[Acute dilatation of the stomach in children. Apropos of 6 cases].

Six cases of acute gastric dilatation in children are reported. Clinical features of the syndrome are gastric distension, respiratory failure, dehydration and electrolyte disturbances. This potentially lethal pathology is well controlled by elementary therapeutics : gastric fluid suction and rehydration with electrolyte supplementation. Autonomic nervous system seems directly involved in the occurrence of these troubles, but pathophysiology remains unclear. In this way, one should carefully discuss the diagnosis with the aim of precisely discarding any underlying pathology (as in our case n. 6).

The pathologist's role in a multidisciplinary approach for soft part tissue sarcoma: a reappraisal (39 cases).

A combined treatment modality of radiotherapy, surgery, and monochemotherapy in 39 cases of soft tissue sarcomas is presented. The 39 patients presented initially with local disease only, without evidence of metastases. At the end of the surgical procedure the adequacy of the excision had been evaluated by the pathologist. Surgical procedure was complemented by systematic radiation therapy. The overall recurrence rate was 7.7%; the overall incidence of metastases 31%. In the group in which adequate excision had been obtained, no loco-regional recurrences were obtained, survival was significantly better, and the incidence of metastases was lower than in group in whom adequate excision could not be obtained. The importance of two criteria was stress: the depth and the character of the margins of the tumor which cannot be considered separately. The results suggest that there is a correlation between the general and loco-regional malignancy of soft tissue sarcomas.

Spontaneous perforation of Meckel's diverticulum in a neonate.

A case of a male newborn infant with spontaneous perforation of Meckel's diverticulum is reported. The clinical course consisted of delayed occlusive disorders and bloody stools following 36 h of normal gastrointestinal transit, without pneumoperitoneum and infection. Pathogenesis of the perforation is discussed and the importance of early diagnosis, especially in maternity hospitals, is emphasized.

[Multidisciplinary treatment of soft tissue sarcomas: an improvement in prognosis (author's transl)].

The results obtained in the treatment of 45 cases of soft tissue sarcomas are presented. All cases were reviewed and classified according to the modern criteria of malignancy. The treatment schedule comprised: (1) preoperative irradiation: 2 sessions of 6.50 Gy in 48 hours, target volume: whole limb segment; (2) surgical excision 48 hours later with systematic intraoperative histologic verification, until healthy tissue margins are obtained; (3) postoperative irradiation: 3 weeks later delivering a cumulative total dose of 50.00 Gy to the preoperative volume and 60.00 - 70.00 Gy to a reduced volume encompassing the surgical region with protection of vascular axes where possible; (4) chemotherapy: Actinomycin D 0.3 mg/m2 half and hour before the first 5 sessions of postoperative irradiation; (5) bilateral lung irradiation: 4 sessions of 3.75 Gy in 7 days to the whole chest. The results were as follows: local recurrence rate was 9.5 per cent at 5 years. In 21 cases in whom surgical excision was deemed histologically adequate, no recurrences was seen at 3 years (minimum follow-up). Survival at 5 years was 77 per cent. Deaths were due to metastatic spread, especially to the lungs. These results show an improvement as compared with historical series. New progress should be sought in combining a more aggressive type of chemotherapy for cases with high metastatic risk.

[Method for studying internal tensions at the intervertebral junction in extreme postures. Description of experimental material and preliminary results].

The authors describe a device for measuring the resultant tensions existing at the intervertebral junction (or its constitutive elements) in all spine attitudes. The reported method has been applied to the L4-L5 junction of a spine taken from the corpse of a 50 years old man. Although incomplete, the results suggest the following points: --the L4-L5 intervertebral junction seems to be very deformable, particularly in the sagittal plane; --the intervertebral disk is probably an elastic support which sustains shear stresses during flexion, extension and bending movements in the frontal plane; --during flexion, articular processes seem to be movement leaders as well as supports; --ligaments seem to play a small mechanical role, but their proprioceptive function seems essential.

