers of plaque-forming cells than unfractionated PBMN. Conversely, OKT8-enriched PBMN, prepared by mixing unfractionated and OKT8+ cells in a ratio of 3:1, gave reduced cell growth and B-cell activation. In OKT8-depleted cultures, B cells, OKT4+ cells, OKT8+ cells, and OKM1+ cells increased in number between days 4 and 7 of culture by factors of 9.8, 5.9, 20.1, and 5.6 respectively, whereas growth rates for these subsets were 2.4, 1.0, 2.0, and 1.3 in unfractionated cultures and 0.9, 1.0, 1.2, and 0.6 in cultures enriched for OKT8+ cells. On day 7 of culture, 73 +/- 10% of B cells secreted immunoglobulin in unfractionated cultures, whereas only 21 +/- 10% of B cells were activated in OKT8-enriched cultures. Surprisingly, PWM stimulation of OKT8-depleted PBMN produced only 40 +/- 12% activated B cells.

B cell activation by pokeweed mitogen in cultures of normal peripheral blood lymphocytes depleted of T regulator subsets by treatment with OKT4 and OKT8 monoclonal antibodies.

OKT4+ (T helper/inducer) and OKT8+ (T cytotoxic/suppressor) subsets were depleted from peripheral blood lymphocytes (PBL) by complement-mediated lysis and residual cells examined for responsiveness to pokeweed mitogen (PWM) using a protein A haemolytic plaque assay for immunoglobulin secreting B cells. It was shown that: (1) three cycles of cell killing were required to totally abolish T helper function; (2) OKT4- PBL did not respond to PWM, but in a co-culture system, an equal number of unfractionated normal PBL could entirely reconstitute responsiveness of the residual B cells; (3) OKT8- PBL gave enhanced numbers of PWM-induced plaque forming cells (PFC); (4) addition of 4 micrograms/ml concanavalin A (con A) to PWM stimulated OKT8- PBL failed to suppress PFC generation, but suppression was induced by 12.5 and 25 micrograms/ml con A and (5) kinetics of PWM-induced PFC development were similar in the presence or absence of OKT8+ cells.

The role of abdominal surgery in the treatment of advanced testicular germ cell tumours.

Twenty patients with advanced testicular teratoma and 1 with advanced seminoma have undergone abdominal surgery for the removal of residual disease following treatment by sequential chemotherapy (1). Fourteen out of the 21 patients (66 per cent) have achieved complete remission with a follow-up period of 6--41 months. In 12 patients (57 per cent) the tumour had undergone histological maturation or necrosis following chemotherapy, those patients in whom the tumour remained histologically active having a worse prognosis. Raised preoperative serum levels of the tumour markers human chorionic gonadotrophin (hCG) and alpha-fetoprotein (AFP) indicating active disease, forecast a particularly poor prognosis, 5 out of 9 such patients (55 per cent) having died. Computerized tomography gave false negative results in 3 patients (14 per cent). Postoperative complications were few, and 2 patients (9 per cent) suffered with failure of ejaculation. At present our policy is surgical removal of any residual mass after chemotherapy to determine the histology, and, wherever possible, to achieve complete clinical and biochemical remission. Six patients with an apparently poor prognosis have benefited directly from surgery and remain in long term remission. The selection of patients who benefit most from surgery can only be determined by more experience and longer follow up.

The scapular flap--an anatomical and clinical study.

The scapular flap is a medium-sized flap combining many of the good qualities of others, such as thinness and minimal donor site deficit, with the potentialities of a consistently long pedicle that allows combination with other adjacent flaps. The anatomy of the flap and its application in nine clinical cases is described.

The Laser Doppler flowmeter for microvascular monitoring: a preliminary report.

A preliminary report is presented of a free-flap monitoring technique using the Doppler shift of Laser light. It has provided useful information in five patients with free flaps. The technique is non-invasive and the apparatus is compact, portable and simple to use. Although a long term evaluation with a larger number of patients is required the technique has many advantages over others currently available and is sufficiently promising to warrant further investigation.

In vitro susceptibility of Gardnerella vaginalis and Bacteroides organisms, associated with nonspecific vaginitis, to sulfonamide preparations.

Recent reports suggest that anaerobic Bacteroides organisms are frequently found with Gardnerella vaginalis in nonspecific vaginitis. Specimens taken from 96 women with vaginal discharge were tested simultaneously for these organisms. G. vaginalis was found in 73% of the specimens, Bacteroides was found in 53%, and both organisms were found in 47%. Sulfonamides have been widely used in the successful treatment of vaginitis. Paradoxically, G. vaginalis is reported to be resistant, and it has been suggested that it could be the vehicle of the drugs which effects the cure. Little is known of the susceptibility of vaginal anaerobes to the sulfonamides. G. vaginalis and Bacteroides isolates were therefore tested in vitro against the individual excipients of sulfonamide tablets, and minimal inhibitory concentration tests were also performed against the three active drugs. The excipients had no effect on G. vaginalis, but Bacteroides strains were susceptible to the urea component. All strains of both organisms were susceptible to at least two of the three sulfonamides at high concentrations.

Balanoposthitis associated with Gardnerella vaginalis infection in men.

Fourteen of 194 (7.2%) consecutive unselected men had positive culture results from genital swabs for Gardnerella vaginalis. A higher yield of isolates was obtained from preputial (93%) than from urethral swabs (64%). Of the 14 men, two had no detectable genital abnormality, eight non-gonococcal urethritis, and nine balanoposthitis. The urethral isolation rates for G vaginalis in men with and without non-gonococcal urethritis were not significantly different, but preputial isolation rates were significantly higher (p less than 0.001) in men with balanoposthitis than in those without. The prevalence rate for G vaginalis in men with non-candidal balanoposthitis was 31%. In a second study, concomitant Bacteroides species were isolated in preputial swabs from nine of 12 (75%) men with G vaginalis-associated balanoposthitis and may play a role in its pathogenesis.
