Serum amino acids in experimentally induced endotoxic shock: the prognostic significance of hyperalaninemia.

Serum amino acid levels were determined in 40 male rabbits before and 2, 6, and 24 hr after shock induction with 50 micrograms kg-1 endotoxin from Escherichia coli 0 111 and compared with amino acid levels of 32 age-matched control animals. Whereas total amino acid concentration in the control group decreased continuously over the 24 hr period due to fasting of the animals, the corresponding concentration in the endotoxin-treated animals remained at the initial level for the first 6 hr after shock induction. During this period, the release of amino acids from peripheral tissues was apparently enhanced, as evidenced by elevated levels of serum phenylalanine and tyrosine. Endotoxin administration exerted a salient effect on serum alanine levels. In all endotoxin-treated rabbits, alanine concentration was increased significantly 2 hr (P less than 0.00001) and 6 hr (P less than 0.00001) after injection of the toxin, whereas alanine levels in the control group remained essentially unchanged. In most animals, peak alanine concentrations were observed at the 6 hr sampling time. Blood glucose measurements indicated that by this time the hyperglycemic period, which is typical of the initial phase of endotoxic shock, was already finished. A high level of alanine following shock induction alluded to the impending death of the animal. In all rabbits in which alanine concentration rose by more than 600 mumols l-1 during the first 2 hr after endotoxin administration, the increase correlated significantly with the rate of lethality (P less than 0.0001). In contrast, alanine levels of surviving animals increased only moderately. It was concluded from these results that monitoring serum alanine levels might be a valuable and practical tool to assess the severity and outcome of endotoxic shock.

Plasma lipids and plasma lipoproteins in diabetics with and without proliferative retinopathy.

The single most important factor related to the development of diabetic retinopathy is the duration of diabetes. Little is known about the underlying mechanisms, but many factors have been suggested to be involved, among them derangements in plasma lipids and plasma lipoproteins. In the present study we examined the relation between plasma lipids, plasma lipoproteins, and the duration of diabetes in Type I diabetics with and without proliferative retinopathy. The duration of diabetes in the two groups was 12.2 +/- 2.8 and 21.5 +/- 9.0 years, respectively (mean +/- SD; p less than 0.01). Except for moderately low HDL levels, plasma lipid and lipoprotein concentrations were normal in both groups of patients. The levels of lipids and lipoproteins did not correlate with the duration of diabetes. Furthermore, no differences were seen between patients with and without proliferative retinopathy. Thus, the present study does not indicate that plasma lipids and plasma lipoproteins play any major role in the development of diabetic proliferative retinopathy.

Treatment planning for conformation therapy using a multi-leaf collimator.

In high energy photon therapy an optimum dose distribution is achieved with an irradiation from several directions, thus adapting the field shape to the target volume. Some methods of irradiation planning using these techniques are presented. The result of such a treatment planning is demonstrated.

Effects of Flumethrin Pour-on against Glossina palpalis gambiensis (Diptera, Glossinidae).

1920 males and females of Glossina palpalis gambiensis were fed 3 times on a Zebu treated with Flumethrin Pour-on. This resulted in a significant increase of the mortality and abortion rate of the flies. The fly-specific effects of Flumethrin Pour-on were time-dependent: Clear effects on the performance of the flies were observed during the first 20 days after treatment of the Zebu; an initial 90% "knock down" effect decreased to 40% during the first 15 days.

[Effect of age at the time of illness, tumor grade and Karnofsky value on the therapy result in patients with glioma of the cerebral cortex treated with combined therapy].

49 patients with gliomas were investigated by means of life table analysis with regard to the influence of age, grade of the tumor and Karnofsky-score on the result of the treatment. The grade of the tumor and the age are factors of high prognostic value. They significantly influence the survival time of patients with gliomas treated by a combination treatment (operation, radiation, polychemotherapy; median survival times: grade 2: 50.4 months; grade 3: 28.5 months; grade 4: 18.9 months; group younger than 40 years: 49.2 months; older than 40 years: 21.9 months). Patients with different Karnofsky-scores show no differences concerning their median survival times (Karnofsky-score 50-70: 34.2 months; 80-100: 31.8 months).

[Comparative follow-up study in vagotomized and conservatively treated duodenal ulcer patients].

We observed 87 patients with duodenal ulcer disease about one year. 46 patients (first group) after proximal gastric vagotomy were observed. A second group (41 patients) were treated with antacids only. We compared our patients for their complaints, consumption of antacids, relapse rate and clinical, social and genetic aspects. The clearly better results of the surgically treated patients allow the conclusion for a better quality of life in patients after proximal gastric vagotomy.

[Early sonographic recognition of renal cell carcinoma].

Of 19 patients with renal cell carcinoma diagnosed by ultrasound, nine already had symptoms of the tumor at the time; in the other ten it was discovered by chance during an upper-abdominal ultrasound screening. The latter group differed from the former by a lower average age (52.7 compared with 63.1 years), lower mean tumor size (4.5 and 7.0 cm, respectively), less advanced tumor stage, further course with fewer complications, and higher survival rate. The findings underline the great importance of routine abdominal ultrasound screening in the early diagnosis of tumors.

[Therapeutic possibilities in diseases caused by intestinal parasites].

