The structure of the capsular antigen from Pneumococcus type 26 has been determined by using methylation analysis, periodate-oxidation studies, and n.m.r. spectroscopy of the original and the dephosphorylated product. It is considered that the polysaccharide is composed of repeating-units having the following structure. (formula: see text): The only difference between this structure and that of the type-6 antigen is that the alpha-L-rhamnopyranosyl residue is linked to O-4 of D-ribitol in the former, but to O-3 in the latter.
The structures of the O-antigens of all known serotypes and subserotypes of Shigella flexneri have been reinvestigated. The results support the assumption that these antigens are composed of a basic tetrasaccharide repeating unit (1), to which alpha-D-glucopyranosyl and/or O-acetyl groups are attached at different positions. Leads to 3)-beta-D-GlcNAcp-(1 leads to 2)-alpha-L-Rhap-(1 leads to 2)-alpha-L-Rhap-(1 leads to 3)-alpha-L-Rhap-(1 leads to The immunological determinants responsible for O-factors I, II, IV, V and 7, 8 contain alpha-D-gluco-pyranosyl groups, the locations of which have been determined. O-Factor 6 is due to O-acetyl groups, linked to O-2 of the 3-substituted alpha-L-rhamnopyranosyl residue in unit 1 and O-factor III seems to be due to the same groups. The chemical natures of the determinants responsible for O-factors 4 and 3, 4 are still obscure. The structural studies indicate that the immunological classification of Sh. flexneri serotypes and subserotypes, as regards these O-factors, may need revision.

Treatment with glucagon in addition to blood transfusion was compared with blood transfusion alone after one hour of hemorrhagic shock in the rat. In liver tissue Na+ increased and K+ decreased during haemorrhagic shock. After treatment the initial values were restored equally in both groups within ten minutes. Incubation of liver slices in cold Krebs' solution resulted in a pronounced increase in Na+ and decrease in K+, the values being partially restored to initial levels after subsequent incubation at 37 degrees. Thirty minutes after treatment the liver slices obtained from rats given glucagon showed a more normal ion composition after leaching and rewarming than slices from rats not given glucagon. ATP decreased and glucose and lactate increased in liver tissue during hemorrhagic shock. These variables were partially restored 30 minutes after treatment. No difference between the treatment groups was noted. Animals trreated with glucagon were, however, more efficient in reducing the elevated blood lactate level. The results suggest that the use of glucagon in the treatment of hemorrhagic shock might be of benefit for cellular function in the liver.

Fistula formation between the appendix and adjacent organs is a rare condition. Cutaneous fistulas occur even more seldom. In this paper a case will be described where a fistula was formed between the appendix and the right buttock.

Previous studies of different Shigella flexneri O-antigens indicate that their O-specific region is composed of oligosaccharide repeating units containing a basic tetrasaccharide structure, to which alpha-D-glucopyranosyl groups and O-acetyl groups may be attached to different positions. Structural studies of O-antigens from variant X, type 5a and type 5b lend further support to this assumption. These antigens contain terminal alpha-D-glucopyranosyl groups, one each per repeating unit in X and 5a, two in 5b. The location of these groups in the repeating unit has been determined.

Labour was induced by the intravenous infusion of prostaglandin F2alpha in 106 patients at 36-44 weeks of pregnancy. The induction was successful in 80% of the women. The total dose needed ranged from 0.1 to 14.2 mg of PGF2alpha. The uterine activity and fetal heart rate were recorded by cardiotocography. The contraction pattern and induction-delivery time were the same as reported for induction with oxytocin. In one case uterine hyperactivity occurred after rupture of the membranes. No serious adverse effects were seen, but in a few cases local irritation was noted at the site of infusion. The condition of the infants was generally good. It might be concluded that PGF2alpha seems valuable for the induction of labour, but due to the risk for over-stimulation careful supervision is needed.
