[Prognostic factors in operated non-small cell cancer of the lung. Study from a randomized therapeutic trial].

This article presents the results of a prognostic study of primary resected lung cancer (non-small cell). The data result from a randomised clinical trial of immunotherapy with a non-specific adjuvant; the follow-up was between four to seven years. Thirty-five clinical, biological and anatomo-pathological parameters were gathered at the time of inclusion in the trial. The response criteria used were survival without recurrence and total survival. A multivariate analysis using the Cox's model was carried out for each criterion. At the reference date of the 1st April 1985, 125 relapses and 132 deaths were counted amongst 219 patients; there was only one patient lost to follow-up and only 39 missing data were observed. The negative therapeutic results of the immunotherapy used were confirmed by this new intermediate analysis. The rate of survival without recurrence at 5 years was 43% and the overall survival at five years was 42%. The use of Cox's model to show the prognostic information at the 5% level for survival without recurrence could be summarised by five factors: main staging (the prognostic factor), leucocytosis, the cutaneous reaction to proteus, Karnofsky index and presence of physical signs. For stages I and II the outcome was identical and no factor was predictive at the 5% level. For stage III the cutaneous reaction to proteus and leucocytosis were prognostic. For overall survival, the prognostic information at the 5% level could be summarised by five factors: staging (main prognostic factor), leucocytosis, Karnofsky index, presence of physical signs and lymphocytosis. For stages I and II whose outcome was identical only Karnofsky index and lymphocytosis were predictive at the 5% level.(ABSTRACT TRUNCATED AT 250 WORDS)

[Deep candidiasis in agranulocytic patients: mycological and serological surveillance in 91 febrile episodes and management of an antifungal and antibacterial protocol].

This study took place between February 1984 and February 1985 in 76 febrile neutropenic patients. We evaluated the whole mycological and serological results, in 91 febrile episodes. We separated two groups: group I (acute leukemia, chronic myeloïd leukemia, medullar aplasia, lymphomas) and group II (chronic lymphoïd leukemia, Hodgkin disease, myelomas, solid tumors). Patients were included in this study if they developed a temperature of 38.5 degrees C or greater and if they had less than five hundred neutrophils per mm3. They were treated with mono or bi-antibiotherapy. If the patient remained febrile more than 72 hours, an antifungal therapy was added (Amphotericin B IV). Through these weekly results, we conclude: the high frequency of digestive tract candidiasis in such patients; disseminated candidiasis occurs only in the first group patients with fungal digestive colonization; the overall response rate in this study was 77 per cent. This underlines the interest of empiric treatment with amphotericin B IV.

[Inhalation pneumonia presenting as a pneumocystis infection].

A case of Pneumocystis carinii pneumonia is presented. Following presentation a chronic alveolitis was uncovered, which was due to ultimately repeated and prolonged inhalation of sweets containing gum arabic. The diagnosis was confirmed by a trans-bronchial biopsy and by chemical analysis of centrifugation of the alveolar lavage deposit. After cessation of the inhalation the progress was satisfactory both in terms of clinical status and lung function measurement.

[Tobacco detoxication by acupuncture and group psychotherapy].

In the out-patient tobacco detoxication unit created, in 1977, within the pneumology department of the Hôtel-Dieu hospital, Paris, the method used is a combination of acupuncture and group psychotherapy. Its effectiveness has been evaluated, over a 2-year period, by means of a questionnaire mailed to each patient and followed, if necessary, by up to 3 reminders. A computer-assisted assessment of the data thus collected provided a follow-up of varying duration of the 555 patients concerned; 488 patients were treated, then contacted, and 371 (76 p. 100) answered the questionnaire. Weaning--defined here as complete detoxication with a 2 to 28 months follow-up when the questionnaire was filled--was obtained in 24 p. 100 of the patients who answered. Other evaluation criteria would have given figures ranging from 18 p. 100 to 58.6 p. 100. The main reason for failure mentioned by the patients was lack of will. An exhaustive study of 84 publications concerning more than 10,000 candidates to weaning showed that similar results are obtained with several types of psychotherapy, acupuncture and auriculotherapy: a success rate of about 60 p. 100 at the end of treatment, 40 p. 100 at 3 months and 30 p. 100 at 6 months and one year. The results obtained with drugs are not as good, perhaps because their use betrays a lesser desire to give up smoking.