Compatible and efficient drugs for the therapy, caused by protozoal- or helminthic-infections, are available today. Many of them are not registered in Austria. Metronidazole, Mebendazole and Praziquantel are particularly effective. Most protozoal-infections can be treated with metronidazole. Mebendazole can be used against numerous nematode-infections (Ascariasis, Trichuriasis, Oxyuriasis, Ancylostomiasis), Niclosamide against cestode-infections (Taeniasis, Hymenolepiasis, Diphyllobothriasis), whereas, Praziquantel is applied against trematode-infections.

Platelet aggregation in type I diabetics with and without proliferative retinopathy.

The cause of retinopathy in diabetes mellitus is unknown. Among factors suggested to be involved in the development of retinopathy is altered platelet function. In the present study, platelet aggregation was measured in vitro after stimulation with adenosine diphosphate (ADP) and collagen in patients with and without proliferative retinopathy. The results show that patients with proliferative retinopathy have an increased platelet aggregation in vitro after stimulation with collagen and ADP. However, the increased platelet aggregation was also found to be correlated to the duration of diabetes. Thus, the present study does not support the opinion that abnormal platelet function can be regarded as a primary cause of diabetic retinopathy. Increased platelet aggregation seems to be coupled to the duration of diabetes and to still unknown factors developing with prolonged duration of the disease.

Trilateral malignant lymphoma: primary malignant B-cell lymphoma of the eyes and brain. Diagnosis by transvitreal retinochoroidal biopsy.

A 59-year-old woman with therapy-resistant bilateral involvement of posterior uvea and retina of some months' duration was examined. Diagnostic vitrectomy was inconclusive, but subsequent transvitreal retinochoroidal biopsy gave ample material for the immunohistopathological diagnosis of a malignant B cell lymphoma. Despite systemic and intrathecal cytostatics and ocular X-ray therapy, central nervous system involvement was observed 2 months later. The ocular lesions regressed after treatment and have not recurred.

[Vascular spasms in microsurgery].

Human arteries and veins contract with hypoxia and deprivation of substrate provoked by increasing calcium inflow into the cell and reduced energy. Such spasm may be eliminated by phosphoenolpyruvate which loads up the cell's energy, blocking glycogen reduction at the same time. Reperfusion will then guarantee a sufficient energy stroke. Therapy should pursue the following steps: 1. Phosphoenolpyruvate infusion (1 X 10(-6) up to 1 X 10(-3) gm/ml in tyrodes solution pH 7.3) into the arterial branch, proximal and distal to the injury. 2. Subsequent treatment with vasodilating drugs and rheologically active substances. 3. Failed therapy after more than three hours warm ischemia could be due to autolytic processes and requires resection of the affected vessel. 4. The imbalance of the thrombolytic system with the so-called no reflow phenomenon could be due to a plasminogen activator's inhibitor released during hypoxia. Such cases may reasonably be treated by urokinase or by streptokinase plasminogen complex.

[Pneumonia in acute leukemias].

Acute leukaemia was complicated by pneumonia in 38 (34.8%) of 109 patients treated between 1979 and 1983; in 39.5% of the patients pneumonia occurred more than once. In 23 patients (60.5%) pneumonia occurred during cytostatic therapy, and 25 patients (65.8%) had less than 1000 mm2 granulocytes. Antibiotic therapy had no or only little effect in 70%. A total of 21 patients (55.3%) died of pneumonia. In 15 patients a direct relationship could be seen between pneumonia and the bacterial spectrum in the sputum. A prevalence of gram-negative bacteria was found (24 of 40 bacteria isolated, especially Enterobacteriaceae (19). Fungi were cultivated in 10 cases. Each of the typical pneumonia bacteria was only seen once respectively. It is most important that therapy begin immediately, even before the bacteria have been identified. Only then is there hope that the survival time of patients with acute leukaemia can be influenced.

Normal eyes in type 1 diabetics stay normal after one year of treatment with continuous subcutaneous insulin pump.

Seven patients with type 1 diabetes mellitus were restored to near normoglycaemia by treatment with continuous subcutaneous insulin infusion pumps (CSII). The patients were examined with ophthalmoscopy, fundus photography and fluorescein angiography before and one year after the start of CSII treatment. In addition, ophthalmoscopy was performed after 6 months of treatment. All 14 eyes were normal prior to the CSII treatment and none had developed any signs of retinopathy after 6 months or 1 year. It is concluded that metabolic control can be near normalized with CSII treatment without any risk for development of diabetic microangiopathy in type 1 diabetics with normal eyes.

[Subjective quality of life of surgically treated glioma patients undergoing polychemotherapy].

25 patients with glioma were questioned as to how they apprehended and appreciated their state of health and their psychosocial situation 1 1/2 years (s = 4 months) after the start of their postoperative polychemotherapy. In their opinion, their health was relatively good, that is, they claimed to suffer little pain, few complaints, little feeling of inadequacy, few fears for the future. They regarded the mode of treatment as being somewhat more inconvenient. Such an unexpectedly positive response, in view of the poor prospect, raises the question as to its provenance. A significant cause is seen in the organic psycho-syndrome, and its significance is made the subject of the concluding discussion, with reference to adaptation mechanisms and specifics of the illness.
