Transcription of biliary glycoprotein I gene in malignant and non-malignant human liver tissues.

Biliary glycoprotein I (BGP I) is a member of carcinoembryonic antigen (CEA) gene family consisting of at least 11 related genes. The transcription of BGP I gene was analysed in malignant and non-malignant human liver tissues with a 396-bp 3'-untranslated region probe from a cDNA clone 4-13 which was newly isolated from an adult human colon cDNA library. Among 21 tissue samples from 14 patients with hepatocellular carcinoma, 16 samples were clearly shown to express a single 3.9-kb message. This message was also found in the hepatoma cell line HuH-7. When the malignant tissues were compared to the non-malignant ones for the intensity of the band, no significant difference was observed. mRNAs of CEA and non-specific cross-reacting antigen (NCA) were not detected in 5 samples which were shown to have the message of the BGP I gene. These data suggest that the human hepatocyte and its malignant transformant produce BGP I, and that this could correspond to the cross-reacting antigen previously detected in the liver.

Significance of adenocarcinoma-associated antigen YH206 levels in the pancreatic juice.

We measured the pancreatic juice levels of antigen YH206, which is a new tumor marker of adenocarcinomas detected by monoclonal antibody YH206 (Hinoda et al., Int. J Cancer 24(5):653-658, 1988). Sandwich enzyme immunoassay revealed that samples from patients with pancreas cancer (n = 21) showed significantly higher values (P less than 0.01) than those of healthy controls (n = 15). Eight out of 21 (38.1%) samples from patients with pancreas cancer showed more than 100 U/ml, whereas only one out of 20 (5.0%) from patients with chronic pancreatitis exhibited more than 100 U/ml of antigen YH206. Simultaneous measurement of antigen YH206 and CA19-9 demonstrated that although a higher incidence of positivity in the case of pancreas cancer was obtained for both antigens, antigen YH206 showed much lower incidence of positivity (14%) than CA19-9 (57%) in patients with chronic pancreatitis. Therefore, the measurement of antigen YH206 in the pancreatic juice could be of use for the diagnosis of pancreas cancer.

Fibronectin-related substance located in the chief cells of human and rat gastric mucosa.

A novel substance located in the chief cells of human and rat gastric mucosa, which was detected immunologically by either polyclonal or monoclonal antihuman fibronectin (FN) antibodies, is reported. All three polyclonal antihuman FN antibodies used in this study reacted immunohistologically exclusively with the chief cells. Monoclonal antibody against C-terminal peptide or cell binding peptide reacted clearly with the human chief cells, but monoclonal antibodies against FN N-terminal and midmolecule failed to react with the cells. Western blot analysis of the rat gastric mucosal extract with polyclonal antihuman FN antibody showed that this substance has a molecular weight of about 70,000 Da. Therefore, this substance appears to be a fragment containing the C-terminal peptide of whole molecule FN and thus in the present study is named FN-related substance (FNRS). In a further study with ethanol-induced ulcer model of the rat, the physiological significance of FNRS was examined. The FNRS decreased remarkably, in a dose-dependent manner, in the fundic mucosa of the rats that ingested ethanol. The FNRS appeared to be associated with development of mucosal damage and repair, subsequently playing, in part, an important role in the gastric mucosal protection mechanism.

Detection of mRNAs of carcinoembryonic antigen and nonspecific cross-reacting antigen genes in colorectal adenomas and carcinomas by in situ hybridization.