[Radiological appearances of osteochondritis of the hip in children].

Radiological examinations are of primary importance in the diagnosis of osteochondritis of the hip in the children, and enable appropriate therapy to be selected. The classification described by Catterall is preferred to that of Taillard, which describes the classical stages of the disease, as it has prognostic value when frontal and profile hip x-rays are taken.

[Anatomy of the right atrioventricular valve (valva atrio-ventricularis dextra or valva tricuspidalis). Description of a new type of chordae: the mixed chordae].

The authors, continuing their work concerning the tricuspid valve which was communicated at the Fourth European Anatomical Meeting (Bále, 1977), now present a study of twenty anatomical preparations according to the Mac ALPINE technique of perfusion-fixation, giving to the preparation a morphology comparable to that of the heart at the end of the diastole. The present work consists in a descriptive and morphological study of one type of tendinous cord, not described until now, and denominated mixed cord. It defines itself by its valvular insertions which are situated on the four areas of the tricuspid valve described by SILVER and al. It is a cylindric tendinous cord, always ramified either into two branches which are in turn greatly ramified, or into a fan shaped spreading. Each ramification is attached to the valve by an expansion which is either perpendicular or parallel to the ring. This work specifies: the number of tendinous cords, their distribution to the three cusps of the tricuspid valve, the number of their ramifications and their valvular insertions.

[Hydroxy 9-methyl 2-ellipticinium acetate (NSC 264-137). Toxicologic study and therapeutic effect in 100 cancers (author's transl)].

A new derivative of ellipticine, hydroxy-9-methyl-2-ellipticinium acetate, was found to be a useful anti-tumor drug in advanced cancers which could not be treated any longer successfully by any other procedure. In our series of 100 patients, the best results were obtained with bone metastases from breast carcinomas and with anaplastic thyroid carcinomas. Most patients usually received a weekly perfusion of 80 mg/m2. The main characteristic of this drug is its lack of hematologic, and hepatic toxicity. No renal trouble was observed during the first year, but 2 deaths from renal insufficiency occured during the 18th and 15th month of treatment. The most frequent side effect consists of digestive troubles (nausea, vomiting) which rarely compelled to stop the treatment (4 times in 100 patients).

[Electrical study and functional approach to the peroneal muscles].

After an attempt to systematize and to give a terminology to the movement of the foot, the authors have studied the role of the muscle peroneus longus and muscle peroneus brevis. This work includes a brief anatomical reminder, and an analysis of electrostimulation and of electromyography. It appears to them that:--the muscles peroneus longus and peroneus brevis have no motor action on the talocrural articulation, but they are synergic with foot flexor muscles as well as with foot extensors, giving lateral stability to this articulation, avoiding varus deviation on a weight bearing limb;--the muscle peroneus longus deepens the plantar arch, enabling impulsion to stance when walking, avoiding weight bearing of the foot on its lateral side. Lastly, the authors think that plastic operations using the muscle peroneus brevis, for rupture of the Achille's tendon, that are performed on an athlete, are not prejudicial to the lateral stability of the foot since a quite inactive muscle is involved.

[Anatomical study and systematization of veins in the foot].

From the dissection and the injection of the foot veins on 70 anatomical pieces, the authors have found the 4 systems usually described:--Lejar's plantar venous rete;--the supra-fascial dorsal venous rete;--the dorsal veins of the foot;--the median and lateral plantar veins. All these systems communicate largely with one another and are practically without any valvula. Among the most interesting systems, one should notice the closely woven plantar rete predominating at the level of the anterior and posterior weight-bearing points, and the importance of the muscular plantar veins close to the weight-bearing points. From those anatomical data 3 conclusions follow: 1) the major importance of the superficial venous drainage in comparison with the deep one, which makes one think of the preponderant role of that system as venous motor at the level the weight-bearing points, while walking; 2) the easy opacification of all those systems in phlebography; 3) the uselessness of any complementary surgical action i the submalleolar regions during the cure of varices.
