Evaluation of some electrical methods for objective assessment of oral mucosal dryness.

The aim of this study was to evaluate different electrical methods (resistance, capacitance and polarization resistance) for clinical and objective registration of dryness of the oral mucosa. Measurements were performed in the buccal and in the lip mucosa before and every 30 min after injection of 1.0 ml methylscopolamine nitrate submucosally in the labial sulcus in 10 healthy subjects. For comparison, a dental mirror sliding test was used. About 1 h after injection, all three methods showed statistically significant decreases of values in the buccal as well as the lip mucosa except for polarization resistance, which showed such a difference only in the buccal mucosa. The deviations from initial values in the individual subject showed no agreement between measurements with the electrical instruments and oral mucosal dryness assessed with the mirror test. Neither was there any agreement between values registered with the three electrical methods. It seems that these electrical methods are not suitable for registration of individual variations in oral mucosal dryness.

Evaluation of a new device for measuring oral mucosal surface friction.

For the objective measurement of oral mucosal dryness or moisture, a device registering oral mucosal surface slide friction has been developed. Two prototypes, Probe I and Probe II, have been tested. Probe I was constructed for initial testing of the method and was based on easily accessible electrical components. Probe I was computerized and developed for more accurate registration and also for easy handling. Reliability and validity tests were carried out on Probe I as well as on Probe II. In repeated in vitro measurements, the probes showed good reproducibility. Validity was assessed on healthy subjects injected with methylscopolamine nitrate submucosally in the labial sulcus. All subjects experienced a pronounced oral mucosal dryness within half an hour. Registration with the surface slide friction device showed maximum friction values 1-2 h after injection. These reliability and validity tests gave good results for both Probe I and Probe II, but Probe II had several practical advantages over Probe I. Both probes were considerably more sensitive to changes of the oral mucosal surface than the previously used simple friction test using the back of a mouth mirror.

Epidemiological studies of geographic variations of cancer incidence in Sweden. Choice of variables and statistical units.

The Swedish Cancer-Environment Registry has been used for studies of the parts of cancer incidences and of their variations between subpopulations, which, in a statistical sense, can be 'explained' by environmental variables. In previous studies the dependence of age-standardized incidences in municipalities (279 in Sweden) on population density, socio-economic variables, smoking habits and a variable for 'diagnostic intensity', assumed to allow for variations in under-diagnosing and under-reporting, was studied in, i.a., multiple-regression analyses. Due to intrinsic variation in potentially etiologic factors and in size, municipalities are less suitable as geographic units. Therefore a system was developed to build up, from parishes (about 2,600), pseudo-municipalities standardized with respect to size and some environmental variable(s), in the present study population density. This paper shows, for a number of cancer diseases and for total cancer, the superiority of pseudo-municipalities to municipalities with respect to power of explaining variation and, preliminarily, also incidence.

Additive effect of isradipine in combination with captopril in hypertensive patients.

The effects of combined treatment with the calcium antagonist isradipine and the angiotensin-converting enzyme inhibitor captopril were investigated in a randomized, placebo-controlled parallel-group trial comprising 28 patients with essential hypertension. The average age was 50 years (range, 31 to 65 years). After all patients were given captopril 50 mg twice daily plus placebo for four weeks, they were randomly assigned into groups receiving in addition either placebo or isradipine 1.25 mg twice daily in increasing doses at four-week intervals. During Weeks 20 to 24, the captopril plus placebo group was given hydrochlorothiazide 12.5 mg per day. Blood pressure was measured in the morning, 12 hours after tablet intake. Supine blood pressure was reduced in the captopril plus isradipine group by -8/-6, -14/-9, -16/-8, and -11/-7 mm Hg compared with the placebo group. Changes in diastolic blood pressure were statistically significant at Week 8, whereas changes in systolic blood pressure were statistically significant at Weeks 12, 16, and 20. With the addition of hydrochlorothiazide (Weeks 20 to 24), only supine systolic blood pressure was significantly reduced. One patient was withdrawn from the trial due to a rash. The results indicate that combined treatment with a calcium antagonist and an angiotensin-converting enzyme inhibitor is effective in lowering blood pressure and that the combination is well tolerated during long-term therapy. The combination of captopril and isradipine was more effective than captopril given with a low dose of hydrochlorothiazide.

