Reports of two cases of selective adrenocorticotropin (ACTH) and growth hormone (GH) deficiency: differential diagnosis from cases with isolated ACTH deficiency associated with transient GH insufficiency.

An acquired partial pituitary insufficiency with selective ACTH and GH deficiency was demonstrated in two men aged 47 and 54, for which the clinical course over many years corresponds to Addison's disease. In one of the 2 cases, antibodies to anterior pituitary cell membrane, assayed by an immunofluorescence method with GH3 cells (rat GH and prolactin secreting cell) and AtT-20 cells (mouse ACTH secreting cell) as antigens, were positive. We also present a 55-year-old man with isolated ACTH deficiency associated with transient GH deficiency. In this case, hydrocortisone replacement corrected his subnormal, pre-therapy GH response to insulin tolerance and glucagon propranolol tests, although there was no response of serum GH to L-dops and arginine stimulation test before therapy. Selective ACTH and GH deficiency are very rare and the finding of transient GH insufficiency in a patient with isolated ACTH deficiency suggests that repeated testing while on hydrocortisone replacement therapy is of great diagnostic importance in order to distinguish between selective ACTH and GH deficiency and isolated ACTH deficiency accompanied by transient GH insufficiency.

[New application of monoclonal antibody for cancer diagnosis].

Clinical significance of serodiagnosis of adenocarcinoma-associated antigen which were detected in our laboratory was discussed. Anti-idiotype monoclonal antibody against monoclonal antibody YH206 was prepared and found useful for the detection of anti-YH206 in patient sera. The simultaneous measurement of anti-YH206 antibody and YH206 antigen is of value and a new application of anti-idiotype antibody to the cancer diagnosis. The third point of the report was a result on mRNA expression in cancerous and non-cancerous tissues by Northern blotting method using CEA- and NCA-cDNA probes. Finally, it was reported that we prepared monoclonal antibodies to synthetic peptide which was identical to a part of CEA domain I (AAs 181-194). The reactivities were characterized and the usefulness to a detailed analysis of CEA epitope and the possible application to serodiagnosis were discussed.

[Clinical evaluation of PIVKA-II as a marker of hepatocellular carcinoma].

PIVKA-II (protein induced vitamin K absence or antagonist-II) levels in plasma were measured using ELISA in patients with hepatocellular carcinoma (HCC). PIVKA-II was detected in 53 of 97 patients (54.6%) of HCC, and the combination assay of PIVKA-II and AFP identified 69 of 97 patients (71.1%) with HCC. However, the positive rate of PIVKA-II (6.1%) was lower than that of AFP (37.5%) in patients with HCC less than 2 cm in diameter. The concentration of PIVKA-II was markedly reduced within 2 weeks after treatment in patients with HCC showing a favorable response to Lp-TAE or Lp-TAI. Although PIVKA-II is a valuable marker for the diagnosis and follow-up of patients with HCC, it must be noted that the change to negative concentration of PIVKA-II after treatment does not mean complete necrosis of the tumor.

[Clinical studies on the relation of abnormal growth hormone secretions to hepatic diabetes mellitus in patients with hepatocellular carcinoma].

Paradoxical growth hormone (GH) responses in 50 g or 75 g oral glucose tolerance tests (OGTT) have been demonstrated in 24 patients with hepatocellular carcinoma, whereas no significant changes in serum GH levels after OGTT were shown in 10 normal controls, 6 patients with cirrhosis of liver, and with chronic active hepatitis. There were no significant difference in the GH responses in OGTT as well as in the incidence of paradoxical GH responses between diabetic and non-diabetic patients with HCC. Informatively, the basal somatomedin C level was very low in all cases examined.

[Malignant lymphoma displaying rearrangements of both immunoglobulin and T-cell receptor genes].