Expression of mRNAs of carcinoembryonic antigen (CEA) and nonspecific cross-reacting antigen (NCA) genes in colorectal carcinomas and adenomas was investigated by in situ hybridization (ISH) with specific biotinylated probes. Hybridization was clearly detected throughout the cytoplasm of 7 out of 15 adenomas and 13 out of 15 carcinomas with the CEA cDNA probe, and in 6 out of 15 adenomas and 10 out of 15 carcinomas with the NCA cDNA probe. The intensity of signal appeared to be stronger in carcinomas than that in adenomas, and the CEA and NCA mRNAs were expressed together in most of the positive tissue specimens. On the other hand, noninvaded tissues adjacent to the carcinoma did not show any signal except for 4 cases faintly stained with the NCA probe. This finding was partly confirmed by Northern blot analysis which indicated that the specific bands for the CEA and NCA mRNAs were more intense in RNAs from carcinoma tissues than in those from adjacent noninvaded tissues. These data suggest that the ISH technique with biotinylated probes could be of use for analyzing expression and localization of CEA and related genes on tissue sections.

Recognition of the polypeptide core of mucin by monoclonal antibody MUSE11 against an adenocarcinoma-associated antigen.

Monoclonal antibody (MAb) MUSE11 detects an adenocarcinoma-associated antigen and is useful for the serodiagnosis of pancreas cancer. We established a sandwich enzyme immunoassay using MAb MUSE11 and MAb DF3 against a breast cancer-associated mucin core protein as a catcher and a tracer, respectively. With this assay system, the binding of the tracer MAb DF3 to an antigen in the human kidney tissue lysate was clearly inhibited by MAb MUSE11. In addition, MAb MUSE11 showed a significant binding activity to the synthetic peptide corresponding to the tandem repeat of a human epithelial mucin core protein. These data suggest that MAb MUSE11 could detect the polypeptide core of a mucin, and may be of use for studying mucin as a gene product.

Preparation of anti-T idiotype monoclonal antibody reacting with human T leukaemic cell lines and with a small percentage of peripheral T lymphocytes.

Monoclonal antibody (MoAb) KT38 raised against a human T leukaemic cell line TALL-1, reacted with another T leukaemic cell line Jurkat, but not with any other cell lines tested. The co-modulation of CD3 and KT38 antigen was observed with stimulations of either MoAb T3 or KT38 on both TALL-1 and Jurkat. Upon radioimmunoprecipitation and SDS-PAGE analysis, MoAb KT38 precipitated the heterodimer of 40-60 kD from Jurkat and TALL-1 under reducing conditions. Thus, MoAb KT38 is considered to be an anti-T idiotype (Ti) antibody to TALL-1 and Jurkat cells. MoAb KT38 was also shown to react with a minor population of peripheral blood lymphocytes (PBL) and with very few cells (0.5-2.0%) in the paracortical area of the lymph node. When PBL were stimulated in a KT38-coated culture flask for 5 days, the percentage of KT38-positive PBL was markedly increased. The CD3 antigen on these cultured PBL in the flask was modulated by the stimulation with MoAb KT38. Thus, it is suggested that a common idiotope exists on the T cell receptor of Jurkat, TALL-1 and a small percentage (1.9-6.1%) of PBL.

[Comparison of three methods for detecting Campylobacter pylori and measurement of human antibody titers against the whole organisms].

In order to detect Campylobacter pylori in the gastric mucosa, three different methods as 1) culture of the organisms, 2) immunostaining by monoclonal antibody against the cells, and 3) urease test were compared. In the disease group, positive % of each methods was 64, 75 and 74. However, positive % common to the three methods was only 48. The corresponding rate of culture and urease tests, and that of culture and staining methods was 81% and 60%, respectively. Therefore, it was concluded that urease test was a useful method, and that the three methods were necessary to confirm the existence of the organisms in the gastric mucosa. Total positive % of culture and staining methods in disease and control groups was 77 and 31, respectively, indicating that positive % of the disease group was much higher than that of the control group. Anti C. pylori titer was compared between culture-positive and culture-negative patients by ELISA. The titer of the former was significantly higher than that of the latter.

Mechanism of clearance of circulating CA19-9 in rats.