Resolved 6,7,8,9-tetrahydro-N,N-dimethyl-3H-benz[e]indol-8-amine: central dopamine and serotonin receptor stimulating properties.

The enantiomers of 6,7,8,9-tetrahydro-N,N-dimethyl-3H-benz[e]indol-8- amine (1a) were prepared and tested for their actions on central dopamine and serotonin (5-HT) receptors. The dopaminergic effects were shown to reside in the (1)-R enantiomer. It was shown that compound 1a and its (+)-R enantiomer possess potent central 5-HT1A receptor stimulating properties.

Chronic recurrent annular neutrophilic dermatosis. An entity?

Two cases with recurrent annular lesions of similar clinical appearance and course are described. The histopathological investigations showed in both cases dense and diffuse infiltrates mainly of polymorphonuclear neutrophilic leucocytes in the mid dermis. No fever, leucocytosis or elevated ESR were observed. Systemic corticosteroid treatment controlled symptoms but recurrence after treatment was withdrawn occurred in both cases. It is discussed if we are dealing with a variant of Sweet's syndrome or a new entity.

The nevoid basal cell carcinoma syndrome--Gorlin's syndrome. Multiple jaw cysts and skin cancer.

The nevoid basal cell carcinoma (NBS) syndrome with its characteristic multiple cysts of the jaws and skin cancer is of great interest to the dentist, the dermatologist and the oncologist. The dentist may have the opportunity of making an early diagnosis as the multiple cysts appear mostly in the ramus of the lower jaw. There is a great risk of recurrence as surgical removal is often very complicated. The disease is hereditary and is transmitted through females. The skin cancer can in most cases be treated surgically. It is essential that the family with the syndrome is investigated regularly and at periods of about six months.

Diltiazem in hypertensive patients with type II diabetes mellitus.

Twenty-three patients with essential hypertension and diabetes mellitus type II were treated with the calcium antagonist diltiazem (120 to 180 mg twice daily). The mean dose was 307 mg/day. The study was a double-blind, placebo-controlled, crossover design. All measurements were performed 12 to 14 hours after drug intake. Blood pressure, heart rate and forearm blood flow were measured noninvasively. Platelet function was studied by measuring adenosine diphosphate-induced platelet aggregation and the platelet specific proteins, beta thromboglobulin and platelet factor 4. Thromboxane B2 formation in serum and the plasma concentration of diltiazem and its metabolites N-demethyldiltiazem, deacetyldiltiazem and N-demethyldeacetyldiltiazem were measured both during placebo and diltiazem treatment. Diabetic control was evaluated by following HbA1C, fasting blood glucose and urinary glucose. Diltiazem reduced both systolic and diastolic (supine and standing) blood pressure significantly. Forearm blood flow was significantly increased by 32%, p less than 0.05. Supine heart rate decreased significantly, while no such change was seen in the standing position. No significant changes were observed in platelet function during diltiazem treatment. There was no relation between the observed blood pressure reduction and the plasma concentration of diltiazem or its metabolites. A positive correlation between the change in heart rate and the metabolite N-demethyldeacetyldiltiazem was observed (r = 0.647, p = 0.005). Three patients were excluded during diltiazem treatment (skin exanthema, headache and atrial fibrillation) and 1 during placebo treatment (angina pectoris). No negative effect on diabetes control was observed. Thus, diltiazem could be used for treatment of hypertension in diabetic patients.

Antihypertensive effect of felodipine or hydralazine when added to beta-blocker therapy.

In a double-blind randomized study, hydralazine (n = 59) or the new dihydropyridine calcium antagonist felodipine (n = 61) was added to previous treatment with beta-adrenoceptor blocking agents in a group of 120 patients with essential hypertension. Active treatment with either hydralazine or felodipine was given for 8 weeks after a 4-week placebo run-in period, at the end of which all patients had supine diastolic blood pressures greater than 95 mm Hg. Assessment of the results according to the intention to treat principle showed that felodipine was significantly more effective than hydralazine at the doses employed, reducing systolic blood pressure 10-19 mm Hg more than hydralazine and reducing diastolic blood pressure 5-11 mm Hg more than hydralazine (95% confidence intervals). The number of patients complaining of side effects, the number of complaints, and the number of patients that had to be withdrawn from treatment were numerically higher during treatment with hydralazine than with felodipine, but these differences were not statistically significant. Against this background it is concluded that felodipine is superior to hydralazine when added to an antihypertensive regimen consisting of beta-adrenoceptor blocking agents.