Asymptotic growth, egg production and trivariate allometry in Esox masquinongy Mitchill.

Multivariate growth models based on the Lotka-Volterra equations seem more complicated than necessary for a fruitful analysis of relative growth data. A simpler and more practical approach combines multivariate allometry with parabolic or asymptotic growth with respect to time. A modified von Bertalanffy curve, X = A [1-exp (-t/D)]C, is compatible with allometry and yields a satisfactory description of somatic growth and egg production in the female muskellunge (Esox masquinongy). A multiplicative variation model appears adequate and partial linearization can be used. While the muskellunge reaches a large size, the present analysis suggests that its growth pattern is limited (asymptotic, determinate). The allometry exponents inferred from the multivariate asymptotic growth model differ slightly from those obtained from principal component analysis or from a trivariate linear structural relationship, but the latter may reflect physiological age rather than physical time.

Pulmonary polymorphic centroblastic type malignant lymphoma in a patient with lymphomatoid granulomatosis, Sjögren syndrome and other manifestations of a dysimmune state.

A case of lymphomatoid granulomatosis arising in a patient with a long history of dysimmunity is reported. The autopsy reviewed the final evolution into a high grade malignant lymphoma with IgM K immunoglobulin production and discovered in deep lymph nodes modifications similar to angio-immunoblastic lymphadenopathy.

[Chemotherapy of bronchopulmonary and pleural adenocarcinoma. Apropos of 31 cases].

Between January 1978 and June 1983, 31 patients aged between 35 and 73 years, with an adenocarcinoma of the respiratory system (25 lung cancers and 6 pleural cancers) were treated by chemotherapy, either exclusively or following failure of surgery. In 21 patients, the treatment consisted of a combination of cyclo-phosphamide (600 mg/m2), adriamycin (40 mg/m2) and 5-fluoro-uracil (600 mg/m2) by intravenous injection, every four weeks. The other 10 patients received different treatments, but platinum salts were administered in only 2 cases. The treatment was fairly well tolerated (only 1 death due to toxicity), but the effectiveness remained disappointing: only 1 complete response (alive after 48 months), 2 partial responses and 4 stabilisations. The overall median survival was 9 months, with a mean survival of 11.6 months. The mean survival of the responder and stabilised patients was more than 21 months. These results lead us to recommend that, apart from randomised trials, chemotherapy should be stopped after the second course in the absence of at least a stabilisation of the disease, in which prolonged survivals are sometimes observed when treatment is limited to purely symptomatic measures.

Adjuvant immunotherapy with nonviable Mycobacterium smegmatis in resected primary lung carcinoma. A randomized clinical trial of 219 patients.

Two hundred and nineteen patients with resected lung carcinoma were randomized 3 weeks after surgery between two treatment arms: a control group (110 cases) and an immunotherapy group (109 cases). The immunostimulant was a nonviable saprophytic mycobacterium, M. smegmatis, given monthly by subcutaneous injection in four sites. The two groups were equivalent in terms of prognostic factors, including a nonsignificant difference favoring the control group based on the N (node) classification. This interim analysis was carried out on June 1, 1981. Treatment comparison by the log-rank test did not show any significant differences between these two groups in regards to disease-free interval and overall survival. There was no significant difference between the two groups after stratification of the comparison according to the N classification or adjustment with a subset of eight prognostic parameters through the Cox model. The initially expected difference (20% 1-year survival) will probably not be achieved, given these interim results, but patients will continue to be treated and followed-up according to the protocol as to allow further evaluation of this nonspecific immunotherapy.

[Negative results of a randomized therapeutic trial of nonspecific immunotherapy in primary, surgically-treated non-small cell bronchial cancer].

Between March 1978 and May 1981, 219 patients suffering from non-small cell primary bronchial carcinoma underwent surgical excision which was intended to be curative. Three weeks later the patients were randomised into two groups: 1. A control group, with no other treatment following excision (110 patients). 2. A non-specific immunotherapy group (109 patients). The immunostimulant used was an aqueous suspension of heat killed mycobacterium smegmatis administered subcutaneously once a month. The trial was analysed on December 1, 1982. There were 117 recurrences and 112 deceased. There was no significant difference as regard survival without relapse or overall survival; all causes of death were included.

[Small-cell anaplastic bronchopulmonary cancers. Prognostic and therapeutic review apropos 49 cases].

