49 patients with gliomas were investigated by means of life table analysis with regard to the influence of age, grade of the tumor and Karnofsky-score on the result of the treatment. The grade of the tumor and the age are factors of high prognostic value. They significantly influence the survival time of patients with gliomas treated by a combination treatment (operation, radiation, polychemotherapy; median survival times: grade 2: 50.4 months; grade 3: 28.5 months; grade 4: 18.9 months; group younger than 40 years: 49.2 months; older than 40 years: 21.9 months). Patients with different Karnofsky-scores show no differences concerning their median survival times (Karnofsky-score 50-70: 34.2 months; 80-100: 31.8 months).
We observed 87 patients with duodenal ulcer disease about one year. 46 patients (first group) after proximal gastric vagotomy were observed. A second group (41 patients) were treated with antacids only. We compared our patients for their complaints, consumption of antacids, relapse rate and clinical, social and genetic aspects. The clearly better results of the surgically treated patients allow the conclusion for a better quality of life in patients after proximal gastric vagotomy.
Of 19 patients with renal cell carcinoma diagnosed by ultrasound, nine already had symptoms of the tumor at the time; in the other ten it was discovered by chance during an upper-abdominal ultrasound screening. The latter group differed from the former by a lower average age (52.7 compared with 63.1 years), lower mean tumor size (4.5 and 7.0 cm, respectively), less advanced tumor stage, further course with fewer complications, and higher survival rate. The findings underline the great importance of routine abdominal ultrasound screening in the early diagnosis of tumors.
Compatible and efficient drugs for the therapy, caused by protozoal- or helminthic-infections, are available today. Many of them are not registered in Austria. Metronidazole, Mebendazole and Praziquantel are particularly effective. Most protozoal-infections can be treated with metronidazole. Mebendazole can be used against numerous nematode-infections (Ascariasis, Trichuriasis, Oxyuriasis, Ancylostomiasis), Niclosamide against cestode-infections (Taeniasis, Hymenolepiasis, Diphyllobothriasis), whereas, Praziquantel is applied against trematode-infections.
The cause of retinopathy in diabetes mellitus is unknown. Among factors suggested to be involved in the development of retinopathy is altered platelet function. In the present study, platelet aggregation was measured in vitro after stimulation with adenosine diphosphate (ADP) and collagen in patients with and without proliferative retinopathy. The results show that patients with proliferative retinopathy have an increased platelet aggregation in vitro after stimulation with collagen and ADP. However, the increased platelet aggregation was also found to be correlated to the duration of diabetes. Thus, the present study does not support the opinion that abnormal platelet function can be regarded as a primary cause of diabetic retinopathy. Increased platelet aggregation seems to be coupled to the duration of diabetes and to still unknown factors developing with prolonged duration of the disease.
A 59-year-old woman with therapy-resistant bilateral involvement of posterior uvea and retina of some months' duration was examined. Diagnostic vitrectomy was inconclusive, but subsequent transvitreal retinochoroidal biopsy gave ample material for the immunohistopathological diagnosis of a malignant B cell lymphoma. Despite systemic and intrathecal cytostatics and ocular X-ray therapy, central nervous system involvement was observed 2 months later. The ocular lesions regressed after treatment and have not recurred.
Human arteries and veins contract with hypoxia and deprivation of substrate provoked by increasing calcium inflow into the cell and reduced energy. Such spasm may be eliminated by phosphoenolpyruvate which loads up the cell's energy, blocking glycogen reduction at the same time. Reperfusion will then guarantee a sufficient energy stroke. Therapy should pursue the following steps: 1. Phosphoenolpyruvate infusion (1 X 10(-6) up to 1 X 10(-3) gm/ml in tyrodes solution pH 7.3) into the arterial branch, proximal and distal to the injury. 2. Subsequent treatment with vasodilating drugs and rheologically active substances. 3. Failed therapy after more than three hours warm ischemia could be due to autolytic processes and requires resection of the affected vessel. 4. The imbalance of the thrombolytic system with the so-called no reflow phenomenon could be due to a plasminogen activator's inhibitor released during hypoxia. Such cases may reasonably be treated by urokinase or by streptokinase plasminogen complex.