Predictors of stroke in the elderly.

Hypertension, diabetes mellitus, coronary heart disease and cigarette smoking have repeatedly been identified as risk factors for stroke in young and middle-aged individuals. In order to find predicting factors for stroke in the elderly we assessed health characteristics in 55 stroke victims in the age range 65-75 years (mean 70.7 +/- 2.7) allocated to our stroke unit at Ostra University Hospital in Gothenburg. For comparison we used data from 2,009 individuals participating in the ongoing longitudinal population study "70-year-old people in Gothenburg, Sweden". Among the stroke victims we found a higher prevalence of hypertension (63.5% vs. 27.8%, p less than 0.001), diabetes mellitus (21.8% vs. 6.2%, p less than 0.001) and a history of previous myocardial infarction (12.7% vs. 4.8%, p less than 0.01), thus confirming previous findings. There was no difference with regard to smoking habits (32.7% vs. 27.5%, NS), which is at variance with findings in the young and middle-aged.

Prognosis and atopic background of juvenile plantar dermatosis and gluteo-femoral eczema.

A 10-year follow-up of patients with juvenile plantar dermatosis and gluteo-femoral eczema disclosed a worse prognosis than previously assumed, with a course in some cases protracted into adult age. The presence of atopy was established with a clinical point score system. It was found that 52% of the patients with juvenile plantar dermatosis and 80% of those with gluteo-femoral eczema were atopic. In many cases the childhood dermatitis was followed in adult age by a hand eczema. This was the case in 14% of the gluteo-femoral eczema patients, and no less than 26% of the juvenile plantar dermatitis patients.

Hand eczema: an evaluation of the frequency of atopic background and the difference in clinical pattern between patients with and without atopic dermatitis.

In a prospective study of 63 patients with long-lasting hand eczema, a diagnosis of atopic dermatitis could be established in 49%. Comparison between patients with atopic dermatitis and those without atopic dermatitis showed that the hand eczema pattern differed very little between the groups. Hyperlinearity was significantly more common in the palms of the patients with atopic dermatitis.

Familial occurrence of eccrine tumours in a family with ectodermal dysplasia.

In two brothers and their sister, ectodermal defects including hypodontia, onychodystrophia, trichodysplasia and palmoplantar keratosis were observed. In addition to the ectodermal dysplasia a diffuse palmoplantar eccrine hyperplasia was noted as well as tumours and cysts of eccrine origin. This seems to be the first report of familial occurrence of eccrine tumours.

A method for the quantitation of intestinal metaplasia of the stomach by morphometry.

A novel method to quantitate the extent of intestinal metaplasia in gastrectomy specimens is presented. Morphometric measurements of histochemically labeled mucin-producing goblet cells were done in three selected gastrectomies (one having a peptic ulcer, one with adenocarcinoma of the intestinal type, and one with adenocarcinoma of the diffuse type). The sectioning of the gastrectomy specimens was standardized. The results indicated that the intestinal metaplasia was significantly higher in the specimen with adenocarcinoma of the intestinal type (as compared to the other two specimens), both along the lesser and greater curvatures as well as in the fundic area. These quantitative results confirm nonquantitative reports based on subjective visual impressions. This quantitative histochemical method for measuring the actual length as well as the topographical distribution of intestinal metaplasia in resected stomachs will be used in future studies of intestinal metaplasia with the aim of disclosing possible differences among populations with disparate incidences of gastric carcinoma.

Comparison of a gelation and a chromogenic Limulus (LAL) assay for the detection of gram-negative bacteria, and the application of the latter assay to milk.

When a chromogenic Limulus Amoebocyte Lysate (LAL) assay and a tube gelation LAL assay were compared for the detection of Gram-negative bacteria using a strain of Pseudomonas putida, the detection level (approximately 10(3) cfu/ml) and cost of the assays were approximately the same for both assays but the reading was more precise for the chromogenic substrate assay. A modified chromogenic assay was devised for detection of Ps. putida in milk.