Molecules bearing the carbohydrate antigen 19-9 (CA19-9) epitope were prepared from serum and ascites of a patient with colon cancer or from the SW 1116 cell culture supernatant. Their plasma clearance mechanism was investigated in rats. The clearance curves of the molecules from three different sources showed a biphasic decrease. The coexistence of epitopes of CA19-9 and desialylated CA19-9 (Lea) in the same molecule was demonstrated by double-determinant enzyme immunoassays. CA19-9-bearing molecules with a low expression of Lea epitope, which were derived from the serum of a patient with colon cancer, showed mainly a slow disappearance, while the molecules with a high expression of Lea epitope from SW 1116 cells showed mainly a rapid disappearance. Thus, the clearance rate of the CA19-9-bearing molecules might be determined by their degrees of sialylation. This observation will be helpful for understanding the in vivo metabolism of CA19-9-bearing molecules.

Transient subclinical hypothyroidism in early pregnancy.

In the present study, a new clinical state of transient subclinical hypothyroidism in 12 early pregnant women is documented. The incidence of transient subclinical hypothyroidism was 18 (0.19%) among 9,453 pregnant women examined in this series in Sapporo. The characteristics of transient subclinical early gestational hypothyroidism in our study may be summarized as follows: temporarily increased TSH in the blood (11.7 +/- 6.3 microU/ml; mean +/- S.D.) in early pregnant women at 8.5 +/- 2.4 weeks of gestation, accompanied with or without reduced FT4 which spontaneously return to normal at 17.9 +/- 7.1 weeks; no subjective complaints and no previous history of thyroid disease; small struma; positive titers of antimicrosome antibody and antithyroglobulin antibody; normal serum hCG; negative results for TSH receptor antibody. None of the infants show any physical abnormality such as struma and none of the patients had neck pain or fever suggesting subacute thyroiditis. The presence of autoantibody to the thyroid gland and echographical findings strongly suggest the existence of Hashimoto's thyroiditis in early pregnant women with transient subclinical hypothyroidism, although the cause of transient subclinical early gestational hypothyroidism remains obscure.

[Fundamental and clinical studies of changes in endocrine conditions in cancer patients].

The aim of the present study was to investigate hypothalamic-pituitary function, pituitary-thyroid function as well as pituitary-adrenal function in cancer patients. 1. Hypothalamic-pituitary function: We already reported a tumor-related paradoxical increase of serum growth hormone (GH) in oral glucose tolerance test (OGTT) and TRH test, which seemed to be due to changes in pituitary GH cell, not hypothalamus. The high incidence of the paradoxical increase of GH in OGTT was found in patients with hepatocellular cancer and pancreatic cancer, whereas in TRH test it was observed in patients with pancreatic, colonic and gastric cancer. The highly sensitive GH enzyme immunoassay demonstrated in 12 non-responder patients in OGTT a significant reduction of serum GH at 30 minutes compared to basal values. 2. Pituitary-thyroid function: We already demonstrated that a high incidence of low T3 syndrome in elderly cancer patients compared to younger patients and metabolic changes in glucose must be involved in the pathogenesis. In addition, this syndrome is significantly related to prognosis. There were no significant changes in thyroid function before and after curative operation for tumors except in patients with poor prognosis. 3. Pituitary-adrenal function: It was shown that cancer cachexia is characterized by hypercortisolemia, which must be due to a defect of negative feedback control.

[A senile case of B-cell lymphoma with high titers of an EB virus-associated antibodies].

An 83-year-old man suffering from general malaise and discomfort in the epigastrium was admitted to hospital. On testing, peripheral blood and bone marrow smears disclosed a number of abnormal lymphocytoid cells, and a CT and a linear echo scan revealed splenomegaly and a multiple intraperitoneal lymphadenopathy, although no superficial lymph nodes were palpable. Further, abnormal lymphocytoid cells were positive for B1, B4, OKIa1, Leu1, Tac, anti-IgM, anti-IgD, and anti-lambda. Thus, a diagnosis of malignant lymphoma of the intermediate B-cell type was made. Surprisingly, serological examination reveared extremely high titers of EB-virus associated antibodies (VCAIgG 20, 480x, VCAIgA 320x, EAIgG 2,560x), but no EBNA or virus particles were detected in the tumor cells, so that no direct relationship was established between the development of the tumor and the EBV infection.