Acute leukaemia was complicated by pneumonia in 38 (34.8%) of 109 patients treated between 1979 and 1983; in 39.5% of the patients pneumonia occurred more than once. In 23 patients (60.5%) pneumonia occurred during cytostatic therapy, and 25 patients (65.8%) had less than 1000 mm2 granulocytes. Antibiotic therapy had no or only little effect in 70%. A total of 21 patients (55.3%) died of pneumonia. In 15 patients a direct relationship could be seen between pneumonia and the bacterial spectrum in the sputum. A prevalence of gram-negative bacteria was found (24 of 40 bacteria isolated, especially Enterobacteriaceae (19). Fungi were cultivated in 10 cases. Each of the typical pneumonia bacteria was only seen once respectively. It is most important that therapy begin immediately, even before the bacteria have been identified. Only then is there hope that the survival time of patients with acute leukaemia can be influenced.
Seven patients with type 1 diabetes mellitus were restored to near normoglycaemia by treatment with continuous subcutaneous insulin infusion pumps (CSII). The patients were examined with ophthalmoscopy, fundus photography and fluorescein angiography before and one year after the start of CSII treatment. In addition, ophthalmoscopy was performed after 6 months of treatment. All 14 eyes were normal prior to the CSII treatment and none had developed any signs of retinopathy after 6 months or 1 year. It is concluded that metabolic control can be near normalized with CSII treatment without any risk for development of diabetic microangiopathy in type 1 diabetics with normal eyes.

Reference values for free amino acids in male rabbits (n = 145) were determined by gas chromatography. In addition, reference values for glucose, serum transaminase activities (aspartate aminotransferase, alanine aminotransferase), and serum protein fractions are reported.
25 patients with glioma were questioned as to how they apprehended and appreciated their state of health and their psychosocial situation 1 1/2 years (s = 4 months) after the start of their postoperative polychemotherapy. In their opinion, their health was relatively good, that is, they claimed to suffer little pain, few complaints, little feeling of inadequacy, few fears for the future. They regarded the mode of treatment as being somewhat more inconvenient. Such an unexpectedly positive response, in view of the poor prospect, raises the question as to its provenance. A significant cause is seen in the organic psycho-syndrome, and its significance is made the subject of the concluding discussion, with reference to adaptation mechanisms and specifics of the illness.
In an attempt to improve visualization of the position of the guidewire and dilatation balloon during coronary angioplasty, a method was developed called digital coronary roadmapping. With this method a digitally acquired coronary angiogram is interlaced with the live fluoroscopic image of the guidewire and balloon catheter. The digital coronary angiogram is superimposed at the same magnification and radiologic projection as the live fluoroscopic image onto the video monitor above the catheterization table. The digital roadmap image thus provides immediate feedback to the angiographer to assist in directing the guidewire into the appropriate coronary artery branch and to help in placement of the balloon so that it straddles the site of stenosis.
The personal characteristics of 80 clients with elevated 2-4 (D-PD) scales on their MMPIs were compared with those of 109 clients without these elevations. Clients with the 2-4 elevated were significantly more depressed, had lower self-esteem, and were more likely to come from disturbed families. They also had poor relationships with the opposite sex, became more dependent on the therapist, and often made little improvement in therapy. In comparison to previous descriptions of hospitalized patients with similar profiles, the counseling center clients continue to function in the community and show less pathological behavior. Implications for therapy are discussed.
The ability of certain neuropeptides (glucagon, somatostatin, leu-enkephalin and neurotensin) to release known neurotransmitters (glycine, GABA, dopamine and 5-hydroxytryptamine) was tested in the chicken retina. Tritiated neurotransmitters were injected intravitreally in chicken eyes. After excision, the retina was stimulated in vitro with the neuropeptide in micromolar concentrations while monitoring the efflux of radioactivity from the retina. A rise of the efflux represents a stimulus dependent release. Neurotensin release [3H] glycine, [3H]dopamine and [3H]5-hydroxytryptamine. Leu-enkephalin released [3H]dopamine and somatostatin released [3H]5-hydroxytryptamine. Glucagon was without effect. [3H]GABA was not released by any of the neuropeptides.

On 47 borderline hypertensives discriminative reaction tests (17 test persons) and ergometries (30 test persons) were carried out. It could be proved that both in psychic and in physical demand by beta-blocker therapy heart rate and blood pressure significantly remained below the initial value. The calculation of the interior heart work resulted in an economization of the heart work. By means of the beta-blocker therapy of the borderline hypertensives a more favourable situation in an increased functional demand and a decreased reaction of the blood pressure on psychic stress was achieved.