A 65-year-old male was admitted to our hospital due to recurred malignant lymphoma of left tonsil origin. Studies using flow cytometry on mononuclear cells in peripheral blood revealed the appearance of intermediate B cells, and examinations on gastrointestinal tract showed diffuse infiltration of medium-sized lymphoid cells into stomach and colon, that had the same phenotype as tumor cells in peripheral blood. They suggested leukemic change and gastrointestinal tract infiltration of malignant lymphoma. Southern blot analysis revealed T-cell receptor beta-chain gene rearrangement as well as immunoglobulin gene rearrangement. Analysis by histo in situ hybridization on infiltrated gastric mucosa specimen showed diffuse expression of immunoglobulin mRNA in tumor cells, but no message of T-cell receptor beta-chain gene. Northern blot analysis showed the same result. It suggests that in this case, the rearrangement of T-cell receptor beta-chain gene is ineffective rearrangement without transcription to mRNA.

[CD33-positive acute lymphoblastic leukemia with breast tumor and cavernous sinus tumor].

A 38-year-old woman was diagnosed as acute lymphoblastic leukemia (L2) in Oct. 1985. After VP and AdVEMP therapy, complete remission was obtained. In Oct. 1987, she noticed bilateral breast tumors and leukemic cell infiltrations were shown in a biopsy specimen of the breast tumor. Bone marrow was occupied with 94 percent blasts. The second complete remission was achieved by the AdVP therapy. In Nov. 1988, she developed double vision and photophobia. The examinations of CT and MRI showed cavernous sinus tumor, and 20 percent blasts were recognized in a bone marrow aspirate. The leukemic cells were negative for peroxidase, but were positive for both lymphoid and myeloid cell surface markers (CD2, CD5, CD7, CD33). The two color flowcytometry showed that CD5 and CD33 were simultaneously expressed on the leukemic cells.

[Clinical application of anti-idiotypic antibody].

The concept of idiotype networks are supposed to play important roles in regulation of the immune response, and anti-idiotypic (anti-Id) antibodies have enabled to analyze the pathogenesis of autoimmune diseases and structural relations of receptors and ligands in clinical fields. We applied anti-idiotypic monoclonal antibodies (MoAb) to tumor immunology to analyze idiotype network system of tumor associated antigen (TAA) and to investigate the fine structure of TAAs and their corresponding antibodies. New assays were established to detect circulating TAAs, anti-TAA antibodies and their immune complex regarding YH 206 antigen and carcinoembryonic antigen (CEA) systems. The measurement of anti-TAA antibodies and immune complex was found to be of value for the cancer diagnosis. The results about immune response with use of anti-Id monoclonal antibodies suggest that some of the anti-Id antibodies might be useful for cancer therapy, since anti-Id antibody could induce anti-anti-Id antibodies reactive with tumor antigen. In this review, the usefulness of anti-Id MoAb to serodiagnosis and immunotherapy for cancer was discussed.

Detection of novel proteins associated with secondary amyloidosis and Alzheimer's disease by monoclonal antibody.

We have established a monoclonal antibody (MoAb) AM34 (IgG1) which was prepared by a hybridoma constructed from fusion between murine myeloma cells and murine splenocytes. Crude amyloid proteins which were used as immunogen, were extracted from the kidney of a patient with rheumatoid arthritis by the distilled water method. This antibody strongly reacted with all 8 cases of secondary amyloidosis, but did not react or very weakly reacted with 17 tissue sections of primary or myeloma-associated amyloidosis. Other amyloid tissues did not give any positive reaction. Interestingly, 6 brain tissues of Alzheimer's disease clearly showed positive staining with this antibody, whereas two apparently normal brain tissues exhibited negative staining. Senile plaque cores, neurofibrillary tangles (weakly stained) and cerebrovascular amyloid in Alzheimer's disease were stained. Absorption of the MoAb AM34 with the crude amyloid proteins abolished the immunoreactivity of the MoAb AM34 not only with the kidney tissue section of the secondary amyloidosis, but also with the above mentioned portions of the brain in the case of Alzheimer's disease. Therefore, these immunohistological data suggest that the MoAb AM34 recognizes common epitope which exists in amyloid deposits of both secondary amyloidosis and Alzheimer's disease. An inhibition test on the kidney section showed that the reactivity of MoAb AM34 was not at all inhibited by the pretreatment of the section with 10 times higher concentration of anti-human amyloid A (AA) MoAb KM268 which was prepared against synthetic peptides of AA protein, suggesting that MoAb AM34 might react with amyloid-related protein other than AA protein. In addition, MoAb KM268 did not react with any lesions in Alzheimer's disease.(ABSTRACT TRUNCATED AT 250 WORDS)

Immunochemical characterization of adenocarcinoma-associated antigen YH206.