This retrospective study of 49 patients with small-cell broncho-pulmonary carcinoma showed the principal prognostic factor to be the response to chemotherapy: an initial treatment association of cyclophosphamide (800 mg/m2), adriamycin (60 mg/m2), methotrexate (40 mg/m2) and CCNU (60 mg/m2) every four weeks was used in 40 patients, out of 46 patients who underwent chemotherapy. All cases were followed up for a minimum of 6 months except 2 who were lost to follow-up; there are 9 survivors, all of whom initially had an apparent complete remission: the extension of the disease seemed to be of less prognostic significance than the response to treatment. The response rate to this protocol was 84 p. cent with 52 p. cent apparent complete remissions. The mean global survival rate was 9 months (mean survival rate of treated patients: 11.46 months). Some patients with disseminated carcinoma initially had very prolonged survival. The association of a final session of thoracic radiotherapy seemed to be useful (the 4 patients who underwent radiotherapy after 8 to 12 sessions of chemotherapy are still alive after an average of 18 months follow-up with no signs of recurrence).

[Transbronchial biopsy of the lung : value for diagnosis of sarcoïdosis. A study in 53 patients (author's transl)].

Data yielded by bronchial and transbronchial biopsy specimens removed during fiberoptic bronchoscopy in 53 patients with sarcoïdosis (24 stage I, 27 stage II, and 2 stage III) was analyzed. Bronchial biopsies (3 to 5 specimens) were positive in 49% of patients (41,6% stage I, 55,5% stage II, 1/2 stage III). Transbronchial biopsies removing bronchiolar and alveolar specimens yielded 88,6% positive results (91.6% stage I, 85,1% stage II, 2/2 stage III). Lymphocyte counts bronchioalveolar washings exceeded normal values by 15% in all 42 patients studied. We conclude : 1) that transbronchial lung biopsies yield significantly more information than proximal bronchial biopsies ; 2) that results of bronchioloalveolar washings reflect granulomatous immunologic changes in the lung and do not allow unequivocal diagnosis.

[Cytology of broncho-alveolar lavage samples in sarcoidosis (author's transl)].

The cytology of samples from broncho-alveolar lavage was studied in 18 patients with stage I or II sarcoidosis, three of whom were smokers (stage II). Cell content was 213 +/- 111.10(3)/ml with 30.8 +/- 15.7 p. cent of lymphocytes for stage I, and 338 +/- 98.10(3)/ml with 34.1 +/- 8.7 p. cent of lymphocytes for stage II patients who were non smokers. In the three patients in stage II who smoked, cell content was 206 +/- 107.10(3)/ml with 15.3 +/- 9.4 p. cent of lymphocytes. Measuring angiotensin conversion enzyme in 12 cases demonstrated elevated (7 cases) or normal (5 cases) levels unrelated to the radiological stage or the lymphocytosis. Diagnosis was confirmed by the presence of characteristic epithelioid granulomas in all cases, in 14 patients after trans-bronchial pulmonary biopsy. In comparison, cell content in 3 cases of extrinsic allergic alveolitis associated with histologically confirmed granulomas was 690 +/- 120.10(3)/ml with 50.3 +/- 10.7 p. cent of lymphocytes, and in 9 cases of different types of intestinal affections was 327 +/- 157.10(3)/ml with 6.33 +/- 7 p. cent of lymphocytes. Constantly elevated lymphocytosis in sarcoidosis constitutes a diagnostic aid; perhaps it can be used during follow-up examinations as a means of assessing progression of the disease. No correlation with the conversion enzyme was noted.

[Clinical studies of the erythrocytosis produced by a new hemoglobin with oxygen affinity : Hb Hôtel-Dieu alpha 2A beta 2 Asp replaced by Gly (author's transl)].

A new case of erythrocytosis associated with a high oxygen affinity hemoglobin is reported : the substitution of beta 99 of aspartic acid by glycine, characterized this abnormal hemoglobin, named "hemoglobin Hôtel-Dieu". An important associated smoking habit, probably responsible of an abnormal lung X-ray and of a disturbed CO transport, contributed to increase the difficulty of diagnosis. Familial inquiry, the search of an abnormal hemoglobin by standard or isoelectrofocusing electrophoresis, and the 2,3-DPG assay should be systematically performed in case of polycythemia in young people or in the absence of obvious aetiology. A corpuscular abnormality can be thus detected in about 10 p. cent of such polycythemias.