Malignant Zollinger-Ellison syndrome. Stabilization of liver metastasis after gastrectomy with resection of primary tumor.

A case of malignant gastrinoma of the jejunum and jejunal mesentery, with findings of metastasis in the liver was studied. Tumor resection and remnant gastrectomy, as well as liver biopsy, were performed. Of all the tumors removed, the metastatic tumor had the lowest gastrin content, which was 1.5 times that found in normal human antral mucosa. The gastrin distribution was of four peaks and one peak for the main tumor, versus two peaks for the metastatic tumor in which G17 was the main component. Seven years after the operation, computed tomography (CT) scan indicated there was stabilization of the liver metastasis, and now, ten years after the total gastrectomy, the patient is well and working full-time. Such an analysis can be of value in determining the prognosis in cases of malignant gastrinoma with liver metastasis.

Immunohistological and immunochemical characterization of a novel pancreatic cancer-associated antigen MUSE11.

Using the ascites fractions of a cancer patient as an immunogen, we have recently prepared an interesting monoclonal antibody (MoAb), MUSE11 (IgG1), to a circulating pancreatic cancer-associated antigen (antigen MUSE11). Immunoperoxidase study revealed that antigen MUSE11 was detected in virtually all adenocarcinomas (especially pancreatic carcinoma), whereas it was either negative or faintly positive in their normal counterparts among the digestive organ tissues. Western blotting analysis and periodic acid-Schiff carbohydrate staining revealed that antigen MUSE11 is a Mr 300,000 glycoprotein that is clearly different from other high-molecular-weight glycoprotein antigens which have been used for detection of circulating antigens in pancreatic cancer patients. MoAb MUSE11 appears to react with a peptide epitope of this molecule, since periodic acid and neuraminidase treatment of the antigen did not affect the reactivity of MoAb MUSE11 with the antigen, but trypsin and protease treatment abolished the reactivity. The antigen MUSE11 was purified using affinity chromatography in conjunction with MoAb MUSE11. This antigen appears to be highly glycosilated with N-linked carbohydrate moieties, since treatment of the pancreatic carcinoma Panc-1 cells with tunicamycin reduced the molecular weight band to Mr 110,000. Once antigen MUSE11 has been purified by affinity chromatography, neither CA19-9 nor pancreatic oncofetal antigens can be detected, although they were present at very high levels in the crude ascitic starting material.

Application of monoclonal antibody KH2 against lactosyl and neolactotetraosyl ceramide to immunohistochemical study of human cancers.

Monoclonal antibody KH2 (IgM) reactive with lactosyl ceramide and neolactotetraosyl ceramide was applied for the first time to the staining of human cancerous and noncancerous tissues which were fixed with formalin and embedded in paraffin. Digestive-tract cancers originating from stomach, colon, pancreas, gallbladder and bile duct were clearly stained. The staining intensity was especially strong in the apical portion and the intraluminal content of adenocarcinomas. In noncancerous tissues, it was of interest that gastric intestinal metaplasia and certain fetal tissues were stained. A part of the tubules of the kidney, pancreatic ductal cells, lung alveolar epithelial cells and polymorphonuclear leukocytes were stained in normal adult tissues in our study. Enhancement of the intensity of immunostaining by neuraminidase treatment indicated that most of the antigenic determinants should be sialylated and the antigenicity exposed after neuraminidase treatment in several kinds of cancerous and noncancerous tissues. These data suggest that the monoclonal antibody KH2 may be useful in the immunohistologic diagnosis of formalin-fixed human cancerous tissues.
