Fatal adult respiratory distress syndrome in a scalded child after immunization with attenuated virus (measles, mumps and rubella).

A fatal adult respiratory distress syndrome (ARDS) occurred in a 15-month-old child who had suffered minor scalding during the febrile response to combined attenuated virus immunization (measles, mumps and rubella [MMR]). Despite vigorous efforts the child died 26 days after the accident. It is suggested that the scalding suppressed the normal immune response to the viremia and that the latter (i.e. most likely the measles viremia) caused the lung damage which, in turn, led to the ARDS. Histologically the lung presented a peculiar change with fibroblastic nodules, vessel wall inflammation and signs as observed in ARDS.

Prediction of the histological type of glomerulonephritis. Multiple discriminant analysis of clinical and laboratory characteristics at time of diagnosis.

We performed a prospective study on 68 subjects with biopsy-proven clomerulonephritis using a discriminant analysis of 12 clinical and laboratory characteristics. Among these, the selectivity of proteinuria, the presence of hematuria and the plasma level of C3 proved to be the most useful. An equation based on these three variables discriminated without overlap between 17 patients with minimal change glomerulonephritis (MCGN) and 30 patients with either membranoproliferative or rapidly progressive glomerulonephritis. However, some overlap existed between MCGN and focal sclerosis or membranous glomerulonephritis. In addition, the formula seems to identify steroid-resistant patients with MCGN.

Papilloma of the bronchus.

Based on the observation of a recurring bronchial papilloma in a 56-year-old male, the authors aim at directing the interest of pneumologists, and especially endoscopists, to this group of rare lesions. The tumor observed obviously developed in a lung lobe affected by chronic tuberculosis, and presented with luminal obstruction. The histopathological definition of solitary papilloma, its differential diagnosis from from papillomatosis, the probable dignity of this type of lesions and the clinical features are discussed.

[Lymphatic manifestation forms in the terminal phase of chronic myelocytic leukemia].

A blastic transformation in chronic myeloic leukaemia (CML) is reported, which, on the one hand, is characterized by the "lymphoid" feature of blasts and, on the other hand, by the appearance of great lymph node swellings. The clear distinction from (secondary) malignant lymphomas may be difficult. The inclusion of such forms of blastic transformation in CML is based on cytogenetic findings. Their non-granulocytic or "lymphoid" nature emerges from their cytological behavior. The fact that these lymphoid transformations respond to vincristin and prednison emphasize that their recognition as special entities is justified. They also demonstrate that the neoplastic process in CML may not only involve myelopoietic but pluripotent stem cells as well.

Adjuvant chemotherapy with VM 26 and CCNU after operation and radiotherapy of high-grade supratentorial astrocytomas.

Thirty-one patients with histologically proven malignant (Grade III and IV) astrocytomas were randomly placed in one of two treatment schedules two weeks after gross total or subtotal tumor removal. One group (15 patients) received radiation therapy and adjuvant chemotherapy with VM 26 and CCNU once every four weeks, commencing four weeks after the completion of radiotherapy. The second group (16 patients) received radiotherapy alone. The duration of the progression-free interval from operation to recurrence was measured to evaluate the effect of the adjuvant chemotherapy. The median free interval calculated by the computer analysis according to the method of Kaplan and Meier was 65.5 weeks for the chemotherapy group and 40.0 weeks for the control group. The difference is not statistically significant. From our experience, adjuvant chemotherapy should only be given to patients with residual or progressive tumors following radiation therapy.

[Injury to the inferior alveolar nerve caused by osteosynthesis screws].

The regeneration and degeneration of the proximal and distal parts of the nerve resulting from damage to the alveolaris nerve by osteosynthesis screws was studied in animal experiments. Twenty-four rabbits were divided into two groups of twelve animals each. In group A, the osteosynthesis screws were left in the nerve during the three-month study; in group B, these screws were removed after one week. A significant difference in the regenerative ability of the inferior alveolaris nerve was noted between group A and group B. The clinical experience that early removal positively affects the regenerative ability of the nerve was confirmed.

