[Neuroradiological experiences iopamidol, a new non-ionic hydro-soluble contrast medium. Comparison to metrizamide with special considerations of side effects and pharmacodynamics with regard to clinical use (author's transl)].

A report is given about the non-ionic hydrosoluble contrast medium Iopamidol, which is compared to metrizamide for lumbosacral radiculography and computed tomography cisternography (CTC). The side effects are discussed. A raised attenuation value of brain parenchyma is the effect of a migration of contrast medium into extracellular space. A quantitative measurement is possible through density profiles at different times after the contrast application together with planimetry, which are characteristic for the normal and the various pathologic kinds of CSF dynamics. These results and the simultaneously registered EEG-changes are giving informations of the pathophysiology of intrathecally used contrast media. Therefore is indispensable to keep the amount of contrast substance as low as possible. The paper gives hints for the dosage.

[Cisternography via computerised tomography: technique and pathophysiology as causes of the side effects (author's transl)].

The article reports on experiences collected with intrathecal contrast amplification in computerised tomography, using the nonionic contrast media metrizamide and iopamidol in a total of 80 examinations. The side effects are caused by the examination technique - which is discussed in detail -, the pathophysiology of CSF dynamics and the pharmacodynamics of the contrast media, but such side effects can be kept low if the relevant principles are observed. This makes the method both feasible and valuable.

[The value of compute tomography in "sciatica" (author's transl)].

13 cases of therapy-resistant lumboischialgia without herniated disk, caused in 12 cases by a tumour and in 1 case by an abscess, were examined by computed tomography of the lumbar and pelvic region. This method is indicated immediately after insufficient results of conventional X-ray methods (including tomography) and of lumbosacral radiculography. The compute tomography is indispensable also in patients with "sciatica" with a known malignoma. The informations given by computed tomography are essential for the therapy planning.

[Fluorouracil, mitomycin-C and adriamycin in the treatment of metastasizing gastrointestinal adenocarcinomas].

28 patients with advanced adenocarcinomas were treated with combinations of 5-fluorouracil and mitomycin-C (FM, 21 patients) or of 5-fluorouracil, adriamycin and mitomycin-C (FAM, 7 patients). 5 patients responded, 3 showing an objective response of more than 50% lasting 3-5 months. Toxicity was primarily hematologic, chiefly comprising severe thrombopenias. The results indicate that with these combinations a limited palliative effect can be achieved in only a few patients.

Experiences with intrathecally enhanced computed tomography.

Computed tomographic cisternography (CTC) shows small normal and pathological structures in the extracerebral CSF spaces in an excellent way and is able to differentiate communicating from non-communicating CSF-containing cysts in congenital malformations and acquired lesions. The main value of the CTC is in the examination of CSF dynamics. A report is given about our experiences with 121 patients. The dynamic flow patterns are divided into eight groups. The results are discussed in detail with suggestions as to the treatment.

Gardner's syndrome with glandular cysts of the fundic mucosa.

The present study describes a family (mother and son) with Gardner's syndrome. Aside from the classical findings, gastric polyposis was found; histologically glandular cysts of the fundic mucosa were diagnosed. The occurrence of gastric polyps in Gardner's syndrome has recently been described with increasing frequency. These lesions have various histological aspects. A number of authors have noted several cases of Gardner's syndrome with glandular cysts of the fundic mucosa, but to our knowledge this is the first case of familial occurrence.

[Venous angiomas of the brain. Diagnosed by CT and angiography (author's transl)].

Report on three cases of the relatively rare venous angioma of the brain presenting a classical angiographic pattern. On CT, one could not be demonstrated; the other two were detected because they showed a linear hyperdensity after contrast enhancement. Gross and microscopic pathology are described of one of these cases in a patient who died of an endocrine crisis following a sudden hemorrhage into a coexisting tumor of the pituitary gland. Only angiography provides a specific diagnosis; CT alone may only be able to suggest the presence of a vascular malformation. The angiographic, CT- and clinical findings and their value are discussed.

[Diagnosis of infratentorial malformations in infancy (author's transl)].

In a newborn boy with hydrocephalus the computed tomography (CT) demonstrates hypoplastic hemispheres of the cerebellum, the inferior part of the vermis is lacking and the enlarged cisterna magna is directly communicating with the fourth ventricle. The ventricles 1--3 are markedly enlarged. Only the additional studies after the application of Metrizamide intrathecally could demonstrate the direct communication of the spinal CSF system with the fourth ventricle, determining the diagnosis: Dandy-Walker-Variant. The computed tomography-cisternography (CTC) is an important additional investigation with only minimal risks to the patient.

[Lumbosacral radiculography (myelography) (author's transl)].

A report is given on lumbosacral myelography (radiculography) with special reference to technique and contrast agents used--all based on clinical and experimental experience. The use of water-soluble contrast material is recommended. The non-ionic metrizamide ("Amipaque") seems to be the agent of choice because of its inertia to cause adverse leptomeningeal changes. Also, another non-ionic contrast agent "Jopamidol" seems very promising. The non-ionic contrast media are to be applied in a concentration isotonic to that of the CSF; this is also sufficient to even show fine structures. Though the new contrast media exhibit low neurotoxicity, they are not inert. Therefore, the application of very careful technique and the limitation of amount and concentration of the contrast medium to as low as possible are indispensable. The radiation dose to spine and gonads was found to be tolerable.

Report on a post-traumatic meningioma diagnosed 37 years after the trauma. Reference to relevant cases and the value of computed tomography for medico-legal opinion in post-traumatic sequelae.

[Computer tomographic diagnosis of a mega-dolicho basilar artery (author's transl)].

Four mega-dolicho basilar arteries are described, which were found by computer tomography and confirmed by angiography. Two patients showed additional abnormalities or anomalies of other vessels. The computer tomographic features are described. The elongated vessels can produce so-called normal pressure hydrocephalus by impressing the floor of the third ventricle, although the hydrocephalus cannot be confirmed by computer tomographic cisternography.

[Inspection of 1 to 5-year-old amalgam, composite, and cast gold fillings].

On 103 randomly chosen patients composite-, amalgam-, and cast gold fillings were checked and examined after various criteria. Also, the gingival changes were noted and vitality tests made. As was to be expected, composites, particularly class 2-fillings, showed more loss of substance than amalgams. The latter, however, had a worsening marginal adaptation with increasing age. Secondary caries was generally more frequent in subgingival fillings. The quality of the material was of smaller importance than the marginal leakage. Inflammatory processes of the gingiva were more frequent with composites than with inlays. Gradually increasing discolorations were the rule. Very few teeth lost their vitality after treatment, irrespective of the bases (normal, direct and indirect capping).
