[Myelography and headache].

The side effects associated with the use of Metrizamide, Iopamidol and Iotrol in two double blind studies on lumbar myelography were determined. The cause of headache is explained on the one hand as the result of the distribution of the contrast substance in the CSF space (early headache) and on the other hand due to the CSF leak through the puncture lesion. Iotrol seems to be the safest contrast substance for intrathecal use, however it should be used in the smallest possible amount to reduce even further contrast-related effects in myelography.

[Therapy of stomach ulcer with low-dose antacid gel and cimetidine. A multicenter double-blind study].

In a multi-centre double-blind (double-dummy) trial the effectiveness of low-dose antacid gel (6 X 12 ml/d; neutralisation capacity 120 mmol) was compared with that of a standard dose of cimetidine (1 g/d) in the curative treatment of gastric ulcer. Antacid gel was given to 65 patients, cimetidine to 60. Diagnosis was confirmed by endoscopic biopsy, which was also employed in a serial follow-up. After 4 weeks antacid gel and cimetidine produced cures in 43% and 52%, respectively; after 8 weeks 76% and 89%, respectively, the difference between the two methods not being statistically significant. There was also no statistically significant difference with regard to ulcer pain. In one case each in the antacid and cimetidine groups, the treatment had to be stopped because of side effects. Diarrhoea was more common on cimetidine than on antacid gel. It is concluded that both low-dose antacids gels and cimetidine are suitable in the treatment of gastric ulcers.

Iotrol, a new water-soluble non-ionic dimeric contrast medium for intrathecal use.

The side effects associated with the use of Iotrol were determined in 100 patients and compared with metrizamide and iopamidol in two double-blind studies. The relevance of the distribution of the contrast medium in the CSF space to the occurrence of headache is discussed. The low persistence of iotrol in the brain parenchyma is demonstrated on CT density profiles, compared with metrizamide and iopamidol. Iotrol seems to be the safest contrast substance for intrathecal use.

Analyses of vegetables from the highlands of Papua New Guinea.

Ten vegetable foods of the highlands were analysed for crude protein, amino acids and trypsin inhibitor. Pandanus nut, lima bean, fern, greens, pitpit and choko had good to acceptable crude protein contents and the proteins of fern, pitpit, Rungia klossii and Indian mustard were of good quality. Lima beans had a very high content of trypsin inhibitor and all others had small or zero amounts, including fern, which is used at pig feasts and hence does not contribute to pig bel.

[Cerebrovascular insufficiency: full diagnosis or ultrasonics alone?].

The possibilities and the value of all methods which are applicable for a global diagnosis in neurological patients with vascular diseases are being compared in more than 1000 cases with operations of the carotid arteries. The necessity of an overall information for an optimal treatment is shown. But one can not renounce cerebral panangiography altogether.

[EEG controls in recurrences of supratentorial brain-specific tumors].

The purpose of this study was to find out the value of postoperative EEG controls in the early detection of recurrence of supratentorial gliomas (the majority being astrocytomas, stage II to IV). 29 cases with verified tumour recurrence were examined and in all but one the EEG showed a reactivation of the focus in accordance with the development of the glioma. At least one of the following parameters had to be established: a further spreading of the focal changes, a reduction in frequency, an increase in amplitudes and focal depression of amplitudes. At least 3 postoperative EEG controls were made in each case. The duration of tumour treatment was 3 to 59 months. In 3 cases temporary focus activation was found without evidence of tumour recurrence; in one of these cases the activation was preceded by an epileptic seizure. Epileptic seizures, thus, seem to have a focus activating effect. Focus activation as a result of radiotherapy or cytostatic treatment was not observed. On the basis of our findings it appears that regularly conducted postoperative EEG controls seem to be highly suited as a non-invasive and economical method for the early detection of recurrence of this type of tumour. In the case of malignant types of gliomas involving rapid growth EEG controls should be made monthly.

[Should contrast media be used in CT studies of cerebral infarct?].

The value of contrast enhancement for the diagnosis of the ischemic brain infarct is discussed critically. In some publications there were hints to possible deterioration of the clinical prognosis of patients with brain infarcts if contrast medium had invaded into the brain parenchyma. The pathophysiological base is briefly discussed. The leading idea is the restrictive use of the contrast medium. Our opinions of different publications are controversial, and therefore useless for the diagnostic consequences. We have gained by working out detailed rules for the indication of a intravasal contrast application in patients with brain infarcts.

Breast carcinoma originating in cystosarcoma phyllodes.

Only three cases of carcinoma infiltrating the stroma of cystosarcoma phyllodes have been reported. An additional case is presented. Unlike the treatment of cystosarcoma phyllodes which is limited to complete removal of the tumor, carcinosarcomas should be treated in the same manner as carcinoma by mastectomy and axillary dissection.

[Treatment of reflux esophagitis with ranitidine. A multicentric prospective study].

Thirty-eight patients with erosive-ulcerative reflux esophagitis were treated in an open trial with the histamine-H2-receptor antagonist ranitidine (150 mg twice daily). At endoscopy after 6 weeks there was evidence of complete healing of all epithelial defects in 20 of 38 patients (53%). Continuation of treatment for another 6 weeks in 13 patients was followed by healing in 6 additional patients as judged by endoscopy. Although the symptoms of the group as a whole improved significantly during treatment, there was no correlation between the degree of symptomatic improvement and healing of esophagitis. Also, the healing rate did not depend on age, sex, smoking and drinking habits or on the severity of esophagitis. It is concluded that medical treatment of reflux esophagitis with ranitidine is promising, even in severe cases previously considered candidates for surgery.

[The value of intathecally contrast enhanced computerized tomography (CT-myelography) in spine injury ].

The computerized tomography gives optimal information in cases of spinal injuries. If a spinal traumatic stenosis and/or neurological symptoms exist the computerized tomography-myelography shows the intraspinal structures and completes the nativa scans to a high degree. The therapeutic measures are influenced substantially. The CT-myelography should be made as soon as possible, especially before an operation. In case of doubt there should be decided to the intrathecally contrast enhanced computerized tomography.