The incidence of congenital adrenal hyperplasia in Switzerland--a survey of patients born in 1960 to 1974.

The incidence of congenital adrenal hyperplasia (CAH) in Switzerland was calucated for patients born in 1960 to 1974. Information was obtained by means of questionnaires sent to all Pediatric Hospitals and Institutes of Pathology in Switzerland. The number of patients and the incidences of the different enzyme defects were: all cases 110 (1:13785 livebirths), 21-hydroxylase 98 (1:15472), 11 beta-hydroxylase 5 (1:303260), 3 beta-dehydrogenase 3 (1:505433). The incidence of females--which is 1:12098 for all cases and 1:13924 for 21-hydroxylase deficiency--presumably appropximates more exactly the real incidence, because of incomplete diagnosis in the male. Salt loss was observed in two thirds of patients with 21-hydroxylase deficiency. A more reliable estimate of the incidence of CAH could be obtained by neonatal screening programs with measurement of serum-17-hydroxyprogesterone.

[Aneuploidia in malignant tumors of testis and their lymphatic metastases (author's transl)].

DNA-estimations, done by flow-through-cytophotometry, were performed in 18 malignant tumors of testis. Aneuploid stemlines which are specific for cancers were found in 17 cases. The only exception was a so called "occult seminoma". It showed only a small difference between the stem-line of tumor and normal cells of testis. So, by using DNA-estimation, it was possible to make a correct diagnosis all 18 patients, suffering from tumor of the testis. The aneuploid stem-lines of seminomas did not differ from those of other cancers. The type of proliferative behavior seems to be a criterion for prognosis. Lymphatic nodes with and without formation of metastases very clearly showed differences in the DNA-content of their nuclei.

[Middle ear tuberculosis (author's transl)].

With the decrease in tuberculosis in general that of the middle ear has become a rarity, such that it is almost unknown to the younger generation of doctors. Nevertheless one must consider this diagnosis in all relapsing and treatment-resistant inflammations of the middle ear, especially when tympanoplasty procedures have been unsuccessful. Bacteriological findings are less reliable than histological proof and therefore examination of granulation tissue is always indicated. A precise history into earlier tuberculosis is important, since the disease is mostly disseminated haematologically. Two clinical examples with pathological descriptions are presented. In the first case fresh exudative disease similar to that of tubotympanitis occurred and quickly passed on to a chronic destructive middle ear inflammatory process. One year previously the patient had pleurisy. A complete cure followed surgery and antituberculosis therapy. In the second case a chronic proliferative and destructive inflammatory process was present with obviously less favourable reparative properties since two tympanoplasties were ineffectively performed.

The prognostic power of flow-through cytophotometric DNA determinations for testicular diseases.

Flow-through cytophotometric DNA determinations with the ICP 11 were carried out in nuclei of testes stained with ethidium bromide. The histograms of healthy testicles showed two haploid peaks (1 CI/1CII), a diploid (2C) and a tetraploid peak (4C). The more spermatogenesis was histologically reduced, the more the height of the haploid and tetraploid peaks and of the graph in the area of the S-phase was decreased. It resulted from a relative increase of diploid cells of testicular parenchyma. Conclusions can be drawn from the DNA histogram on the degree of inhibition of spermatogenesis, but not on its etiology (inflammation, malposition, endocrime disorders). In torsions of tests, the histogram showed differences from normal graphs, increasing with the degree of necrosis. The histograms of malignant tumors of testes had many more signals in the area of the S-phase and in the hypertetraploid region than those of testes without malignant tumors. In addition, they all had aneuploid peaks, although not in each of the multiple samples of one tumor. Nonseminomas differed more from tumor-free testes than did seminomas. The DNA determinations in nuclei of testes gave objective information about spermatogenesis and, in torsions of testes, about the rate of necrosis. It facilitated the intraoperative decision as to whether a testis should be conserved or extirpated. Malignancy could be established with a high degree of certainty. The difference between histograms of testicular carcinomas and healthy testes seemed to give prognostic information.