The antigen recognized by MAb YH206 is mainly expressed in adenocarcinomas and is also detected in the sera of cancer patients (Hinoda et al., 1987). This antigen (antigen YH206) was chemically characterized and purified by column chromatography. SDS-PAGE analysis revealed a broad component in the high-molecular-weight range which was clearly detectable by carbohydrate (PAS) but not by protein (silver) stain. Agarose gel electrophoresis and immunoblotting of antigen YH206 indicated that it consists of a high-molecular-weight component (more than 2,000 kDa). Treatment of antigen YH206 with alkali suggested that the antigenic determinant consists of carbohydrate chains of mucin type. Density gradient ultracentrifugation revealed that the activity of antigen YH206 is localized at a density of 1.45 g/ml, suggesting that antigen YH206 is a mucin. Neuraminidase treatment of antigen YH206 indicated that the epitope is cryptic and contains an asialocarbohydrate chain. Once antigen YH206 has been purified by affinity chromatography, neither CA19-9 antigen nor DU-PAN-II antigen can be detected, although they were present at very high levels in the crude ascitic starting material; these last two are representative carbohydrate antigens which are widely used for serodiagnosis to detect adenocarcinomas.

Stress-induced vascular damage and ulcer.

Factors controlling gastric mucosal blood flow are now better understood, and improved techniques have been established to study microcirculation in the gastrointestinal tract. In this paper, four areas are reviewed: gross and microscopic vessels that supply blood to the stomach; factors that control gastrointestinal blood flow; pathogenic role of disturbed mucosal blood flow circulation induced by stress; and findings in early vascular permeability and injury caused by cold restraint stress in the rat.

A case of early cancer in the gallbladder: immunohistological analysis with monoclonal antibodies.

A case of early cancer in the gallbladder is reported. The tumor was disclosed as an echogenic nodular lesion with irregular margin by ultrasonography and endoscopic retrograde cholangiography. Among several tumor markers, CA19-9 showed a slightly higher level (51 U/ml). An operation was carried out because of the tumor size (over 10 mm) and the coexistence of a gallstone. The tumor was a well differentiated tubular adenocarcinoma. In the carcinoma tissue, CA19-9 and YH206 antigens showed different staining patterns, the latter antigen being detected by the monoclonal antibody, YH206, established in our laboratory. Although CA19-9 was strongly positive in the non-cancerous gallbladder epithelial cells, the YH206 antigen was negative. A slight elevation of the CA19-9 level in the patient's serum was probably caused by a diffusion of the CA19-9 antigen from the gallbladder epithelial cells to the serum, due to inflammation. On the other hand, it is interesting to note that the YH206 antigen was not demonstrated in the non-cancerous gallbladder epithelial cells but was in the cancer tissue.

Effect of thyrotropin releasing hormone injection on blood growth hormone (GH), TSH and growth hormone releasing hormone (GHRH) concentrations in cancer patients.

In order to investigate whether endogenous GHRH and somatostatin were involved in the mechanism of the paradoxical GH rise after TRH injection, changes in serum GH and plasma GHRH were examined before and after TRH injection in 12 cancer patients and changes in serum TSH and GH were similarly studied in 76 cancer patients including 31 GH-responders and 45 GH-nonresponders to TRH. TRH stimulated GH secretions without altering the circulating GHRH concentration in 4 of the 12 cancer patients. There was neither a significant correlation between the increase from the basal to maximum GH and GHRH after TRH injection in the 12 cancer patients nor a reciprocal relationship between the increase in GH and TSH after TRH injection in the 76 cancer patients. These findings suggested that the paradoxical GH rise after TRH injection in cancer patients was exerted by its direct action at the pituitary level, and not mediated through the hypothalamus.
